Three-Dimensional Deformations of Pulmonary Collapse for Intraoperative Augmented Reality Guidance: A Proof-of-Concept Study.

OBJECTIVE: During pulmonary surgery, the lung is deflated to facilitate the procedure. This study aimed to assess the deformation of the bronchial tree and pulmonary parenchyma during lung collapse, for eventual use in augmented reality (AR) guidance during pulmonary resections. METHODS: The concept was first tested in 2 porcine models by analyzing paired computed tomography scans of collapsed and inflated lungs, then applied to 6 human patients. Bronchus and parenchyma were segmented, and a bronchus centerline was calculated. The diameter, length differences, angular deformations, and volume

Myelomatous Pleural Effusion: A Case Report.

Multiple myeloma is a relatively common hematologic malignancy; however, serous cavity effusions occur infrequently, and bona fide myelomatous effusions are exceedingly rare, typically representing an advanced, biologically aggressive variant associated with dismal outcomes. We report a compelling case of a 50-year-old patient with established multiple myeloma who developed leukemic transformation with concurrent pleural cavity infiltration while receiving systemic chemotherapy. The patient exhibited rapidly progressive dyspnea, a nonproductive cough, and debilitating fatigue in the days prece

Fatal traumatic injury patterns in free-roaming cats: an anatomopathological study of 40 cases.

BACKGROUND: Trauma is an important cause of death in domestic cats, particularly in animals with unrestricted outdoor access. However, necropsy-based studies focusing specifically on immediately fatal trauma in free-roaming cats remain limited. This study aimed to characterize fatal traumatic injuries in free-roaming cats through a retrospective anatomopathological review of feline necropsies performed over a 10-year period at the Veterinary Pathology Laboratory of the Universidade Federal da Paraíba, Brazil, based on gross findings, histopathology, and complementary imaging when available. A prognostic tool for pulmonary collapse: nomogram-based prediction of 28-day mortality.

BACKGROUND: Pulmonary collapse is a common and serious respiratory condition, but there is no dedicated bedside tool to estimate prognosis. This study aimed to develop a nomogram to predict 28-day mortality in patients with pulmonary collapse. METHODS: We extracted data for patients with pulmonary collapse from MIMIC-III, identified predictors using regression analyses, and used MIMIC-IV for temporal validation. We then built a nomogram based on the selected predictors. Model performance was evaluated using the area under the receiver operating characteristic curve (AUC), AUC comparisons usin

Respiratory Physiotherapy Techniques in Neonatal Intensive Care Units: A Scoping Review.

BACKGROUND AND PURPOSE: Certain physiological characteristics increase the susceptibility of newborns, particularly preterm infants, to pulmonary collapse. In addition, newborns are prone to the accumulation of airway secretions, which, even in small volumes, can compromise respiratory function. Consequently, breathing therapy plays a crucial role in promoting airway clearance and supporting effective ventilation in this population. This scoping review aimed to map the breathing therapy methods used for newborns admitted to intensive care units and also to identify the clinical outcomes assess

Possible Paraneoplastic Amyotrophic Lateral Sclerosis Presenting as Respiratory Failure in a Breast Cancer Patient: A Case Report.

A 63-year-old female with metastatic breast cancer presented with acute hypercapnic respiratory failure secondary to severe diaphragmatic dysfunction and mixed upper and lower motor neuron signs. Serum onconeural and neural surface antibody panels were negative, and initial cerebrospinal fluid analysis revealed albuminocytologic dissociation. Immunotherapy with intravenous immunoglobulin (IVIG) produced no clinical improvement, necessitating invasive mechanical ventilation and gastrostomy. Applying the 2021 PNS-Care consensus criteria, the case fulfills criteria for possible paraneoplastic amy

Alcohol-related disorders are associated with short-term outcomes in lung cancer patients undergoing VATS lobectomy: a 5-year retrospective analysis.

BACKGROUND: Alcohol abuse/dependence has been associated with an increased risk of postoperative complications. This study aims to evaluate the impact of alcohol abuse/dependence on short-term surgical outcomes in lung cancer patients undergoing video-assisted thoracoscopic surgery (VATS) lobectomy. METHODS: We analyzed data from the National Inpatient Sample (NIS) covering the years 2016 to 2020, focusing on 13,273 lung cancer patients who had undergone VATS lobectomy. Patients were categorized based on their history of alcohol abuse/dependence. A 1:2 nearest-neighbor propensity-score matchi

Navigating Anesthesia for Thoracoscopic Biopsy in a Six-Year-Old Child With Eosinophilic Granulomatosis With Polyangiitis, Stroke Sequelae, and Pulmonary Collapse.

Eosinophilic granulomatosis with polyangiitis (EGPA), formerly known as Churg-Strauss syndrome, is a rare systemic vasculitis that affects small- to medium-sized vessels, characterized by asthma, hypereosinophilia, and multiorgan involvement. Pediatric cases are sporadic, and anesthetic management poses significant challenges due to airway hypersensitivity, allergy risk, and systemic complications. We describe the anesthetic management of a six-year-old girl with EGPA presenting with left-sided hemiparesis secondary to a prior intracerebral bleed, severe eosinophilia, and left lung collapse wi

Risk Estimates of Five Adverse Outcomes Following Hip and Spine Surgery Among Children with Cerebral Palsy to Inform Peri-operative Care.

BACKGROUND: Children with cerebral palsy (CP) are vulnerable to post-operative adverse outcomes following orthopaedic surgery; yet clinically usable risk estimates of these outcomes remain underreported. The number of anti-seizure medications (ASMs) may capture a child's underlying medical complexity, thus serving as a simple, easily attainable measure for risk stratification. The objective of this study was to generate post-operative risk estimates of five adverse outcomes based on the number of ASMs among children with CP undergoing hip or spine surgery. METHODS: Using the IBM® MarketScan®

An exceptional case of sudden death due to pseudo-Meigs syndrome.

Pseudo-Meigs syndrome is a rare clinical entity characterized by the triad of ascites, pleural effusion, and a benign pelvic tumor other than an ovarian fibroma. Although typically considered benign and reversible following tumor removal, the syndrome can result in severe life-threatening compromise if unrecognized or untreated. Unlike the classical Meigs syndrome, which has occasionally been associated with fatal outcomes, no deaths related to pseudo-Meigs syndrome due to benign tumors have been previously reported. This report presents the sudden death of a young adult woman who complained o

Acute respiratory distress following an otherwise normal, uneventful, screening colonoscopy.

A 62-year-old man with a past history of sleep apnea syndrome, umbilical and left inguinal hernia repairs, was referred to the emergency room for acute respiratory distress. He had underwent a screening colonoscopy 12 h earlier for a family history of colonic adenoma. This colonoscopy was complete, normal, and uneventful. A plain chest X-ray showed a distended colon extending to the upper third of the right side of the chest (Fig. 1). Further anamnesis helped the patient to remember a right diaphragmatic hernia, well-documented by CT-scan years ago. He had not previously mentioned this conditi

Phenotyping Down syndrome: discovery and predictive modelling with electronic medical records.

BACKGROUND: Individuals with Down syndrome (DS) have a heightened risk for various co-occurring health conditions, including congenital heart disease (CHD). In this two-part study, electronic medical records (EMRs) were leveraged to examine co-occurring health conditions among individuals with DS (Study 1) and to investigate health conditions linked to surgical intervention among DS cases with CHD (Study 2). METHODS: De-identified EMRs were acquired from Vanderbilt University Medical Center and facilitated creating a cohort of N = 2282 DS cases (55% females), along with comparison groups for

Impact of pneumothorax-like pulmonary collapse caused by rapid bronchial obstruction: A case report of pneumothorax ex vacuo.

We report two cases of pulmonary collapse that simulated pneumothorax on computed tomographic images and were caused by rapid complete bronchial obstruction. One patient was a 77-year-old woman with sudden dyspnea, and the other was an 83-year-old woman with sudden dyspnea who was infected with influenza A virus. Chest computed tomography revealed lobular complete atelectasis with an almost complete expansion of the other lobes of the right lung. Some air space in the right pleural cavity was also observed. Both cases were diagnosed as "pneumothorax" by primary doctors. We noted the disappeara

Esketamine opioid-free intravenous anesthesia versus opioid intravenous anesthesia in spontaneous ventilation video-assisted thoracic surgery: a randomized controlled trial.

BACKGROUND: Opioid-free anesthesia (OFA) provides adequate analgesia and can reduce postoperative opioid consumption, but its efficacy in spontaneous ventilation video-assisted thoracic surgery (SV-VATS) has not been demonstrated. We aimed to investigate the hypothesis that OFA could provide the same perioperative pain control as opioid anesthesia (OA), maintain safe and stable respiration and hemodynamics during surgery, and improve postoperative recovery. METHODS: Sixty eligible patients (OFA group: n=30; OA group: n=30) treated between September 15, 2022, and December 15, 2022, at The Firs

Nationwide Outcomes After Thoracoscopic Versus Open Resection of Congenital Pulmonary Airway Malformations in Newborns.

Purpose: Elective resection of congenital pulmonary airway malformations (CPAM) has been debated for decades and varies significantly between individual surgeons. However, few studies have compared outcomes and costs associated with thoracoscopic and open thoracotomy approaches on a national level. This study sought to compare nationwide outcomes and resource utilization in infants undergoing elective lung resection for CPAM. Materials and Methods: The Nationwide Readmission Database was queried from 2010 to 2014 for newborns who underwent elective surgical resection of CPAM. Patients were str

One-lung ventilation with a bronchial blocker in thoracic patients.

BACKGROUND: Lung isolation is a technique used in a multitude of surgeries to ensure single-lung ventilation with collapse of the contralateral lung, as to achieve improved access and visualization of relevant anatomical structures. Despite being accepted and having favorable outcomes, bronchial blockers (BBs) are not to this day the main device of choice among anaesthesiologists. METHODS: In this retrospective and descriptive study, we analyzed the safety and efficacy of a BB in all types of thoracic surgeries in our centre between 2015 and 2022, excluding patients with massive hemoptysis or

Lung Recruitment Maneuvers Assessment by Bedside Lung Ultrasound in Pediatric Acute Respiratory Distress Syndrome.

The use of recruitment maneuvers (RMs) is suggested to improve severe oxygenation failure in patients with acute respiratory distress syndrome (ARDS). Lung ultrasound (LUS) is a non-invasive, safe, and easily repeatable tool. It could be used to monitor the lung recruitment process in real-time. This paper aims to evaluate bedside LUS for assessing PEEP-induced pulmonary reaeration during RMs in pediatric patients. A case of a child with severe ARDS due to Haemophilus influenzae infection is presented. Due to his poor clinical, laboratory, and radiological evolution, he was placed on venovenou

BLUNT TRAUMA INTERCOSTAL LUNG HERNIATION AND DELAYED EXTRA PLEURAL HEMATOMA.

Blunt chest trauma is an important cause of morbidity and mortality in traumatized emergency patients. We report the case of a 74-year-old man who suffered a glenohumeral joint dislocation, trans trochanteric femur fracture, multiple rib fractures, diaphragmatic rupture with chest herniation of the spleen and stomach associated with herniation of the lung through an anterior chest wall defect after blunt trauma. Although immediate surgical repair was performed, he developed a delayed complication of multiple rib fracture in the form of large extrapleural hematoma that had to be surgically remo

Emergency application of extracorporeal membrane oxygenation in a pediatric case of sudden airway collapse due to anterior mediastinal mass: A case report and review of literature.

Mediastinal masses can compress the respiratory or cardiovascular system, especially when anteriorly located. Obtaining histological material for diagnosis poses a challenge due to the major risk of cardiorespiratory collapse following anesthetic procedure. Our case shows the utility of rescue with venovenous extracorporeal membrane oxygenation (VV-ECMO) after occurrence of such an event and demonstrates the feasibility of administering chemotherapy during VV-ECMO. A 4-year-old boy was referred to the pediatric oncology clinic of our hospital after a large mediastinal mass was observed on ches

Retrospective outcomes analysis of tracheostomy in a paediatric burn population.

INTRODUCTION: Previous analyses of tracheostomy in paediatric burns was hindered by a lack of multi-institution or nationwide analysis. This study aims to explore the effects of tracheostomy in paediatric burn patients in such an analysis. De-identified data was obtained from the TriNetX Research Network database. METHODS: Two cohorts were identified using ICD and CPT codes: paediatric burn patients with tracheostomy (cohort 1) and paediatric burn patients without tracheostomy (cohort 2). Cohorts were matched according to age at diagnosis and pulmonary condition, specifically influenza and pn
