Surgical stabilization of rib fractures in a high-altitude trauma cohort: in-hospital clinical outcomes and resource use in a propensity score-weighted retrospective cohort study.

PURPOSE: Surgical stabilization of rib fractures (SSRF) is increasingly used in the management of traumatic rib fractures, but its in-hospital outcomes in high-altitude trauma settings remain uncertain. We evaluated the association of SSRF with in-hospital pulmonary complications and length of stay in a high-altitude (≈ 3,500 m) trauma system. METHODS: We conducted a single-center retrospective cohort study of adults with CT-confirmed traumatic rib fractures admitted from 2015 to 2023 and managed with SSRF or non-operative management. Confounding was addressed using stabilized inverse probabi

Nirsevimab provides age-specific protection against RSV hospitalisation in the first year of life: a population-level study in Queensland, Australia.

Nirsevimab, a long-acting monoclonal antibody with activity against respiratory syncytial virus (RSV), was introduced into the Queensland paediatric RSV prevention program in 2024. Using Queensland Notifiable Conditions Register (NoCS) data, we found that the infant cohort eligible for the nirsevimab program had lower RSV hospitalisation rates than comparable pre-program cohorts. The population-level impact of nirsevimab was greatest during the first month of life (incidence rate ratio [IRR]: 0.22; 95% confidence interval [95% CI]: 0.16-0.31; representing a 78% reduction in hospitalisation rat

Experiences and Expressions of Stress Among US Food Industry Workers During the 2020 Covid-19 Pandemic.

INTRODUCTION: The Coronavirus pandemic (Covid-19) exposed legacies of deep inequality, including who could afford protection from exposure. As part of the essential workforce, food industry workers continued fully in-person work throughout the pandemic and associated shutdowns. OBJECTIVE: To qualitatively examine US food worker experiences and stressors during the 2020 Covid-19 pandemic. METHODS: Thirty-four open-ended phone interviews were conducted from May through September 2020 with people who grew (n = 2), processed (n = 5), cooked (n = 9), sold (n = 17), and/or delivered (n = 5) food i

Sex-divergent trends in cardiorespiratory endurance after the COVID-19 pandemic: a 21-year interrupted time series analysis of Chinese university students.

INTRODUCTION: Studies of COVID-19 and physical fitness in Chinese university students rarely extend beyond 2022, and none has formally tested whether post-pandemic trends differ by sex. METHODS: Using 21 years of routinely collected fitness data (2005-2025), we examined indicator-specific change and sex differences in population-level trends in a repeated cross-sectional design. The cardiorespiratory analytic sample comprised 297,565 valid records from 91,686 full-time undergraduates at one comprehensive university in Zhejiang Province, China. Six indicators were classified by data availabili

Comparative histopathological and immunohistochemical features of central nervous system and pulmonary tuberculosis: an exploratory retrospective study.

Central nervous system tuberculosis (CNS-TB) is a rare and particularly severe form of extrapulmonary tuberculosis (EPTB). Early diagnosis and precise treatment are crucial in managing this condition. Pathological and immunohistochemical features may differ between CNS-TB and pulmonary tuberculosis (PTB). This study included 9 patients of CNS-TB, and analyzed their clinical manifestations, pathological features, and the immunohistochemical(IHC) features. The findings were then compared with PTB to explore the similarities and differences in how tuberculosis manifests in the brain versus the lu

Pathogen-targeted quick multiplex PCR identification of Chlamydia psittaci and Haemophilus influenzae coinfection in pneumonia: a case report.

This study describes a rare case of pneumonia induced by coinfection with Chlamydia psittaci (C. psittaci) and Haemophilus influenzae (H. influenzae). The patient had a history of chronic nephritis and long-term pigeon breeding. The initial manifestation was slurred speech, followed by rapid progression to high fever and respiratory failure. Chest computed tomography revealed bilateral interstitial pneumonia, predominantly in the right lung. Pathogen-targeted quick multiplex polymerase chain reaction (pt-qPCR) confirmed the pathogens as C. psittaci and H. influenzae, and metagenomic next-gener

Use of Metagenomic Next-Generation Sequencing to Investigate Respiratory Pathogens in Influenza-Negative Nasopharyngeal Swabs From Cameroon's National Influenza Surveillance.

BACKGROUND: In Cameroon, national influenza surveillance identifies influenza-positive cases, but the causes of non-influenza ARIs remain poorly characterized. We applied metagenomic next-generation sequencing (mNGS) to investigate respiratory pathogens in influenza-negative nasopharyngeal swabs collected through national influenza surveillance. METHODS: We analyzed 199 influenza-negative nasopharyngeal swabs collected between September and December 2023 from patients with influenza-like illness or severe acute respiratory infection. Mixed DNA/RNA libraries were prepared and sequenced on an I

[Recognizing and managing post-exertional malaise in primary care].

Post-exertional malaise (PEM) is a syndrome characterized by a worsening of symptoms after physical, cognitive, or emotional exertion. It presents as increased fatigue, usually accompanied by other clinical symptoms, such as muscle pain or cognitive impairment. First described in patients with chronic fatigue syndrome/myalgic encephalomyelitis (CFS/ME), PEM is now recognized as a hallmark of post-infectious syndromes that have received increasing attention through the emergence of long COVID. PEM is a distinguishing symptom of these conditions, and its early detection and appropriate managemen

Gut microbiota vesicle reprogramming by Ganoderma lucidum polysaccharide alleviates mycoplasmal pneumonia via the gut-lung axis.

Respiratory infections remain a major global health challenge, further compounded by antibiotic resistance. Among these, mycoplasmal pneumonia represents a particularly prevalent and refractory form. Here, using targeted enzymatic fragmentation combined with multidimensional NMR analysis, we resolved the structure of a homogeneous Ganoderma lucidum polysaccharide, GLFP2, and identified β-(1→6)-linked galactosyl motifs as key structural features. Despite minimal systemic absorption, oral GLFP2 alleviated mycoplasmal pneumonia in mice. Instead, GLFP2 reshaped the gut microbiota and functionally

Bridging spatial scales in respiratory viral infection.

Respiratory viruses spread within the host through a combination of local expansion and occasional long-range dissemination that seeds new infection foci. We present LEAP (Lotka-Euler Airway Pathogen model), an analytically tractable within-host model that captures this two-scale process. The model takes the form of an age-dependent branching process and yields a closed-form expression for the exponential growth rate during early infection. Using empirical data to parameterize the model, we find that productive dissemination requires only a small number of successful long-range seeding events

Comparing a straight-to-biopsy versus pleural aspiration-first approach in malignant and tuberculous pleural effusions.

BackgroundPleural aspiration has limited diagnostic yield in malignant (MPE) and tuberculous pleural effusions (TPE), more so when requiring molecular profiling in MPE and due to the paucibacillary nature of TPE. In our institution, patients with suspected MPE or TPE are offered a straight-to-biopsy approach with pleural biopsy as the initial diagnostic procedure, or a pleural aspiration-first approach, through shared decision-making.ObjectivesTo compare clinical outcomes between a straight-to-biopsy strategy and a pleural aspiration-first approach in patients diagnosed with MPE or TPE.DesignW

Respiratory Viral Positivity Is Associated with Decreased Risk of Serious Bacterial Infections in Febrile Infants.

INTRODUCTION: Febrile infants ≤ 60 days of age are at increased risk for serious bacterial infections (SBIs), often necessitating extensive diagnostic evaluation and hospitalization. Although respiratory viral infections are common in this population and may influence the likelihood of concurrent bacterial infection, the relationship between respiratory viral panel (RVP) results and SBI risk remains an important consideration in clinical decision-making. The objective of this study was to evaluate the association between RVP results and the occurrence of SBIs among febrile infants ≤ 60 days of

Analytical and clinical performance evaluation of the STANDARD™ M10 MTB/NTM assay for the simultaneous detection of Mycobacterium tuberculosis and nontuberculous mycobacteria.

The global surge in nontuberculous mycobacteria (NTM) infections, which clinically mimic Mycobacterium tuberculosis (MTB) infections, underscores the need for rapid differential diagnosis. This study evaluated the analytical and clinical performance of the STANDARD™ M10 MTB/NTM assay, a fully automated, cartridge-based, sample-to-result real-time PCR system. Analytical performance was assessed using contrived artificial sputum samples to evaluate inclusivity (3 MTB-complex, 12 NTM species), cross-reactivity (65 respiratory pathogens), and interference (33 substances). The north-south divide in English mortality, 1981-2024: a longitudinal population study.

BACKGROUND: The north-south mortality divide in England, UK, has persisted for several decades. Previous longitudinal analyses up to 2016 have highlighted a widening mortality gap among those aged 25-44 years since the mid-1990s. We aimed to update mortality trends in England and evaluate how trends differed by age and sex up to 2024. METHODS: We did a segmented longitudinal population study of the north-south mortality divide in England (1981-2024). Using registered deaths and mid-year population estimates by 5-year age group and sex, we calculated standardised mortality rates and estimated

Effectiveness of respiratory syncytial virus immunisation in practice.

Respiratory syncytial virus (RSV) affects all ages, with infants and older adults most affected by severe disease. The RSV prevention landscape includes 39 candidates in clinical development, alongside setbacks such as the discontinuation of a promising paediatric live-attenuated vaccine candidate. Several products have been approved, including a maternal vaccine, infant monoclonal antibodies, and vaccines for older adults and adults at high risk. Due to high cost, implementation of these products remains limited to higher-income settings. Real-world evidence confirms high effectiveness, with

Safety outcomes of semaglutide by chronic kidney disease severity in the FLOW trial.

INTRODUCTION: People with type 2 diabetes (T2D) and chronic kidney disease (CKD) often experience medication intolerance and side effects that worsen with lower estimated glomerular filtration rate (eGFR). This prespecified secondary analysis assessed the safety of semaglutide by baseline eGFR. METHODS: FLOW (NCT03819153) was a double-blind, randomized, placebo-controlled trial testing the efficacy of subcutaneous semaglutide 1.0 mg once-weekly on a primary kidney outcome (50% or more eGFR reduction, kidney failure, or death due to kidney or cardiovascular causes) in participants with T2D and

A multi-objective sequence engineering framework for rational genetic vaccine design.

Genetic vaccine performance is determined not only by the delivery platforms and antigen choice but also by the coding sequence that influences antigen expression, innate immune sensing, transcript stability and sequence-associated risks. However, the design principles governing these sequence-level effects remain insufficiently defined. Thus, we consider vaccine coding-sequence design as a multi-objective engineering problem that requires simultaneous consideration of multiple, potentially competing biological properties. We present a multi-parameter computational framework for analysing SARS

Delayed introduction, transmission variation and susceptible dynamics explain spatial asynchrony during Republic of Korea's large pertussis outbreak.

Infectious disease outbreaks typically exhibit a large-scale spatial synchrony, reflecting coupling through shared environmental forcing and human mobility. Exceptions to this theoretical expectation have been reported, but the conditions under which synchrony breaks down remain poorly understood. Republic of Korea's 2024-2025 pertussis outbreak provides a striking example, during which epidemic trajectories diverged substantially throughout the country: notably, the decline in spatial synchrony with distance was more pronounced than that reported in many previous outbreaks. Integrating high-r

Investigation of the effect of simulated gastrointestinal digestive fluids on the infectivity of influenza A virus H1N1.

The recent emergence of highly pathogenic avian influenza (HPAI) in dairy cattle has raised concerns about potential non-respiratory exposure routes including ingestion of contaminated raw milk. In this study, we assessed the survival of influenza A virus (IAV) in milk during simulated human digestion and estimated infection risk using quantitative microbial risk assessment (QMRA). Whole milk artificially inoculated with IAV A(H1N1) was exposed to human saliva, fed-state simulated gastric fluids (pH 3-6) containing pepsin, and fed-state simulated intestinal fluids containing pancreatin, in the
