Real-Time MRI With Deep Learning for Efficient Evaluation of Neuromuscular Breathing Impairment.

Efficient detection of breathing impairment is critical for treatment and prognosis in neuromuscular disorders. However, standard pulmonary function tests often yield ambiguous results. This prospective study evaluates whether advanced real-time MRI (RT-MRI) combined with deep learning-based image segmentation provides sensitive outcome measures for respiratory dysfunction in late-onset Pompe disease (LOPD), a model disease for diaphragmatic weakness. Eleven Pompe patients (mean age 52.2 years; 55% female) and 11 controls (mean age 50.9 years; 55% female) were included. RT-MRI with a temporal

A preclinical rat model for bilateral phrenic nerve stimulation during mechanical ventilation.

Phrenic nerve stimulation (PNS) may preserve diaphragm activation and mitigate multiorgan injury during mechanical ventilation (MV); however, a minimal invasive rat model integrating PNS with MV is lacking. We established an omohyoid muscle-based PNS rat model combined with MV. Bilateral nerves were exposed within 20 ± 2 min by transection at the intermediate tendon of omohyoid muscle, minimizing trauma and bleeding. Threshold stimulation (0.6 ± 0.2 mA) correlated with body weight. Ventilator-synchronized stimulation increased compound muscle action potentials by ~30%, whereas histology confir

Bronchial epithelial cell-derived extracellular vesicles drive inflammasome activation and NTHi infection in COPD.

Extracellular vesicles (EVs) are lipid-membrane bound vesicles that can be beneficial or detrimental depending on the content they carry. As epithelial cells are the first line of defense against harmful particles, this work explored the role of bronchial epithelial cell-derived EVs (CepEVs) in the pathogenesis and progression of chronic obstructive pulmonary disease (COPD). RNA sequencing of macrophages stimulated with CepEVs revealed the upregulation of various inflammasome-related genes, alongside significant IL-1b and IL-18 release, which could be attenuated with caspase-1 or NLRP3 inhibit

Acute Effects of Positive Expiratory Pressure on Breathing Pattern and Chest Wall Volumes During Cycle Exercise in COPD Patients.

BACKGROUND: Patients with COPD experience air flow limitation, reduced expiratory capacity, and dynamic hyperinflation (DH), which causes dyspnea and decreased exercise tolerance. Expiratory positive airway pressure (EPAP) is a potential intervention that can reduce DH and improve respiratory muscle efficiency, but its impact on exercise tolerance remains unclear. The aim was to evaluate the acute effects of EPAP during exercise on the respiratory pattern, chest wall volumes, thoracoabdominal asynchrony (TAA), and DH in individuals with COPD using optoelectronic plethysmography. METHODS: This

Automatic Detection of Onset and Offset of Respiratory Electromyographic Activity in Severe COPD Patients on Non-Invasive Mechanical Ventilation.

OBJECTIVE: Accurate detection of inspiratory onset and offset in the diaphragm electromyographic signal (EMGdi) is clinically relevant to assess patient-ventilator interaction in COPD patients undergoing non-invasive ventilation (NIV). Manual annotations are time-consuming and subject to inter-observer variability, highlighting the need for reliable automatic methods. METHOD: We developed a fully automatic algorithm to detect EMGdi activity cycles and their onset/offset timing in overnight NIV recordings. Four ECG suppression approaches were combined with root mean square (RMS) and fixed samp

Protective role of fatty acid oxidation against epithelial barrier dysfunction in allergic asthma.

BACKGROUND: Fatty acid oxidation (FAO) is implicated in lung diseases, but its role in bronchial asthma is not fully understood. We investigated its effect on airway epithelial barrier integrity. METHODS: Using a house dust mite (HDM)-induced murine asthma model and HDM, IL-4, IL-13, or TNF-α stimulated human primary bronchial epithelial cells (BECs) and bronchial epithelial (Beas-2b) cells, we modulated FAO with L-carnitine (agonist) and Etomoxir (inhibitor). BECs and Beas-2b cells were infected with lentivirus-mediated CPT1A shRNA prior to stimulation. Barrier function, mitochondrial oxidat

Ten-year trends in pediatric hypnotic prescribing before and during the COVID-19 pandemic in Japan.

Sleep problems, particularly insomnia, are common among children and adolescents, and may have been exacerbated during the COVID-19 pandemic. However, long-term trends in pediatric hypnotic prescribing and changes associated with the pandemic remain poorly characterized. Using the JMDC Claims Database, we conducted an observational study of children and adolescents aged 2-18 years in Japan between January 2014 and December 2023 to examine hypnotic prescribing by age, sex, and pharmacological class and changes following the April 2020 COVID-19-related state of emergency. The proportion of child

Acetazolamide to prevent ventilatory drive withdrawal in REM sleep apnoea: a randomised controlled trial.

BACKGROUND: Obstructive sleep apnoea (OSA) pathogenesis during rapid-eye movement (REM) sleep has been linked to dips in ventilatory drive and downstream genioglossus hypotonia. The carbonic anhydrase inhibitor acetazolamide is known to increase ventilatory drive and improve OSA severity. Therefore, we tested the effect of acetazolamide on REM-predominant OSA severity (apnoea hypopnoea index (AHI) and hypoxic burden, co-primary outcomes) and underlying physiological mechanisms (ventilatory drive, ventilation and pharyngeal muscle activity). METHODS: 11 participants with REM-predominant OSA pe

CCN1 drives asthmatic airway remodeling through amplification of TGF-β1/Smad3 signaling.

BACKGROUND: Airway remodeling contributes significantly to chronic airflow limitation in asthma, but effective therapies are lacking. The matricellular protein CCN1 is implicated in fibrosis across multiple organ systems; however, its specific contribution to the pathogenesis of asthmatic airway remodeling has not been clearly delineated. This study was therefore designed to elucidate the functional role and regulatory mechanisms of CCN1 in this disease context. METHODS: To establish clinical relevance, we first compared CCN1 expression levels in bronchial biopsies and serum samples obtained f

Primary Ciliary Dyskinesia With Situs Inversus in Twin Premature Infants: Two Case Reports.

AIMS/BACKGROUND: Primary ciliary dyskinesia (PCD) is a rare autosomal recessive disorder characterised by structural or functional abnormalities of the cilia, which prevent proper oscillation. Patients typically present with symptoms such as chronic respiratory infections, bronchiectasis, and, in approximately half of the cases, inversion of internal organs. Due to its diverse clinical manifestations and lack of specificity, PCD is often misdiagnosed or underdiagnosed, and diagnosis in the neonatal period remains notably challenging. Therefore, this study aimed to analyse the clinical characte

Bridging structure and function: artificial intelligence-based modelling of kidney proteins.

The ability to predict the three-dimensional structure of a protein from its amino acid sequence has potential to provide insights into its function, and in the context of disease, its pathogenic mechanisms and potential drug targets. Artificial intelligence (AI)-driven algorithms, particularly AlphaFold and RoseTTAFold, have revolutionized the field of protein modelling, enabling rapid, high-confidence predictions of protein structures. In nephrology, these advances have clarified the molecular architecture of key renal systems such as podocyte slit diaphragm complexes, the conformational sta

Clinical characteristics of diffuse idiopathic pulmonary neuroendocrine cell hyperplasia.

RATIONALE: Diffuse idiopathic pulmonary neuroendocrine cell hyperplasia (DIPNECH) is characterised by diffuse bronchial hyperplasia of pulmonary neuroendocrine cells, which are situated within the walls of bronchi and bronchioles. Presenting symptoms are nonspecific and the clinical course varies, making diagnosis challenging. We sought to describe the clinical characteristics of patients with DIPNECH in a large multinational case series to guide and inform future care and research. METHODS: Data were collated from 18 international centres. Information collected included disease presentation,

Heightened trait and state-level Fatigue and Fatigability in patients with post COVID across cognitive, motor, and emotional domains.

Fatigue (self-reported exhaustion) and Fatigability (performance decline) are core symptoms of post COVID but remain poorly understood due to their multidimensional nature. It is unclear how the symptoms manifest at state and trait levels across cognitive, motor, and emotional domains. Our test paradigm included questionnaires addressing cognitively, emotionally and physically perceived Fatigue, and we assessed alertness (cognitive Fatigability), and hand grip strength (motor Fatigability). These were administered before and after experiencing either cognitive (attention tasks) or emotional (s

Acoustic guide pad: a simple innovation that dramatically reduces the risk of misdiagnosis in respiratory muscle ultrasound examinations.

BACKGROUND: Respiratory muscle ultrasound is increasingly applied in clinical practice, but its heterogeneity and operator dependency limit quantitative assessments. This study first introduces the use of acoustic guide pads to improve reproducibility, and further evaluates their clinical applicability in the stratification of COPD. METHODS: This single-center, prospective study was conducted at the respiratory medicine and ultrasound departments of a tertiary hospital over a period of 18 months, from January 2024 to July 2025. This study comprised two parts. Firstly, a prospective reproducib

Outcomes of hiatal closure with surgical gastropexy alone for large paraesophageal hernia without significant reflux.

BACKGROUND: Paraesophageal hernia (PEH) repair is usually performed with fundoplication. Historically, hiatal hernia repair without fundoplication was proposed but abandoned due to high anatomical recurrence and gastroesophageal reflux disease (GERD) rates. Modern comparisons are limited among dissimilar groups with mixed conclusions. We compared objective and symptomatic outcomes of hiatal hernia repair with gastropexy (HH-G) to a matched group of hiatal hernia repair with fundoplication (HH-F) for patients with large PEH without significant reflux. METHODS: We conducted a propensity matched
