Handheld lung ultrasound for assessing cystic fibrosis lung disease in children: a diaphragm-ıntegrated scoring approach.

UNLABELLED: Lung ultrasound (LUS) is a radiation-free imaging modality with potential value for monitoring cystic fibrosis (CF) lung disease. However, data on handheld LUS performed by pediatric pulmonologists, as well as the integration of diaphragmatic assessment into LUS scoring, remain limited. This prospective cross-sectional study included clinically stable children with CF (cwCF). LUS was performed independently by a pediatric radiologist using a conventional ultrasound device and by a pediatric pulmonologist using a handheld device. Three LUS scores were evaluated: radiologist-assessed

AMAP-APP: Efficient Segmentation and Morphometry Quantification of Fluorescent Microscopy Images of Podocytes.

BACKGROUND: Automated quantification of podocyte foot process morphology is essential for unbiased, high-throughput analysis in kidney disease research. The previously established "Automatic Morphological Analysis of Podocytes" (AMAP) method allows for such analysis but is limited by high computational demands requiring high-performance computing systems, the lack of a user interface, and restriction to Linux operating systems. This study aimed to develop AMAP-APP, an optimized, cross-platform desktop application designed to overcome these hardware and usability barriers while maintaining anal

Delayed diagnosis of selected thoracic and mediastinal conditions in young adults: a focused narrative review of clinical pitfalls, cognitive biases, and system barriers.

BACKGROUND: Thoracic and mediastinal conditions affecting the esophagus, diaphragm, mediastinum, and airway carry considerable morbidity when diagnosis is delayed. Young adults are often perceived as low risk for serious thoracic disease, and low clinical suspicion, atypical symptoms, and cognitive and healthcare-system barriers can delay recognition. METHODS: This focused narrative review searched PubMed and Scopus, supplemented by Google Scholar, for studies published between January 2000 and May 2026, with selected earlier landmark studies retained where essential. Search terms included "t

AAVcc47-μDys Gene Therapy Ameliorates Respiratory Pathology in mdx Mouse Model of Duchenne Muscular Dystrophy.

Duchenne muscular dystrophy (DMD) is a progressive X-linked neuromuscular disorder caused by deficiency of dystrophin - a protein essential for muscle integrity. Loss of dystrophin results in global muscle weakness, including in the muscles of respiration. As a result, respiratory failure is a leading cause of morbidity and mortality in patients with DMD. Gene therapy using adeno-associated virus (AAV) is ideal for monogenetic diseases. However, the dystrophin gene is too large to be packaged into AAV; therefore, microdystrophin (μDys) containing important elements of the dystrophin gene is us

ACR Appropriateness Criteria® Suspected Diaphragm Dysfunction.

Diaphragm dysfunction includes diaphragmatic eventration, weakness, and paralysis, a set of disorders which are often challenging to diagnose clinically due to their varied presenting symptoms including dyspnea, positional orthopnea, cough, or even chest pain. Given the clinical challenge in diagnosing these conditions, diaphragm dysfunction is often suggested incidentally on initial imaging for workup of these symptoms. Imaging can play an important role both in raising the initial suspicion for diaphragm dysfunction as well as in making the diagnosis through real-time evaluation of the diaph

Chemometrics-assisted FTIR and machine learning for biochemical profiling of human skeletal muscles in relation to type 2 diabetes.

Skeletal muscle is the major site of insulin-stimulated glucose uptake, making it highly susceptible to metabolic disorders. This study investigates Type 2 diabetes mellitus-associated alterations in the macromolecular composition of functionally distinct human skeletal muscles. These modifications are crucial to understanding the molecular basis of diabetic myopathies but remain underexplored. Five skeletal muscles from 80 individuals (40 controls and 40 individuals with Type 2 diabetes) were analysed. To overcome the limitations of traditional histochemical assays, we employed infrared spect

From molecular correction to functional rescue: delivery, tissue state, and immunobiology as the principal constraints on dystrophin-restoring therapy in Duchenne muscular dystrophy.

Duchenne muscular dystrophy (DMD) arises from thousands of distinct mutations at a single X-linked locus that converge on loss of functional dystrophin, and this convergence on one molecular endpoint-rather than any genetic uniformity-has long made the disease appear a candidate for straightforward molecular correction. Three decades of work indicate that the difficulty lies less in restoring dystrophin than in restoring it adequately along four axes: amount, retained domains, breadth of coverage across skeletal muscle, diaphragm, heart and brain-the constraint being extent of coverage rather

Carbon dioxide as a promising alternative to iodinated contrast media for lower extremity endovascular procedures (Review).

Carbon dioxide (CO2) angiography has emerged as an alternative contrast agent for arterial endovascular procedures, particularly in patients at increased risk of complications from iodinated contrast media (ICM). The present review evaluates the clinical circumstances in which carbon dioxide (CO2) angiography can reduce the use of iodinated contrast media (ICM) during arterial endovascular procedures, with particular attention to renal-risk reduction, image quality, anatomical limitations, and procedure-related complications. A narrative literature review was performed using PubMed, EMBASE, th

Emerging genetic mechanisms of glomerular proteinuria: a podocyte-centered multi-layer network perspective.

Podocyte injury is widely recognized as a central driver of glomerular proteinuria and progression to chronic kidney disease. Although proteinuria can originate from multiple components of the glomerular filtration barrier, genetic and experimental studies increasingly implicate podocyte dysfunction as a dominant and convergent mechanism in many disease contexts. In recent years, the application of next-generation sequencing has broadened the spectrum of genes linked to proteinuric kidney disorders. These genes are no longer confined to structural elements of the slit diaphragm or cytoskeleton

Dosimetric comparison of organs at risk exposure in multicatheter high-dose-rate interstitial brachytherapy vs. VMAT-based APBI for real patient treatment plans.

PURPOSE: To dosimetrically compare multicatheter interstitial high-dose-rate brachytherapy (MIBT) with two volumetric modulated arc therapy (VMAT) techniques, i.e., continuous (cVMAT) and tangential (tVMAT), for accelerated partial breast irradiation (APBI), specifically focusing on organs at risk (OARs) exposure. MATERIAL AND METHODS: Treatment plans of 50 early-stage breast cancer patients treated with interstitial multicatheter HDR brachytherapy (MIBT) were analyzed. Using identical CT datasets with catheters in situ and standardized contours, MIBT, cVMAT, and tVMAT plans were generated fo

An Uncommon Cause of Dyspnea: Parsonage-Turner Syndrome With Bilateral Phrenic Nerve Involvement.

Parsonage-Turner syndrome, also called neuralgic amyotrophy, typically affects the brachial plexus, while involvement of the phrenic nerve is far less commonly documented. When present, it can compromise diaphragmatic function, producing breathlessness with activity, orthopnea, or, in more advanced presentations, respiratory failure. These features are easily mistaken for other cardiac or pulmonary conditions, which often delays correct diagnosis. A 51-year-old woman presented with a five-month course of worsening breathlessness on exertion and inability to breathe comfortably while supine. Ca

Determinants of contraceptive discontinuation among sexually active women in Nigeria: a systematic review.

INTRODUCTION: Contraceptive use is critical for improving reproductive health and reducing unintended pregnancies, yet many women discontinue its use despite the need for it. In Nigeria, the total fertility rate remains high at 4.8 children per woman, while modern contraceptive prevalence among women of reproductive age is only 17-20%, highlighting persistent gaps in family planning. This systematic review aimed to synthesize evidence on the prevalence, patterns, and determinants of contraceptive discontinuation among Nigerian women. METHODS: A systematic review was conducted focusing on stud

Isolation of Immune Cells from Skeletal Muscles for Flow Cytometry.

Inflammation is a characteristic of skeletal muscle wound healing and of many muscle diseases. Analysis and isolation of the immune cell populations infiltrating skeletal muscles and their molecular signatures allow optimal use of anti-inflammatory treatments and identification of novel therapeutic targets. Here we describe a protocol for unbiased isolation of skeletal muscle immune cells. This technique allows for downstream assessment of immune cell populations or their isolation for downstream molecular or cellular assays from individual skeletal muscles during normal wound healing or disea

Assessment of Contractile and Kinetic Properties of Skeletal and Cardiac Multicellular Preparations in Mouse Models: A Comprehensive Methodological Guide.

The mechanical functions of skeletal and cardiac muscles are fundamentally defined by their contractile and passive properties, reflecting their unique physiological roles and operational dynamics. Contractile properties, which include the generation of force during muscle contraction, the duration required to reach peak force, and the subsequent relaxation period, are pivotal in assessing muscle performance, strength, injury, fatigue resistance, and overall health. These properties are crucial for understanding muscle function across various physiological and pathological conditions, includin

Effects of Body Position on Respiratory Pressure and Muscle Activity in Amyotrophic Lateral Sclerosis and Healthy Subjects.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a fatal heterogeneous neurodegenerative disease characterized by the degeneration of both upper and lower motor neurons and spinal cord. Measurement of respiratory muscle strength has been shown to be an important assessment in the decision-making process and can be assessed by maximum inspiratory (PImax) and expiratory pressures (PEmax), sniff nasal inspiratory (SNIP) and expiratory (SNEP) pressures. Body position appears to have a significant effect on respiratory muscle strength. The aim of this study was to observe the difference in peak v
