Idiopathic pleuroparenchymal fibroelastosis: diagnosis and management.

INTRODUCTION: Idiopathic pleuroparenchymal fibroelastosis (iPPFE) is a rare progressive interstitial lung disease characterized by upper-lobe fibrosis, severe restrictive impairment, and poor prognosis. Unlike idiopathic pulmonary fibrosis, in which acute exacerbations, chronic respiratory failure, and lung cancer are the major causes of death, iPPFE primarily leads to progressive respiratory failure, often complicated by malnutrition and recurrent pneumothorax. Despite growing recognition, its pathogenesis remains unclear and no effective treatments exist. AREAS COVERED: This review summariz

Conducting a registry-based randomised trial (REDOX) in chronic respiratory failure: experiences and advice.

Registry-based randomised controlled trials (R-RCTs) represent a paradigm shift in research, with the potential to accomplish pragmatic but large trials with high external validity. In this paper, we review our experiences from planning and performing the REgistry-based randomised controlled trial of treatment Duration and mortality in long-term OXygen therapy (REDOX) trial, the first R-RCT within respiratory medicine. The REDOX study compared the two established treatment options of home oxygen 15 and 24 h per day. Previous recommendations to use oxygen for at least 15 h but preferably 24 h p

Exploring the role of serum adiponectin and its holigomerization in fibrotic interstitial lung diseases: results from a cross-sectional study.

Intestitial lung diseases (ILDs) include a group of inflammatory and fibrotic pulmonary disorders with different etiologies which in several patients might lead to a progressive reduction of respiratory capacities and chronic respiratory failure. Nowadays, biomarkers for predicting the ILD progression and response to therapies are lacking. Adiponectin, the most abundant peptide secreted by adipocytes, has emerged as a potential response biomarker in fibrotic progressive ILDs. The aim of this observational prospective single-center cross-sectional study is therefore to verify whether serum adip

The Impact of Comorbidities and Intensive Care Unit (ICU) Admissions on Survival in Kyphoscoliosis-Related Respiratory Failure: A Retrospective Cohort Study.

Background: Kyphoscoliosis is a restrictive thoracic disorder frequently associated with chronic respiratory failure. While home non-invasive ventilation (NIV) improves short-term outcomes, long-term mortality predictors remain underexplored in this context. Methods: This retrospective cohort study evaluated 88 kyphoscoliosis patients with chronic respiratory failure who were initiated on home-based NIV between 2008 and 2018 at a tertiary ICU outpatient clinic. The demographic, clinical, and laboratory data were analyzed. Survival was assessed using Kaplan-Meier analysis, and independent predi

An underestimated factor for therapeutic decision-making in rare diseases: parents' (un)knowledge-the example of Duchenne muscular dystrophy caregivers and non-invasive ventilation.

BACKGROUND: Duchenne muscular dystrophy (DMD) is a progressive genetic disease that leads to degeneration of muscles, including respiratory muscles, and requires early introduction of non-invasive ventilation (NIV). Parental knowledge and management strategies for pulmonary care are essential when respiratory function is compromised, particularly in conditions involving sleep apnea and the progression of chronic respiratory failure. The aim of this study was to assess parental knowledge of key aspects of pulmonary care in DMD and to identify knowledge gaps that may influence therapeutic decisi

Moyamoya Disease in a Patient With Cerebral Palsy Presenting With Intraventricular Hemorrhage and Hydrocephalus Requiring Ventriculoperitoneal Shunt Placement: A Case Report.

Moyamoya disease (MMD) is a chronic cerebrovascular disorder characterized by progressive stenosis of the internal carotid arteries (ICA) and the development of fragile collateral vessels. These abnormal vessels can lead to ischemic or hemorrhagic strokes, with intracranial hemorrhage being a notable complication. Although rare, hydrocephalus may also occur in MMD, particularly in the setting of intraventricular hemorrhage (IVH). We present the case of a 42-year-old male with a history of cerebral palsy (CP) and MMD, diagnosed at the age of seven, who developed IVH and hydrocephalus. Despite p

Myositis associated with antimitochondrial autoantibodies presenting with early respiratory failure.

Myositis associated with antimitochondrial M2 antibodies (AMA) is a distinct subtype of inflammatory myopathy characterized by axial muscle weakness and, occasionally, respiratory failure. This report highlights two cases involving patients presenting with severe respiratory failure while ambulant. Patient 1: An 82-year-old man was referred to our hospital for respiratory failure and truncal muscle weakness. Shortly after hospitalization, the patient succumbed to respiratory failure. Autopsy findings revealed inflammation predominantly affecting the respiratory muscles. Later, AMA were detecte

A lung function threshold for survival? - FEV1Q and mortality in patients with COPD and chronic respiratory failure.

INTRODUCTION: The FEV1 quotient (FEV1Q), calculated as the index between FEV1 and a theoretical lower survivable FEV1 threshold of 0.4L for females and 0.5L for males, has been investigated as a novel method of interpreting results from lung function testing. The applicability of the FEV1Q in populations with chronic respiratory failure has not been studied, and the continuous association between the FEV1Q and mortality is unknown. METHODS: Longitudinal analysis of data from the DISCOVERY database. First percentile values of FEV1 were determined. The predictive ability of FEV1Q and FEV1 %-pre

The lost chILD: a case report of delayed diagnosis of surfactant protein C deficiency in a 15-year-old African male.

BACKGROUND: Childhood interstitial lung disease (chILD) encompasses a heterogeneous group of rare disorders characterized by respiratory distress, hypoxemia, exercise intolerance, and distinctive radiological findings. Despite the variable age of onset, these conditions often present with overlapping symptoms and variable progression, even with identical genetic mutations. Surfactant protein deficiencies fall under the category of chILD, with Surfactant Protein-C (SP-C) deficiency posing significant diagnostic challenges due to its rarity and the variable severity of clinical presentation. [Should patients with respiratory failure and obesity be made to lose weight?].

Patients suffering from chronic respiratory failure may present with obesity, which can cause respiratory failure, particularly in cases of Obesity Hypoventilation Syndrome (OHS) or certain forms of asthma. It can also aggravate symptoms such as dyspnea. In this context, patients are often asked to change their lifestyle, or even to undergo bariatric surgery, the common objective being to help them lose weight. That said, a question may be raised: is weight loss always desirable? In the present review of relevant articles, we report on: (a) the current possibilities for achieving significant a

Postoperative atrial arrhythmias after bilateral lung transplantation with intraoperative V-A extracorporeal membrane oxygenation: a single-center experience.

INTRODUCTION: Lung transplantation (LT) is the standard treatment for end-stage chronic respiratory failure that does not respond to other therapies. Advances in surgical techniques and perioperative care have improved survival rates. However, postoperative complications, particularly atrial arrhythmias (AA) remain clinically significant. Although AAs are frequently observed in the early postoperative period, data regarding their incidence and impact on outcomes are scarce. This observational study aims to: (i) assess the incidence of new-onset postoperative AA within one month of bilateral LT

Transitioning Between Volume-Assured Pressure Support Modalities on Home Mechanical Ventilators for Chronic Respiratory Failure.

Clinicians who care for patients with chronic respiratory failure are faced with complex medical decisions when initiating and managing home mechanical ventilation. The technological advances and complexities of home mechanical ventilators (HMVs) have outpaced the education and resources provided to clinicians who manage this unique patient population. Many clinicians are familiar with 1 brand and depend on medical equipment suppliers when prescribing and managing home ventilators. A recent national safety recall in the United States of 1 of the most commonly used HMVs has led to an increase i

Non-Invasive Ventilation: When, Where, How to Start, and How to Stop.

Non-invasive ventilation (NIV) is a cornerstone in the management of acute and chronic respiratory failure, offering critical support without the risks of intubation. However, successful weaning from NIV remains a complex, high-stakes process. Poorly timed or improperly executed weaning significantly increases morbidity and mortality, yet current clinical practice often relies on subjective judgment rather than evidence-based protocols. This manuscript reviews the current landscape of NIV weaning, emphasizing structured approaches, objective monitoring, and predictors of weaning success or fai

Adverse Events in Non-invasive Ventilation Approaches: Systematic Review.

Non-invasive ventilation (NIV) plays a critical role in the management of acute and chronic respiratory failure, offering benefits over invasive mechanical ventilation. However, its use is associated with various adverse events that may impact clinical outcomes. This systematic review aimed to evaluate the types, frequencies, and clinical consequences of complications related to NIV. A systematic search of PubMed, EMBASE, and Cochrane Library databases was conducted according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses 'PRISMA' guidelines, covering studies published b
