Necrotising fasciitis of the neck, chest and abdomen arising from odontogenic Ludwig's angina in a healthy young adult.

Odontogenic infections are commonly considered a cause of Ludwig's angina; their progression to necrotising fasciitis (NF) is rare and often associated with significant morbidity and mortality. We report a case of a previously healthy patient in his 20s, a heavy smoker, who developed extensive cervicothoracic NF after wisdom tooth extraction. He initially presented with neck swelling and pain, with stable vital signs and no airway compromise. Within 24 hours, he deteriorated abruptly with respiratory distress and septic shock. CT showed widespread fascial involvement with emphysema tracking in

Blepharoconjunctivitis mimicking conjunctival tumor associated with Streptococcus intermedius sinusitis: case report and literature review.

Streptococcus intermedius, a commensal bacterium in the human oral cavity, can occasionally cause severe infections in deep tissues. The patient was referred because of a conjunctival tumor. She had severe nasal cavity and periocular tissue inflammation that persisted for over a year. Microbiological examination of the nasal and ocular specimens identified S. intermedius as the pathogenic strain. The inflammation and the conjunctival mass subsided after systemic and topical administration of a susceptible antibiotic. Smear microscopy of the eye and nasal discharge was useful for the differenti

Bacterial profile and antimicrobial resistance in patients with pulmonary infection: a retrospective tertiary hospital-based study.

BACKGROUND: Pulmonary infections remain a major cause of global morbidity and mortality, particularly in low- and middle-income countries (LMICs). The rapid emergence of antimicrobial resistance (AMR) has significantly limited therapeutic options and increased healthcare burden. This study aimed to investigate the bacterial profile and AMR patterns among patients with confirmed pulmonary infections at a tertiary referral hospital in Mongolia, where data from tertiary care settings and internationally disseminated studies on AMR are still scarce, especially in the context of pulmonary infection

Fluoroquinolone Use and Risk of Pneumothorax in Adults Hospitalized With Community Acquired Pneumonia: A Retrospective Cohort Study.

OBJECTIVES: The objective of this study is to examine the relationship between fluoroquinolone exposure and pneumothorax risk in patients hospitalized with pneumonia. METHODS: This retrospective cohort study used the TriNetX US Collaborative Network. We defined two mutually exclusive cohorts of hospitalized adults based on exposure to antibiotic classes commonly used to treat community acquired pneumonia: fluoroquinolone (ciprofloxacin, moxifloxacin, levofloxacin, and ofloxacin) versus nonfluoroquinolone (amoxicillin, azithromycin, ceftriaxone, and doxycycline). Patients with connective tissu

Pulmonary Mucormycosis Presenting as Non-resolving Pneumonia in an Elderly Patient With Uncontrolled Diabetes Mellitus: A Case Report.

Pulmonary mucormycosis is an uncommon but highly aggressive opportunistic fungal infection associated with significant morbidity and mortality, particularly in immunocompromised individuals. Early diagnosis remains challenging due to its nonspecific clinical and radiological presentation, often mimicking bacterial pneumonia or other pulmonary infections. We report the case of a 78-year-old woman with poorly controlled type 2 diabetes mellitus who presented with a prolonged history of productive cough, pleuritic chest pain, and intermittent fever. Despite treatment with broad-spectrum antibioti

A Cross-sectional Study on Sputum Bacteriology and Antibiogram with Inflammatory Marker, Clinical, and Functional Profile in Exacerbators of Chronic Obstructive Pulmonary Disease.

INTRODUCTION: Globally, chronic obstructive pulmonary disease (COPD) ranks third in terms of morbidity and mortality. Chronic obstructive lung disease is a respiratory illness caused by partial or complete obstruction of the airflow. It is one of the joint conditions that can be treated, and it is marked by tissue degradation and a progressive restriction of airflow. It was linked to structural alterations in the lungs brought on by long-term inflammation caused by exposure to harmful particles or gases. AIM: To determine the sputum bacteriology in Exacerbations of hospitalized COPD patients

Application of multiplex PCR-based nanopore sequencing for rapid detection and surveillance of antimicrobial-resistant Mycoplasma bovis in Japanese dairy cattle.

UNLABELLED: Mycoplasma bovis causes mastitis and respiratory disease, leading to economic losses in the dairy industry. Conventional culture and antimicrobial susceptibility testing require approximately 2 weeks, promoting empirical antimicrobial use and emergence of resistant strains, highlighting the need for rapid detection methods. Resistance-associated mutations have been detected in rrl and rrs (for macrolides, lincosamides, tetracyclines, and phenicols) and gyrA and parC (for fluoroquinolones), providing targets for a rapid prediction. We developed a multiplex PCR-based nanopore sequenc

Longitudinal epidemiology of respiratory viruses in hospitalized patients in Oman (2017-2025).

BACKGROUND: Respiratory virus epidemiology in hospitalized patients is influenced by host susceptibility, seasonality, and health system disruptions; however, longitudinal patterns across major population-level events remain incompletely defined. METHODS: We conducted a single-center longitudinal study of 6776 hospitalized patients in Oman from 2017 to 2025. Respiratory viruses were detected using multiplex real-time PCR and analyzed descriptively by age group, season, and epidemiological period (pre-pandemic, pandemic, post-pandemic), without adjustment for testing intensity over time. [Mechanisms of traditional Chinese medicine in combating drug-resistant bacterial infections].

Drug-resistant bacterial infections have emerged as a critical global public health challenge, characterized by high mortality rates, substantial healthcare costs, and rapid transmission characteristics that pose a severe test to modern medical practice. Traditional Chinese medicine(TCM), with its comprehensive theoretical framework and extensive clinical experience, demonstrates unique advantages in infectious disease management. Particularly in addressing drug-resistant bacterial infections, TCM has achieved remarkable progress by integrating its fundamental principle of "strengthening

L-type pyocins inhibit the BAM complex to kill without cell entry.

Many antibiotics are ineffective against the Gram-negative pathogen Pseudomonas aeruginosa because of intrinsic defence mechanisms, such as the impermeable bacterial outer membrane. Here, we show that protein antibiotics called L-type pyocins kill P. aeruginosa by inhibiting the β-barrel assembly machinery (BAM) complex at the cell surface, halting outer-membrane protein assembly. Using single-particle cryo-electron microscopy, we show that L-type pyocins bind a surface-exposed region of BamA and deploy a C-terminal peptide that competitively inhibits the BAM complex, demonstrating that cell e

Direct viral invasion and tumor-like pulmonary nodules: A fatal case of mpox in a patient with advanced HIV disease.

While mpox is typically a self-limiting zoonosis, individuals with advanced human immunodeficiency virus type 1 (HIV-1) infection are at increased risk for severe visceral complications and high mortality. We report a fatal case of fulminant mpox pneumonia in a 38-year-old male with advanced HIV-1 Infection and severe immunosuppression (CD4+ T-cell count <100 cells/µL). The patient initially presented with characteristic cutaneous lesions but rapidly progressed to dyspnea and respiratory failure. Serial chest imaging revealed diffuse, solid perivascular nodules and patchy consolidations were h

YWHAZ acts as a regulatory node in Streptococcus pneumoniae-induced pulmonary inflammation.

BACKGROUND: Tyrosine 3-monooxygenase/tryptophan 5-monooxygenase activation protein zeta (YWHAZ, 14-3-3ζ), a key regulator of intracellular signaling and inflammatory responses, has not been investigated in Streptococcus pneumoniae (S.pn) infection. METHODS: We first employed Western blotting to detect the expression and activation of YWHAZ in murine RAW264.7 macrophages and a mouse model of S.pn pneumonia. A small-molecule inhibitor BV02 was used to pharmacologically block YWHAZ function both in vitro and in vivo. Subsequent evaluations included the production of inflammatory cytokines via EL
