A real-world cohort study comparing chemotherapy and surgery in the treatment of stage IVA thymomas with pleural dissemination.

BACKGROUND: Chemotherapy is the guideline-recommended first-line therapy for stage IVA thymomas (TMs) with pleural dissemination, but its efficacy has never been specifically assessed. We aim to evaluate the efficacy of chemotherapy, and to compare the results of surgery and chemotherapy without surgery in these patients. METHODS: This is a real-world retrospective study. Consecutive patients with stage IVA TM with pleural dissemination who received chemotherapy or upfront surgery between 2007 and 2018 were enrolled. Propensity-score matching (PSM) was used to balance selection bias between g

Pathobiology of thymic epithelial tumours and the treatment strategy based on immuno-oncological characteristics: a narrative review.

BACKGROUND AND OBJECTIVE: Thymic epithelial tumours (TETs), including thymomas and thymic carcinomas, are relatively rare anterior mediastinal malignancies. Thymomas are well known to be associated with various autoimmune diseases, such as myasthenia gravis (MG), pure red cell aplasia, and hypogammaglobulinemia. The mainstay of treatment for all TETs is complete surgical resection, although recent advances in irradiation and anti-tumour drug therapies are increasingly indicated for advanced or recurrent disease. The pathobiology of TETs was reviewed to consider optimal treatment strategies bas

Cryoablation as a downsizing strategy enabling R0 resection in a giant mediastinal desmoid tumor: a case report.

BACKGROUND: Endothoracic desmoid tumors are rare, locally aggressive soft-tissue neoplasms whose intrathoracic location often precludes safe surgical resection. The historical paradigm of upfront radical surgery has shifted toward a more conservative and stepwise approach, prioritizing active surveillance and non-surgical treatments whenever feasible. Surgical resection is now typically reserved for selected cases. Percutaneous cryoablation (PCA) has emerged as a promising minimally invasive treatment and valid therapeutic alternative for selected patients, although its role as a tumor downsiz

Tracheal Squamous Cell Carcinoma in an Adult Cat.

A 7-year-old, neutered male domestic shorthair cat presented to a tertiary referral hospital for respiratory distress. Thoracic radiographs revealed severe tracheal stenosis, a cranial mediastinal mass, pleural effusion, and a perihilar bronchial pulmonary pattern. Neoplasia involving the trachea and cranial mediastinum was prioritized. Additional antemortem diagnostics were not pursued. With few reported treatment options, the owners elected euthanasia, and necropsy confirmed tracheal squamous cell carcinoma. Tracheal squamous cell carcinoma is a rare feline neoplasm associated with a poor pr

Diagnostic Reclassification From Parkinson's Disease to Multiple System Atrophy Based on Longitudinal Clinical and Imaging Findings: A Case Report.

It is often challenging to differentiate Parkinson's disease (PD) from multiple system atrophy (MSA) - particularly, the parkinsonian subtype (MSA-P) - in the early disease stages because of overlapping clinical features. Here, we report the case of a 72-year-old man who was initially diagnosed with PD but whose diagnosis was later changed to MSA-P following the emergence of autonomic dysfunction and characteristic magnetic resonance imaging (MRI) findings. He presented with tremors, rigidity, bradykinesia, and postural instability, and dopamine transporter imaging demonstrated bilateral presy

Mediastinal tumors.

Mediastinal tumors represent a wide spectrum of epithelial, mesenchymal, neuroectodermal, embryonal, germ cell, and mixed neoplasms. Nevertheless, their incidence is low, accounting for approximately 1% of all neoplasms, and clinical presentation is often nonspecific usually reflecting advanced-stage disease. Primary mediastinal tumors must be distinguished from secondary tumors and so-called pseudotumors. From the perspective of their classification, diagnostics, and therapy, the anatomical--topographical division of the mediastinum into compartments is essential, as it may facilitate identif

Widely Invasive Follicular Thyroid Carcinoma With Extensive Nodal Metastases and Subsequent Atypical Locoregional Recurrences: A Case Report.

Follicular thyroid carcinoma (FTC) is a well-recognized thyroid malignancy classically known for hematogenous spread, making extensive lymph node involvement at diagnosis an atypical finding. Here, we present the case of a 49-year-old man with widely invasive FTC presenting with an extraordinary burden of metastatic cervical lymph nodes at diagnosis. He subsequently developed two sequential locoregional recurrences at atypical locations, the retropharyngeal space and the superior mediastinum, and ultimately achieved no evidence of disease. This case highlights the potential for widely invasive

Local Control by Mediastinal Lymph-Node Dissection in Stage I Non-Small-Cell Lung Cancer.

OBJECTIVES: Mediastinal lymph-node dissection (MLND) is recommended for resectable non-small-cell lung cancer (NSCLC); however, its prognostic impact remains controversial. We evaluated whether MLND improves prognosis in patients with radiologically solid-dominant clinical stage I NSCLC. METHODS: We retrospectively reviewed 2212 patients who underwent lobectomy for solid-dominant clinical stage I NSCLC between 2010 and 2022. Prognosis and local control were compared between the MLND and non-MLND groups. RESULTS: The MLND and non-MLND groups included 1912 and 300 patients, respectively. After

Mediastinal Extrapleural Air Mimicking a Bulla in a Non-Expandable Lung.

In a patient with pleuroparenchymal fibroelastosis and a non-expandable lung, CT and three-dimensional rendering showed a bulla-like mediastinal air pocket that regressed on follow-up imaging and lacked an autopsy correlate, supporting intrathoracic extrapleural air rather than a true pulmonary bulla.

When Asthma Leads to Air: Pneumomediastinum in a Child.

Background: Pneumomediastinum is a clinical condition characterized by the presence of free air within the mediastinum. Methods: We report the case of a 12-year-old boy with recurrent wheezing who developed pneumomediastinum associated with pneumorrhachis during a Rhinovirus-triggered asthma exacerbation, in the context of a family history of spontaneous pneumothorax, alongside a literature review covering the past ten years. Results: Laboratory tests were unremarkable, except for neutrophilia and a positive Rhinovirus test. Chest X-ray revealed a large pneumomediastinum, extending into cervic

The Three-Y and Trachea View (3Y + T): A Conceptual Extension of the Three-Vessel and Trachea View for Comprehensive Fetal Echocardiographic Assessment.

Introduction: Congenital heart disease remains the most common congenital anomaly and a leading cause of infant morbidity and mortality. Although the three-vessel and trachea view has substantially improved prenatal assessment of the great arteries and aortic arch anatomy, detailed evaluation of the distal arch, aortic isthmus, and arch convergence may remain challenging in routine fetal echocardiography. Methods: A structured literature review of PubMed-indexed studies was conducted to examine the anatomical basis, clinical applications, diagnostic performance, and technological developments

Thoracic SMARCA4-Deficient Undifferentiated Tumor Presenting as a Giant Mediastinal Mass: A Case Report.

BACKGROUND Thoracic SWI/SNF-related matrix-associated actin-dependent regulator of chromatin subfamily A member 4 (SMARCA4)-deficient undifferentiated tumor (SMARCA4-UT) is a rare and highly aggressive thoracic malignancy. It predominantly affects male smokers and typically arises in the mediastinum, where rapid tumor growth and early metastasis contribute to a poor prognosis. Recent evidence suggests that this tumor exhibits biological heterogeneity and variable responses to therapy, underscoring the need for further clinical characterization. CASE REPORT A 67-year-old man presented with dysp

Allogeneic hematopoietic stem cell transplantation achieves complete response in refractory primary mediastinal large B-cell lymphoma after the failure of chemotherapy, CAR-T therapy, and PD-1 blockade.

Primary mediastinal large B-cell lymphoma (PMBCL) typically responds to chemotherapy, and immunotherapies such as chimeric antigen receptor T-cell (CAR-T) therapy and the programmed cell death protein 1 (PD-1) inhibitor pembrolizumab have shown promise in relapsed or refractory cases. However, the sequential use of multiple immunotherapies for PMBCL remains poorly characterized. Here, we report a case of a 19-year-old female with PMBCL involving the mediastinum and multiple extranodal sites. After a partial response to chemotherapy, she developed a central nervous system relapse that responded

[Extrapulmonary lymphangioleiomyomatosis with retroperitoneal lymph node involvement].

Lymphangioleiomyomatosis (LAM) is a rare disease of unknown etiology that occurs almost exclusively in women, primarily of reproductive age. This disease is characterized by smooth muscle cell proliferation, most commonly in the lungs. However, cases of extrapulmonary LAM have been reported, such as in the lymph nodes of mediastinum, abdominal cavity, and retroperitoneum. The clinical manifestations of pelvic lymph node LAM are subtle manifesting as symptoms of compression due to a large tumor mass but more often diagnosed incidentally in lymph node specimens removed during gynecologic oncolog

Four-Point Support Frame Positioning for Obese Patients Undergoing Minimally Invasive Esophagectomy. A Preliminary Institutional Experience.

Prone minimally invasive esophagectomy offers several technical advantages; however, in obese patients, increased abdominal pressure may contribute to diaphragmatic elevation, leading to difficulty in first-port placement and compromised operability in the lower mediastinum. These technical challenges can be particularly relevant in robot-assisted procedures, where adequate working space and sufficient inter-arm distance are required. We describe a positioning modification using a 4-point support frame in obese patients undergoing prone minimally invasive esophagectomy. By supporting the thora

Classic Hodgkin Lymphoma: A Review.

IMPORTANCE: Hodgkin lymphoma (HL) is a lymphoid malignancy derived from B cells. Globally, there were 82 409 new cases of HL in 2022, associated with 22 701 deaths. In the US, 8720 incident cases of HL and 1150 deaths due to HL were reported in 2025. Classic HL (cHL) accounts for 90% to 95% of HL cases. OBSERVATIONS: Classic HL has a bimodal age distribution, with an initial peak at age 15 to 35 years and a second, smaller peak at approximately age 70 years. The median age at cHL diagnosis is 33 years. Most patients present with supradiaphragmatic lymphadenopathy (cervical, mediastinal, supra

Bridging the gap: intraoperative esophageal stent used as a scaffold for the treatment of complete postsurgical gastroesophageal anastomosis dehiscence.

BACKGROUND AND AIMS: Esophagogastric (EG) anastomotic leaks are serious adverse events after esophagectomy. Endoscopic stent placement has become an effective, minimally invasive option that promotes healing. While esophageal stents maintain luminal continuity, technical success rates are as high as 90.9%. A 62-year-old man with esophageal adenocarcinoma presented 12 days after McKeown esophagectomy with dyspnea and empyema. Imaging and chest tube output suggested an anastomotic leak. The aim of this study was to display the utility of esophageal stent placement even in cases of complete EG de

Recognizing Descending Necrotizing Mediastinitis in Adults: A Narrative Review of Diagnostic Dilemmas and Barriers.

Unrecognized, untreated, or treatment-resistant deep neck infections (DNIs), commonly originating from oropharyngeal or odontogenic sources, can rapidly lead to life-threatening complications such as airway obstruction, systemic sepsis, and descending necrotizing mediastinitis (DNM). Severe infections may spread downward from the head and neck through and along fascial planes into the mediastinum. This process continues to be associated with high mortality rates and severe associated morbidity. Early diagnosis remains particularly difficult and challenging, and delayed recognition is linked to

Endobronchial Ultrasound-Guided Fine Needle Biopsy in the Diagnosis of Mediastinal Lymphadenopathy: A Randomized Trial.

BACKGROUND: The comparative sampling efficacy of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) needles in mediastinal lymphadenopathy remains unclear. Would the fine needle biopsy (FNB), with its Franseen design, provide better results than the fine needle aspiration (FNA) during EBUS-TBNA? METHODS: We performed a single-center, investigator-initiated, single-blinded, randomized, parallel-group, superiority trial between August 2022 and July 2023. Consecutive patients were randomly assigned (1:1) to undergo EBUS-TBNA with either 19-gauge (G) FNA or 22-G FNB. The

Myxoid Pleomorphic Liposarcoma: A Clinicopathological and Cytogenomic Study of 24 Cases.

Myxoid pleomorphic liposarcoma (MPLPS), a rare, aggressive liposarcoma subtype, is thought to occur chiefly in adolescents with a mediastinal predilection. MPLPS lack DDIT3 rearrangements or MDM2 amplification; a small number have recently been shown to harbor widespread copy-neutral loss of heterozygosity (cnLOH). We studied a large series of MPLPS and compared them with other sarcomas, particularly other liposarcomas. Available slides/blocks for cases coded as "MPLPS" were retrieved (2008-2026). We also searched our single-nucleotide polymorphism assay records for tumors demonstrating (1) hi
