Unicentric hyaline-vascular Castleman disease of the jejunal mesentery mimicking a gastrointestinal stromal tumor or neuroendocrine tumor: a case report.

BACKGROUND: Castleman disease (CD) is a rare lymphoproliferative disorder classified as unicentric (UCD) or multicentric (MCD). The hyaline vascular variant of UCD most commonly arises in the mediastinum, whereas mesenteric involvement is exceedingly rare. As it can mimic hypervascular mesenteric tumors like neuroendocrine tumors (NETs) and gastrointestinal stromal tumors (GISTs), preoperative diagnosis remains challenging. CASE PRESENTATION: A 36-year-old woman presented with a 3-month history of vague abdominal discomfort, early satiety, and an unintentional 9-kg weight loss. Contrast-enhan

The role of thoracic interventional radiologists in the management of chest infections: a pictorial review.

Thoracic infections remain a major cause of morbidity and mortality worldwide, requiring accurate and timely diagnosis to guide appropriate treatment. This pictorial review illustrates the role of the interventional thoracic radiologist in the management of infectious processes involving the lung, pleura, mediastinum, and chest wall. We describe a range of non-vascular image-guided percutaneous diagnostic and therapeutic techniques. These include targeted sampling for microbiological identification in complicated pneumonias and cavitary lesions, as well as image-guided drainage of pulmonary, p

Delayed diagnosis of selected thoracic and mediastinal conditions in young adults: a focused narrative review of clinical pitfalls, cognitive biases, and system barriers.

BACKGROUND: Thoracic and mediastinal conditions affecting the esophagus, diaphragm, mediastinum, and airway carry considerable morbidity when diagnosis is delayed. Young adults are often perceived as low risk for serious thoracic disease, and low clinical suspicion, atypical symptoms, and cognitive and healthcare-system barriers can delay recognition. METHODS: This focused narrative review searched PubMed and Scopus, supplemented by Google Scholar, for studies published between January 2000 and May 2026, with selected earlier landmark studies retained where essential. Search terms included "t

Direction of discordance between PET/CT, EBUS-TBNA, and multidisciplinary tumor board nodal assignment in non-small cell lung cancer: A retrospective observational study.

Mediastinal nodal classification in non-small cell lung cancer (NSCLC) at the N0/N1-N2/N3 treatment-defining boundary guides therapeutic strategy and is commonly finalized in multidisciplinary tumor board (MDT) meetings. We compared positron emission tomography/computed tomography (PET/CT) and endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) against MDT assignment and quantified discordance at this threshold. We retrospectively analyzed 114 consecutive NSCLC patients who underwent both PET/CT and EBUS-TBNA before MDT review. Nodal stage was dichotomized as N0/N1 vs

Tortuous Descending Thoracic Aorta in a Patient with a History of Ascending Aorta Replacement as a Confounder in Transesophageal Echocardiographic Evaluation of the Left Atrium: A Procedural Challenge.

HIGHLIGHTS: Descending thoracic aorta tortuosity, sometimes present as a vascular mass in the mediastinum, distorting nearby cardiac chambers, which may confuse physicians during transesophageal echocardiography.Recognizing the potential echocardiographic presentation of aortic tortuosity can aid in the timely identification of this condition during transesophageal echocardiography. UNLABELLED: Descending thoracic aorta tortuosity results from elongation of the descending aorta and is associated with aging. It acts as a risk factor for challenging endovascular procedures and has prognostic si

Fetal hemothorax due to intrapleural rupture of a small anterior thoracic lymphatic malformation: a case report.

BACKGROUND: Fetal pleural effusion is usually chylous, whereas hemothorax is considerably rarer. Trauma, iatrogenic complications, and rupture of intrathoracic tumors have been reported as causes of fetal hemothorax. Lymphatic malformations often enlarge during the fetal period; however, these benign lesions may regress spontaneously after birth or be managed with sclerotherapy. Accurate prenatal diagnosis is therefore crucial for determining whether fetal intervention or postnatal management is the most appropriate strategy. Here, we present a rare case of a fetus with hemothorax presumably c

Local repetitive transcranial magnetic stimulation (rTMS) induces whole-brain functional dynamic remodeling.

OBJECTIVE: To explore the features of various mediastinal lymphadenopathies using computed tomography perfusion (CTP). METHODS: CTP parameters (CTPs) of the selected mediastinal nodes from 59 patients with pathology-proven malignant lymph nodes and of those from 29 patients with clinically diagnosed or pathology-proven inflammatory lymphadenopathies were collected. Patients were divided into subgroups by etiology and phase of primary disease, including different pathological malignant nodes and diverse inflammatory nodes. CTPs were defined as blood flow (BF), blood volume (BV), mean transit t

Safety of internal thoracic artery use in patients with prior mediastinal radiation undergoing coronary artery bypass grafting: a Maryland statewide propensity-matched analysis.

BACKGROUND: The safety of internal thoracic artery (ITA) grafting in patients undergoing coronary artery bypass grafting (CABG) with prior mediastinal radiation remains controversial due to concerns regarding compromised sternal perfusion and radiation-induced injury. This study evaluated whether prior mediastinal radiation is associated with adverse perioperative outcomes in patients undergoing CABG with ITA grafting. METHODS: We conducted a retrospective cohort study using the Maryland Cardiac Surgery Quality Initiative (MCSQI) database. A total of 29 206 patients who underwent CABG with IT

A case of antineutrophil cytoplasmic antibody-associated vasculitis and sclerosing mediastinitis.

Sclerosing mediastinitis (SM) is a rare condition characterised by extensive fibrous proliferation within the mediastinum. While some patients remain asymptomatic, others may present with chest pain, dyspnoea, haemoptysis, or complications such as superior vena cava syndrome or pulmonary hypertension. The aetiology of SM may be caused by infections, malignancies, autoimmune diseases, radiation therapy, or have idiopathic origins. We present a case of a 55-year-old man diagnosed with SM and granulomatosis with polyangiitis. The patient initially presented from an outside hospital with a large p

Comparison of Diagnostic Utilities in a Canine Retrobulbar Large B-Cell Lymphoma With Multifocal Involvement.

Lymphoma is the most common malignant neoplasm in dogs. However, retrobulbar lymphoma, a type of orbital lymphoma, is rarely reported in dogs. An 8-year-old neutered male Shih Tzu dog presented with bilateral exophthalmos. Computed tomography and magnetic resonance imaging revealed bilateral retrobulbar masses and enlargement of the salivary glands and cranial mediastinum. Histopathological examination and immunohistochemistry of the retrobulbar tissue confirmed large B-cell lymphoma. On the cytologic analysis, intermediate-to-large malignant lymphocytes were predominant in the retrobulbar tis

Clinical outcomes with perioperative nivolumab by nodal status in patients with stage III resectable NSCLC: phase 3 CheckMate 77T exploratory analysis.

Individuals with non-small-cell lung cancer (NSCLC) with metastases to the ipsilateral mediastinum or subcarinal lymph nodes (N2 disease) have poor long-term survival. This exploratory analysis from the randomized phase 3 CheckMate 77T study assessed clinical outcomes by nodal status in individuals with stage III NSCLC who received neoadjuvant nivolumab plus chemotherapy followed by surgery and adjuvant nivolumab (nivolumab) versus neoadjuvant chemotherapy followed by surgery and adjuvant placebo (placebo). Here we show that among patients with N2 disease, nivolumab versus placebo improved eve

Multicenter development and validation of a probability-based model to diagnose Lewy body disease using ¹²³I-meta-iodobenzylguanidine.

PURPOSE: We previously proposed that a probability-based sympathetic 123I-meta-iodobenzylguanidine (mIBG) index (SMILe) could distinguish the presence or absence of Lewy body disease (LBD) based on findings at a single center. However, whether the model would be useful in the real world remained uncertain. Therefore, we updated and evaluated its performance at five Japanese and three European institutions. METHODS: We compared data from 967 patients with suspected LBD with 62 controls from a normal database (NDB). Of 815 patients with guideline-based diagnoses, 483 had LBD (Parkinson disease

A rare primary mediastinal mesenchymal tumor diagnosed by endobronchial ultrasound (EBUS).

Primary mesenchymal tumors of the mediastinum are extremely rare and often pose considerable diagnostic challenges owing to their histological diversity and anatomical location. We report the case of a 51-year-old man with a 45 pack-year smoking history who was referred for evaluation of a mediastinal mass initially detected by chest radiography. Thoracic computed tomography (CT) and positron emission tomography-CT revealed a 3 × 3 cm lesion extending from the right upper and lower paratracheal regions into the right upper lobe parenchyma, demonstrating intense FDG uptake.
