Evaluating detection of Histophilus somni immunoglobulin-binding protein A DR2 Fic: A species-specific gene target for recombinase polymerase amplification relative to long-read sequencing of respiratory samples from feedlot calves.

Histophilosis is an important cause of morbidity and mortality as well as antimicrobial use in feedlot cattle across North America. Detection of Histophilus somni by culture is challenging, and there is no standardized tool for distinguishing isolates that carry virulence factors most likely to contribute to disease. The DR2 repeat of H. somni-associated virulence factor 'immunoglobulin-binding protein A' (ibpA DR2) harbors a Fic domain that mediates host cell cytotoxicity and is essential for histophilosis. For rapid detection of ibpA DR2 in extracted DNA, we developed a real-time recombinase

Optimizing Clinical Decision Support for Antibiotic Prescribing in Pediatric Acute Respiratory Tract Infections: A Usability Study.

BACKGROUND: Inappropriate antibiotic prescribing contributes to antimicrobial resistance. Antimicrobial stewardship (AMS) programs increasingly rely on electronic health record (EHR)-based clinical decision support (CDS) alerts to guide evidence-based care, yet poorly designed alerts can increase cognitive burden and contribute to burnout. Usability testing offers a structured approach to improving alert design and adoption. OBJECTIVES: This study aimed to evaluate and iteratively refine interruptive CDS alerts developed to support appropriate antibiotic prescribing for pediatric acute respir

Bacterial respiratory infections in children: advances in diagnostic strategies.

INTRODUCTION: Respiratory tract infections (RTIs) are among the most common causes of medical consultation in children. Although the majority of upper respiratory tract infections are viral and self-limiting, bacterial pathogens remain responsible for a relevant proportion of cases and may require targeted antimicrobial therapy. Accurate microbiological diagnosis is therefore essential to guide clinical management and support antimicrobial stewardship. AREAS COVERED: This review discusses the etiology and clinical presentation of pediatric respiratory infections and examines current diagnosti

Dynamics of clinical Klebsiella pneumoniae strains over the COVID-19 pandemic in Qingdao, China.

The dynamics of multidrug-resistant pathogens in response to large-scale societal and environmental disruptions remain poorly understood. Here, we investigated 528 Klebsiella pneumoniae clinical isolates collected from a tertiary hospital in Qingdao, China, spanning 2016 to 2023, with a primary focus on 2018-2023, across pre-, during-, and post-COVID-19 pandemic periods. All isolates underwent whole-genome sequencing. We identified SL258 and SL23 as dominant sequence lineages, with prevalent serotypes including K1 and O1. A wide range of resistance and virulence determinants were detected, par

Establishment of a chinchilla model of Achromobacter xylosoxidans-induced otitis media.

UNLABELLED: Achromobacter xylosoxidans (Ax) is an emerging opportunistic pathogen associated with chronic respiratory infections, particularly in individuals with cystic fibrosis; however, its role in otitis media (OM) and the mechanisms that underlie middle ear infection remain poorly defined. Despite its initial isolation from middle ear effusions decades ago, no experimental models of Ax-induced OM have been established. Here, we report the development of a reproducible chinchilla model of Ax-induced OM, which, to our knowledge, represents the first experimental model of Ax-induced middle e

Intraspecific diversity of Staphylococcus aureus populations isolated from cystic fibrosis respiratory infections.

UNLABELLED: Long-term associations within an individual host allow bacterial populations to diversify over time, sometimes resulting in the coexistence of multiple species or variants of the same species. Traditional methods for identifying infective agents generally involve isolating a single pathogen and potentially even a single colony from a given sample. While screening samples for virulent or difficult-to-treat pathogens is an important part of informing clinical treatment and correlative research, these reductive methods alone do not provide researchers or healthcare providers with the

[Otorhinolaryngologic infections in the post-pandemic context: What can we learn from the literature?].

BACKGROUND: The epidemiology of various respiratory infections changed due to the impact of the COVID-19 pandemic. Lockdowns and other containment measures (nonpharmacologic interventions, NPI) led to a decline in virus circulation, resulting in fewer cases at hospitals and practices. Once restrictions were eased, a rebound effect was observed worldwide, resulting in an increase in viral and severe invasive bacterial infections. OBJECTIVE: Various case series on different ear, nose, and throat (ENT) infections have been conducted in the context of the pandemic in Germany. These studies should
