Association Between Dead Space to Tidal Volume Ratio and Duration of Respiratory Support After Extubation in Children With Congenital Heart Disease.

BACKGROUND: Children with cardiac disease liberated from mechanical ventilation often receive noninvasive respiratory support (NRS) postextubation via high-flow nasal cannula, CPAP, or noninvasive ventilation. Predicting the type and duration of postextubation NRS can be challenging due to a lack of objective tools to guide decision-making. The dead space to tidal volume ratio (VD/VT) is a potential tool to guide this decision. We hypothesized that an elevated VD/VT would be associated with longer duration and higher level of NRS following extubation in children with cardiac disease. METHODS:

Baseline and 4-year associations between prediabetes and kidney impairment: Insights from the ILERVAS cohort.

AIMS: Prediabetes is a prevalent condition associated with increased cardiovascular and renal risk. However, the long-term impact of haemoglobin A1c (HbA1c) within the range defined as prediabetes on kidney function remains unclear. This study aimed to evaluate the association between prediabetes and renal function (estimated glomerular filtration rate [eGFR] and albuminuria) through cross-sectional and prospective analyses over 4 years in a cohort of individuals with no diagnosis of diabetes or established cardiovascular disease. MATERIALS AND METHODS: Analysis using data from 8153 participa

Transcriptomic response of skeletal muscle to acute aerobic versus combined exercise in chronic kidney disease.

BACKGROUND: Chronic kidney disease (CKD) affects approximately 14% of the UK population and is associated with significant exercise intolerance, partly due to skeletal muscle dysfunction. While exercise is a potential therapeutic strategy, the molecular response of skeletal muscle to exercise in CKD remains poorly understood. This study aimed to characterise transcriptomic changes in skeletal muscle 24 hours after aerobic (AE) or combined aerobic and resistance exercise (CE) in non-dialysis CKD. METHODS: This study utilised muscle biopsies from participants in the ExTRA CKD trial with stage 3

Exploring the role of chemerin in skeletal muscle phenotype in those with kidney disease.

INTRODUCTION: People with chronic kidney disease (CKD) often experience poor muscle quality and elevations in skeletal muscle wasting and dysfunction, contributing to a reduced quality of life and an increased risk of morbidity and mortality. The adipokine chemerin is associated with CKD progression and is involved in inflammatory-related signalling processes. With chronic inflammation recognised as a driving factor in uraemic sarcopenia, we aimed to investigate the potential role of chemerin as a uraemic toxin in CKD and identify the mechanisms of action to elucidate therapeutic opportunities

Predictive Value of Albumin-Corrected Anion Gap for Secondary Acute Myocardial Injury in Patients With Acute Pesticide Poisoning.

AIMS/BACKGROUND: Acute pesticide poisoning frequently results in secondary acute myocardial injury. Identifying predictive biomarkers, such as albumin-corrected anion gap (ACAG), is crucial for early intervention. This study aimed to explore the predictive value of ACAG for secondary acute myocardial injury in patients with acute pesticide poisoning. METHODS: A retrospective analysis was conducted on 205 patients with acute pesticide poisoning admitted between March 2017 and December 2024. Patients were classified into a myocardial injury group (n = 81) and a non-myocardial injury group (n =

[Procedural options in the prescription of outpatient intensive care and potential assessment in children and adolescents.].

The care and treatment of children and adolescents requiring long-term ventilation pursues several overarching goals and is ideally carried out through interdisciplinary collaboration within an experienced, multidisciplinary team.In children and adolescents, neuromuscular diseases resulting in respiratory insufficiency are the primary cause of long-term dependence on a ventilator. In addition to the diseases that necessitate long-term ventilation, airway obstructions, trauma, malformations associated with genetic syndromes, and, rarely, insufficient secretion clearance or severe dysphagia with

Real-world faceoff: EBRT vs interventions for malignant central airway obstruction.

BACKGROUND: Malignant central airway obstruction (MCAO) is a life-threatening complication in thoracic oncology. While external beam radiation therapy (EBRT), stenting, bronchoscopic debulking, and combination approaches are widely used, real-world comparative survival data are limited. METHODS: Using the TriNetX US Collaborative Network (2008-2023), we identified adults with MCAO undergoing EBRT (n = 11 478), stenting (n = 11 478), debulking (n = 7855), or stenting plus EBRT (n = 2985). Patients were 1:1 propensity-matched by demographics, cancer type, Eastern Cooperative Oncology Group (ECO

The role of COPD and inhaled corticosteroids in major adverse cardiovascular events in cardiovascular-kidney-metabolic populations.

BACKGROUND: Cardiovascular-kidney-metabolic (CKM) disease and chronic obstructive pulmonary disease (COPD) are associated with major adverse cardiovascular events (MACE). Whether COPD further increases MACE risk within CKM populations, and whether this potential risk is modifiable through inhaled corticosteroids (ICS), is unknown. Within CKM populations, we investigated the relationship between (1) COPD and subsequent MACE, and (2) amongst concurrent CKM-COPD populations, we investigated the relationship between ICS and subsequent MACE. METHODS: We used Clinical Practice Research Datalink (CP

Bilateral Lung Transplantation for Lymphangioleiomyomatosis With Secondary Pulmonary Hemosiderosis: A Case Report.

BACKGROUND: Pulmonary lymphangioleiomyomatosis (LAM) is a rare and progressive disease characterized by abnormal proliferation of smooth muscle-like cells leading to diffuse cystic destruction of the lung parenchyma and respiratory compromise, predominantly affecting young women. Idiopathic pulmonary hemosiderosis (IPH), a rare disorder characterized by recurrent diffuse alveolar hemorrhage and iron deposition in the lungs, may present with nonspecific respiratory symptoms and radiologic findings that can overlap with other diffuse lung diseases, potentially leading to diagnostic confusion. Association between acute respiratory acidosis and hyperkalemia during esophageal cancer surgery in the prone position: A multicenter retrospective observational study protocol.

INTRODUCTION: Esophagectomy in the prone position can induce acute respiratory acidosis. While it is known that metabolic acidosis can significantly elevate serum potassium, the effect of respiratory acidosis is conventionally considered to cause minimal or no elevation. However, clinical practice in this surgical setting sometimes reveals a greater-than-expected degree of hyperkalemia. The objective of this study is to investigate the association between this acute respiratory acidosis and hyperkalemia, and to explore its clinical implications. METHODS AND ANALYSIS: This is a retrospective,

Differential associations of transient hyperuricemia and transient hypouricemia with annual changes in estimated glomerular filtration rate in healthy participants: an observational study.

BACKGROUND: Dysuricemia, encompassing both hyperuricemia (serum uric acid: SUA > 7 mg/dl) and hypouricemia (SUA ≤ 3 mg/dL), has been linked to cardio-renal risks, but it remains unclear whether consistent or transient dysuricemia contributes to the risk of chronic kidney disease (CKD). PURPOSE: To investigate whether consistent and transient dysuricemia influences annual changes in the estimated glomerular filtration rate (eGFR) in healthy participants. METHODS: We retrospectively analyzed 1142 healthy participants who underwent health checkups with at least four consecutive annual measureme

Rationale for and approach to establishing a multidisciplinary acute pulmonary embolism expert care team.

Patients with acute pulmonary embolism (PE) may present with cardiac arrest, overt or impending cardiogenic shock and/or severe respiratory insufficiency. Immediate evaluation and management of these patients require high clinical suspicion along with (bedside) imaging to confirm the diagnosis, targeted haemodynamic and/or respiratory support, appropriate anticoagulant treatment, and in many cases reperfusion therapy. The immediate treatment decision-making is largely driven by local expertise and resources and should be guided by the individual patient's characteristics such as cardiopulmonar

COPD and kidney disease: not so far apart?

This review synthesizes current literature on the epidemiology, mechanistic links, and clinical consequences of the COPD-CKD association. In addition to evaluating established risk factors such as age and tobacco smoking, we discuss emerging insights into the roles of novel biomarkers and epigenetic modifications in bridging the lung-kidney axis. The review further examines how the presence of CKD influences clinical management in COPD patients, including impacts on drug dosing, electrolyte balance, acid-base homeostasis, and outcomes during acute exacerbations, and conversely, how COPD can ad

Anabolic and catabolic responses to different modes of exercise in patients with chronic kidney disease.

BACKGROUND: Muscle wasting is common in chronic kidney disease (CKD), contributing to impaired function and poor health outcomes. Exercise is recommended, but the molecular responses of skeletal muscle to different exercise modalities remain unclear. This study examined anabolic, catabolic, and myogenic responses to aerobic exercise (AE) versus combined exercise (CE; aerobic plus resistance) in CKD. METHODS: Muscle biopsies were collected from participants in a 12-week randomized controlled trial (the ExTra CKD trial) at baseline, 24 hours after initial exercise bout (untrained), and 24 hours

Non-invasive ventilation support during feeding tube placement in amyotrophic lateral sclerosis patients with moderate to severe ventilatory impairment: an update.

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease characterized by motor neuron degeneration, leading to muscle weakness and respiratory issues. Enteral nutrition is used in ALS patients when they experience severe weight loss, dysphagia, dehydration, or a risk of aspiration pneumonia. Noninvasive ventilation (NIV) is essential for managing respiratory failure in ALS patients, especially during feeding tube placement procedures. This narrative review compares percutaneous endoscopic gastrostomy (PEG), percutaneous radiologic gastrostomy (PRG), and radiologically in

[Progress in research of effects of exposure to different-sized particulate matter on chronic kidney disease].

Air pollution is a significant public health problem, and particulate matter, one of the important air pollutants, has attracted global attention for its impact on human health. Although a large number of studies have been conducted to investigate the impacts, previous studies of particulate matter usually focused on the diseases of respiratory and cardiovascular systems, and less attention was paid to the effects of exposure to particulate matter on renal system. This paper systematically analyzes the differences in the effects of exposures to particulate matter with different particle sizes
