Chronic kidney disease (CKD) is a complex condition where the kidneys are damaged and progressively lose their ability to filter blood, 10% of the world population have the disease that often goes undetected until it is too late for intervention. Using the UK Biobank (UKBB) we constructed a CKD cohort of patients (n = 46,986) with genomic, clinical and demographic data available, a subset (n = 2,151) having also whole body Magnetic Resonance Imaging (MRI) scans. We used this multimodal cohort to successfully predict, from initially healthy patients, their 5-year outcomes for End-Stage Renal Di

Congenital diaphragmatic hernia (CDH) is a severe congenital anomaly characterized by impairment of both diaphragm and lung development in utero. CDH presents as a spectrum of forms and severities, with diaphragm defects arising in the dorsal/posterior region typically correlating with more severe pulmonary disease and higher risk of mortality than those appearing in ventral/anterior regions. The genetic etiology underlying CDH is complex, with many genes implicated showing variable expressivity and incomplete penetrance in both human patients and mouse models. Here we present in vivo validati

ObjectiveUltrasonography has been commonly employed during the Coronavirus Disease 2019 (COVID-19) pandemic to assess lung and heart conditions; however, diaphragm ultrasonography (DU) has not been as widely acknowledged. This study aimed to assess the prevalence of diaphragm dysfunction (DD) in non-intubated critically ill patients with COVID-19 pneumonia on intensive care unit (ICU) admission and to examine the correlation between DD and various prognostic factors during their ICU stay.MethodsThis prospective observational cohort study included 79 consecutive patients admitted to the intensi

Chronic respiratory disorders (CRDs) are the third most common cause of cancer- and cardiovascular disease-related mortalities. Non-pharmacological interventions for CRDs effectively slow disease progression, improve quality of life, ameliorate symptoms, and potentially extend survival. Pulmonary rehabilitation faces limitations in respiratory training efficacy owing to methodological constraints, poor patient adherence, and questions about long-term benefits. Transcranial magnetic stimulation (TMS) has been used to demonstrate the capacity of cortical excitability modulation to cause neuronal

Duchenne muscular dystrophy is a fatal disease characterized by persistent skeletal muscle degeneration, inflammation, and fibrosis. Gene therapy using an adeno-associated virus-derived vector and a microdystrophin transgene is currently under investigation in patients, but the impact of physical activity on long-term therapeutic outcome remains poorly understood. Recently, we reported that 21 weeks of voluntary wheel running complemented the positive endurance and muscle function outcomes of gene therapy in mdx mice. In the present study, we performed a transcriptomic analysis of the gene exp

Gastric antral web (GAW) is a mucosal diaphragm causing gastric outlet obstruction. The etiology of this rare condition is heterogeneous, and clinical presentation ranges from asymptomatic disease to abdominal distention, early satiety, and weight loss. Diagnosis, while requiring a high index of suspicion, can be made by upper endoscopy, the gold standard, which reveals a tight antral web. Therapeutic options can be endoscopic, using multiple methods such as balloon dilatation and endoscopic web incision, or a surgical approach when endoscopic techniques fail. We present a case of gastric outl

Rosai-Dorfman disease (RDD) can present with lymphadenopathy. However, it is important to note that RDD can also coexist with hematologic malignancies, including non-Hodgkin lymphomas. We present a case of an older male patient with generalized lymphadenopathy who was first diagnosed with RDD. High-dose steroid therapy was rendered without a satisfactory response. Thus, the tissue specimens were re-examined, and the patient was found to have diffuse large B-cell lymphoma (DLBCL) in the background of RDD. The laboratory tests are notable for a slightly elevated serum lactate dehydrogenase (LDH)

BACKGROUND: Stroke is frequently accompanied by respiratory dysfunction, which limits ventilatory reserve and may hinder functional recovery. Respiratory muscle training (RMT) can improve selected ventilatory indices, but overall benefits remain modest. This pilot trial aimed to evaluate the clinical effects of calcium-regulating pulsed electromagnetic fields (Ca-R-PEMF) combined with RMT on poststroke respiratory function.
METHODS: This single-center, prospective, double-blind pilot trial was conducted at Yongsen Hospital of China Medical University, China, from July to December 2025. Thirty

CD2-associated protein (CD2AP) is a multifunctional adaptor linking membrane receptors to the actin cytoskeleton, ensuring slit diaphragm integrity, regulating synaptic proteostasis, and facilitating cytoskeletal remodeling. Dysregulation of CD2AP contributes to proteinuric kidney disease, Alzheimer's disease, and tumor progression. This review provides a comprehensive overview of the structural and functional characteristics of CD2AP, its phosphorylation landscape, and potential regulatory roles. By integrating 3,825 publicly available phosphoproteomic research articles with data obtained fro

Hiatal hernia repair is one of the most commonly performed surgical procedures for the treatment of gastroesophageal reflux disease (GERD), yet postoperative pain remains a clinically relevant issue despite the widespread adoption of minimally invasive techniques. Dissection and suturing of the diaphragmatic crura may contribute significantly to early postoperative discomfort, and intraoperative local anesthetic infiltration of the crura under direct visualization has been proposed as a simple pain-modulating strategy. In this retrospective two-center study, 196 patients who underwent laparosc

BACKGROUND: Hormonal contraceptives (HCs) are widely used and have well-documented population-level statistics. Previous studies with short follow-ups have focussed on individual HC use and side effects. However, the same aspects over longer periods, HC formulation switching, and the impact of genetic factors on HC side effects remain understudied due to the limited availability of suitable datasets. We investigated whether the Estonian Biobank (EstBB) is suitable for studying genetic risk for HC side effects.
METHODS AND FINDINGS: This is a longitudinal descriptive study combining prescribed

Interstitial lung diseases may affect the diaphragm due to continuous muscle overload caused by increased lung elastance, and functional alterations have already been described in patients with interstitial lung disease. However, the histological remodeling of the diaphragm is unknown. Our objective was to compare the histological and ultrastructural findings of the diaphragm in patients with and without interstitial lung disease. In this cross-sectional study with internal and external controls, we evaluated diaphragmatic and peripheral muscle (internal controls) biopsies from patients with a

BACKGROUND: Transthoracic echocardiography (TTE) is the most commonly used modality for the diagnosis and follow-up of cardiac diseases. During routine TTE, surrounding extracardiac structures are often visualized, including the lungs, pleural spaces, oesophagus, diaphragm, and sub-diaphragmatic structures such the liver, stomach, bowel, and abdominal aorta. Pathologic conditions affecting these organs may exert extrinsic compression on the cardiac chambers, leading to diagnostic uncertainty or mimicking cardiac disease by producing symptoms, such as dyspnoea, fatigue, chest pain, or palpitati

BACKGROUND: Postoperative diaphragmatic dysfunction (DD) is a frequent complication following thoracic surgery, which can significantly impair respiratory function, prolong recovery, and increase morbidity. Video-assisted thoracic surgery (VATS), a minimally invasive surgical approach, has gained widespread use due to its advantages over traditional open thoracotomy, including smaller incisions, reduced postoperative pain, and faster recovery times. However, despite these benefits, DD remains a notable concern in VATS patients, potentially due to surgical manipulation of the diaphragm and surr

BACKGROUND: While early rehabilitation is widely implemented in intensive care units (ICUs) for patients requiring mechanical ventilation (MV), the clinical effectiveness of external diaphragm pacing (EDP) in this context remains unclear. This systematic review and meta-analysis aimed to evaluate the impact of EDP on clinical outcomes in patients receiving MV and to provide evidence to inform future research.
METHODS: A comprehensive search of nine electronic databases was conducted from inception to January 2026, for randomized controlled trials (RCTs) comparing EDP with control in MV patien

BACKGROUND: Skeletal muscle dysfunction, a common extrapulmonary manifestation in patients with chronic obstructive pulmonary disease (COPD), contributes to decreased physical activity and poor prognosis. Among skeletal muscles, erector spinae muscle (ESM) is crucial in posture and respiratory support. However, the association between the diaphragmatic function and ESM remains unclear. This study aimed to determine whether diaphragmatic excursion (DEmax) measured using ultrasonography is associated with longitudinal changes in the cross-sectional area of the ESM (ESMCSA) in patients with COPD.
