The Septoplasty Healthcare Monitor: An Outcome Assessment Infrastructure to Enhance the Quality and Transparency of Care.

OBJECTIVE AND DESIGN: The growing demand for transparency about the efficacy of healthcare has accelerated the use of Patient-Reported Outcome Measures (PROMs), but their integration into daily practice is challenging. This observational study describes how the Septoplasty Healthcare Monitor (SHM) addresses these challenges and highlights the benefits of standardised outcome assessments and visualisation for various stakeholders, including physicians and patients. MAIN OUTCOME MEASURES: Since 2014, all eligible septoplasty patients have been included in the SHM. Patients are automatically off

Explainable machine learning for postoperative respiratory failure prediction in open-heart surgery patients - a study based on the MIMIC-IV database.

BACKGROUND: Postoperative respiratory failure (PRF) is a severe complication after open-heart surgery, associated with increased mortality and prolonged ICU stays. While machine learning (ML) models have shown promise in predicting PRF, existing models often rely on fragmented data and lack interpretability. This study aimed to develop an interpretable ML model for early prediction of PRF using data from the first 24 h of ICU admission. METHODS: We analyzed data from the MIMIC-IV database, focusing on patients undergoing open-heart surgery with cardiopulmonary bypass (CPB). Patients with preo

Understanding the Control of Breathing to Optimize Patient-Ventilator Interactions.

Control of breathing depends on a complex and intricate feedback control system that integrates both automatic and volitional aspects of ventilation. A "central pattern generator" integrates inputs from peripheral and central chemoreceptors, as well as receptors located in the lungs, airways, muscles, and joints, in conjunction with signals from various brain regions. The output of the central pattern generator-referred to as the respiratory drive-determines the characteristics of breathing, including magnitude, timing, and the specific effector muscles involved. Several conditions frequently

Characteristics of the Five Different Types of Intermittent Mandatory Ventilation.

Intermittent mandatory ventilation (IMV) has evolved substantially from its original form, developing into 5 distinct types intended to enhance patient safety, comfort, and liberation from mechanical ventilation. Each form of IMV has different potential advantages and disadvantages that should be considered when selecting the mode of ventilation. IMV(1): the set mandatory breath rate is always delivered. IMV(2): mandatory breaths may be suppressed by spontaneous breaths if their frequency is high enough. IMV(3): mandatory breaths may be suppressed by spontaneous breaths if they achieve some pr

Challenges in interpreting leukocyte and nucleated red blood cell counts in neonatal hemolytic disease: A case report on hematology analyser performance.

We present a case of hemolytic disease of the newborn (HDN) due to maternal alloimmunisation with anti-E and anti-c antibodies, resulting in severe anaemia, respiratory insufficiency, and hyperbilirubinemia in a term male neonate. Haematological evaluation using the automated analyser Sysmex XN-3100 (Sysmex Corporation, Kobe, Japan) yielded an erroneously elevated white blood cell (WBC) count of 1 6 3 x 1 0 / L , later manually corrected to 2 8 x 1 0 / L due to extreme nucleated red blood cell (NRBC) interference (> 2,000 NRBCs per 100 WBCs). This case illustrates the analytical limitations

High-Flow or Standard Oxygen in Acute Hypoxemic Respiratory Failure.

BACKGROUND: Data are needed on the effect of oxygen delivered through a high-flow nasal cannula, as compared with standard oxygen therapy, on intubation and mortality in patients with acute hypoxemic respiratory failure. METHODS: In this multicenter, open-label trial, we randomly assigned patients who had acute hypoxemic respiratory failure to receive high-flow-oxygen or standard-oxygen therapy. All the patients had a ratio of the partial pressure of arterial oxygen to the fraction of inspired oxygen of 200 or less, a respiratory rate of more than 25 breaths per minute, and pulmonary infiltra

From Physiology to Intervention: Endoscopic Adenoid-to-Choanae Ratio Thresholds for Symptomatic Adenoid Hypertrophy in Preschool Children.

ImportanceAdenoid enlargement in preschool children is commonly cited as a cause of nasal obstruction and sleep-disordered breathing, yet age-related lymphoid development complicates differentiation between physiological growth and clinically significant hypertrophy. Existing endoscopic grading schemes lack consensus on age-specific normative limits and clear management thresholds.ObjectiveThis study aimed to determine endoscopic adenoid-to-choanae (A/C) ratio cut-offs that discriminate symptomatic from asymptomatic preschool children and to propose a practical, management-oriented grading sca

Clinician Perspectives on the Extracorporeal Membrane Oxygenation Decision-Making Process.

IMPORTANCE: Venovenous extracorporeal membrane oxygenation (VV ECMO) is a resource-intensive, life-sustaining technology to support patients with severe refractory respiratory failure. Its precise indications and contraindications are not standardized, and expert opinions are frequently changing, leading to variation in why and to whom VV ECMO is offered. OBJECTIVE: To characterize the ways clinicians approach candidacy selection, the criteria considered, and the relative weight given to such variables. DESIGN, SETTING, AND PARTICIPANTS: This qualitative study conducted virtual semistructure

ACOD1-itaconate in macrophage attenuates oxidative stress and inflammation in benign airway stenosis by upregulating and transferring FTH1.

The oxidative stress of macrophage plays pivotal roles of acute and chronic inflammation and chronic fibrotic phases, in which the metabolic mechanism needs to be further explored. In our research, multi-omics analyses of human and murine during Benign airway stenosis (BAS) biopsy identified ACOD1 as a hallmark of immunometabolic regulation during acute inflammation stage. ACOD1 knockout aggravated both acute and chronic inflammation, which increased the granulation tissue formation. The ACOD1-itaconate axis, along with its derivative, 4-octyl itaconate (4-OI), orchestrated acute and chronic i

Correction of Patient-Ventilator Discordance.

Optimizing the interaction between the patient and the ventilator should be a cornerstone for clinicians to pursue. Disruptions in these interactions can lead to what is called patient-ventilator discordance. Patient-ventilator discordance is a mismatch between what the patient desires in a breath and what the ventilator provides. Asynchrony is also a common term used to describe these unhealthy interactions. Different types of asynchronies make up the more global term of patient-ventilator discordance. Asynchronies can occur at the onset of a breath (trigger asynchrony), during breath deliver

Defining and Measuring Patient-Ventilator Interactions: 10 Fundamental Maxims.

Patient-ventilator interaction (PVI) has been extensively addressed in the literature, but there is no agreement on terms and definitions used. The result is that articles and books present concepts and terms that are inconsistent, confusing, and many times based on obsolete legacy ideas. To solve this problem, we propose a set of basic rules and standard nomenclature from which to establish a formal taxonomy and basis for its measurement. We also highlight some areas for research on PVI definitions, measurement, and clinical needs. Taxonomies have 2 components: a set of definitions relevant t

How to start non-invasive ventilation for COPD-associated type 2 respiratory failure.

Exacerbations of chronic obstructive pulmonary disease (COPD) can result in type 2 respiratory failure, which can be effectively and safely treated with non-invasive ventilation (NIV). NIV involves delivering oxygen to the patient via a mask and ventilator, so it does not require the use of an endotracheal tube. NIV is commonly used in the emergency department for patients presenting with type 2 respiratory failure caused by a COPD exacerbation. It is important that emergency department nurses are confident in the steps required to start patients on NIV. • NIV can feel overwhelming for patient

Discordance, Pendelluft, and Electrical Impedance Tomography.

Pendelluft, the internal redistribution of gas within the lung, was first described as a phenomenon related to high breathing frequencies and regional differences in time constants. The availability of electrical impedance tomography has transformed our understanding of this form of intrapulmonary discordance, revealing that effort-related pendelluft occurs commonly in mechanically ventilated patients and may contribute to patient self-inflicted lung injury. Emerging evidence suggests associations between pendelluft and adverse clinical outcomes, though meaningful gaps in our understanding rem

Modeling insights into potential mechanisms of opioid-induced respiratory depression within medullary and pontine networks.

The opioid epidemic is a pervasive health issue and continues to have a drastic impact on the healthcare system. This is primarily because opioids cause respiratory suppression and can lead to respiratory failure. Opioid administration can affect the frequency and magnitude of inspiratory motor drive by activating µ-opioid receptors, located throughout the respiratory control network in the brainstem. However, the precise neural mechanisms that suppress breathing are not fully understood. Previous research suggests opioids affect medullary and pontine inspiratory neuron activity by disrupting

Obesity and Metabolic Complications in Cystic Fibrosis.

OBJECTIVE: Cystic fibrosis (CF) is an autosomal recessive disorder caused by mutations in the CF transmembrane conductance regulator gene, resulting in impaired chloride and bicarbonate transport across epithelial cells. This defect results in multisystem involvement, characterized primarily by chronic pulmonary infections, obstructive lung disease, and respiratory failure, alongside extrapulmonary manifestations including pancreatic insufficiency, CF-related diabetes, and liver disease. Historically, malnutrition represented a major clinical challenge due to chronic catabolism, increased ener

Comparison of Ventilatory Ratios Calculated Using Predicted Versus Actual Body Weight.

BACKGROUND: Ventilatory ratio (VR) is a surrogate marker of dead-space fraction that is increasingly used to assess ventilation impairment and predict clinical outcomes. However, the current VR equation uses on predicted body weight (PBW) to estimate reference minute ventilation, which may lead to an overestimation of dead-space fraction in patients with extreme body weights. We hypothesized that VR calculated using PBW and actual body weight (ABW) might differ in their ability to predict outcomes, particularly in obese patients. METHODS: This retrospective study compared the discrimination c

A multi-centre quality improvement project to assess the impact of a standardised NIV care bundle on mortality outcomes in patients with acute type 2 respiratory failure.

The evidence for non-invasive ventilation (NIV) is clear; however, real-world outcomes fall short of those demonstrated by clinical trials. We developed a five-step care bundle in line with BTS quality standards to standardise management and guide clinicians through the first few hours of NIV care. This was combined with staff training initiatives. The project aimed to reduce acute NIV mortality to 10%. Although this target was not met, the project delivered a 7% reduction in mortality from 28% to 21%, equating to 143 fewer deaths in 2023 compared to 2022. The project also delivered increased

Identification and Characterization of Reverse Triggering in Acute Hypoxemic Respiratory Failure Using an Automated Tool.

BACKGROUND: Reverse triggering is a highly common ventilatory asynchrony in sedated critically ill patients. When accompanied by high respiratory effort, it may cause diaphragmatic and lung injury. However, assessing its incidence and the associated effort remains limited, as it relies on expert review of waveform segments representing <5% of the full recording. Automated methods to assess and better characterize this patient-ventilator interaction are needed. METHODS: We conducted an observational study in subjects with acute hypoxemic respiratory failure. Using ventilator and esophageal tra

Successful Multidisciplinary Management of Uterine Carcinoma Treated With Chemotherapy Under Mechanical Ventilation.

We report a rare case of stage IVB endometrial carcinoma complicated by respiratory failure requiring mechanical ventilation. A 61-year-old woman with severe obesity developed bilateral pleural effusions and was admitted to the intensive care unit (ICU) for respiratory management. Because the organ failure was considered primarily tumor-related and hepatic and renal functions were preserved, weekly paclitaxel-carboplatin chemotherapy was initiated under mechanical ventilation through close multidisciplinary collaboration. The treatment was effective, leading to improvement in the pleural effus
