Exercise-induced dynamic hyperinflation in chronic obstructive pulmonary disease.

Chronic obstructive pulmonary disease (COPD) is an inflammatory lung disease caused by inhalation of noxious particles, most commonly cigarette smoking. The consequent changes in airways, lung parenchyma and pulmonary vasculature lead to increased resistive, elastic and threshold loads and impaired capacity of the respiratory muscle pump. COPD is characterized by progressive expiratory flow limitation. During exercise, increases in respiratory rate lead to shortening of expiratory time with consequent gas trapping. The resultant increase in end-expiratory lung volume is referred to as dynamic

Piezo1-Mediated Mechanotransduction: Orchestrating the Dynamic Response of Podocytes and Parietal Epithelial Cells to Mechanical Stress.

AIM: The glomerulus is a specialized microvascular unit that filters plasma through the coordinated function of podocytes and parietal epithelial cells (PECs). From this perspective, the glomerulus functions like a living hydrogeological filtration system. This review aims to integrate mechanobiology and hydrogeology, reframing podocytes and PECs as active regulators in a pressure-driven network, with Piezo1 central to glomerular homeostasis, adaptation, and pathology. METHODS: This review integrates existing literature on glomerular biology, mechanosensitive signaling, and epithelial cell fu

WNK1 Kinase Activity Is Required for the Functional Maintenance of Podocyte Structure.

The filtration function of glomeruli requires slit diaphragms formed by interdigitating podocyte foot processes, which are actin-based membrane protrusions. Failure in the maintenance of these cytoskeletal structures leads to foot process effacement, proteinuria, and progression to chronic kidney disease. We report WNK1 kinase activity is required for normal glomerular function in vivo and test the hypothesis that WNK1 kinase activity affects the structure of podocyte foot processes through modulation of actomyosin activity and focal adhesion complexes. Perturbation of cytoskeletal structure a

CPAP and high-flow nasal oxygen also reduce lung, diaphragm, and accessory muscle injury in experimental self-inflicted lung injury.

Strenuous respiratory effort has been proposed as a second hit, introducing the concepts of “patient self-inflicted lung injury” (P-SILI) and underassistance myotrauma. We investigated the effects of continuous positive airway pressure (CPAP) and high-flow nasal oxygen (HFNO) on inspiratory and expiratory effort, lung injury, and myotrauma of obligatory and accessory respiratory muscles in experimental P-SILI. In rats, lung injury was induced through surfactant depletion, followed by 3 h of standard oxygen therapy (2 L/min, positive control) or supported ventilation (CPAP 6 cmH2O, HFNO 4 L/min

Three-dimensional geometric morphometric analysis of diaphragmatic dome motion in COPD patients.

Diaphragmatic dysfunction is a hallmark of chronic obstructive pulmonary disease (COPD), especially in emphysema, where hyperinflation alters diaphragm geometry and impairs inspiratory mechanics. However, quantitative three-dimensional (3D) assessments of diaphragmatic dome shape and motion across COPD phenotypes are limited. This study applied 3D geometric morphometrics (3DGM) to characterize diaphragmatic kinematics and curvature in COPD patients compared to controls. We prospectively enrolled 126 adults: 40 COPD patients (17 bronchitis-predominant, 23 emphysema-predominant) and 86 asymptoma

Molecular characteristics of Sarcocystis spp. in sheep from slaughterhouse in Xinjiang, China.

Sarcocystis is an important zoonotic protozoan parasites. This study collected 354 tissue samples (22 skeletal muscle samples, 74 diaphragm samples, 171 heart samples, and 87 esophagus samples) from 354 sheep across seven slaughterhouses in seven cities of Xinjiang. Screening for Sarcocystis spp. was performed using PCR targeting the 18S rRNA gene, with confirmation PCR targeting the COX 1 gene. The infection rate was determined to be 30.2% (107/354). All seven cities tested positive for Sarcocystis, with the highest infection rate in Tacheng (95.5%, 21/22) and the lowest infection rate in Kuq

Surface tracking-assisted multi-cycle 4D MRI motion modeling for lung radiotherapy: a preliminary validation study.

Objective. To validate a respiratory motion model that uses real-time electromagnetic (EM) surface tracking acquired concurrently with time-resolved multi-cycle 4D MRI (TRMC-MRI) to estimate respiration-induced changes within the entire irradiated volume.Approach. Four volunteer participants with no self-reported history of lung cancer or other pulmonary disease underwent TRMC-MRI using a golden-angle stack-of-stars 3D GRE sequence while breathing freely for 2 min. Concurrently, real-time thoracoabdominal surface motion was recorded using four MR-compatible electromagnetic (EM) sensors (EndoSc

CITED2 deficiency drives pulmonary hypoplasia via miR-200b and inflammatory pathways.

BACKGROUND: CITED2 is a transcriptional co-activator with widespread roles in development. We recently found that the nitrofen rat model of congenital diaphragmatic hernia (CDH) showed CITED2 dysregulation at a late stage of abnormal lung development, but the role of CITED2 in lung development remains unexplored. Here, we study potential pathways of CITED2 in multiple models of lung hypoplasia. METHODS: We used CRISPR/Cas9-generated CITED2-deficient rats to examine fetal lung growth, lung morphometry and inflammatory gene expression. CITED2 and its potential up- and downstream mediators were

Retroperitoneal Bronchogenic Cysts: A Case Series.

BACKGROUND Bronchogenic cysts are a rare congenital malformation of primitive foregut development, predominantly located in the thorax. However, they can also be found in various atypical locations, including the retroperitoneal space. Retroperitoneal bronchogenic cysts are exceedingly rare. The diagnosis of retroperitoneal bronchogenic cysts presents considerable difficulty owing to their rarity and the absence of distinctive clinical and imaging characteristics. CASE REPORT This study reports 6 surgically confirmed cases of retroperitoneal bronchogenic cysts at our institution. One patient p

Ultrasonographic diaphragmatic assessment as an emergency mortality predictor in acute exacerbation of COPD.

OBJECTIVES: To evaluate the role of diaphragmatic ultrasound as a mortality predictor in acute exacerbation of chronic obstructive pulmonary disease (AECOPD), compared to the DECAF and BAP65 scoring systems. BACKGROUND: The diaphragm is the primary muscle of respiration and is often compromised during AECOPD. Bedside ultrasound offers a non-invasive method to assess diaphragmatic function. Early identification of patients at high risk of mortality upon presentation may aid in appropriate triage and management. METHODS: This observational pilot study included 50 patients presenting to the emerg

Imaging-Based Diagnosis of a Ruptured Isolated Dissecting Abdominal Aortic Aneurysm: A Case Report.

Background and Clinical Significance: Acute aortic dissection is the most common and most severe manifestation of acute aortic syndrome. An isolated dissecting aneurysm of the abdominal aorta is defined as a dissecting aneurysm distal to the diaphragm and is an extremely rare disease. Detection of an intimal flap between two lumens using different imaging methods is a definitive diagnostic sign of aortic dissection. A number of studies have validated ultrasound, including point-of-care ultrasound, as the standard initial imaging modality for the diagnosis of aortic dissection. Case Presentatio

Endoscopic endonasal surgery for infradiaphragmatic craniopharyngiomas: the impact of diaphragma sellae resection.

OBJECTIVE: Infradiaphragmatic craniopharyngiomas (ICs) are anatomically distinct lesions bounded by an elevated diaphragma sellae (DS). While endoscopic endonasal surgery (EES) has emerged as their primary treatment, achieving complete resection remains challenging due to frequent tumor-DS adhesions, potentially contributing to higher recurrence rates. Authors of this retrospective study evaluated the efficacy and safety of DS resection (DSR) during EES for IC. METHODS: The data for patients with ICs treated with EES between March 2011 and December 2024 were retrospectively reviewed. Accordin

Skeletal muscle in spinal muscular atrophy: Critical insights from pathogenesis to therapeutic strategies.

Spinal muscular atrophy (SMA) is a genetic neuromuscular disorder caused by loss of the survival motor neuron (SMN) protein. While SMA was originally viewed as a pure motor neuron disease, it is currently considered a multi-system disorder in which skeletal muscle plays a pathogenic role. Muscular defects, such as impaired myogenesis and mitochondrial dysfunction, contribute to pathogenesis partly independently of denervation. Accumulating evidence suggests that the SMN deficit impairs muscle development from the earliest stages of fetal life, with delayed myotube maturation and modification o

Duodenal Small Bowel Diaphragm Disease: A Rare and Underrecognized Complication of Chronic NSAID Use.

Nonsteroidal anti-inflammatory drugs (NSAIDs) are among the most commonly used medications. Although common adverse effects such as gastrointestinal bleeding, ulcers, and kidney injury are widely known, small bowel diaphragm disease, an exceedingly rare complication of long-term NSAID use, remains underrecognized. We report an uncommon case of a 56-year-old patient on long-term high-dose NSAIDs who developed diaphragm disease involving the duodenum, a rarely reported location, manifesting as intestinal obstruction. This case highlights the potential for unusual anatomical involvement to mask d

Effect of Steroids on Anti-nephrin Autoantibodies and B cells in Minimal Change Disease.

INTRODUCTION: The identification of autoantibodies against nephrin, a key signaling protein of the podocyte slit diaphragm, and their pathogenic role in patients with minimal change disease (MCD) has substantially changed our understanding of primary podocytopathies. However, details on underlying immune pathophysiological mechanisms such as the relation of anti-nephrin autoantibodies with immune cell subsets and therapy response remain to be determined. METHODS: In this prospective study, we evaluated blood samples from adults with newly diagnosed, biopsy-proven MCD for circulating anti-neph

Persistent Hiccups as an Atypical Sole Manifestation of ST-Segment Elevation Myocardial Infarction: An Interesting Case Report.

BACKGROUND: Myocardial infarction (MI) commonly presents with chest pain, dyspnea, diaphoresis, and nausea; however, atypical and nonclassical presentations are increasingly recognized and may delay diagnosis and management. Persistent hiccups as the sole presenting manifestation of ST-elevation myocardial infarction (STEMI) are exceedingly rare, with only a few cases reported. Awareness of such unusual presentations is essential to avoid missed or delayed diagnoses. CASE PRESENTATION: A 60-year-old male presented to the outpatient clinic with a 1-month history of persistent hiccups without c

Functional and structural pathologies in skeletal muscle of a rat model of Duchenne muscular dystrophy.

BACKGROUND: Duchenne muscular dystrophy (DMD) is a lethal pediatric degenerative muscle disease for which there is no cure. Robust preclinical models that recapitulate major clinical features of DMD are required to investigate efficacy of potential DMD therapeutics. Rat models of DMD have emerged as promising small animal models to accomplish this; however, there have been no comprehensive studies investigating the functional skeletal muscle decrements associated with the modeling of DMD in rats. METHODS: CRISPR/Cas9 gene editing was used to generate a dystrophin-deficient Sprague-Dawley musc
