Airway Stenting for Benign and Malignant Central Airway Obstruction: A Case Series.

Central airway obstruction (CAO) is a life-threatening condition caused by malignant or benign processes involving the trachea and main bronchi. Bronchoscopic airway stenting is an established intervention to restore airway patency, relieve symptoms, and stabilize patients, particularly in urgent or critical settings. However, its clinical role varies according to underlying etiology and disease context. We report a single-center case series of seven adult patients with symptomatic CAO who underwent bronchoscopic airway stenting. Six cases were associated with malignant disease, and one involv

Amyotrophic Lateral Sclerosis.

OBJECTIVE: This article describes the nerve loss underlying amyotrophic lateral sclerosis, its clinical manifestations, examination findings leading to the diagnosis, the expanding role of genetics, and available treatment and patient management. LATEST DEVELOPMENTS: The cause of neuron death in patients with amyotrophic lateral sclerosis is not known, but many genes are strongly linked to the disease, in both patients with a family history and those with no history. A multistep pathologic process helps to explain the onset of symptoms later in life, even in patients with associated genes. Po

Trends in mortality with chronic obstructive pulmonary disease as the underlying cause and chronic kidney disease as a contributing cause among US adults, 1999-2020: a CDC WONDER analysis.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) and chronic kidney disease (CKD) frequently coexist, yet mortality trends in patients with both conditions remain poorly characterized. METHODS: Using the CDC WONDER Multiple Cause of Death database, we analyzed death certificates from 1999 to 2020 for adults aged ≥ 25 years with COPD (ICD-10: J40-J44) as the underlying cause and CKD (N18) as a contributing cause. Age-adjusted mortality rates (AAMRs) per 100,000 population were calculated using 2000 US standard population. Trends were assessed via Joinpoint regression, with stratificati

Machine learning-based early prediction of severe pneumonia risk in hospitalized leukemia patients: a dual-center retrospective cohort study.

BACKGROUND: Severe pneumonia is a major infectious complication in patients with leukemia and may rapidly progress to respiratory failure or septic shock. Disease- and treatment-related immunosuppression increases susceptibility, yet population-specific early risk-stratification tools are limited. OBJECTIVES: To develop and externally validate a machine learning model for predicting severe pneumonia in hospitalized patients with leukemia. METHODS: Data from 2,978 leukemia patients at two Chinese hospitals were retrospectively analyzed. The derivation cohort was divided into training and inte

Aspergillus tubingensis pneumonia with pulmonary oxalosis in a virologically suppressed person with HIV: A case report.

Opportunistic aspergillosis has occasionally been reported in individuals with advanced HIV infection but remains rare overall and is particularly uncommon under well-controlled conditions. Here, we present a case of Aspergillus tubingensis pneumonia in a virologically suppressed person with HIV (PWH) in his late sixties. He was receiving bictegravir-based antiretroviral therapy, with a CD4 cell count of 368/μL and undetectable HIV RNA two months before symptom onset. His symptoms began with a cough, followed 20 days later by a fever and dyspnea, which required hospitalization. Computed tomogr

Clinical Predictors of High-Flow Nasal Cannula Failure in Children with Acute Respiratory Illness: A Single-Center Experience.

High-flow nasal cannula (HFNC) therapy is increasingly used for managing pediatric respiratory distress; however, early identification of children at risk of treatment failure remains challenging. This retrospective observational study evaluated clinical predictors of HFNC failure among children aged 1 mo to 12 y admitted with moderate-to-severe respiratory illness at a tertiary care center between April and June 2025. Fifty children received HFNC therapy, of whom 44 (88%) had successful outcomes, while six (12%) required invasive mechanical ventilation, among these two died. HFNC failure was

Early onset scoliosis: A comprehensive review of aetiology, diagnosis and classification.

Early-onset scoliosis (EOS) encompasses spinal deformities that develop before ten years of age, occurring during the critical period of thoracic and pulmonary growth. Beyond spinal curvature, EOS represents a developmental disorder in which structural imbalance compromises thoracic expansion and lung maturation, predisposing to thoracic insufficiency syndrome (TIS). The historical evolution of the term-from infantile and juvenile scoliosis to the comprehensive framework of EOS-reflects a shift from age-based categorization to one integrating growth dynamics and physiological impact. The Class

BACKGROUND: Immune checkpoint inhibitors (ICIs) have transformed cancer treatment, but immune-related adverse events remain clinically challenging. Tislelizumab is a humanized anti-PD-1 monoclonal antibody widely used in China, and tislelizumab-associated secondary adrenal insufficiency (SAI) remains insufficiently characterized, particularly with regard to how and when it is recognized. Pyogenic Granuloma in the Sinonasal Cavity: A Potential 38-Year Latency Following Foreign Body Entrapment.

BACKGROUND: Lobular capillary hemangioma (LCH), also known as pyogenic granuloma (PG), is a benign vascular lesion typically associated with minor trauma or irritation, predominantly occurring in the oral cavity and in females. Its sinonasal occurrence, particularly in males, is rare, and an association with remote childhood trauma remains a speculative hypothesis. Clinically, it must be differentiated from other vascular sinonasal lesions such as angiofibroma, hemangioma variants, and malignant mimics. CASE PRESENTATION: This rare case report descripts a highly unusual case of an inflammator

[Nucleoside analogues in etiotropic therapy of hantavirus fevers].

Hantavirus fevers (HF) are caused by pathogenic orthohantaviruses and are classified into two clinical forms: hemorrhagic fever with renal syndrome (HFRS), reported in Eurasia, and hantavirus pulmonary syndrome (HPS), reported in the Americas. HF are characterised by an acute course with predominant involvement of internal organs: HFRS is mainly associated with kidney damage, while HPS is characterised by severe lung injury. Both diseases may also affect the central nervous and endocrine systems. In addition, hantavirus fevers are marked by severe complications during the acute phase and a pro

Veterinarians' attitudes towards the treatment and surgical management of brachycephalic dogs - a survey among small animal veterinarians in Austria.

BACKGROUND: The rising popularity of flat-faced dog breeds has intensified discussions around brachycephaly and its associated health risks. The structural shortening of the skull coupled with a lack of a concurrent reduction in the volume of the soft tissues in these breeds often results in chronic health issues, notably Brachycephalic Obstructive Airway Syndrome (BOAS), which remains incurable but may be alleviated through corrective surgical interventions. In light of these challenges, veterinarians are increasingly called upon to provide informed guidance and support to pet owners navigati

Hantavirus infection in emergency radiology: Imaging manifestations, pathophysiology, and infection control considerations.

In April 2026, several cases of severe respiratory illnesses were reported aboard the cruise ship MV Hondius during its voyage from Argentina. Subsequent laboratory testing identified Andes virus (ANDV), a hantavirus subtype, with rare but documented human-to-human transmission. Although hantavirus infections are uncommon, both hantavirus pulmonary syndrome (HPS) and hemorrhagic fever with renal syndrome (HFRS) are associated with substantial morbidity and mortality. Their rapid progression and potential need for intensive respiratory support continue to pose important challenges for emergency

Reversible bilateral cerebellar ataxia resulting from hypercapnic encephalopathy in an undiagnosed patient with obstructive sleep apnoea.

A man in his 40s with morbid obesity and type 2 diabetes mellitus presented with early-morning drowsiness and respiratory distress after several days of nocturnal dyspnoea and unsupervised loop diuretic use. Initial assessment showed hypoglycaemia, acute-on-chronic hypercapnic respiratory acidosis with metabolic alkalosis and obstructive ventilatory mechanics, requiring intubation. Within 24 hours of extubation, he re-presented with recurrent drowsiness and striking bilateral cerebellar ataxia despite normal neuroimaging. Repeat arterial blood gases again demonstrated acute-on-chronic hypercap

Disease severity as the dominant clinic-available marker of myasthenic crisis occurrence: a single-centre retrospective study.

INTRODUCTION: Myasthenic crisis is the most feared acute complication of myasthenia gravis (MG), yet simple clinic-based approaches to risk stratification remain limited. We examined which routinely recorded clinical features were associated with crisis occurrence across the observed disease course and whether a parsimonious model could provide retrospective group-level stratification. METHODS: This single-centre retrospective study included 86 adults with MG. Myasthenic crisis was defined as respiratory failure requiring mechanical ventilation. Disease severity was graded using the Myastheni

Ecological and Individual-Level Factors Associated With Hantavirus Cardiopulmonary Syndrome Notifications and in-Hospital Mortality Among Hospitalized Patients in Chile, 2005-2022.

INTRODUCTION: Hantavirus cardiopulmonary syndrome (HCPS) is a severe zoonotic disease caused by Orthohantavirus andesense (Andes virus, ANDV), with significant public health burden in Chile. Despite decades of surveillance, knowledge gaps remain regarding the ecological drivers of notification burden and the individual-level determinants of in-hospital mortality among hospitalized patients. This study aimed to address both dimensions simultaneously, providing a multilevel characterization of HCPS epidemiology in Chile from 2005 to 2022. METHODS: Two complementary analyses were conducted. Firs

[Summary IgA nephropathy, or Berger's disease].

IgA nephropathy (IgAN), or Berger's disease, is a common primary glomerulonephritis that presents a variety of clinical manifestations. The most typical presentation consists of episodes of gross hematuria in young adults, often occurring concurrently with upper respiratory tract infections. However, it may also present microscopic hematuria, varying degrees of proteinuria, renal insufficiency, or hypertension of varying severity. The diagnosis is based on kidney biopsy, which reveals IgA mesangial deposits. This condition may be associated with other systemic diseases that should be investiga
