MTHFR, Pneumonia, and Thrombosis in Children: A Gene-Infection Interaction.

Severe pediatric pneumonia associated with thromboembolism is notable, yet its pathogenic mechanism remains elusive, suggesting a potential nonnegligible role of genetic susceptibility. Five previously healthy children were admitted with severe pneumonia, primarily caused by Mycoplasma pneumoniae and other pathogens. All patients exhibited markedly elevated inflammatory markers and D-dimer levels. Crucially, whole-exome sequencing identified the methylenetetrahydrofolate reductase (MTHFR) c.665C>T mutation in all patients (2 homozygous, 3 heterozygous). Computed tomographic angiography confirm

Immunogenicity and efficacy of a Mycoplasma hyopneumoniae/porcine circovirus type 2 combination vaccine against experimental Mycoplasma hyopneumoniae infection in pigs.

Vaccination is an important control measure against infections with Mycoplasma hyopneumoniae (M. hyopneumoniae), the primary pathogen of enzootic pneumonia in pigs. The present study investigated local and systemic humoral and cell-mediated immune responses and efficacy upon vaccination of piglets with a combined vaccine containing inactivated M. hyopneumoniae and porcine circovirus 2d (Cirbloc® M Hyo, Ceva), followed by experimental M. hyopneumoniae infection (D21). Forty-five piglets were allocated to three groups, namely the not-vaccinated and infected (C) (n = 20), vaccinated and infected

Co-infection with macrolide-resistant and macrolide-susceptible Bordetella pertussis strains in an infant: a case report.

BACKGROUND: Bordetella pertussis infection causes severe respiratory illness, particularly in young infants. Although macrolides are recommended as the first-line treatment, macrolide-resistant strains have emerged in several countries. In Japan, a nationwide pertussis epidemic occurred in 2025, during which numerous infections were reported not only due to macrolide-susceptible B. pertussis (MSBP) but also due to macrolide-resistant strains (MRBP). CASE SUMMARY: A 2-month-old female developed a persistent cough and was admitted to the pediatric intensive care unit (PICU), where she received

Helicobacter pylori outer membrane vesicle-induced hsa-miR-302a-3p and hsa-miR-184 promote the occurrence and development of gastric cancer.

Helicobacter pylori (H. pylori) is a major gastric cancer pathogen. Recent studies have linked H. pylori infection to microRNA (miRNA) dysregulation. H. pylori outer membrane vesicles (OMVs) support bacterial survival and pathogenesis, but the OMV-miRNA interactions remain unclear. To address this lack of understanding, we co-cultured the H. pylori strain NCTC11637 and its OMVs with the gastric epithelial cell line GES-1 and gastric cancer cell lines SGC-7901 and HGC-27. Reverse transcription quantitative polymerase chain reaction (RT-qPCR) showed hsa-miR-302a-3p upregulation in the GES-1 line

Serum IL-6, IL-8, and Th/Ts ratio for severity assessment and short-term clinical outcome risk stratification in pediatric Mycoplasma pneumoniae pneumonia: insights into inflammation-immune imbalance.

Mycoplasma pneumoniae pneumonia (MPP) exhibits significant clinical heterogeneity in children. This study investigated the clinical utility of serum interleukin-6 (IL-6), interleukin-8 (IL-8), and the T helper/T suppressor (Th/Ts) cell ratio for evaluating disease severity and 1-month short-term clinical outcomes in pediatric MPP. We retrospectively enrolled 194 hospitalized children with MPP without co-infection (102 mild, 92 severe; 171 favorable short-term clinical outcome, 23 poor short-term clinical outcome). Core biomarker levels were compared between groups. Diagnostic performance was e

Stenotrophomonas mexicanensis subsp. fluminensis subsp. nov., a new subspecies isolated from a clinical sample at a hospital in Rio de Janeiro, Brazil, during COVID-19 pandemic.

The genus Stenotrophomonas exhibits wide genetic diversity, and the Stenotrophomonas maltophilia complex (Smc) is formed by multiple cryptic species. In this study, we performed taxonomic characterization of SM006 strain, isolated from lung tissue of an 83-year-old female patient at a hospital in Rio de Janeiro. Initially identified as S. maltophilia by Matrix-Assisted Laser Desorption/Ionization - Time of Flight Mass Spectrometry and 16 S rRNA gene sequencing, the strain was phylogenetically positioned within Smc. Multilocus sequence typing identified a novel allelic profile, leading to the a

Metagenomic investigation of VIM-type metallo-β-lactamase-producing multidrug-resistant Pseudomonas aeruginosa associated with a hospital outbreak across multiple hospital units in Saudi Arabia.

BACKGROUND: Healthcare-associated infections (HAIs) caused by multidrug-resistant (MDR) Pseudomonas aeruginosa (P. aeruginosa) represent a public health challenge, particularly when associated with VIM-type metallo-β-lactamases (MBLs), which limit therapeutic options. Conventional microbiological methods may underestimate resistance determinants and transmission dynamics. Long-read metagenomic sequencing is a promising approach for Pathogen detection, resistome characterization, and genomic surveillance. OBJECTIVES: The study's objectives were to characterize the resistome, including detectio

Distribution and genetic diversity of Mannheimia species, including recently described taxa, in cattle in Japan.

The genus Mannheimia comprises Gram-negative bacteria that primarily inhabit the upper respiratory tract of ruminants. Currently, 10 species are recognized within the genus Mannheimia, among which M. haemolytica has been extensively studied due to its association with bovine respiratory disease (BRD). In contrast, information on other species, particularly the four species proposed since 2021 (M. pernigra, M. bovis, M. cairinae, and M. indoligenes), remains extremely limited. In this study, we retrospectively analyzed 210 Mannheimia isolates obtained from nasal swabs of cattle in Japan between

Hypereosinophilia With Pseudomonas aeruginosa Infection in Patients With Asthma on Anti-Inteleurkin-5 Biologics.

Biologic therapies targeting IL-5 or its receptor reduce eosinophilia and exacerbations in asthma. Breakthrough eosinophilic attacks are usually considered biologic failure. We report 2 patients with severe asthma receiving mepolizumab or benralizumab who developed relapsing hypereosinophilia associated with Pseudomonas aeruginosa airway infection. Case 1 was a 71-year-old woman in Canada with severe eosinophilic asthma and COPD overlap whose eosinophils suddenly rebounded to 1,400 cells/μL during an acute exacerbation while on mepolizumab, coinciding with P aeruginosa infection and resolving

Staphylococcus aureus superantigens drive a concentration-dependent neutrophilic or eosinophilic pulmonary inflammation in mouse models of cystic fibrosis.

Staphylococcus aureus (SA), the most common cystic fibrosis (CF) respiratory pathogen, is uniquely capable of producing superantigen (SAg) exotoxins, which are recognized as the most potent activators of the immune system and inducers of inflammation. Although a high frequency of CF SA isolates can produce SA-SAgs, their role in the immunopathogenesis of CF lung disease is unexplored. Therefore, to investigate the role of SA-SAgs in pulmonary inflammation in vivo, we crossed HLA-DR3 transgenic mice with CF gut corrected (HLA-DR3.CFGC) mice and βENaC transgenic (HLA-DR3.ENAC) mice expressing th

Immune protection and persistence in Pseudomonas aeruginosa infections by pyruvate cross-feeding.

Chronic infections by Pseudomonas aeruginosa in people with cystic fibrosis are characterized by persistent inflammation and oxidative stress, yet the mechanisms enabling bacterial persistence are not fully understood. Here, we identify a persistence mechanism mediated by pyruvate secretion resulting from mutations in the pyruvate dehydrogenase complex in clinical isolates of P. aeruginosa, with putative analogous mutations also identified in Staphylococcus aureus, Haemophilus influenzae and Stenotrophomonas maltophilia. These mutations lead to elevated extracellular pyruvate, which dampens ho

Pulmonary mRNA-LNP vaccines for rapid and durable protection against bacterial infection.

Pulmonary bacterial infections remain a major clinical challenge. Although vaccination reduces infection rates and mortality, the vulnerable post-vaccination immunity gap can still result in infection and vaccine failure. In addition, effective vaccines are unavailable for many clinically important bacterial pathogens. Here, we report a pulmonary mRNA-lipid nanoparticle (mRNA-LNP) vaccine incorporating an ionizable lipid engineered for localized high-level expression, which elicits both rapid and durable protection against bacterial lung infections in female mice, effectively bridging this cri

Pediatric empiric antibiotic use and resistance patterns at a single tertiary care hospital in Tanzania.

BACKGROUND: Despite growing microbiology capacity in Tanzanian public hospitals, pediatric-specific antibiograms remain rare, and patterns of empiric antibiotic use in emergency care are poorly described compared to high-income settings. Understanding resistance profiles and empiric prescribing practices is essential for developing successful pediatric antimicrobial stewardship (AMS) programs. OBJECTIVES: To assess baseline pediatric empiric antibiotic use in the emergency medical department (EMD) and microbiology results from children at Muhimbili National Hospital (MNH), a tertiary care cen

Novel High-Efficacy Antimicrobial Peptides Derived from Myxinidin and their Therapeutic Efficacy in Bacterial Pneumonia.

Myxinidin is a natural antimicrobial peptide with a short sequence and low inherent toxicity, though its activity remains suboptimal for clinical application. To overcome this limitation, we performed rational modifications by substituting key residues to stabilize the α-helical conformation while increasing hydrophobicity and net positive charge. Among the designed analogs, M14 and M21 exhibited potent broad-spectrum activity against all tested bacterial strains, along with high selectivity for bacterial over mammalian cells. Both peptides demonstrated significant therapeutic efficacy in a mo
