Familial factors in SARS-CoV-2 susceptibility and COVID-19 severity: a nationwide Danish study of adult twins and their spouses.

BACKGROUND: Transmission of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) often clusters in families, suggesting role of shared environmental and genetic factors. We performed a nationwide Danish study of twins, their spouses, and matched population controls with the aim of disentangling the host genetic and environmental mechanisms underlying the familial clustering of SARS-CoV-2 susceptibility and coronavirus disease 2019 (COVID-19) severity. METHODS: This registry-linkage study included 83 902 twins and 32 719 spouses, matched to 839 020 population controls and 327 190 spous

Disparities in the Epidemiology of COVID-19 in Irish Travellers in the Republic of Ireland.

INTRODUCTION: Irish Travellers, a distinct minority ethnic group, make up 0.64% of the population in the Republic of Ireland (n = 32,949) according to the national census. Major health disparities exist between Travellers and the general population including shorter life expectancy and higher mortality rates from respiratory and cardiovascular disease. METHODS: Here, the epidemiology of COVID-19 in Irish Travellers was investigated. Public health surveillance data on 238,907 notified COVID-19 cases in Ireland from March 2020 to April 2021 were analysed. RESULTS: The crude incidence rate (CIR

Electrical impedance tomography in neonatal respiratory diseases: a clinical review of applications and evidence.

Neonatal respiratory diseases remain a leading cause of morbidity and mortality in intensive care settings, with management complicated by the limitations of current monitoring tools. Chest radiography provides only static snapshots and cannot capture the dynamic evolution of lung aeration, while lung ultrasound, though portable, remains operator-dependent and incapable of quantifying regional ventilation. Electrical Impedance Tomography (EIT) has emerged as a noninvasive, radiation free imaging modality that addresses these gaps by enabling real time, continuous visualization of regional lung

Needs among persons with advanced COPD and their informal caregivers: a multi-perspective qualitative study focusing on the potential role of palliative care.

OBJECTIVES: To explore needs among persons with advanced COPD and informal caregivers, and to explore how palliative care can address those needs. BACKGROUND: COPD is a common, progressive disease affecting lung function and represents a leading cause of death. Advanced COPD is associated with a high symptom burden and poor quality of life. Persons with advanced COPD may benefit from palliative care, but its use among persons with COPD remains limited, also in Germany. METHODS: A qualitative, multi-perspective interview study was conducted with persons suffering from advanced COPD, informal

Post-translational modifications in the insoluble fraction of platelet-poor plasma distinguish pre-COVID-19 Postural Orthostatic Tachycardia Syndrome, Long COVID, and Long COVID-POTS: a preliminary study.

BACKGROUND: Pre-COVID-19 Postural Orthostatic Tachycardia Syndrome (PC-POTS), Long COVID with initial orthostatic hypotension (LC-IOH), and Long COVID-POTS (LC-POTS) are primarily chronic post-viral conditions with debilitating symptoms, despite routine laboratory findings often being normal. We previously identified amyloidogenic fibrinaloid microclot complexes (FMCs) in the insoluble fraction of platelet-poor plasma (PPP-IF) in Long COVID, but it remains unclear whether similar abnormalities occur in PC-POTS and LC-POTS, or whether post-translational modifications (PTMs) contribute to diseas

Immunosuppression is associated with loss of airway microbiome resilience in severe COVID-19.

BACKGROUND: The role of the lower airway microbiome in COVID-19-associated mortality remains poorly understood despite evidence implicating secondary pathogens and non-airway commensals. The influence of prior immunosuppression on the lower airway microbiome and its contribution to mortality in mechanically ventilated patients has been largely unexplored. Given that host-microbiome interactions are dynamic, we assessed longitudinal patterns in the respiratory microbiome associated with mortality and immunosuppression status. RESULTS: We conducted longitudinal forecasting, differential abundan

Prevalence of SARS-COV-2 infection, Relationship between SARS-CoV-2 and HIV among Sex Workers in Cotonou.

BACKGROUND: Since the outbreak of the COVID-19 pandemic, few studies have been carried out on the prevalence of the severe acute respiratory syndrome of the coronavirus 2 (SARS-CoV-2), particularly in groups of patients with comorbidity factors. Moreover, the question of whether HIV increases the risk of contracting SARS-COV-2 or its severity has been the subject of some research. OBJECTIVE: To determine the prevalence of SARS-COV-2 and estabilish the relationship between HIV infection and SARS-CoV-2 infection in female sex workers. METHODOLOGY: Descriptive cross-sectional study of female se

Disseminated BCG disease and recurrent pulmonary infections in a child with trisomy 21 carrying a novel BACH2 variant of uncertain significance: a 5-year case report.

BACKGROUND: Trisomy 21 is associated with combined immune dysregulation, including lymphopenia, impaired responses to vaccines, and heightened susceptibility to respiratory infections, driven in part by triplication of chromosome 21-encoded interferon receptor genes. BACH2 haploinsufficiency is a rare monogenic immunodeficiency characterized by lymphopenia, hypogammaglobulinemia, and susceptibility to infections and autoimmune diseases. Co-occurrence of these two conditions has not been previously described. CASE PRESENTATION: We report a 5-year longitudinal course in a Kenyan girl with triso

Prognostic utility of immune biomarkers for identifying individuals at risk of unfavourable treatment outcomes in pulmonary tuberculosis.

INTRODUCTION: Tuberculosis remains a major global health challenge, with many patients expe- riencing unfavourable treatment outcomes despite standard therapy. Host immune biomarkers, including cytokines, chemokines, and matrix metalloproteinases (MMPs), have shown promise in predicting disease severity and treatment response, but require validation across independent cohorts. METHODS: We validated previously identified immune biomarkers using baseline plasma samples from a nested case-control cohort within the RePORT India consortium (cases: n=74; controls: n=161). Bacteriophage Therapy in Cystic Fibrosis: Emerging Strategies for Managing Chronic Pseudomonas aeruginosa Infections.

Pseudomonas aeruginosa is the leading cause of chronic respiratory infection in people with cystic fibrosis (pwCF) and remains a major driver of morbidity and premature mortality. The unique environment of the CF lung promotes persistent colonisation, biofilm formation, and the emergence of multidrug-resistant (MDR) phenotypes, severely limiting the effectiveness of conventional antibiotic therapies. Despite aggressive and prolonged antibiotic regimens, eradication of chronic P. aeruginosa infection is rarely achieved, and repeated exposure accelerates resistance development. Consequently, the

In vitro antiviral activity of the RdRp inhibitor 4'-fluorouridine against a porcine teschovirus clinical isolate.

Porcine Teschovirus (PTV) is an economically significant swine pathogen with a global distribution that can cause severe neurological, respiratory, and reproductive diseases. However, no specific vaccines or antiviral drugs are currently available. To address the absence of specific antiviral therapies for PTV, this study aimed to identify a clinically promising antiviral candidate through in vitro compound screening. We began by isolating and purified a contemporary PTV strain (PTV/Shandong/2024) from a clinical swab of a dead piglet. We also established a strain-specific RT-qPCR assay to acc

[Specific respiratory tract infections in acute cough-when is targeted diagnosis and treatment required?].

In most cases, acute cough is caused by a self-limiting respiratory infection. However, symptoms, pre-existing conditions, or environmental factors may indicate a cause requiring treatment; red flags point to dangerous courses, the NEWS2 score can assess the risk of septic deterioration, and the CRB-65 score supports decision-making regarding hospital admission in pneumonia. Pneumonia cannot be reliably diagnosed clinically but is unlikely in the absence of clinical indicators. Laboratory tests can support the decision for antibiotic treatment, while the diagnosis is confirmed by chest X‑ray.

Jet-injected combinational adjuvants potentiate naked mRNA vaccines for protection against respiratory viral infections in mice.

The use of lipid nanoparticles (LNPs) to enhance delivery and immunogenicity of mRNA vaccines raises safety concerns due to strong reactogenicity. Developing naked mRNA vaccines, which can be delivered through liquid jet-injection to the skin, may offer a safer alternative, but their immunogenicity has been limited. To address this challenge, we screened clinically relevant adjuvants to enhance the efficacy of N1-methyl-pseudouridine-modified naked mRNA jet-injection vaccines in mouse models. Although many adjuvants showed little or inhibitory effects on inducing humoral or cellular immunity-d

Applying normalized p-control charts for early detection of local COVID-19 outbreaks.

The COVID-19 pandemic underscored the urgent need for early detection of local outbreaks to enable better resource allocation and effective mitigation. Outbreak identification largely depended on local infection rates, which were highly variable and influenced by daily testing volumes. Effective pandemic management requires accurate, adaptive methods that reflect the dynamic and localized nature of these rates, ensuring timely and appropriate interventions. This study seeks to improve the early detection and localization of COVID-19 outbreaks through Statistical Process Control (SPC) and healt

Pandemic Politics: Developing Vaccines in Canada's Anti-vax Heartland.

The COVID-19 pandemic in Saskatchewan is a story of extremes. Through most of 2020, low case numbers, effective policy, and broad agreement among government officials, health care workers, and the public sustained a consensus that strict measures and vaccination offered the best responses; however, that consensus collapsed. The province shifted from a focal point for Canadian vaccine development to a staging ground for an anti-vaccine movement that took root on social media and then in street protests. Premier Scott Moe moved from a pro-vaccine, pro-consensus stance to a confrontational positi
