RNA sequencing significantly improves HYDIN diagnosis and defines the effects of common deletions in primary ciliary dyskinesia.

Primary Ciliary Dyskinesia (PCD) is a rare genetic disorder affecting ~1:7500 individuals characterised by recurrent chronic respiratory infections, bronchiectasis, laterality defects, and infertility. While at least 60 PCD-causing genes are reported, approximately 30% of patients remain without a confirmed molecular diagnosis. PCD caused by biallelic HYDIN variants is difficult to diagnose due to a 98% homologous pseudogene (HYDIN2). Additionally, functional ciliary diagnostic testing can be inconclusive due to the lack of discernible HYDIN ciliary ultrastructure defects by electron microscop

Evidence-based design of vaccine hesitancy information resources for health care providers.

BACKGROUND: The University of Florida Health Science Center Libraries (HSCL) sought to address the increasing challenge of vaccine hesitancy during the COVID-19 pandemic by developing evidence-based information resources for health care providers. To do this, HSCL librarians assessed the vaccine hesitancy information needs of local health care providers to design a resource that could be used to support conversations with vaccine hesitant patients at the point of care. CASE PRESENTATION: A team of six librarians, supported by student research assistants, conducted interviews with 20 health ca

Early functional maturation of anti-Spike antibodies predicts efficient SARS-CoV-2 RNA clearance.

INTRODUCTION: Viral RNA clearance following mild SARS-CoV-2 infection varies considerably among individuals, but the early humoral immune features underlying this heterogeneity remain incompletely understood. METHODS: We investigated whether the kinetics and functional maturation of the antibody response were associated with viral RNA clearance in a longitudinal study of 78 healthcare professionals infected with the ancestral Wuhan SARS-CoV-2 strain and followed for up to three years. Participants were stratified according to viral RNA clearance kinetics as nonpersistent (≤21 days) or persist

"It's Like a Whole Set of Things That We Do": A Qualitative Study of the Roles of Journal Editors-in-Chief.

INTRODUCTION: Journal editors-in-chief (EICs) play a central role in shaping scholarly publishing, yet little is known about how they understand and enact their roles. We explored how medical education journal EICs describe their roles in shaping the publication process. METHODS: We conducted a qualitative study using thematic analysis of interviews with nine medical education journal EICs. The interviews were part of a broader investigation of publication timelines and explored editorial leadership and practices, including experiences during the COVID-19 pandemic. Participants' accounts also

Mixed Pneumonia with Pulmonary Embolism and Multiple Pathogens Detected in Bronchoalveolar Lavage Fluid: A Case Report.

Severe community-acquired pneumonia may involve mixed pathogens and concurrent vascular complications. A 72-year-old man presented with fever, nonproductive cough, vomiting, hypoxemia, markedly elevated inflammatory markers, and an elevated D-dimer concentration. Chest computed tomography showed left lower-lobe consolidation and pleural effusion. Computed tomography pulmonary angiography, obtained because of hypoxemia and suspected pulmonary embolism, showed a filling defect in a right upper-lobe pulmonary artery. The patient received antimicrobial therapy prior to bronchoalveolar lavage (BAL)

Diagnostic yield of endotracheal aspirate and bronchoalveolar lavage in ventilator-associated pneumonia: A single-centre study.

Ventilator-associated pneumonia (VAP) remains a significant cause of morbidity and mortality in critically ill patients. This study aimed to compare the diagnostic accuracy of endotracheal aspirate (ETA) with bronchoalveolar lavage (BAL) culture, considered the reference standard, in identifying the causative pathogens of VAP. This study was conducted as a cross-sectional study at Haji Adam Malik General Hospital, Medan, Indonesia. A total of 95 samples from mechanically ventilated patients were analysed using both ETA and BAL in similar institutions. The results demonstrated that 49 samples w

The bacteriology of fallen leaves (Phylloplane) from council parks and green spaces in Northern Ireland: Clinical significance for at risk people living with cystic fibrosis (CF).

BACKGROUND: People living with cystic fibrosis (CF) may suffer from chronic respiratory infections due to acquired environmental organisms, ultimately leading in increased morbidity and mortality. Consequently, parents of children living with CF frequently ask their CF healthcare professional about sources and potential routes of transmission of environmental organisms to their child. This allows parents to make an informed decision regarding risk assessment/risk management of outdoor/ nature-based activities. Mitigations can then be put in place to help minimise or absolutely avoid exposure o

Fast and inexpensive visualization of genome collection at scale.

BACKGROUND: Investigating genetic progression and lineage using sequence data is a fundamental focus within the realm of viral genome research. Given the substantial increase in publicly accessible genome sequences, tools designed for extensive data analytics, particularly insightful visualizations play an important role in elucidating the spatio-temporal evolution as well as the diversity of viral lineages. Existing genome data analysis methods frequently rely on multiple sequence alignment (MSA) and phylogeny computations, making integration of new sequences computationally expensive. Such m

The presence of Histoplasma capsulatum in environments from the state of Nayarit, Mexico, reveals a potential risk of infection for human populations.

The Histoplasma capsulatum complex clusters cryptic species with genetic diversities linked to their geographic distribution and virulence. The American continent harbours its highest diversity, making this region the main axis for H. capsulatum spread. Species of this complex cause respiratory-systemic mycosis known as histoplasmosis. Infection occurs through the inhalation of fungal propagules that grow in sites such as caves, mines, and abandoned buildings, where environments rich in organic matter containing micronutrients present in bat and bird guano favour the development of these funga

Transcription Regulatory Sequences in Coronaviruses: Mechanistic Foundations for Antivirals and Vaccines.

Coronaviruses employ a distinctive discontinuous transcription mechanism to generate a nested set of subgenomic mRNAs (sgRNAs) from a single large positive-strand RNA genome. Transcription regulatory sequences (TRSs)-comprising a leader TRS (TRS-L) near the 5' end and multiple body TRSs (TRS-Bs) upstream of downstream open reading frames-serve as the primary cis-acting signals that direct polymerase pausing and template switching during negative-strand synthesis. The COVID-19 pandemic spurred intensive investigation of SARS-CoV-2 gene expression, yielding high-resolution transcriptome maps, ge

Evaluating Classifier Derivation and Validation Beyond the Receiver Operating Characteristic Curve.

The area under the receiver operating characteristic curve (AUROC) summarizes a classifier's discrimination across all possible thresholds, but it does not establish calibration, clinical utility, or performance at the threshold used in practice. This editorial explains the interpretation and limitations of AUROC using a hypothetical genomic enzyme test for susceptibility to symptomatic COVID-19. It emphasizes that threshold selection must be justified according to the intended use, outcome prevalence, and consequences of false-positive and false-negative results. Threshold derivation and vali

Global Genomic Surveillance of Middle East Respiratory Syndrome Coronavirus.

Middle East respiratory syndrome coronavirus (MERS-CoV) is a persistent zoonotic and epidemic threat. We analyzed 807 publicly available human- and camel-derived MERS-CoV genomes collected during 2012-2024 to assess global genomic surveillance. Most sequences originated from Saudi Arabia and the United Arab Emirates. Major gaps in surveillance persist across Africa and South and Central Asia despite large camel populations, active trade, and evidence of viral circulation. Phylogenetic analysis confirmed predominance of clade B in the Arabian Peninsula and clade C in East Africa, consistent wit

Surgical stabilization of rib fractures in a high-altitude trauma cohort: in-hospital clinical outcomes and resource use in a propensity score-weighted retrospective cohort study.

PURPOSE: Surgical stabilization of rib fractures (SSRF) is increasingly used in the management of traumatic rib fractures, but its in-hospital outcomes in high-altitude trauma settings remain uncertain. We evaluated the association of SSRF with in-hospital pulmonary complications and length of stay in a high-altitude (≈ 3,500 m) trauma system. METHODS: We conducted a single-center retrospective cohort study of adults with CT-confirmed traumatic rib fractures admitted from 2015 to 2023 and managed with SSRF or non-operative management. Confounding was addressed using stabilized inverse probabi

Nirsevimab provides age-specific protection against RSV hospitalisation in the first year of life: a population-level study in Queensland, Australia.

Nirsevimab, a long-acting monoclonal antibody with activity against respiratory syncytial virus (RSV), was introduced into the Queensland paediatric RSV prevention program in 2024. Using Queensland Notifiable Conditions Register (NoCS) data, we found that the infant cohort eligible for the nirsevimab program had lower RSV hospitalisation rates than comparable pre-program cohorts. The population-level impact of nirsevimab was greatest during the first month of life (incidence rate ratio [IRR]: 0.22; 95% confidence interval [95% CI]: 0.16-0.31; representing a 78% reduction in hospitalisation rat
