Clinical applications of body ventilators.

Interest has been increasing in providing ventilatory support in the home for patients with chronic respiratory failure, mainly with the use of positive pressure ventilation via a chronic tracheostomy. However, body ventilators that assist ventilation by applying intermittent negative or positive pressure to the thorax, abdomen, or airway without requiring an artificial airway, can offer distinct advantages for selected patients over systems requiring a permanent airway. These ventilators include the iron lung, portable lung (Portalung), pneumowrap, chest cuirass, pneumobelt, rocking bed, and

[Intravenous theophylline: adaptation of dosage to blood theophylline levels at admission and to clearance].

Intravenous infusions of aminophylline expose the patient to the risk of overdosage related to the narrow safety margin of the therapeutic concentrations and to the great individual variability of its excretion. The aim of this study was to evaluate a simplified protocol designed to determine the optimal dose of theophylline based on total body clearance. Forty-four patients (average age: 63 years) admitted with decompensation of chronic respiratory failure (N = 33) or with status asthmaticus (N = 11) were studied. Theophylline was administered initially at a constant rate R0 (mg/kg/h) dependi

Clinical applications of mechanical ventilation.

This paper reviews recent applications of mechanical ventilation such as controlled hypoventilation in acute asthma, domiciliary nocturnal ventilation in chronic respiratory failure due to neuromuscular disease and improvement of left ventricular performance by raised intrathoracic pressure. Established uses of mechanical ventilation include control of respiratory failure, intracranial pressure and pulmonary hypertension while other uses such as internal splinting of flail chest, simultaneous ventilation-compression cardiopulmonary resuscitation and prophylactic postoperative ventilation are m

Bioavailability and diuretic effect of furosemide during long-term treatment of chronic respiratory failure.

The bioavailability and diuretic effect of furosemide 40 mg administered orally for at least 6 months have been compared in patients with chronic respiratory failure and in healthy controls. The mean urinary recovery of unchanged drug was 11.5 mg and 9.41 mg in 24 h after pre- and postprandial administration to 10 patients, whereas the recovery was 14.4 mg in 10 healthy subjects. The diuretic effect, in terms of urine flow and sodium ion excretion in the 6 h after administration, was also less in patients than in healthy subjects. This was ascribed to the lower bioavailability of furosemide in

[Long-term treatment of chronic respiratory insufficiency using endotracheal ventilation at home in 118 tracheotomy patients].

118 patients suffering from severe chronic respiratory failure were submitted to domiciliary artificial ventilation (V.A.D.T.) by tracheotomy using a fixed rate volumetric respiration. The daily length of V.A.D.T. was 10 to 16 hours. The improvement on the blood gases, their stability, the disappearance of signs of right heart failure and polycythaemia, as well as a large reduction in mean length of hospital stay per subject and per year after V.A.D.T., underlined the efficacy of this method. In the long term (10 years) there was a 50% survival in patients with restrictive respiratory failure

A simple method of predicting left ventricular function in stabilized chronic respiratory failure.

A simple method of assessing left ventricular function in stabilized chronic respiratory failure (CRF) is proposed. 21 patients with CRF were ranked according to the prediction of left ventricular function derived from eight elementary clinical criteria including standard chest X-ray and conventional basal electrocardiography. Equilibrium radionuclide angiocardiography was performed at rest (rERNA) for the evaluation of left ventricular systolic function (ejection fraction, LVEF; peak ejection rate) and diastolic function (peak filling rate). Right ventricular ejection fraction (RVEF) was also

[Outcome of patients with chronic respiratory insufficiency treated at home with tracheotomy and assisted ventilation].

60 patients were studied with severe chronic respiratory failure (IRC) and a permanent tracheostomy treated with domiciliary ventilation (VADT). The study commenced from the time of the tracheostomy and included length of survival (by the actuarial method) quality of life, subjectively and objectively (progress of blood gases in a stable clinical state and cumulative length of periods in hospital). Factors influencing the decision for tracheostomy and VADT were examined. Restrictive cases benefitted from the technique as much from the improved quality of life as from the duration of survival (

[Compliance with long-term respiratory assistance in patients with chronic respiratory insufficiency].

The domiciliary treatment of severe chronic respiratory failure (I.R.C.) by oxygen or assisted ventilation was accomplished by members of a recently created national organisation (ANTA-DIR, 1980). This work reports the results of a survey carried out in Nice on 19 patients (12 men and 7 women) followed for more than a year. A questionnaire was filled in at the home of each patient, primarily to assess the functional outcome and the level of compliance of the therapy prescribed. We noticed and 86% drop in the number of days spent in hospital over a 2 year period. An analysis of the diary cards

Problems before, during and after mechanical ventilation in chronic bronchitis and emphysema.

Recent work has shown considerable advance in the potential for treatment of patients with acute exacerbations of chronic respiratory failure, and the "blue and bloated" pattern of chronic bronchitis and emphysema. New guidelines for controlled oxygen therapy have been developed, which appear to enable the dangers of hypoxaemia and respiratory acidosis in an acute exacerbation to be reduced. New modalities of therapy, long-term oxygen treatment. Almitrine, and beta 2-agonists, hold promise of prolonging the survival of useful life in these severely afflicted patients.

Inspiratory muscle resistive training in respiratory failure.

In this study, 4 patients who had failed weaning attempts using traditional methods underwent inspiratory muscle resistive training (IRT). Three patients showed improvements in inspiratory muscle strength (mean peak negative inspiratory pressure improved from 38 to 54 cm H2O) and in respiratory muscle endurance (mean PCO2 at the end of the same or a longer period of spontaneous breathing improved from 70 to 52 mmHg). These 3 patients were successfully weaned from mechanical ventilation after 10 to 24 days of IRT. A fourth patient showed no improvement and could not be weaned. This pilot study

[Effect of artificial ventilation with an end-expiratory plateau on gas exchange amd hemodynamics in chronic respiratory failure].

Previous studies of pulmonary models and with animals have shown that in obstructive disease of the airways, ventilation with an end-expiratory plateau improves ventilation distribution. Paradoxically, there has been no data published on patients with obstructive disease. For this reason, we examined the effects of mechanical ventilation with an end-expiratory plateau on gas exchange and haemodynamics in 12 patients presenting acute exacerbations of chronic respiratory failure. Following a period of conventional controlled ventilation, two plateaux of 0.3 and 0.5 s respectively are successivel

Hypercapnic acidosis induced by nutrition in mechanically ventilated patients: glucose versus fat.

Total parenteral nutrition (TPN) increases CO2 production (VCO2) in patients on intermittent positive-pressure ventilation who cannot match their CO2 excretion to the CO2 load, leading to an increase in PaCO2. We studied gas exchange and blood gas values in six patients with chronic respiratory failure, who were ventilated at low (6 +/- .7 L/min) and high (10 +/- 2 L/min) minute ventilation during three randomized nutritional regimens: control (255 kcal/day), glucose TPN (2550 kcal/day), and lipid TPN (3000 kcal/day). At the two levels of ventilation, TPN compared to control increased VCO2 and

Effect of oral high frequency ventilation by jet or oscillator on minute ventilation in normal subjects.

Normal subjects were asked to breathe through an open ended tube while high frequency oscillations were superimposed on tidal breathing via a side arm, either an eight inch (20 cm) loudspeaker or a jet ventilator being used. Both systems were comfortable and well tolerated. Spontaneous minute ventilation fell by 19-46% at frequencies up to 33 Hz without a rise in transcutaneous PCO2. Maximum ventilatory savings occurred at 1.6 Hz with the jet ventilator (p less than 0.01) and at a frequency corresponding to respiratory system resonance with the loudspeaker. This suggests that during oral high

The surgical management of children with familial dysautonomia.

Familial dysautonomia (FD) is a rare incurable genetic disorder with multisystem involvement. Most of its clinical manifestations are related to disorders of the autonomic nervous system. The disease is associated with specific disturbances of the upper gastrointestinal tract: pharyngoesophageal dyskinesia, gastroesophageal reflux, and prolonged gastric emptying. About 40% of the dysautonomic children manifest repeat vomiting crises. In view of the extensive gastrointestinal symptomatology, children with FD are prone to repeated aspiration pneumonia and chronic respiratory failure, while inade

[Incidence of ciliary beats in nasal cells obtained from smears. Study of 40 patients of whom 28 were children with respiratory pathology].

Disturbance of ciliary function are considered as a significant factor in the outcome of chronic respiratory afflictions. Thanks to a simple method of stroboscopic lighting, the ciliary beat frequency was studied on 40 nasal mucus smears obtained by swabbing in 28 children and 12 adults with chronic respiratory failure or recurrent disease. The 28 children were divided into three groups: group A (n = 10), pulmonary disease (broncho-pneumonia); group B (n = 3), Kartagener syndrome; group C (n = 15) pure otorhinolaryngological (ORL) disease (rhino-opharyngitis, sinusitis, otitis). Twelve adults
