Sectional neuroanatomy of the upper thoracic spine and chest.

This is the second in a series of articles on the spine. The first reviewed the anatomy of the neck. Subsequent articles are planned to cover the anatomy of the middle and lower thoracic spine. Procedures and trauma of the upper thoracic spine and chest are fraught with potentially serious complications. Hemothorax, pneumothorax, nerve damage, pulmonary collapse, and thoracic aortic aneurysm are included in the list. This article provides anatomically accurate schematics of innervation of the upper thoracic chest and spine that can be used to interpret magnetic resonance images of the muscles

Respiratory distress in neonates.

Respiratory distress due to either medical or surgical causes occurs commonly in neonates. It is the most common cause of admission to a neonatal surgical intensive care facility in a tertiary care hospital. The distress can be caused by a variety of clinical conditions; common conditions treated in medical intensive care units are transient tachypnea of the new born, respiratory distress syndrome, pulmonary air leak and pneumothorax. In surgical causes of respiratory distress in neonates the underlying mechanisms include airway obstruction, pulmonary collapse or displacement and parenchymal d

A new rabbit model of myocardial infarction without endotracheal intubation.

INTRODUCTION: When establishing a myocardial infarction model in rabbits, artificial ventilation under endotracheal intubation is required to prevent pulmonary collapse caused by pneumothorax. However, it is difficult to carry out endotracheal intubation due to the rabbit anatomy. We therefore attempted to develop a myocardial infarction model in rabbits without endotracheal intubation. MATERIALS AND METHODS: A facial oxygen mask with a flow rate of 200 mL/min was placed on anesthetized rabbits (n = 17). The sternum was carefully incised along the midline to avoid injury to the parietal pleur

Treatment of a well differentiated pulmonary adenocarcinoma in a cat by pneumonectomy and adjuvant mitoxantrone chemotherapy.

Primary lung neoplasia is a rare condition of older cats. This report describes a case of left-sided pulmonary collapse, secondary to pulmonary adenocarcinoma in an 11-year-old cat. Left sided pneumonectomy was performed, and adjuvant chemotherapy with mitoxantrone (Novantrone; Lederle) was administered every 3 to 5 weeks, for 10 doses after surgery. Thirty-four months after surgery, the cat remains well and has no radiographic evidence of pulmonary metastasis.

Conscious volunteers developed hypoxemia and pulmonary collapse when breathing air and oxygen at reduced lung volume.

Ventilation at maximal voluntary reduction of lung volume caused significant desaturation in some healthy subjects breathing air. Saturation rapidly returned to control levels when normal lung volume was regained. These changes are probably due to reversible airway obstruction. During the inhalation of oxygen, ventilation at maximal voluntary reduction of lung volume caused, in one subject, a reduction of arterial Po2 of 243 mmHg. Normal arterial Po2 was not immediately restored on regaining normal lung volume. Chest radiographs showed extensive atelectasis, which persisted for several hours i

Thoracic trauma in foals: post mortem findings.

REASONS FOR PERFORMING STUDY: Thoracic trauma occurs in newborn foals and may cause associated clinical signs; this condition remains poorly documented. OBJECTIVES: The purpose of this study was to describe the pathological features of thoracic trauma in newborn foals presented for necropsy examination between 1990 and 2000. METHODS: Necropsy reports of foals with thoracic trauma from 1990-2000 were reviewed. Subject details, clinical signs, thoracic and abdominal lesions were noted and analysed statistically. RESULTS: Sixty-seven (9%) of 760 necropsied foals had thoracic trauma. In 19 foal

Hantavirus.

When hantaviruses hit the headlines with the advent in May 1993 of a new disease in the USA, and later in the New World from Canada to south Argentina, called "hantavirus pulmonary syndrome" (HPS), speculations in the lay press rose from the very beginning around the possibilities of a biological warfare (BW) weapon. Indeed, the responsible agent of HPS, hantavirus, was almost unknown at that moment in the New World, was airborne, seemed to target preferentially young adults, and induced a devastating cardio-pulmonary collapse with a high case-fatality rate (50%), often within hours. It quickl

[Changes of pulmonary tissue area density and their mechanisms in rats with hypoxic pulmonary hypertension].

OBJECTIVE: To investigate the changes of pulmonary tissue area density and their mechanisms in rats with hypoxic pulmonary hypertension (HPH). METHODS: 54 Wistar rats were divided into hypoxia 10 d (n = 12), 20 d (n = 12), 30 d (n = 12) groups and control group (n = 18). The rats in hypoxia groups were exposed to a simulated hypoxia environment at a height of 5 km above sea level to establish HPH models. The changes in pulmonary tissue area density and pathological morphology were determined by image analysis, optical microscope, electron microscope and histochemistry. RESULTS: After hypoxia

Alveolar recruitment strategy increases arterial oxygenation during one-lung ventilation.

BACKGROUND: Deterioration of gas exchange during one lung ventilation (OLV) is caused by both total collapse of the nondependent lung and partial collapse of the dependent lung. A previous report demonstrated that an alveolar recruitment strategy (ARS) improves lung function during general anesthesia in supine patients. The objective of this article was to study the impact of this ARS on arterial oxygenation in patients undergoing OLV for lobectomies. METHODS: Ten patients undergoing open lobectomies were studied at three time points: (1) during two-lung ventilation (TLV), (2) during OLV befo

A comparison of the effects of manual and ventilator hyperinflation on static lung compliance and sputum production in intubated and ventilated intensive care patients.

BACKGROUND AND PURPOSE: Lung hyperinflation is a technique used by physiotherapists to mobilize and remove excess bronchial secretions, reinflate areas of pulmonary collapse and improve oxygenation. Hyperinflation may be delivered by the ventilator or manually, by use of a manual resuscitation circuit, depending upon the respiratory and cardiovascular status of the patient. The effects of manual hyperinflation, with respect to excess bronchial secretions and static lung compliance, have been well-established. There is, however, only limited evidence as to the efficacy of ventilator hyperinflat

[Effects of the alveolar recruitment manoeuver and PEEP on arterial oxygenation in anesthetized obese patients].

BACKGROUND: Diminished functional residual capacity and pulmonary collapse during general anesthesia lead to alterations in respiratory mechanics and gas exchange. Such phenomena are more pronounced in obese patients. We recently demonstrated the beneficial effects of the alveolar recruitment strategy on oxygenation in anesthetized patients of normal body mass index (BMI). The aim of the present study was to evaluate whether obese patients also benefit from the alveolar recruitment strategy and to determine the level of positive end-expiratory pressure (PEEP) that prevents recollapse in obese

Rescue use of DNase in critical lung atelectasis and mucus retention in premature neonates.

Excessive or thick pulmonary secretions are a common clinical challenge in the neonatal population. Mucus accumulation can cause many life-threatening complications, including plugging of the endotracheal tube and increasing the risk of pulmonary infections. We report 3 premature neonates who had critical pulmonary collapse secondary to mucous plugging. Different conventional methods to liquefy mucus and facilitate removal of secretions were exhausted to no avail. The rescue use of DNase was effective in reestablishing airway patency. Thus, this drug could be a valuable tool in treating atelec

Surgically treated Swyer-James syndrome.

Because patients with Swyer-James syndrome have almost always been treated conservatively, few reports exist of pathological findings of the lung in this syndrome. We report a case of this rare disease treated surgically and discuss pathological findings. A 36-year-old woman repeatedly contracted bronchitis and pneumothorax since adolescence, until April 26, 1997, when she reported chest pain and dyspnea. Chest X-ray on admission showed left pulmonary collapse with a slight deviation of the mediastinum toward the right. Chest computed tomography showed an apical bulla and emphysematous change

Atelectasis formation during anesthesia: causes and measures to prevent it.

Pulmonary gas exchange is regularly impaired during general anaesthesia with mechanical ventilation. This results in decreased oxygenation of blood. A major cause is collapse of lung tissue (atelectasis), which can be demonstrated by computed tomography but not by conventional chest x-ray. Collapsed lung tissue is present in 90% of all subjects, both during spontaneous breathing and after muscle paralysis, and whether intravenous or inhalational anaesthetics are used. There is a correlation between the amount of atelectasis and pulmonary shunt. Shunt does not increase with age. In obese patien

[Nonmalignant asbestos-related diseases].

Non malignant asbestos diseases are dominated numerically by pleural plaques. This form of circumscribed fibrosis of the parietal pleural is generally asymptomatic and its potential for evolution is weak. More rarely diffuse pleural fibrosis results from disease of the visceral pleura with a secondary fusion of the two pleural layers. Usually it follows a pleural effusion which resolves spontaneously. Its functional effects are sometimes important. The generalised practice of computerized tomographic scanning as a work up for asbestos related disease has revealed the frequent association of th

[Pulmonary re-expansion edema. Description of two cases and observations about its management].

Two cases of Reexpansion pulmonary edema (RPE), an uncommon complication of the treatment of chronic lung collapse secondary to pneumothorax or pleural effusion, are described. RPE is generally unilateral and occurs when the lung is rapidly reexpanded by active evacuation of large amounts of air or fluid. Nevertheless, both cases observed confirm that RPE can be seen when the pulmonary collapse is of short duration and the lung is reexpanded without suction. The pathogenesis of RPE is still unclear and is probably multifactorial. Implicated in the etiological process of RPE are chronicity of c

An unusual presentation of suspected oedema disease of swine in Kenya.

From a group of 11 recently weaned pigs, 4 were reported to be sick. Clinical examination of the sick pigs revealed marked dyspnoea, bluish-red discolouration of the skin, incoordination and difficulty in walking. Bacteriological examination of the gut contents of 2 pigs that had died earlier yielded pure cultures of haemolytic Escherichia coli. Post mortem examination of the remaining 2 pigs that died subsequently revealed progressive pulmonary collapse. One of these also showed subcutaneous oedema of the head and marked oedema of the mesentery of the spiral colon and oedema of the brain. Mic

Absorption of intubation-related lidocaine from the trachea during prolonged cardiopulmonary resuscitation.

The purpose of this study was to determine whether lidocaine is absorbed from the trachea during the artificial circulation of cardiopulmonary resuscitation. The tissue distribution of lidocaine was investigated in eight individuals (Cases 1-8) who underwent cardiopulmonary resuscitation before being pronounced dead. In Cases 1-4, there was no restoration of heart beat during cardiopulmonary resuscitation. Heart massage had been continued for 5 min in Cases 1 and 2, and for 60 min in Cases 3 and 4. Relatively high concentrations of lidocaine (more than 0.1 mg/L) were detected in the blood left
