Primary hyperparathyroidism is most commonly caused by a single adenoma, with multiglandular disease accounting for a smaller proportion of cases. A recurrent hyperparathyroidism decades after prior parathyroidectomy is uncommon and often presents with nonspecific multisystem symptoms that delay diagnosis. Ectopic parathyroid tissue, particularly in intrathymic or retrosternal locations, represents a challenging but important cause of persistent or recurrent disease. A 43‑year‑old woman with a history of three‑gland parathyroidectomy 20 years earlier presented with a year of progressive fatigu

Peritoneal dialysis (PD)-related leaks are usually pleuroperitoneal; mediastinal extension is exceptionally rare. A 38-year-old man with ESKD due to immune complex-mediated focal segmental glomerulosclerosis on long-term automated PD presented with acute exertional chest pain mimicking an acute coronary syndrome. Cardiac biomarkers were elevated, but electrocardiography and coronary angiography showed no ischemic disease. Computed tomography pulmonary angiography excluded pulmonary embolism and revealed a paraesophageal fluid collection. Contrast-enhanced computed tomography after intraperiton

BACKGROUND: Guidelines recommend invasive mediastinal nodal staging (IMNS) for patients with synchronous primary lung cancer (SPLC) who are candidates for curative resection. We investigated the prevalence of occult nodal disease and selective use of IMNS in patients with SPLC.
METHODS: Patients with SPLC suspected on preoperative imaging who underwent resection from 2000 to 2022 were included. Exclusion criteria included metachronous and recurrent lung cancer, cStage III-IV, carcinoid histology, absence of preoperative PET/CT, and receipt of neoadjuvant therapy. Mediastinal masses with pleural effusion in dogs typically raise suspicion for thymoma or mediastinal lymphoma, necessitating careful differentiation. This paper describes a 12-year-old neutered male dog presenting with acute-onset coughing. Radiographic imaging revealed mild pleural effusion with an expanded cranial mediastinum. Subsequent ultrasonographic and computed tomographic examinations identified multiple masses extending from the cervical region to the cranial mediastinum. Cytologic analysis of the pleural fluid revealed findings suggestive of a salivary origin, with mild inflammator

BACKGROUND: Castleman disease (CD) is a rare lymphoproliferative disorder that commonly involves the abdomen, head and neck, and mediastinum. Primary involvement of the pleura is rare and prone to misdiagnosis. CASE PRESENTATION: We report the case of an asymptomatic 58-year-old male presenting with an incidental left pleural mass detected during a physical examination. While imaging studies initially suggested a solitary fibrous tumor or vascular neoplasm, definitive diagnosis of pleural hyaline vascular unicentric Castleman disease (UCD) was confirmed via pathological and immunohistochemical

Robotic-assisted thymectomy has become a well-established treatment option for acetylcholine receptor-positive myasthenia gravis. This review provides a synthesis of current evidence regarding its technical evolution, surgical outcomes, neurological efficacy, and future directions. Robotic thymectomy, particularly using platforms such as the da Vinci system, offers high-definition three-dimensional visualization, tremor filtration, and articulating instruments that enable precise dissection within the confined space of the mediastinum. Several surgical approaches are available, including unila

Over the past several decades, the management of nonmetastatic nonsmall cell lung cancer (NSCLC) has centered on identifying patients eligible for upfront surgical treatment. This selection has traditionally relied on multidisciplinary assessments of clinical operability and oncologic resectability, with the latter depending primarily on mediastinal lymph node evaluation, a key prognostic factor in determining whether patients should be directed toward upfront surgery or definitive chemoradiotherapy. The recent incorporation of immune checkpoint inhibitors (ICIs) into standard neoadjuvant ther

OBJECTIVE: The thyroid gland receives sympathetic innervation. 123I-meta-iodobenzylguanidine (123I-MIBG) is a noradrenaline analog taken up by adrenergic nerve terminals. Although thyroid uptake can be detected on 123I-MIBG scintigraphy, its quantitative value is poorly understood. This study examined the clinical relevance of thyroid 123I-MIBG uptake in Parkinson's disease (PD).
METHODS: A total of 233 de novo patients were enrolled in a longitudinal cohort and underwent 123I-MIBG scintigraphy. Early and late heart-to-mediastinum ratios (HMRs) and early thyroid-to-mediastinum ratios (TMRs) w

Fibrosing mediastinitis is a rare disease caused by benign proliferation of fibrous tissue within the mediastinum encircling the mediastinal organs, leading to compression of the surrounding bronchial and blood vessels. We report a case of fibrosing mediastinitis with rapidly progressive pulmonary vein stenosis evaluated using multimodality imaging. Computed tomographic pulmonary angiography demonstrated multiple pulmonary vein stenoses, while intravascular optical coherence tomography provided high-resolution characterization of the lesions, showing smooth vessel walls without evidence of thr

BACKGROUND: While EBUS-TBNA is the first-line modality for sampling mediastinal lymphadenopathy, its diagnostic yield is variable, and sample often inadequate for advanced molecular testing. EBUS-guided mediastinal cryobiopsy (EBUS-MCB) enables acquisition of larger, intact tissue specimens. This study assesses the diagnostic yield and safety of EBUS-MCB in cases with inconclusive smear, or atypical cells on rapid on-site evaluation (ROSE).
METHODS: We retrospectively analysed patients who underwent EBUS-MCB between March 2022 and August 2025. EBUS-MCB was performed in cases with a non-diagno

A 67-year-old woman with coexisting papillary thyroid carcinoma (PTC) and anaplastic thyroid carcinoma (ATC) harbored an ALK rearrangement on FISH. Despite surgery and 2 courses of 131 I treatments, 18 F-FDG PET/CT showed an intensely avid right lower-neck mass extending into the superior mediastinum. The patient was considered radioiodine-refractory (RAIR) and started ensartinib (225 mg once daily). CT at 3 months showed marked regression, with sustained disease control at 12 months. This case illustrates an objective imaging response to ALK-directed therapy in RAIR thyroid carcinoma.

INTRODUCTION: The World Health Organization classification of tumors of hematopoietic and lymphoid tissues recognizes acute erythroid leukemia as a distinct entity under myeloid neoplasms. Erythroblastic sarcoma, also known as extramedullary erythroid sarcoma, is defined as a rare extramedullary tumor composed of erythroid precursors, typically occurring outside the bone marrow. While both entities involve immature erythroid precursors and share similar histology, acute erythroid leukemia is a systemic marrow-based leukemia, whereas erythroblastic sarcoma is a localized mass-forming lesion. BACKGROUND: The ninth TNM edition distinguishes single-station (N2a) from multistation (N2b) disease, increasing concern for cross-contamination between N2 lymph nodes during EBUS staging when ROSE is unavailable. The absolute probability and clinical impact of this phenomenon are uncertain. We estimated a theoretical upper bound on false upstaging and the number of needle changes required to prevent one false upstage.
METHODS: A Monte Carlo decision model simulated mediastinal staging in 100,000 virtual patients across published estimates of occult N2 prevalence, probability of a single-stat

Carcinoma of the rete testis is an extremely rare neoplasm, which has not been previously reported in cattle. Therefore, the aim of this report is to describe a case of carcinoma of the rete testis in a bull, including clinical, ultrasonographic, histopathological, and immunohistochemical findings. An eight-year-old Nellore bull was referred for evaluation following a history of hemospermia. In the ultrasonographic evaluation, dilation of the left mediastinum testis was observed, as well as the presence of anechoic cystic structures. Unilateral orchiectomy was performed on the affected testis,

OBJECTIVE: Lung cancer screening (LCS) is complicated by a high prevalence of incidental findings (IFs), defined as abnormalities detected on low-dose computed tomography (LDCT) of the chest that are outside the scope of LCS. Although the detection of some abnormalities might lead to the diagnosis of conditions of clinical relevance, LDCT protocols are not adequate for the characterisation of most IFs and thus, further investigations are usually necessary. Thus, the detection of such findings is perceived as a double-edged sword, and radiologists still widely disagree on which IFs should be re

Epcoritamab, a subcutaneous CD3×CD20 bispecific antibody, has shown high efficacy in relapsed or refractory large B-cell lymphoma (R/R LBCL). However, treatment options are limited for patients who fail to achieve durable remission after immune-based therapy. Here, we describe a case of refractory diffuse large B-cell lymphoma in an 87-year-old woman who achieved long-term remission through concurrent epcoritamab and radiotherapy. During the fifth cycle of third-line epcoritamab therapy, positron emission tomography/computed tomography revealed persistent fluorodeoxyglucose-avid lesions in the
