Role of elexacaftor-tezacaftor-ivacaftor therapy on fecal elastase-1 and fat-soluble vitamins in children with cystic fibrosis.

Clinical studies have shown that Elexacaftor-Tezacaftor-Ivacaftor (ETI) improves lung disease and body weight in individuals with cystic fibrosis (CF); however, gastrointestinal system effects remain unclear. The purpose of this study was to evaluate exocrine pancreatic function using fecal elastase-1 (FE-1) levels in CF patients receiving ETI therapy and to assess changes in fat-soluble vitamin levels specifically within the pancreatic-insufficient (PI) subgroup. We retrospectively evaluated FE-1 levels before and during ETI treatment in the entire study group. Additionally, sweat chloride le

Impact of interscalene versus superior trunk blocks as sole anesthetics on respiratory outcomes for shoulder arthroscopy: a randomized controlled trial.

BACKGROUND: The interscalene block (ISB) provides effective analgesia for shoulder surgery but commonly results in hemidiaphragmatic paralysis (HDP). The superior trunk block (STB) is an alternative technique to reduce this complication while maintaining effective analgesia. This randomized trial compared ISB and STB used as sole anesthetic techniques, without general anesthesia, for shoulder arthroscopy, focusing on diaphragmatic excursion, spirometry, and clinical respiratory outcomes. METHODS: This prospective randomized observer-blinded study included 123 patients allocated to either ISB (

Analysis of Surgical Outcomes During a Period of Intravenous Fluid Restriction as a Result of a National Shortage.

BACKGROUND: Hurricane Helene severely damaged the Baxter Pharmaceutical Plant in North Cove, North Carolina, in the fall of 2024, resulting in a national shortage of IV fluids. Our institution implemented an IV fluid restriction (IVFR) protocol to conserve supply across many domains of the hospital, including the perioperative space. STUDY DESIGN: Gynecologic, plastic, urologic, and general surgery procedures performed during the IVFR period were reviewed in our local NSQIP and MBSAQIP datasets. These cases were matched to historical controls from the pre-IVFR period using random 1:1 propensi

UQCRC1 deficiency impairs mitophagy via PINK1-dependent mechanisms in Parkinson's disease.

Oxidative phosphorylation (OXPHOS) and mitophagy are functionally interconnected cellular processes, the defects of which are considered key driving forces behind the pathogenesis of Parkinson's disease (PD). UQCRC1, a core subunit of the mitochondrial respiratory chain complex III, is a recently identified familial PD gene whose pathogenic mutations result in OXPHOS stress. Given its importance, however, the role of UQCRC1 in idiopathic PD as well as mitophagy has not been investigated. In this study, we collected 19 datasets comprising postmortem substantia nigra from 150 cases of non-diseas

The impact of preoperative oropharyngeal microflora, decontamination, and postoperative nosocomial and opportunistic infections on the occurrence of respiratory complications in patients undergoing esophagectomy for esophageal cancer after chemoradiotherapy. A single-center cohort.

INTRODUCTION: Surgical oesophagectomy remains the primary curative treatment for oesophageal cancer. It is a challenging procedure that carries the possibility of serious postoperative complications. METHODOLOGY: Retrospective analysis of preoperative oropharyngeal microbiologic cultivation, the role of decontamination, and the postoperative microbiologic cultivation in patients after hybrid esophagectomy with mild or severe respiratory complications (pneumonia and respiratory failure), and anastomotic leakage. RESULTS: Significantly more pneumonias were found in the group without eradication

Internal Nasal Dilators After Septorhinoplasty: A Randomized Controlled Trial of Functional and Aesthetic Outcomes.

BACKGROUND: Postoperative nasal obstruction and asymmetry can compromise satisfaction after rhinoplasty. Internal nasal dilators (INDs) may support early healing by maintaining valve patency and symmetry, but their postoperative efficacy has not been systematically evaluated. OBJECTIVES: The aim of this study was to determine whether IND use after septorhinoplasty improves functional, aesthetic, and symmetry outcomes compared with standard care. METHODS: In this single-center, randomized, single-blinded trial, 128 adults undergoing primary septorhinoplasty were allocated (1:1) to IND plus st

Novel MYL1 Intron Variant With Expanded Phenotype.

Congenital myopathy-14 (CMYO14) is an ultrarare autosomal recessive disorder caused by biallelic variants in MYL1, with only four patients reported to date. We describe what is likely the fifth reported patient, a neonate with severe hypotonia, respiratory insufficiency, and skeletal anomalies showing distinct histological changes of skeletal muscle consistent with all previously described patients. The patient carried a novel homozygous intron variant (c.479-25T>C, p.(?)) in MYL1. RNA analysis of patient muscle demonstrated aberrant splicing, with inclusion of 19 bp from intron 4 in most tran

Phenotyping persistent pulmonary hypertension of the newborn: recognition of its persistence across world symposium on pulmonary hypertension classifications.

The earliest clinical sign of pulmonary vascular disease in neonates is persistent pulmonary hypertension of the newborn (PPHN), traditionally classified as World Symposium on Pulmonary Hypertension (WSPH) Group 1. Typical PPHN is defined by the delayed transition from intra- to extra- uterine life with failure of pulmonary artery pressures to fall after delivery. This results in sustained elevation of pulmonary vascular resistance and contributes to hypoxemic respiratory failure (HRF). However, early HRF can also arise from atypical PPHN phenotypes that may persist beyond the expected resolut

A Clearer Picture: Using Fetal MRI to Diagnose Neck Masses and Predict Airway Compromise.

INTRODUCTION: Fetal neck masses are rare but can be life-threatening if causing airway compromise. Early and accurate diagnosis of these masses allows life-saving interventions to be undertaken at birth in the form of the EXIT procedure. METHODS: A single institution case series of all patients referred for fetal MRI to a tertiary center in the North of England due to presence of a neck mass on antenatal ultrasound. Data concerning the MRI findings for each patient and their final diagnosis were collected to create a flow chart proposing the most likely diagnosis based on fetal MRI features.

Improving internal medicine resident proficiency in non-invasive positive pressure ventilation and oxygen modalities through simulation training.

INTRODUCTION: Simulation-based training is increasingly used to teach respiratory interventions, but limited studies have evaluated its impact on both non-invasive positive pressure ventilation (NIPPV) and supplemental oxygen delivery systems. We assessed changes in internal medicine residents' knowledge and attitudes before and after a simulation-based training program. METHODS: This is a retrospective study of a simulation-based training program. There were 78 internal medicine residents from postgraduate first year (PGY1), postgraduate second year (PGY2), and postgraduate third year (PGY3)

Clinical effects of the use of a novel oropharyngeal airway in painless gastroscopy in patients with obstructive sleep apnoea-hypopnoea syndrome: a randomised controlled trial protocol.

INTRODUCTION: Obstructive sleep apnoea-hypopnoea syndrome (OSAHS) is a sleep-related breathing disorder characterised by partial or complete obstruction of the upper respiratory tract. Owing to their relevant physiological anatomy, patients with OSAHS often experience severe respiratory complications, such as airway obstruction and hypoxaemia, during painless gastroscopy. In addition, the use of the shared airway approach increases the difficulty of airway management. At present, simple and effective tools for airway management for use during gastroscopy in clinical practice are lacking. There

Association of Airflow Obstruction and Left Atrial Conduit Strain in Patients with Known or Suspected Coronary Artery Disease.

PURPOSE: Airflow obstruction (AO) and coronary artery disease (CAD) share smoking as a risk factor. They may be associated with alterations in ventricular and atrial volumes and functions, and global coronary flow reserve (CFR), all of which can be assessed by cardiovascular MR (CMR). The purpose of this study was to evaluate the association between AO severity and CMR parameters in patients with known or suspected CAD. METHODS: Eighty patients with known or suspected CAD who underwent CMR, including cine, stress and rest perfusion, stress and rest coronary sinus flow measurements by phase-co

Nasal Air-Jet Sensitivity Differentiates Empty Nose Syndrome and Turbinate Reduction Patients: A Pilot Study.

OBJECTIVES: Abnormal nasal mucosal function has been frequently implicated in the symptomatology of empty nose syndrome (ENS), yet with limited evidence. This prospective pilot study aims to compare nasal mucosal sensitivity to air-jet stimuli between ENS patients and patients who underwent septoplasty/turbinate reduction (septo-turb) surgery, but without ENS symptoms. METHODS: Seven ENS and seven septo-turb subjects were recruited through a sample of convenience (from 06/2023 to 09/2024) due to the rarity of ENS. A precise, 0.25 s air jet ranging from 0.5 to 5 L/min was delivered via a 25-ga

[Weaning Classification Systems: WIND, ICC and Prolonged Weaning in light of the updated German S3-Guideline on invasive mechanical ventilation].

Weaning from invasive mechanical ventilation represents a critical transition in intensive care, accounting for a substantial proportion of total ventilation time in successfully extubated patients. Inadequate assessment may lead either to prolonged ventilation with associated complications or to premature extubation attempts with an increased risk of reintubation. To systematically describe weaning processes, several classification models have been proposed. The established International Concensus Classification (ICC) weaning classification ("Budapest classification") categorized weaning cour
