Evolution of 3D-Printed Microneedles toward Closed-Loop Theranostic Platforms.

Rheumatoid arthritis (RA) is an autoimmune disorder that induces joint inflammation, cartilage injury, and bone damage. Thus far, methotrexate (MTX) is a primary DMARD drug to treat RA. Despite high efficiency, its clinical application is compromised due to delivery-associated systemic side effects. This investigation reports a Quality-by-design (QbD; Box Behnken Strategy) assisted production of a novel, innovative, and multipurpose polycation-templated approach for producing stable albumin MTX nanoparticles (pT-AMTX-NP). This approach formed a highly biocompatible MTX formulation with reduced

Interstitial pneumonia and lymphocytic bronchiolitis with interstitial emphysema of unknown cause in Western Canadian fetal and neonatal beef calves.

Interstitial pneumonia and lymphocytic bronchiolitis with interstitial emphysema is an unusual reaction pattern in fetal and neonatal calves. These changes often are thought to suggest a chronic bacterial infection acquired in utero, and an associated placentitis is expected. Viral agents can also be implicated. Specific known pathogens that can induce a similar inflammatory response include Ureaplasma diversum, Mycoplasmopsis bovis, Brucella abortus, bovine viral diarrhea virus, and bovine parainfluenza virus 3. We describe a series of 15 cases of interstitial pneumonia and lymphocytic bronch

Strain-induced pulmonary interstitial emphysema: Exploring trauma-associated injury mechanisms.

The present study examines the effects of direct blunt impact trauma on deformation, strain, and subsequent mechanical tissue damage within fresh, inflated, ex vivo porcine lungs. As a result of the applied impact, adjacent segmental bronchi can either converge or diverge depending on their location relative to the impact site. This motion generates localized tensile and shear strains that exceed those measured at the impact site. Histological analysis of tissue excised from these regions found air-filled cystic lesions characteristic of pulmonary interstitial emphysema (PIE). These lesions we

Initial CPAP of 5 versus 8 cmH2O during delivery room stabilisation of very preterm infants: the PHILODENDROOM pilot randomised trial.

UNLABELLED: The objective was to evaluate the feasibility and safety of conducting a randomised controlled trial comparing two initial continuous positive airway pressure (CPAP) levels during delivery room (DR) stabilisation of very preterm infants at risk for respiratory distress syndrome (RDS) and to explore their early physiological respiratory effects. In this pilot randomised controlled trial conducted at a tertiary neonatal intensive care unit, preterm infants born between 26 + 0 and 29 + 6 weeks' gestation requiring respiratory support at birth were randomised to receive CPAP at either

Choking in Down Syndrome: Clinical and Forensic Perspectives.

Choking is a serious yet often overlooked risk among individuals with Down syndrome (DS), primarily due to their anatomical and neuromuscular vulnerabilities. This case report discusses the sudden choking-related death of an 11-year-old child with DS, emphasizing both clinical and forensic perspectives. The child developed respiratory distress while eating and became unresponsive, leading to a medicolegal investigation. A comprehensive postmortem examination, including verbal autopsy, postmortem computed tomography (PMCT), and traditional autopsy, was performed. PMCT revealed a foreign body ob

Evaluating remote post-mortem inspection in bovines: Challenges, limitations, and future potential in assessing post-mortem outcomes in cattle offal.

In the European Union, effective meat inspection is crucial for safeguarding public health and animal welfare, particularly through post-mortem inspections, which act as the final barrier against zoonotic and animal diseases. This study explores the feasibility of using remote video transmission as a modern solution to enhance oversight in meat inspections. Furthermore, it aims to collect and analyze data from routine post-mortem veterinary inspections of bovine viscera, as there is still limited knowledge in this area. A comparative study involving the routine post-mortem inspection of bovine

Pulmonary interstitial emphysema in newborns.

Pulmonary interstitial emphysema (PIE) is a rare case of air leak syndrome in newborns, with air escaping from the tracheobronchial tree into the lung tissue. We observed 3 cases of PIE. The three newborns were moderately premature. The common initial presentation was respiratory distress, which subsequently worsened. The treatment was conservative, with positioning and oxygen therapy. Medium- and long-term outcomes were good, with patients remaining clinically symptom-free. However, follow-up CT scans revealed the persistence of sequelae bullae.Systematic medium- and long-term follow-up may be

Acquired interstitial emphysema in a premature patient. Treatment with unilateral angioplasty balloon pulmonary blockage.

INTRODUCTION: Pulmonary interstitial emphysema is a severe rare complication associated with mechanical ventilation in pre-term patients. It induces alterations both in ventilation and pulmonary perfusion, and it may cause progressive overdistension of the side involved and atelectasis of the contralateral lung. Management is uneasy and requires changing ventilation strategies, with invasive procedures such as blockage and selective pulmonary ventilation being potentially necessary. CLINICAL CASE: Premature female patient born at gestation week 26+0, with an initially good clinical progressio

Pulmonary interstitial emphysema: A rare complication in adults (Report of two cases).

Pulmonary interstitial emphysema (PIE) is a rare and potentially life-threatening complication in mechanically ventilated adults. While extensively described in neonates, its occurrence in adults remains exceptional. We report two cases of PIE in polytrauma patients requiring invasive ventilation after severe road traffic accidents. Both cases are remarkable for early detection on CT within hours after intubation and the occurrence of PIE despite moderate ventilator settings. Initial CT imaging revealed characteristic findings of PIE, including air tracking along the interlobular septa and pul

Case Report: Neurally adjusted ventilatory assist as an effective rescue treatment for pulmonary interstitial emphysema in extremely low birth weight infants.

Pulmonary interstitial emphysema (PIE) is a complication observed in extremely low birth weight (ELBW) infants on mechanical ventilation. Despite various proposed therapeutic interventions, the success rates have shown inconsistency. Neurally adjusted ventilatory assist (NAVA) stands out as a novel respiratory support mode, offering lower pressure and tidal volume in comparison to conventional ventilation methods. In this case report, we present five ELBW infants with refractory PIE who were transitioned to NAVA ventilation. Following the switch to NAVA, all cases of PIE gradually resolved. In

Significant Spontaneous Pneumomediastinum and Extensive Subcutaneous Emphysema in a COVID-19 Patient.

A 64-year-old man, who had no pre-existing health conditions, was admitted to the intensive care unit due to progressive shortness of breath resulting from COVID-19. Initially, the patient responded to non-invasive mechanical ventilation, which improved his breathing function. However, after six days, his respiratory function worsened significantly, requiring invasive ventilation. Out of nowhere, the person experienced spontaneous pneumomediastinum and extensive subcutaneous emphysema. The next day, a spontaneous pneumothorax occurred and was successfully drained later. It became evident that

Pulmonary interstitial emphysema and spontaneous pneumomediastinum are more prevalent in sighthounds than other dog breeds undergoing thoracic CT.

Spontaneous pneumomediastinum is an uncommon condition described in veterinary medicine with concurrent respiratory disease. It can be caused by the Macklin effect, which is when gas leaks from the alveoli into the surrounding interstitial lung tissue. Pulmonary interstitial emphysema (PIE) is the presence of gas within the pulmonary vascular sheaths and indicates the presence of the Macklin effect. In the authors' experience, spontaneous pneumomediastinum and PIE are more prevalent in sighthound dogs than in other breeds and are often considered incidental findings. This retrospective, observ

Percutaneous Drainage for Giant Pulmonary Interstitial Emphysema in a Tiny Infant with a Birth Weight of 327 g.

Giant pulmonary cyst in extremely low birth weight (ELBW) infants has been described as one of severe pulmonary diseases. Any definitive therapy for refractory cases, where conservative methods of treatments are not effective, has not been established as a standard. Herein, we report an ELBW infant with a giant pulmonary cyst cured by percutaneous drainage without any adverse events. A female infant was born with a birth weight of 327 g. Surfactant was administered on days 1 and 2 of life to treat respiratory distress syndrome. Tracheal intubation was performed and synchronized intermittent ma

Air leak syndrome in animals: definition and pathogenesis.

Air leak syndrome (ALS) is described in human medicine as a constellation of clinical disorders including pneumomediastinum, pneumopericardium, pulmonary interstitial emphysema, pneumothorax, pneumoperitoneum, pneumoretroperitoneum and subcutaneous emphysema. The pathogenesis of ALS depends on the anatomy of the mediastinum and its associations with thoracic, abdominal and cervical connective tissues, as well as a physical phenomenon referred to as the Macklin effect. Various animal species develop diverse combinations of these lesions, although ALS has not been recognized in animals. However,

Spontaneous pulmonary interstitial emphysema, pneumomediastinum and cervical subcutaneous emphysema secondary to the Macklin effect in dogs with no clinical signs: 12 cases.

Pneumomediastinum denotes the presence of gas within the mediastinum and generally occurs by leakage of air from an aerated viscus that traverses or abuts the mediastinal plane. The Macklin effect has been described in several veterinary studies and describes gas tracking along the perivascular interstitium following alveolar rupture causing interstitial emphysema, pneumomediastinum and subsequently cervical subcutaneous emphysema. This retrospective case series describes incidental spontaneous pulmonary interstitial emphysema, pneumomediastinum and cervical subcutaneous emphysema secondary to
