Evaluating skeletal muscle dysfunction and recovery in a zymosan model of critical illness in mice.

Muscle wasting and weakness are common complications associated with critical illness and admission to the intensive care unit (ICU), which contribute to increased mortality and health deficits post discharge. The mechanisms underlying ICU-acquired muscle weakness (ICU-AW) are incompletely understood, and small-animal models can help address this shortfall and provide experimental platforms for devising therapeutic strategies. We used zymosan treatment to induce wasting and weakness in C57BL/6J mice and evaluated recovery of hindlimb muscles and diaphragm on day (D)4, D7, D14 and D28 after ind

Fibrotic differentiation profile of skeletal and cardiac muscle fibroadipogenic progenitors in D2-mdx mouse.

Muscle fibrosis is a key pathological feature of Duchenne muscular dystrophy (DMD) and is closely associated with disease progression. Fibroadipogenic progenitors (FAPs) are major contributors to fibrosis, yet the precise mechanisms remain unclear. To investigate FAP dynamics and lineage specification, we generated dual-reporter mice (PRURD2) by crossing D2.B10-Dmdmdx/J (D2-mdx) mice with FAP and brown/beige adipose tissue (BAT) reporter lines. Corresponding control mice (PRURDBA) were established on the DBA/2J background. At 12 months, heart, diaphragm, and tibialis anterior (TA) muscles were

Investigating the efficacy of osteopathic manipulative treatment for intractable hiccups: a pilot study.

CONTEXT: Intractable hiccups is a rare medical condition defined as daily hiccups continuing for more than 30 days and is recognized by the National Organization for Rare Disorders (NORD) and NIH Genetic and Rare Diseases (GARD) Information Center. Hiccups are spasmodic contractions of the respiratory muscles, triggered by activation of the hiccup reflex arc. Intractable hiccups can occur if this arc is chronically or aberrantly activated. The hiccup reflex arc has three components: (1) afferent pathways involving the phrenic, sympathetic, and vagal nerves; (2) central processing centers withi

Longitudinal characterization of Gaac.1826dupA mice reveals the cardiac, myopathic and biochemical phenotypes of Pompe disease.

Pompe disease (PD) is a rare autosomal recessive disorder caused by acid α-glucosidase (GAA) deficiency, leading to lysosomal glycogen accumulation. Pathogenic GAA variants result in enzyme dysfunction and glycogen storage in cardiac, skeletal and smooth muscle, as well as in the central nervous system, driving both systemic and neurological manifestations. We have previously characterized a transgenic knock-in (KI) mouse carrying the Gaa c.1826dupA variant to 12 weeks of age, showing that it recapitulates key biochemical and phenotypic features of PD. Here, we extend this analysis to present

First report of Anaerococcus tetradius in bilateral tubo-ovarian abscess following intra-uterine device insertion.

Tubo-ovarian abscess (TOA) is a life-threatening complication of pelvic inflammatory disease. A 30-year-old woman presented with bilateral tubo-ovarian abscesses, one week after the insertion of a levonorgestrel-releasing intrauterine device. The patient required intensive care management with intravenous antibiotics, along with ultrasound-guided abscess drainage. Anaerobic culture of the aspirated pus samples (3) yielded Anaerococcus tetradius. The patient recovered fully after 17 days of hospital stay. This is the first documented case of Anaerococcus tetradius isolated from a bilateral TOA,

Asynchrony Related to Mechanical Ventilation Exacerbates Diaphragm Dysfunction in a Mouse Model of Duchenne Muscular Dystrophy.

INTRODUCTION/AIMS: In dystrophic mice (mdx, a genetic homolog of Duchenne muscular dystrophy: DMD), previous studies showed that mechanical ventilation (MV) induces ventilator-induced diaphragmatic dysfunction (VIDD). However, susceptibility to mechanical stress caused by asynchrony remains unknown. Our aims were to investigate whether MV exacerbates diaphragm vulnerability to eccentric stress and to evaluate the role of RyR1 remodeling and NOX2 activation. METHODS: Male mdx and wild-type (WT) mice (10-12 weeks) were assigned to non-ventilated or 6-h MV groups. Diaphragm strength and suscepti

Impaired phrenic nerve axon development and diaphragm neuromuscular junction formation in embryonic Cyfip2-null mice.

Pathogenic variants in CYFIP2 cause developmental and epileptic encephalopathy 65 (DEE65) and have been predominantly investigated in the context of central nervous system dysfunction. However, emerging clinical evidence suggests that peripheral nervous system (PNS) involvement may also contribute to disease manifestations. To explore this possibility, we examined the role of CYFIP2 in the development of the phrenic neuromuscular system, which is essential for neonatal respiration. Because conventional Cyfip2-null (Cyfip2-/-) mice exhibit perinatal lethality, we analyzed phrenic nerve axon dev

Daily hyperbaric oxygen therapy increases diaphragm electromyographic activity and alters diaphragm gene expression after cervical spinal cord injury.

Hyperbaric oxygen (HBO) therapy can enhance motor recovery when initiated shortly after spinal cord injury (SCI). HBO can also induce a biphasic ventilatory response that includes hypo- and hyperventilation, but this has not been evaluated after SCI. We studied adult Sprague-Dawley rats with cervical SCI to determine how an acute bout of HBO impacts diaphragm activation and to test the hypothesis that daily HBO treatment can accelerate recovery of diaphragm activation. Indwelling electromyogram (EMG) electrodes were placed in the mid-costal diaphragm prior to lateral mid-cervical contusion inj

An Anterior Transdiaphragmatic Approach for En Bloc Resection of a Giant Thoracolumbar Chordoma: A Case Report.

CASE: A 54-year-old man presented with a giant T10-L2 chordoma. The tumor encased the thoracoabdominal aorta, presenting a significant challenge for en bloc resection. We used a novel anterior transperitoneal and transdiaphragmatic approach. By longitudinally incising the diaphragm along the aortic hiatus, we obtained direct visualization to safely dissect the aorta from the tumor in both thoracic and abdominal cavities. This facilitated a successful 2-stage en bloc spondylectomy and reconstruction with a three-dimensional-printed vertebral body. At 24 months, the patient was neurologically in

Case Report: Hemidiaphragm Paralysis Results in Reduced Blood Oxygen Saturation, Increased Respiratory Rate, and Severe Dyspnea in Supine and Prone Positions due to Impaired Abdominal Breathing.

Background: The breathing cycle consists of abdominal breathing (AB), for which the diaphragm is responsible, and thoracic breathing (TB), generated by the intercostal muscles. Contraction of the two portions of the diaphragm accounts for 80% percent of inspiration. While bilateral diaphragmatic paralysis causes severe shortness of breath, hemidiaphragm paralysis (HDP) gives fewer symptoms at rest, making it difficult to recognize and diagnose. Because this condition is rare, little is known regarding its consequences on breathing efficiency. Hypothesis: Based on previous studies, we hypothesi

Ultrasound assessment of diaphragmatic structure and function before and after orthotopic liver transplantation in MNGIE.

BACKGROUND: Mitochondrial Neurogastrointestinal Encephalomyopathy (MNGIE) is an ultra-rare autosomal recessive disorder caused by thymidine phosphorylase (TP) deficiency, leading to systemic nucleoside accumulation and multisystemic dysfunction. While respiratory failure is not a primary feature, severe cachexia, sarcopenia, and respiratory muscle weakness may contribute to morbidity, particularly during acute illnesses. Orthotopic liver transplantation (OLT) has emerged as a disease-modifying therapy, restoring TP activity and normalizing circulating nucleosides, though its impact on respirat

Spontaneous Right-Sided Diaphragmatic Rupture With Liver Herniation After Bilateral Lung Transplantation.

Spontaneous diaphragmatic rupture is a rare but life-threatening complication. We report the case of a 53-year-old female individual who developed acute right-sided diaphragmatic rupture with liver, gallbladder, and transverse colon herniation 6 days after bilateral lung transplantation for end-stage chronic obstructive pulmonary disease. The event followed forceful vomiting and caused cardiac tamponade due to compression. Emergency surgical repair required a combined thoracic and abdominal approach with mesh reconstruction of the diaphragm. Prompt recognition, careful perioperative positionin

[A Case of Esophagogastric Junction Cancer Treated with Perioperative Nivolumab Therapy].

Immune checkpoint inhibitors have shown efficacy in various malignancies. However, reports of conversion surgery after nivolumab therapy for unresectable advanced esophagogastric junction(EGJ)cancers remain rare. We present the case of a 69-year-old male diagnosed with EGJ cancer(cT4a[diaphragm], N2[No. 1×1, No. 7×1, and No. 19×1], M0, cStage Ⅲ). Neoadjuvant chemotherapy with docetaxel plus S-1 regimen was administered, resulting in stable disease(SD). During treatment, microsatellite instability-high(MSI-H)status was identified, and nivolumab monotherapy was subsequently initiated per regulat

Diaphragm hypertrophy and spinal stress response accompany phrenic myelin defects in a murine model of Charcot-Marie-Tooth Disease Type 1E.

Charcot-Marie-Tooth disease type 1E (CMT1E) is a rare, early-onset dysmyelinating neuropathy linked with point mutations in the peripheral myelin protein 22 (PMP22) gene. Respiratory problems are known to impact neuropathic patients, yet existing studies have not characterized this pathophysiology in corresponding animal models. Trembler J (TrJ) mice carry the same Leu16Pro amino acid substitution in the PMP22 protein that is present in families diagnosed with CMT1E. Utilizing electrophysiological, biochemical and morphological approaches, we examined critical sites of the lower respiratory sy

Breath-by-breath parasternal intercostal muscle stiffening tracks inspiratory effort during graded inspiratory threshold loading in humans.

Inspiratory effort is associated with the recruitment of extra-diaphragmatic muscles, including parasternal intercostal muscles (PIMs). The mechanical behavior of PIM and its relationship to indices of inspiratory effort in humans remains poorly characterized. We investigated whether PIM stiffening, assessed using ultrafast ultrasound shear wave elastography (SWE), tracks inspiratory effort breath-by-breath during graded inspiratory loading. Fourteen healthy adults (9 men and 5 women) were studied during quiet breathing and inspiratory threshold loading at 10%, 20%, 30%, and 40% of maximal ins

Endovascular stimulation as a therapeutic strategy for the treatment of cardiopulmonary disorders.

Endovascular electrical stimulation is a minimally invasive technique that delivers electrical impulses through the vascular system to achieve neuromodulation or neuromuscular stimulation. Compared with conventional implants, it reduces surgical risks and recovery time. This narrative review summarizes current applications in the cardiopulmonary system, focusing on cardiovascular regulation and respiratory disorder management. In cardiovascular regulation, endovascular stimulation at sites such as the superior vena cava, pulmonary arteries, coronary sinus, and inferior vena cava has been shown
