Antimicrobial resistance patterns and genetic determinants of emerging Gallibacterium anatis isolates from poultry in West Java, Indonesia.

BACKGROUND: Gallibacteriosis is a disease in poultry caused by the bacterium Gallibacterium anatis that can lead to respiratory disorders or decreased production. This bacterium causes infections that are similar to those caused by other bacteria, which may lead to misdiagnosis. Infections caused by G. anatis are increasingly associated with reduced susceptibility to antibiotics. AIM: This study aimed to investigate the antibiotic resistance, antibiogram profile, and antibiotic resistance genes associated with emerging G. anatis infections in poultry. METHODS: Using a targeted sampling metho

Pseudomonas aeruginosa infection causes lysosomal dysfunction in the cystic fibrosis bronchial epithelium.

Cystic fibrosis (CF) is a genetic disorder that causes chronic airway disease, leading to accelerated aging, inflammation, and susceptibility to opportunistic infections. Pseudomonas aeruginosa (PA) is prevalent in the CF lung, often multidrug-resistant, and affecting lung function decline, highlighting a need for alternative treatment strategies, such as host-directed therapeutics. The PA metabolite pyocyanin can induce senescence-associated β-galactosidase (SA-βgal), a lysosomal cell senescence marker. However, whether PA can induce cell senescence and therefore affect immune functions has n

Infective Endocarditis-Associated Glomerulonephritis Caused by Streptococcus cristatus: A Case Report.

INTRODUCTION: Infective endocarditis (IE) can trigger systemic embolic and immune-mediated complications, including infection-related glomerulonephritis. When the infectious focus is IE, the condition is termed IE-associated glomerulonephritis (IEAGN). A major diagnostic pitfall is that IEAGN may present with purpura, nephritic urinary abnormalities, systemic inflammation, and antineutrophil cytoplasmic antibody (ANCA) positivity, closely mimicking primary small-vessel vasculitis and risking inappropriate immunosuppression if infection is not promptly excluded. CASE PRESENTATION: A 52-year-ol

Therapeutic drug monitoring-guided linezolid salvage therapy for Lacticaseibacillus paracasei infective endocarditis: A case report.

We report a rare case of infective endocarditis (IE) caused by Lacticaseibacillus paracasei (formerly Lactobacillus paracasei) that was managed with intravenous linezolid as salvage therapy. A 38-year-old woman presented with a three-week history of intermittent fever and cutaneous manifestations, and was diagnosed with IE caused by Lacticaseibacillus paracasei. The initial ampicillin/clindamycin therapy was discontinued due to suspected drug fever, and subsequent levofloxacin monotherapy was complicated by a drug-induced rash. Linezolid monotherapy was therefore initiated as salvage therapy,

Lascufloxacin switch therapy versus ampicillin/sulbactam for older patients with community-acquired pneumonia in Japan (STEP study): a study protocol for a multicentre open-label randomised controlled non-inferiority trial.

INTRODUCTION: Community-acquired pneumonia (CAP) is a major cause of morbidity and mortality in older adults in Japan. Prolonged hospitalisation accelerates functional decline, promotes the progression of frailty and increases the need for long-term care. An early switch from intravenous to oral antibiotics helps achieve clinical outcomes comparable to those of continued intravenous therapy while shortening hospital stays. For older adults with frailty, avoiding unnecessary hospitalisation may help prevent further deterioration of functional status. Demonstrating non-inferiority of oral antibi

Emergence and persistence of ESBL- and carbapenemase-producing Klebsiella pneumoniae-related species in Barcelona wastewater treatment plants.

The World Health Organization classifies extended-spectrum beta-lactamase (ESBL) and carbapenemase-producing Klebsiella pneumoniae as critical-priority pathogens due to their high incidence, mortality, transmissibility, rapid resistance acquisition, and limited treatment options. Beyond clinical settings, their detection in wastewater treatment plants (WWTPs) provides an opportunity to assess their prevalence, persistence, and circulation within wastewater systems. This study characterized 37 antibiotic-resistant K. pneumoniae-related species strains isolated from two WWTPs in the metropolitan

Reporting macrolide-resistant Mycoplasma pneumoniae: a diagnostic obligation?

Macrolide-resistant Mycoplasma pneumoniae (MRMP) is increasingly detected worldwide, with increasing prevalence even in the United States. Detection of resistance within M. pneumoniae has meaningful clinical and stewardship implications. Resistance is most commonly mediated by point mutations within 23S rRNA and can be reliably identified by molecular assays, not just on isolates butalso on primary respiratory samples like swabs. Multiple studies associate macrolide resistance with prolonged fever, delayed clinical response, increased healthcare utilization, and greater antibiotic exposure due

Predictors and mortality in PCR-positive pneumocystis pneumonia among non-HIV patients.

BACKGROUND: Pneumocystis jirovecii is a major cause of morbidity and mortality in HIV. It also poses a significant risk to non-HIV immunocompromised patients, especially those with cancer or recent chemotherapy. We evaluated risk factors, diagnostic markers-including lactate dehydrogenase (LDH)-and mortality predictors in PCR-confirmed pneumocystis pneumonia (PJP). METHODS: We retrospectively analyzed patients with hypoxemia, pulmonary involvement and/or fever who underwent PJP PCR testing at Koç University Hospital between January 2020 and November 2023. Cases were defined as patients with a

A novel mouse model of chronic Pseudomonas aeruginosa infection inducing bronchiectasis-like phenotype.

BACKGROUND: Given the need to translate basic research into human therapies, the development and refinement of clinically relevant animal models for bronchiectasis are critically important. To date, there are no well-established animal models for bronchiectasis. Thus, our aim was to develop a novel animal model that accurately recapitulates bronchiectasis-like pathologies. METHODS: To address this question, clinical strains of chronic Pseudomonas aeruginosa (CPA) were embedded in agar beads in vitro; then CPA-loaded agar beads and papain were repeatedly instilled intratracheally in female C57

Pulmonary Exacerbations and Treatment in PCD: A Narrative Review.

Primary ciliary dyskinesia (PCD) is a rare genetic disorder that causes chronic lung disease. Patients with PCD often experience pulmonary exacerbations with worsening symptoms and lung function decline. This review provides an overview of the existing knowledge regarding the PEx in PCD and reveals understudied topics that should be addressed in the future. We describe the different PCD-specific PEx definitions that exist in the literature, and provide data on the frequency of PEx. Few studies targeted the pathophysiology of PEx, with a focus on the most common microorganisms in the airways an

Muc16 contributes to protection against invasive pneumococcal infection originating from nasal colonization.

Streptococcus pneumoniae asymptomatically colonizes upper respiratory mucosa and invades into deeper tissue through mucosal membrane. Invasive pneumococcal disease has a significantly high rate of mortality, making its control an urgent concern. Mucin is a defensive system against respiratory infection. However, the role of transmembrane mucin against the development of invasive infection has been unclear. In this study, we demonstrate the roles of Muc16 on invasive pneumococcal disease. Muc16 knockout mice had significantly increased bacterial loads in the lungs and brain after nasal pneumoco

Whole cell and acellular pertussis vaccines - rethinking the evidence.

INTRODUCTION: Pertussis remains an unsolved public health problem. We have synthesized recent evidence on the immunogenicity, effectiveness, and safety of the two available vaccines, whole-cell (wP) and acellular (aP). We aimed to reorient current thinking and critically address some widely held misconceptions about pertussis vaccines. AREAS COVERED: We conducted a targeted literature review to identify studies published in the last 10 years, describing aspects of effectiveness, safety, and the immune response to wP and aP. EXPERT OPINION: Vaccines containing wP and aP continue to successful

Evaluating the socioeconomic value of adult respiratory immunization in Spain: a benefit-cost analysis.

BACKGROUND: Vaccine-preventable illnesses, such as pneumococcal disease, respiratory syncytial virus, influenza, and COVID-19, place a significant burden on Spain's population health, health system, and economy. However, uptake and funding constraints limit the full value potential of Spain's national immunization programs. METHODS: A benefit-cost analysis was performed to assess the societal return on investment from adult vaccination by estimating benefit-cost ratios and net benefits (NBs). All outcomes were monetized using established methods. Program costs included all direct vaccination-

Evaluating the societal value of adult respiratory vaccination in Sweden: a benefit-cost analysis.

BACKGROUND: In Sweden, older adults and adults in clinical risk groups are recommended vaccination against pneumococcal disease, respiratory syncytial virus, influenza, and COVID-19. However, vaccine uptake remains inadequate. This study estimates the socioeconomic benefits of these immunization programs under current coverage compared to aspirational scenarios, assessing the value foregone due to suboptimal uptake. METHODS: We conducted a benefit-cost analysis from a societal perspective applying life table-based disease models to estimate benefit-cost ratios (BCRs) and net benefits (NBs). H

Projected health and economic impact of PCV20 vaccination in UK children: a dynamic transmission model.

BACKGROUND: Despite the significant impact of longstanding pediatric pneumococcal conjugate vaccine (PCV) use in the United Kingdom (UK), pneumococcal disease burden remains substantial. Higher valent vaccines, such as the 20‑valent PCV (PCV20), could reduce this burden, yet their value in the contemporary UK setting has not been fully assessed using recent data. RESEARCH DESIGN AND METHODS: An age-structured dynamic transmission model used post-COVID-19 UK epidemiology (2001-2023) to compare pediatric PCV20 (2 + 1 and 1 + 1) with PCV13 (1 + 1) and PCV15 (1 + 1). Over 10 years, we assessed co

East Asia as a sentinel: risk-stratified corticosteroid therapy for paediatric macrolide-resistant Mycoplasma pneumoniae pneumonia.

BACKGROUND: Mycoplasma pneumoniae is a major cause of paediatric community-acquired pneumonia. The prevalence of macrolide-resistant M. pneumoniae (MRMP) has exceeded 80% in East Asia, posing significant challenges to clinical management. Increasing evidence suggests that disease progression is driven not only by direct microbial injury but also by host-mediated hyperinflammatory responses. METHODS: This structured narrative review was conducted using PubMed and Embase databases to identify relevant studies published between 2000 and 2025. Clinical studies, biomarker analyses and regional gui

Beware the tick: ehrlichiosis in a lung transplant recipient.

Ehrlichiosis is a rare tick-borne zoonosis caused by Ehrlichia chaffeensis, E. ewingii and related species. In solid organ transplant recipients, immunosuppression can result in atypical presentations, delayed diagnosis and poor outcomes. The disease remains under-recognised among lung transplant recipients despite its potentially fatal course. A man in his early 70s with a history of bilateral lung transplantation for chronic obstructive pulmonary disease presented with a high-grade fever and shortness of breath. Bronchoalveolar lavage (BAL) culture grew Pseudomonas aeruginosa and meropenem w
