Prophylactic Hemostatic Treatment to Control Iatrogenic Bleeding During Endobronchial Biopsy (PROTECT Study): A Multicenter, Randomized, Single-Anonymized, Placebo-Controlled Study.

BACKGROUND: Iatrogenic bleeding related to endobronchial biopsy could be catastrophic and might lead to fatal outcomes. Although some guidelines and studies recommend the prophylactic use of hemostatic agents, there is still insufficient evidence from clinical studies. RESEARCH QUESTION: Is prophylactic hemostatic treatment effective for controlling bleeding in patients undergoing endobronchial biopsy? STUDY DESIGN AND METHODS: In this multicenter, randomized, placebo-controlled trial, eligible participants scheduled to undergo endobronchial biopsy were randomly assigned (1:1:1) to the adren

Nasal Airway Transcriptome Reflects Selected Asthma-Associated Gene Signatures in the Lower Airways.

BACKGROUND: Transcriptomic analysis of bronchial brushes reveals asthma-associated gene signatures but is limited by the invasiveness of bronchoscopy. Based on the "united airways" hypothesis, we evaluated whether and to what extent nasal brushes reflect asthma-associated transcriptomic changes in the lower airways. METHODS: In the ARMS study, we included 26 asthma patients and 28 healthy controls, all fully clinically characterized. Nasal and bronchial brushes were collected for RNA sequencing. We used edgeR and WGCNA to identify asthma-associated genes and modules in bronchial brushes. We a

Predictors for pediatric bronchitis obliterans in Mycoplasma pneumoniae pneumonia with bronchial casts.

Bronchitis obliterans is a major complication of Mycoplasma pneumoniae pneumonia (MPP) with bronchial casts. This retrospective study aimed to identify predictors of pediatric bronchitis obliterans (PBO) in patients with mycoplasma pneumoniae pneumonia (MPP) complicated by bronchial casts. We screened 236 hospitalized children with MPP and bronchial casts on bronchoscopy between June 2018 and June 2023, of whom 197 were included in the analysis, including 49 with PBO and 148 without PBO. The clinical features, laboratory data, radiological findings, and bronchoscopic features of the children i

Multidisciplinary management of endobronchial carcinoid tumours through a combined bronchoscopic and surgical approach.

We report the case of a 44-year-old woman with a typical carcinoid tumour of the left mainstem bronchus managed through a multidisciplinary and combined approach. Initial symptoms included haemoptysis and progressive wheezing. A subsequent bronchoscopy revealed a highly vascular, pedunculated mass nearly occluding the airway. En bloc resection using electrocautery and a cryoprobe enabled airway recanalization and diagnosis. Endobronchial ultrasound-guided transbronchial fine needle aspiration ruled out nodal disease. Histopathological examination confirmed a typical carcinoid tumour.  Robotic-

Severe pulmonary edema after pulmonary hypertension interventional surgery: case report and literature review.

BACKGROUND: Pulmonary hypertension caused by fibrosing mediastinitis often presents with clinical manifestations related to involvement of the pulmonary arteries, pulmonary veins, and bronchi. Interventional therapy has become an important treatment option; however, it carries a significant risk of complications. In this case, the patient developed severe pulmonary edema after intervention, which posed major challenges to clinical management. CASE PRESENTATION: A middle-aged man presented with recurrent cough, expectoration, chest tightness, and dyspnea. He was initially treated for chronic o

Thoracoscopic biopsy should be considered for pulmonary nodules with the halo sign during immunotherapy for hepatocellular carcinoma.

Multiple pulmonary nodules developing during immune checkpoint inhibitor (ICI) therapy pose a broad differential diagnosis that includes infection, vasculitis, malignancy, and immune-related adverse events (irAEs). A 77-year-old man receiving atezolizumab–bevacizumab therapy for recurrent hepatocellular carcinoma developed multiple bilateral pulmonary nodules with surrounding ground-glass opacities (halo sign). Although bronchoalveolar lavage cultured Aspergillus versicolor, fungal markers were negative, and the nodules progressed despite antifungal therapy. Tumor markers for hepatocellular ca

Flexible Bayesian modeling of non-equidispersed counts with penalized complexity priors in disease incidence studies.

Counts in epidemiology often deviate from equidispersion and exhibit spatial, temporal, and nonlinear structure that the Poisson model cannot accommodate. We introduce a gamma-count structured additive regression model that strategically integrates penalized complexity priors in two critical aspects: (i) a principled penalized complexity prior on the dispersion parameter of the gamma-count distribution, which naturally shrinks toward the base Poisson model when the data support equidispersion, and (ii) scale-dependent penalized complexity hyperpriors on the smoothing variances for nonlinear, s

Carcinomas and Carcinoid Tumors of the Lungs and Bronchi in Children and Adolescents: The EXPeRT Recommendations.

Primary lung carcinomas and bronchial carcinoid tumors (BC) are very rare malignancies in childhood. While typical BC and mucoepidermoid carcinomas are mostly low-grade, localized tumors with a more favorable prognosis than in adults, necessitating avoidance of overtreatment, adenocarcinomas of the lung are often diagnosed at advanced disease stages with low survival rates. This paper presents consensus recommendations on the diagnosis and treatment of pediatric patients with primary lung carcinomas and BC, established by the European Cooperative Study Group for Pediatric Rare Tumors (EXPeRT)

Akkermansia muciniphila primes lung-resident antiviral immunity via the gut-lung axis during SARS-CoV-2 infection.

INTRODUCTION: The gut microbiota plays a central role in shaping systemic immunity and modulating the gut-lung axis, which is crucial during respiratory infections such as COVID-19. SARS-CoV-2 infection is known to disrupt the gut microbiome, but the downstream functional impacts on microbial metabolism and host immune responses remain insufficiently understood. METHODS: Using K18-hACE2 transgenic mice, researchers investigated the effects of SARS-CoV-2 variants (WA and Omicron) on the gut microbiome and host immunity. Microbial composition and functional profiles were assessed post-infection

A novel quantitative method for CT assessment of bronchiectasis using AI-based airway segmentation and upper limits of normal airway caliber.

BACKGROUND: Bronchiectasis represents a growing global health burden. While computed tomography (CT) is essential for diagnosis, current criteria rely on broncho-arterial ratios and visual assessments, which are subject to inter-observer variability and can be confounded by pathological changes in arterial caliber. This study aimed to develop a novel quantitative method for assessing bronchiectasis by incorporating artificial intelligence-based airway segmentation and establishing location-specific upper limits of normal (ULN) for airway dimensions derived from healthy controls. METHODS: We an

Endoscopic Debulking in the Management of Osteosarcoma With a Rare Endobronchial Metastasis.

Osteosarcoma most commonly metastasizes to the lung, which requires individualized management based on the location of the disease. Surgical resection has been the cornerstone to improving outcomes in peripheral metastatic lung disease. However, central airway obstruction is rare, resulting in limited evidence regarding the optimal management of this manifestation. We report a case of a 68-year-old man with osteosarcoma and bilateral lung metastasis. A 10 centimeter tumor on the right involved the hilum with extension to the pleura. Bronchoscopy confirmed an endobronchial mass occluding the br

Implications of segmental and lobar tracheobronchial anomalies in congenital heart disease: a 12-year retrospective CT analysis.

OBJECTIVES: This study aims to evaluate the prevalence and anatomical patterns of tracheobronchial anomalies and to analyze their associations with different types of congenital heart disease (CHD) using retrospective CT data. INTRODUCTION: Tracheobronchial anomalies, including variations in the branching patterns of the trachea and bronchi, are relatively rare but clinically significant. These anomalies, which are often associated with CHD, can complicate respiratory function and airway management. Despite their low prevalence, early identification and understanding of these anatomical varia

[Non-respiratory lung function (problem status)].

Non-respiratory lung function (NFL) has been an area of intense scientific research for the past twenty years. Experts from the European Respiratory Society have developed a technical regulation that has made it possible to standardize the study of organic and inorganic volatile compounds both in the portion of exhaled air and in the collection of its condensate. The metabolic function of the pulmonary parenchyma is actively manifested in the regulation of water-electrolyte metabolism, as well as biologically active amines and peptides; unique properties of alveolocytes of the second type and

Management of Adult Bronchiectasis: Consensus-Based Guidelines of the German Respiratory Society.

Bronchiectasis is a chronic, often progressive respiratory disease characterized by irreversible dilation of the bronchi. It is etiologically heterogeneous and frequently associated with a significant symptom burden, multiple complications, and reduced quality of life. In recent years, the global prevalence of bronchiectasis has increased markedly, placing a substantial economic burden on healthcare systems. These consensus-based guidelines are the first German-language guidelines focused on the management of bronchiectasis in adults. They underscore the critical role of thoracic imaging - par

Temporal variation in markers of immune signaling and wound repair in primary human airway epithelial cells following acute versus repeated woodsmoke condensate exposure.

Wildfire smoke (WFS) exposures are becoming more common, and firefighters and community members are often exposed to WFS for days to weeks. Controlled studies assessing the effects of repeated exposure to WFS and woodsmoke (WS; as a surrogate for WFS) or compared acute versus repeated exposure have evaluated a limited number of timepoints and endpoints using rodent models. Here, we leveraged differentiated primary human bronchial epithelial cells (HBECs) to test the hypothesis that different molecular responses occur upon acute versus repeat exposures to WS. HBECs (n = 4 donors) were exposed t
