The local efficacy and influencing factors of ultrasound-guided percutaneous radiofrequency ablation in sub-cardiac hepatic tumor: A 7-year experience at a single center.

PURPOSE: To investigate the effectiveness and safety of ultrasound (US)-guided percutaneous radiofrequency ablation (PRFA) for sub-cardiac hepatic tumors and evaluate the factors influencing local tumor progression (LTP). METHODS: Between January 2016 and January 2023, we included 118 patients (118 lesions) of US-guided RFA for sub-cardiac hepatic tumors (located in S2 or S4a, with a gap between the tumor and the left diaphragm ≤ 10 mm). We retrospectively analyzed patients' electronic medical records and imaging follow-up data to evaluate the success rate, LTP, and complications. We also use

Diaphragm Ultrasound-Guided PEEP Titration in COPD with Dynamic Hyperinflation.

Chronic obstructive pulmonary disease (COPD) is characterized by airflow limitation, dynamic hyperinflation, and intrinsic positive end-expiratory pressure (iPEEP), which increase inspiratory threshold load and complicate ventilatory support. Non-invasive ventilation (NIV) is the preferred initial ventilatory support for COPD exacerbations complicated by acute or acute-on-chronic respiratory acidosis, while invasive mechanical ventilation (IMV) may be required when NIV fails. External PEEP may reduce the inspiratory threshold load in patients with expiratory flow limitation, although excessive

The Most Recent European and American Guidelines on Cardiac Rehabilitation: A Lack of Focus on Diaphragm Training.

Cardiovascular diseases (CVDs) are the leading causes of death and morbidity worldwide. Patients with vascular and cardiac conditions undergo cardiac rehabilitation (CR), a process that may begin before surgery (less frequently), after invasive procedures, or as part of chronic disease management, with durations varying by healthcare facilities or national policies. CR is a valuable non-pharmacological intervention that supports patient health and the resumption of daily activities. These rehabilitation programmes are strongly recommended for cardiac patients and are structured according to gu

Prolonged fever with a distinct rectangular rash: What is going on?

We present the case of an older adult man with prolonged fever of unknown origin and a persistent rectangular erythematous plaque, highlighting the diagnostic value of careful skin examination in patients with systemic inflammatory symptoms. This 70-year-old man with adult-onset endogenous eczema, hypertension, stage II upper rectal carcinoma following resection and adjuvant radiotherapy and chemotherapy and left hip avascular necrosis presented with fever persisting for 1 month following an upper respiratory tract infection. He developed generalised arthralgia and bilateral lower limb oedema,

Fully automated diaphragm segmentation on routine 2D-cine and 3D thoracic MRI: multireader validation.

OBJECTIVE: The diaphragm is central to respiratory mechanics and may be impaired in various diseases. Parametric magnetic resonance imaging (MRI) assessment is limited by time-consuming manual segmentation. We developed and validated fully automated diaphragm segmentation on two-dimensional (2D)-cine and three-dimensional (3D) thoracic MRI. MATERIALS AND METHODS: In this single-center study, eligible adults without known diaphragmatic disorders underwent 2D-cine MRI, 3D MRI, or both. Two U-Net models (2D/3D) were trained using expert manual contours as ground truth. Performance was evaluated

Development and Internal Validation of a Prediction Model Incorporating Respiratory Muscle Function for CAT Score Deterioration in Stable COPD.

OBJECTIVE: The objectives are to investigate the association between respiratory muscle function and CAT score deterioration in stable chronic obstructive pulmonary disease (COPD) and to develop an internally validated prediction model. METHODS: This single-center retrospective cohort study included 203 patients with stable COPD. Clinically significant CAT score deterioration was defined as an increase of ≥ 2 points from baseline within 12 months. Predictors were selected using LASSO Cox regression and incorporated into a multivariable Cox model. Model performance and the incremental value of

TLR4 signaling cascades leading to increases in albumin permeability, nephrin ectodomain shedding, and apoptosis in MPC-5 podocytes.

Proteinuria is a cardinal symptom of kidney disease, but common transient proteinurias may reflect a normal physiological response to distant tissue damage or infection. Here we observed that activation of Toll-like receptor-4 (TLR4) using the selective agonist CRX572 induced three distinct functional effects in cultured podocytes. TLR4 activation evoked a reversible increase in albumin flux (PAlb) across a confluent layer of podocytes in 60 m or less. At 24 h matrix metalloproteinase-dependent shedding of nephrin ectodomains was detected. Finally, apoptosis occurred after 72 h. These effects

Modeling patient variants of Cnot1 and Cdc42bpb results in distinct forms of congenital diaphragmatic hernia in mice.

Congenital diaphragmatic hernia (CDH) is a severe congenital anomaly characterized by impairment of both diaphragm and lung development in utero. CDH presents as a spectrum of forms and severities, with diaphragm defects arising in the dorsal/posterior region typically correlating with more severe pulmonary disease and higher risk of mortality than those appearing in ventral/anterior regions. The genetic etiology underlying CDH is complex, with many implicated genes showing variable expressivity and incomplete penetrance in both human patients and mouse models. Here we present in vivo validati

Evaluation of Outcome of Laparoscopic Nissen Fundoplication with or without Fundophrenicopexia in Surgical Management of Hiatal Hernia.

BACKGROUND: Gastroesophageal reflux disease (GERD) and hiatal hernia frequently coexist, impairing quality of life and often requiring surgical intervention. Laparoscopic Nissen fundoplication (LNF) is the gold standard, but recurrence, dysphagia, and gas-bloat remain concerns. FundophrenicopexiaFundophrenicopexia, fixation of the gastric fundus to the diaphragm, has been proposed to reinforce hiatal closure and reduce recurrence. This study prospectively compared LNF with and without fundophrenicopexia. METHODS: A prospective comparative study was conducted at Suez Canal University Hospitals

Video-assisted thoracoscopic diaphragmatic plication in a ventilator-dependent patient with an elevated and dysfunctional hemidiaphragm.

Unilateral hemidiaphragm dysfunction can reduce ventilatory reserve, but its contribution to weaning failure is difficult to isolate in a critically ill patient. A 61-year-old woman with asthma and a chronically elevated left hemidiaphragm developed severe pneumonia and acute hypercapnic respiratory failure, with an arterial partial pressure of carbon dioxide (PaCO2) of 164.9 millimeters of mercury (mmHg), followed by recurrent extubation failure and tracheostomy. Pulmonary embolism was excluded. Bronchoscopy, microbiological treatment, nutritional support, and treatment for suspected autoimmu

Extraction of atrial tumor thrombus in renal cell carcinoma via a complete transabdominal approach.

Surgical resection is the mainstay treatment of renal cell carcinoma (RCC) offering a 5-year survival rate of 70-80%. Associated vena cava tumor thrombus adds complexity to surgical planning especially when the thrombus extends above the diaphragm. Those who do not undergo treatment of the primary cancer as well as the associated thrombus have poor mortality outcomes. Cardiopulmonary bypass (CPB) has been the mainstay for level IV thrombectomy since first introduced. While it has advantages, it is also associated with multisystem failure, hypothermia, and coagulopathy. We sought to describe ou

Anaplastic lymphoma kinase-positive large B-cell lymphoma presenting as a submucosal tumor-like lesion at the esophagogastric junction: a diagnostic pitfall.

Anaplastic lymphoma kinase (ALK)-positive large B-cell lymphoma (LBCL) is a rare, aggressive variant of diffuse LBCL with an immunoblastic/plasmablastic morphology, a plasmacytic immunophenotype with loss of conventional B-cell antigens, and granular cytoplasmic ALK expression. Involvement of the gastrointestinal tract has been documented only in the stomach, duodenum, and ileum. To the best of our knowledge, we report the first case presenting at the esophagogastric junction. A 71-year-old immunocompetent man with Parkinson's disease was referred for progressive anemia that had been detected

Handheld lung ultrasound for assessing cystic fibrosis lung disease in children: a diaphragm-ıntegrated scoring approach.

UNLABELLED: Lung ultrasound (LUS) is a radiation-free imaging modality with potential value for monitoring cystic fibrosis (CF) lung disease. However, data on handheld LUS performed by pediatric pulmonologists, as well as the integration of diaphragmatic assessment into LUS scoring, remain limited. This prospective cross-sectional study included clinically stable children with CF (cwCF). LUS was performed independently by a pediatric radiologist using a conventional ultrasound device and by a pediatric pulmonologist using a handheld device. Three LUS scores were evaluated: radiologist-assessed

AMAP-APP: Efficient Segmentation and Morphometry Quantification of Fluorescent Microscopy Images of Podocytes.

BACKGROUND: Automated quantification of podocyte foot process morphology is essential for unbiased, high-throughput analysis in kidney disease research. The previously established "Automatic Morphological Analysis of Podocytes" (AMAP) method allows for such analysis but is limited by high computational demands requiring high-performance computing systems, the lack of a user interface, and restriction to Linux operating systems. This study aimed to develop AMAP-APP, an optimized, cross-platform desktop application designed to overcome these hardware and usability barriers while maintaining anal

Delayed diagnosis of selected thoracic and mediastinal conditions in young adults: a focused narrative review of clinical pitfalls, cognitive biases, and system barriers.

BACKGROUND: Thoracic and mediastinal conditions affecting the esophagus, diaphragm, mediastinum, and airway carry considerable morbidity when diagnosis is delayed. Young adults are often perceived as low risk for serious thoracic disease, and low clinical suspicion, atypical symptoms, and cognitive and healthcare-system barriers can delay recognition. METHODS: This focused narrative review searched PubMed and Scopus, supplemented by Google Scholar, for studies published between January 2000 and May 2026, with selected earlier landmark studies retained where essential. Search terms included "t

AAVcc47-μDys Gene Therapy Ameliorates Respiratory Pathology in mdx Mouse Model of Duchenne Muscular Dystrophy.

Duchenne muscular dystrophy (DMD) is a progressive X-linked neuromuscular disorder caused by deficiency of dystrophin - a protein essential for muscle integrity. Loss of dystrophin results in global muscle weakness, including in the muscles of respiration. As a result, respiratory failure is a leading cause of morbidity and mortality in patients with DMD. Gene therapy using adeno-associated virus (AAV) is ideal for monogenetic diseases. However, the dystrophin gene is too large to be packaged into AAV; therefore, microdystrophin (μDys) containing important elements of the dystrophin gene is us

ACR Appropriateness Criteria® Suspected Diaphragm Dysfunction.

Diaphragm dysfunction includes diaphragmatic eventration, weakness, and paralysis, a set of disorders which are often challenging to diagnose clinically due to their varied presenting symptoms including dyspnea, positional orthopnea, cough, or even chest pain. Given the clinical challenge in diagnosing these conditions, diaphragm dysfunction is often suggested incidentally on initial imaging for workup of these symptoms. Imaging can play an important role both in raising the initial suspicion for diaphragm dysfunction as well as in making the diagnosis through real-time evaluation of the diaph

Chemometrics-assisted FTIR and machine learning for biochemical profiling of human skeletal muscles in relation to type 2 diabetes.

Skeletal muscle is the major site of insulin-stimulated glucose uptake, making it highly susceptible to metabolic disorders. This study investigates Type 2 diabetes mellitus-associated alterations in the macromolecular composition of functionally distinct human skeletal muscles. These modifications are crucial to understanding the molecular basis of diabetic myopathies but remain underexplored. Five skeletal muscles from 80 individuals (40 controls and 40 individuals with Type 2 diabetes) were analysed. To overcome the limitations of traditional histochemical assays, we employed infrared spect

From molecular correction to functional rescue: delivery, tissue state, and immunobiology as the principal constraints on dystrophin-restoring therapy in Duchenne muscular dystrophy.

Duchenne muscular dystrophy (DMD) arises from thousands of distinct mutations at a single X-linked locus that converge on loss of functional dystrophin, and this convergence on one molecular endpoint-rather than any genetic uniformity-has long made the disease appear a candidate for straightforward molecular correction. Three decades of work indicate that the difficulty lies less in restoring dystrophin than in restoring it adequately along four axes: amount, retained domains, breadth of coverage across skeletal muscle, diaphragm, heart and brain-the constraint being extent of coverage rather

Carbon dioxide as a promising alternative to iodinated contrast media for lower extremity endovascular procedures (Review).

Carbon dioxide (CO2) angiography has emerged as an alternative contrast agent for arterial endovascular procedures, particularly in patients at increased risk of complications from iodinated contrast media (ICM). The present review evaluates the clinical circumstances in which carbon dioxide (CO2) angiography can reduce the use of iodinated contrast media (ICM) during arterial endovascular procedures, with particular attention to renal-risk reduction, image quality, anatomical limitations, and procedure-related complications. A narrative literature review was performed using PubMed, EMBASE, th
