Economic evaluation of long-term oxygen therapy: 24 hours versus 15 hours per day in severe hypoxemia-the REDOX trial.

RATIONALE: Long-term oxygen therapy (LTOT) given for at least 15 hours/day improves survival in patients with severe chronic hypoxemia. However, the recent REDOX trial showed that LTOT prescribed for 24 hours/day was not superior to 15 hours/day in terms of death, hospitalizations, or self-reported outcomes. OBJECTIVES: We aimed to examine the cost effectiveness of prescribing LTOT for 24 versus 15 hours/day. METHODS: A cost minimization analysis of the REDOX trial data on 241 patients with severe hypoxemic respiratory failure randomized 1:1 to either LTOT 24 hours/day (n = 117) or 15 hours/

Titrating and Discontinuing Noninvasive Respiratory Support in the Emergency Department.

Acute respiratory failure is common in the emergency department (ED), often requiring noninvasive respiratory support (NIRS). NIRS has become a cornerstone in the initial management of acute respiratory failure. Despite its growing use, significant variability in practice remains, and evidence-based guidance specific to the ED setting is limited. This article aims to provide emergency physicians with a practical, evidence-informed framework for initiating and managing NIRS in adults with acute respiratory failure, with a focus on initial settings, titration, monitoring for failure, and guidanc

Clinical Application and Outcomes of Endoscopic-Assisted Autologous Septal Cartilage Technique in Crooked Nose Correction.

BACKGROUND: Crooked nose deformity is challenging when septal deviation coexists with external malalignment. We evaluated an endoscopic-assisted approach using autologous septal cartilage to improve both function and aesthetics. METHODS: We retrospectively reviewed 186 consecutive patients (January 2023-June 2025) who underwent endoscopic-assisted septorhinoplasty with autologous septal cartilage grafting. Rohrich types were Type I (n = 58), C-type (n = 100), and S-type (n = 28). Outcomes included digital photographic measurements (dorsum/tip deviation angles, nasolabial angle, tip projection

TFAM Dependent Mitochondrial Fitness Limits CD8⁺ T Cell Immunopathology and Sustains Protective Immunity during Viral Pneumonia.

During respiratory virus infection, CD8⁺ T cells kill infected cells and establish antigen-specific memory, but mechanisms regulating these functions remain incompletely understood. Here, we identify mitochondrial transcription factor A (TFAM)-dependent mitochondrial fitness as a regulator of CD8⁺ T cell function during influenza infection. Human CD8⁺ T cells exhibited an age-associated decline in TFAM expression and mitochondrial function. To model this physiologically relevant decline and determine its consequences for antiviral immunity, we generated CD8⁺ T cell-specific TFAM-haploinsuffici

Poor control of pulmonary viral replication in COVID-19 patients with early respiratory failure.

BACKGROUND: Severe respiratory failure is the leading cause of intensive care unit (ICU) admission and mortality in critically ill COVID-19 patients. However, whether active viral replication persists in the lower respiratory tract of these patients and contributes to lung injury remains unclear. METHODS: We conducted a prospective cohort study of 159 critically ill COVID-19 patients requiring invasive mechanical ventilation (IMV). A bronchoalveolar lavage (BAL) sample was collected within 24 h of intubation. SARS-CoV-2 RNA load (N1, N2, E, and subgenomic E) and the expression of 18 host immu

Clamp On vs Off Impact of Distal Anastomotic Technique During Ascending Aortic Replacement in Acute Type A Aortic Dissection: IRAD Insights.

OBJECTIVES: Most ascending aortic replacements for acute type A aortic dissection (ATAAD) are performed utilizing hypothermic circulatory arrest with an "open" distal anastomosis ("hemiarch"), whereas some are completed without ever removing the aortic cross-clamp. We sought to determine the impact of distal anastomotic technique in ascending aortic replacement for ATAAD repair. METHODS: All patients in the IRAD Interventional Cohort database who underwent ATAAD repair between 2010 and 2020 were identified (n = 2559). Data for distal anastomotic technique were available in 2031 patients who u

Strategies to Manage Refractory Hypoxemia in the Intubated Patient.

Persistent hypoxemia after intubation is associated with high mortality and demands a systematic approach focused on the underlying pathophysiology. This article emphasizes developing a focused differential diagnosis that includes both mechanical and patient factors to guide management. The authors review key therapeutic strategies, including careful use of positive end expiratory pressure, positioning techniques, medications such as inhaled pulmonary vasodilators, sedation, and neuromuscular blockade, and, when conventional therapies fail, consideration of extracorporeal membrane oxygenation.

Extracorporeal membrane oxygenation for life-threatening measles in children: a case series.

UNLABELLED: Measles is resurgent worldwide, and although vaccine-preventable, severe disease can progress to cardiorespiratory failure refractory to conventional support. This study aimed to determine whether extracorporeal membrane oxygenation (ECMO) is feasible in children with measles. A retrospective case series at a tertiary pediatric intensive care unit (PICU) with a regional ECMO program, of all children younger than 18 years with laboratory-confirmed measles who received ECMO during the August 2025-May 2026 outbreak. Of 49 children with measles admitted to the PICU, 10 received ECMO. F

Guillain-Barré Syndrome.

OBJECTIVE: This article reviews the epidemiology, clinical symptoms, examination findings, pathogenesis, diagnostic criteria, and recent guidelines of Guillain-Barré syndrome (GBS) and its variants, including information on outcome measures, management, and ongoing research. LATEST DEVELOPMENTS: CSF examination and electrodiagnostic testing are useful in some cases to support the diagnosis, particularly when GBS variants are suspected. Antibody testing is of limited value in typical sensorimotor GBS, but useful when GBS variants and nodopathies are considered. According to evidence-based guid

Paradoxical Acute Hypoxemic Deterioration During Treatment of Bilateral Bronchopneumonia: A Case Report.

Community-acquired pneumonia may occasionally be associated with unexpected respiratory deterioration despite apparent clinical, laboratory, and radiological improvement. We report a case of a previously healthy adult male who presented with progressive dyspnea, persistent cough, generalized weakness, malaise, and low-grade fever. Initial chest radiography showed no significant abnormalities despite marked respiratory symptoms, whereas chest computed tomography revealed bilateral bronchopneumonia with multiple centrilobular nodules and patchy ground-glass opacities. The patient was treated wit

A Scoping Review to Map Health Outcomes in Inducible Laryngeal Obstruction.

BACKGROUND: Inducible laryngeal obstruction (ILO) is defined as an inappropriate laryngeal closure causing difficulty in breathing. Outcome measures can be used in ILO to monitor changes in health status over time. A comprehensive review of existing measures is important to understand what the targets of treatment and management are and whether there is a need for development of new tools. AIMS: To systematically evaluate the literature reporting outcomes in individuals with ILO, identifying what is measured, whether there is consistency of measurement to enable synthesis of evidence for trea

Respiratory depression with concomitant use of opioids and cannabinoids: a case report.

BACKGROUND: The United States has had a 15-fold increase in daily cannabis use in the last 30 years. There is a growing importance for palliative care physicians to understand the pharmacological interactions between high-dose opioids and cannabinoids when treating patients with cancer-related pain who are increasingly concomitantly using cannabinoids. We present a case of respiratory depression following high‑dose THC and opioids. We discuss proposed mechanistic interactions, the importance of cannabinoid use in social history, and the role of proactive counseling of patients with concomitant

Acute respiratory deterioration in rheumatoid arthritis-associated interstitial lung disease associated with pulmonary Mycobacterium abscessus infection: a case report.

Nontuberculous mycobacteria (NTM) are important pathogens in patients with chronic lung diseases, including rheumatoid arthritis-associated interstitial lung disease (RA-ILD). Although acute exacerbations of ILD associated with NTM infection have been reported sporadically, cases involving pulmonary Mycobacterium abscessus infection remain extremely limited. We describe a fatal case of acute respiratory deterioration in a patient with RA-ILD following pulmonary infection with M. abscessus. A 76-year-old man with longstanding RA-ILD developed pulmonary NTM disease initially caused by Mycobacter

PVADet: fast patient-ventilator asynchrony detection on waveforms.

Patient-ventilator asynchrony (PVA) is a common and critically import clinical problem in patients receiving mechanical ventilation. However, PVAs are often underrecognized, underestimated and delayed, and there has been minimal success in automating their detection. In this study, we develop an efficient and fast end-to-end model to recognize PVAs on ventilator waveforms: running the model costs 106.5ms on CPUs and 7.8ms on GPUs. We propose label striping and stripe-mask encoding for efficient multi-class multi-target detecting. The model innovatively integrates causal convolutional, depth-wi

Effects of sprint interval training with voluntary hypoventilation on aerobic and anaerobic energy metabolism in young men.

PURPOSE: This study aimed to investigate the effects of sprint interval training with voluntary hypoventilation (VHL) on aerobic and anaerobic energy metabolism in young men. METHODS: Fourteen healthy male university students were randomly assigned to a normal breathing group (CON, n = 7) or a VHL group (n = 7). Both groups performed repeated sprint training for three weeks. Pre- and Post-training, we assessed maximal oxygen uptake (VO₂max) as an index of aerobic capacity, and maximal accumulated oxygen deficit (MAOD) and repeated sprint ability (RSA) as indices of anaerobic energy metabolism
