Total anomalous pulmonary venous connection (TAPVC) is a rare congenital cardiac malformation, and cases complicated with airway stenosis are even less common, posing substantial diagnostic and therapeutic challenges. We report a 4-month-old male infant with supracardiac TAPVC, accompanied by left main bronchial stenosis, a central atrial septal defect (ASD), and severe pulmonary hypertension. Following supportive management, the patient underwent TAPVC repair and atrial septal defect patch repair. Intraoperatively, weaning from cardiopulmonary bypass initially failed due to severe pulmonary h

BACKGROUND: Adenoid hypertrophy is a common condition in children and a leading cause of upper airway obstruction, associated with sleep-disordered breathing, recurrent otitis media, and chronic rhinosinusitis. Although nasal endoscopy is the most widely used diagnostic tool, its invasiveness and limited correlation with functional obstruction in intermediate cases highlight the need for alternative methods. Acoustic rhinometry (AR) is a non-invasive technique that measures nasal and nasopharyngeal geometry and may overcome several limitations of conventional assessments.

Sleep in the ICU is subjectively and objectively poor. Patients in the ICU have short sleep duration and highly fragmented sleep. Poor sleep in the ICU is associated with worse outcomes, particularly the development of ICU delirium. Patients undergoing noninvasive or invasive mechanical ventilation struggle with sleep. For those patients on noninvasive ventilation, settings may need to be adjusted compared to wake to account for the physiological changes that occur with the sleep state. Specifically, upper airway collapsibility (ie, obstructive sleep apnea) must be considered as do changes in

OBJECTIVE: Inferior turbinate (IT) hypertrophy is a major cause of nasal obstruction. Although surgical intervention is effective, it can be associated with morbidity and prolonged recovery. This study evaluated the safety, efficacy, and tolerability of a novel submucosal fractional radiofrequency (FRF) treatment.
METHODS: A prospective, single-arm, open-label study was conducted from March to November 2024. Patients with nasal obstruction caused by IT hypertrophy unresponsive to at least 1 month of medical treatment were included in the study. The procedure was performed endoscopically under

BACKGROUND: Airway stenting is a critical intervention to restore airway patency, yet complications such as stent migration, mucostasis, airway injury, and granulation tissue formation remain major concerns. Although various covering materials have been explored, the effectiveness of amniotic membrane (AM) as a stent cover has not been evaluated. This study is the first to assess airway-related complications of a novel AM-covered self-expandable metallic stent (SEMS) compared with a commercially available synthetic polymer-covered SEMS. To compare the incidence of stent-related complications b

With fungal infections rising worldwide, the use of azoles, polyenes, and echinocandins, the mainstay of antifungal therapy has expanded markedly. With broader use, uncommon adverse effects are increasingly being identified. Azoles, the most widely prescribed class of antifungal agents, inhibit several cytochrome P450-dependent steps in human steroidogenesis, leading to disruption of endocrine pathways. Adrenal and gonadal dysfunctions with ketoconazole, and mineralocorticoid excess with posaconazole and itraconazole, are well-documented. Uncommon manifestations, such as voriconazole-induced s

BACKGROUND: There is limited and conflicting evidence on the impact of obstructive sleep apnoea (OSA) and its treatment with positive airway pressure (PAP) therapy on pulmonary hypertension (PH). Our research question was: does treatment of OSA and obesity hypoventilation syndrome (OHS) with PAP therapy improve pulmonary arterial pressures?
METHODS: A systematic review and random effects meta-analysis was performed (PROSPERO: CRD42024595312). PubMed, Embase, Scopus, Web of Science, EBSCO, Cochrane Central Register of Controlled Trials (CENTRAL) and ClinicalTrials.gov were searched using the P

BACKGROUND: Cervical spinal cord injury (CSCI) often results in respiratory muscle weakness, requiring prolonged mechanical ventilation and tracheostomy. However, the frequency and timing of ventilator-weaning milestones after acute hospitalization are not well described. This study assessed the rates and timing of daytime ventilator weaning, liberation from invasive ventilator support, and decannulation among adults with CSCI managed under a standardized weaning protocol at a rehabilitation center, and the associations of weaning milestones with discharge disposition.

BACKGROUND: Airway stent placement is used to manage malignant airway obstruction, but often these patients cannot receive immune checkpoint inhibitors (ICIs) because of poor performance status (PS). We evaluated whether stenting could enable ICI therapy and improve outcomes.
METHODS: We retrospectively reviewed patients with lung cancer who received anticancer therapy after airway stent placement from January 2016 to July 2025. Outcomes included 1-year overall survival (OS), median survival time (MST), ECOG PS changes, complications, immune-related adverse events, and stent removal.

BACKGROUND: The diagnosis of P. jirovecii pneumonia (PJP) in immunosuppressed, HIV-negative patients is often limited by the inability to perform a bronchoalveolar lavage (BAL) due to respiratory insufficiency. In this study, we evaluated the diagnostic accuracy of combining serum beta-D-glucan (BDG) testing with P. jirovecii polymerase chain reaction (PCR) in oral wash (OW) samples as noninvasive tools for PJP diagnosis.
METHODS: This was a retrospective, single-center study including all immunosuppressed non-HIV adult patients with suspected PJP in whom a Fujifilm Wako β-glucan test BDG and

OBJECTIVES AND HYPOTHESIS: Tonic diaphragmatic activity is common during non-invasive ventilation (NIV), suggesting efforts to increase end-expiratory lung volume. We assessed the feasibility and physiological impact of NeuroPAP, a novel NIV mode continuously adjusting the delivered positive pressure proportionally to diaphragm electric activity (Edi) during both inspiration and expiration, in infants with respiratory failure. We hypothesized NeuroPAP would enable dynamic control of end-expiratory pressures (PEEP).
METHODOLOGY: This prospective crossover study enrolled premature neonates (25-

BACKGROUND: Permanent pacing in pediatric patients is complicated by small body size, vascular access limitations, and the need for durable long-term management. Leadless pacemakers offer an emerging alternative that minimizes infection and lead-related complications, yet vascular access remains a key challenge in young patients.

INTRODUCTION: Rib fractures in the elderly are associated with significant morbidity and each additional rib fracture has shown an increased risk of pneumonia and death. However, the most recent data fully evaluating this effect is outdated, using x-ray imaging from the 1980-1990s. Computed tomography imaging is now the gold standard in diagnosis and management has evolved. We hypothesized that mortality rates would be decreased from those historical reports and sought to evaluate the effect of each additional rib fracture on geriatric outcomes.
METHODS: A 5-year retrospective review of the t

Placental insufficiency is a central pathological condition underlying fetal growth restriction and is strongly associated with adverse outcomes during the neonatal period. Although epidemiological and clinical studies have consistently linked compromised placental function to neonatal respiratory, neurological, gastrointestinal, and infectious morbidity, the mechanisms through which placental insufficiency translates into organ-specific neonatal vulnerability remain unclear. Increasing evidence from human observations and experimental animal models indicates that placental insufficiency disru

RATIONALE: Congenital central hypoventilation syndrome (CCHS) is caused by PHOX2B gene variants, resulting in highly variable cardiac autonomic dysregulation, including abrupt sinus pauses and risk of sudden unexpected death. The relationship between age, PHOX2B genotype, and cardiac autonomic dysfunction remains unknown, and CCHS-related physiomarkers are lacking.
OBJECTIVE: To assess the impact of age and genotype severity on longitudinal heart rate variability (HRV), a noninvasive measure of cardiovascular autonomic function, in CCHS.
METHODS: Longitudinal HRV measures were calculated fro

Pathogenic heterozygous variants in PHOX2B are associated with congenital central hypoventilation syndrome (CCHS), which is characterized by autonomic nervous system dysregulation severely affecting respiratory control. The interpretation of PHOX2B missense variation is challenging due to their rarity and the lack of available functional evidence. Consequently, most PHOX2B missense variants are classified as variants of uncertain significance (VUSs), complicating the timely diagnosis and clinical management of the condition. To generate an improved model for assessments of PHOX2B missense vari

AIM: Rhythmic breathing movements driven by the brainstem rely on chemosensory inputs to homeostatically adjust motor output to the prevailing metabolic demand. The central CO2 chemoreflex is a critical component of this neural circuitry, as defects in these sensors cause hypoventilation syndromes, which are typically difficult to manage pharmacologically. Progesterone has long been known to stimulate breathing in both sexes, and remarkably the progestinic metabolite, etonogestrel (ETO), enhances CO2 chemosensitivity in animal models and female patients affected by congenital central hypoventi

OBJECTIVE: Ex-utero intrapartum treatment (EXIT) procedures are employed during delivery to establish a safe airway on placental support for neonates with potential airway obstruction. While EXITs are typically planned with a multidisciplinary team, these procedures may occur emergently due to preterm labor, late diagnosis, or delayed transfer of care. This systematic review compares outcomes of scheduled versus unscheduled/emergent EXIT procedures.
DATA SOURCES: We searched PubMed, Scopus, and CINAHL in March 2025 to identify observational studies describing EXIT procedures.
