Diaphragm muscle weakness in adults with diabetes.

BACKGROUND: Diabetes-related pulmonary dysfunction, including impaired spirometry and pulmonary gas exchange, is well characterized. However, data on diaphragm muscle weakness(DMW), a predictor of poor clinical outcomes, are limited. OBJECTIVES: We report the percentage change in forced vital capacity(FVC) from sitting to supine position(∆FVC), a surrogate measure of DMW, in adults with diabetes. METHODS: This cross-sectional study included 537 adults with diabetes without primary pulmonary disease managed at Ghana's National Diabetes Management and Research Centre. Spirometry was conducted

Impact of Inspiratory Muscle Training in Individuals with Gastroesophageal Reflux Disease: A Randomized Controlled Trial Protocol.

Gastroesophageal reflux disease (GERD) is a common chronic condition mainly caused by the dysfunction of the antireflux mechanism at the gastroesophageal junction. This is composed of the lower esophageal sphincter and the crural diaphragm. Increasing evidence suggests that diaphragmatic dysfunction and reduced inspiratory muscle strength may contribute to the persistence of GERD symptoms. Although respiratory physiotherapy has shown beneficial effects, the role of a structured inspiratory muscle training (IMT) program has not been sufficiently examined. This study aims to investigate the effe

Postural relief of dyspnoea is associated with improved neuromechanical coupling in patients with advanced COPD.

BACKGROUND: Patients with advanced COPD often report that adopting a specific body posture improves their dyspnoea. Exploring the underlying mechanisms could enhance our understanding of the origin of this symptom and approaches towards its alleviation. METHODS: Pulmonary function tests and detailed sensory-mechanical measurements, including gastro-oesophageal catheter-based crural diaphragm electromyography (surrogate for inspiratory neural drive (IND)) and respiratory manometry, were performed in 16 patients (nine males; mean±sd age 66±7 years) with advanced COPD (mean±sd forced expiratory

Diaphragm assessment by multimodal ultrasound imaging in patients with interstitial lung disease: a prospective observational study.

BACKGROUND: The dyspnea experienced by patients with interstitial lung disease (ILD) is partly attributable to diaphragmatic dysfunction. This study aims to quantitatively evaluate diaphragmatic function in ILD patients by measuring different diaphragm ultrasound parameters, seeking to gain a more comprehensive understanding of diaphragmatic characteristics in this population. METHODS: Forty-four ILD patients and 31 healthy controls were prospectively enrolled and underwent diaphragmatic ultrasound. Parameters measured included diaphragmatic thickness, excursion, and tissue Doppler imaging (T

NRASQ61R Expression in Lymphatic Endothelial Cells Causes Enlarged Vessels, Hemorrhagic Chylous Effusions, and High Mortality in a Mouse Model of Kaposiform Lymphangiomatosis.

BACKGROUND: Kaposiform lymphangiomatosis (KLA) is an aggressive complex lymphatic anomaly. Patients exhibit malformed lymphatic vessels and often develop hemorrhagic effusions and elevated angiopoietin-2 (Ang-2) levels. A somatic NRAS p.Q61R (NRASQ61R) mutation has been associated with KLA. Existing preclinical models do not recapitulate the thoracic effusions that drive morbidity and mortality in patients. The goal of this study was to develop a genetic mouse model that more closely mimics the clinical phenotype of KLA. PROCEDURE: A tamoxifen-inducible Cre-loxP system was used to conditional

Distinct long-term effects on lung function and airway remodeling in ovalbumin and house dust mite mouse models of experimental asthma.

Although mouse models of acute allergic inflammation (AAI) have long been used to study asthma mechanisms, their ability to reproduce human disease remains controversial and long-term studies of chronic asthma remain scarce. We compared lung function and tissue changes in three experimental asthma models after four months of recovery. Using x-ray-based lung function (XLF) analysis, we assessed lung parameters and diaphragm motion, alongside lung expression of α-smooth muscle actin and key extracellular matrix proteins, collagen and elastin. A severe ovalbumin (OVA) model (SAA) produced stronge

Predicting amyotrophic lateral sclerosis stage based on multi-parameter ultrasound: development and validation of an interpretable machine learning model.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) lacks sensitive, objective staging tools to guide clinical management and trials. Existing methods have limited granularity and rely on subjective assessment, while biomarker and imaging approaches can be invasive or impractical for serial use. Ultrasound is a safe, portable imaging modality that can detect neuromuscular changes, but it has not yet been applied to ALS staging. We developed and validated an interpretable ultrasound model for clinical staging and risk stratification in ALS. METHODS: We enrolled 300 ALS patients, classified as earl

Mechanistic study of inspiratory training in childhood Asthma: Rationale and methods of a pediatric clinical trial.

Asthma is a common pediatric disease of the airways. Obesity among children increases the risk of asthma and is associated with a unique non-allergic asthma phenotype that is highly symptomatic and often unresponsive to conventional drugs. However, the mechanisms linking obesity with incident asthma, more frequent and severe symptoms, and resistance to conventional drugs remain unknown. Excess fat on the chest wall and abdomen forces children with obesity to breathe at lower lung volumes where small airway dysfunction is more likely to occur and where the normal length-tension relationship of

Anti-nephrin antibodies in pediatric nephrotic syndrome: a new paradigm.

Circulating anti-nephrin antibodies have recently been identified as mediators in many cases of childhood nephrotic syndrome. After binding of the antibody to nephrin, a key component of the slit diaphragm, different intracellular signaling mechanisms lead to foot process effacement. These new insights into the pathophysiology of nephrotic syndrome may have the potential to influence clinical treatment strategies. First, the measurement provides a pathomechanism-specific serological biomarker of a previously clinically diagnosed renal disease. Second, it may have additional value for diagnosis
