Bacteriophage Therapy in Cystic Fibrosis: Emerging Strategies for Managing Chronic Pseudomonas aeruginosa Infections.

Pseudomonas aeruginosa is the leading cause of chronic respiratory infection in people with cystic fibrosis (pwCF) and remains a major driver of morbidity and premature mortality. The unique environment of the CF lung promotes persistent colonisation, biofilm formation, and the emergence of multidrug-resistant (MDR) phenotypes, severely limiting the effectiveness of conventional antibiotic therapies. Despite aggressive and prolonged antibiotic regimens, eradication of chronic P. aeruginosa infection is rarely achieved, and repeated exposure accelerates resistance development. Consequently, the

Association of Peak Expiratory Flow with Pulmonary Fungal Infection in Hospitalized Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease: A Single-Center Retrospective Study.

OBJECTIVE: Pulmonary fungal infections are increasingly recognized as important complications of acute exacerbations of chronic obstructive pulmonary disease (AECOPD), but early identification remains challenging. This study investigated the independent association between peak expiratory flow (PEF) and pulmonary fungal infection in hospitalized patients with AECOPD. METHODS: This retrospective study enrolled 729 adult inpatients with AECOPD admitted to a tertiary hospital in Chongqing from January to December 2023. PEF was measured after admission and before treatment initiation. Pulmonary f

In vitro antiviral activity of the RdRp inhibitor 4'-fluorouridine against a porcine teschovirus clinical isolate.

Porcine Teschovirus (PTV) is an economically significant swine pathogen with a global distribution that can cause severe neurological, respiratory, and reproductive diseases. However, no specific vaccines or antiviral drugs are currently available. To address the absence of specific antiviral therapies for PTV, this study aimed to identify a clinically promising antiviral candidate through in vitro compound screening. We began by isolating and purified a contemporary PTV strain (PTV/Shandong/2024) from a clinical swab of a dead piglet. We also established a strain-specific RT-qPCR assay to acc

[Specific respiratory tract infections in acute cough-when is targeted diagnosis and treatment required?].

In most cases, acute cough is caused by a self-limiting respiratory infection. However, symptoms, pre-existing conditions, or environmental factors may indicate a cause requiring treatment; red flags point to dangerous courses, the NEWS2 score can assess the risk of septic deterioration, and the CRB-65 score supports decision-making regarding hospital admission in pneumonia. Pneumonia cannot be reliably diagnosed clinically but is unlikely in the absence of clinical indicators. Laboratory tests can support the decision for antibiotic treatment, while the diagnosis is confirmed by chest X‑ray.

Jet-injected combinational adjuvants potentiate naked mRNA vaccines for protection against respiratory viral infections in mice.

The use of lipid nanoparticles (LNPs) to enhance delivery and immunogenicity of mRNA vaccines raises safety concerns due to strong reactogenicity. Developing naked mRNA vaccines, which can be delivered through liquid jet-injection to the skin, may offer a safer alternative, but their immunogenicity has been limited. To address this challenge, we screened clinically relevant adjuvants to enhance the efficacy of N1-methyl-pseudouridine-modified naked mRNA jet-injection vaccines in mouse models. Although many adjuvants showed little or inhibitory effects on inducing humoral or cellular immunity-d

Applying normalized p-control charts for early detection of local COVID-19 outbreaks.

The COVID-19 pandemic underscored the urgent need for early detection of local outbreaks to enable better resource allocation and effective mitigation. Outbreak identification largely depended on local infection rates, which were highly variable and influenced by daily testing volumes. Effective pandemic management requires accurate, adaptive methods that reflect the dynamic and localized nature of these rates, ensuring timely and appropriate interventions. This study seeks to improve the early detection and localization of COVID-19 outbreaks through Statistical Process Control (SPC) and healt

Pandemic Politics: Developing Vaccines in Canada's Anti-vax Heartland.

The COVID-19 pandemic in Saskatchewan is a story of extremes. Through most of 2020, low case numbers, effective policy, and broad agreement among government officials, health care workers, and the public sustained a consensus that strict measures and vaccination offered the best responses; however, that consensus collapsed. The province shifted from a focal point for Canadian vaccine development to a staging ground for an anti-vaccine movement that took root on social media and then in street protests. Premier Scott Moe moved from a pro-vaccine, pro-consensus stance to a confrontational positi

Clinical Insights Into Bloom Syndrome With Dentinogenesis Imperfecta: A Report of a Rare Case.

Bloom syndrome, a rare autosomal recessive genetic disorder, is characterized by a predisposition to various types of cancer, a distinctive facial appearance, and a range of other physical and developmental abnormalities. Individuals afflicted with this condition typically exhibit proportional growth deficiency, with adult heights rarely exceeding 5 feet and characteristic narrow facies with a prominent nose and ears. The syndrome is also marked by photosensitive facial skin, which can be exacerbated by exposure to sunlight, and an increased susceptibility to respiratory tract infections and o

Community health workers and promotores de salud: a tiered framework for infectious disease prevention and care among California agricultural workers.

Agricultural workers in the United States face a disproportionate burden of infectious diseases, including tuberculosis, HIV, coccidioidomycosis, viral hepatitis, and COVID-19, driven by occupational exposures, crowded housing, poverty, and limited healthcare access. Despite this burden, farmworkers have among the lowest rates of healthcare utilization of any US working population, and traditional clinic-based delivery models are poorly suited to the realities of agricultural labor. Community health workers (CHWs), known as promotores de salud in Latino communities, offer a well-established st

The mechanism of lentinan in preventing and treating acinetobacter baumannii pneumonia via immunomodulation.

BackgroundAcinetobacter baumannii pneumonia (ABP) presents a clinical challenge due to drug resistance and immune evasion. The specific mechanism by which the natural immunomodulator lentinan (LNT) exerts anti-infective effects remains unclear. This study aimed to investigate whether LNT functions through the PINK1/Parkin-mediated mitophagy pathway.MethodsAn immunosuppressed mouse model of ABP was established, and the mice were treated with LNT alone or in combination with the mitophagy inhibitor mdivi-1. Therapeutic efficacy was evaluated through survival analysis. The bacterial load in lung

Respiratory Viral Infections in Febrile Children With Lymphoreticular Malignancies: A Single-Centre Descriptive Study.

Children with lymphoreticular malignancies are highly vulnerable to respiratory viral infections (RVIs) because of disease- and chemotherapy-related immunosuppression, yet data on their epidemiology, clinical spectrum, and outcomes from resource-limited settings remain scarce. This study aimed to characterize the seasonal distribution, clinical presentation, and outcomes of RVIs other than SARS-CoV-2 in febrile children with lymphoreticular malignancies at a tertiary care centre in India. In this prospective, descriptive, cross-sectional study (May 2023-November 2024), 33 children under 12 yea

KLRG1+CD28⁻CD57+ Ag85A-specific CD8+ T cells mark a severity-associated terminal-differentiation imprint in pulmonary tuberculosis.

BACKGROUND: Pulmonary tuberculosis (PTB) is characterized by prolonged host-pathogen interaction, chronic granulomatous inflammation, and sustained exposure to Mycobacterium tuberculosis (Mtb) antigens. How Mtb antigen-specific CD8+ T cell differentiation states relate to clinical disease activity and severity remains incompletely defined. This study investigated the clinical relevance of a terminal differentiation-associated phenotype within Ag85A-specific CD8+ T cells across clinically defined states of Mtb infection and disease. METHODS: A total of 124 participants were enrolled, including

Late cardiopulmonary morbidity in patients with Hodgkin lymphoma: a retrospective nationwide registry study.

Hodgkin lymphoma (HL) is a B-cell neoplasm with a cure rate of 90%. Previous data on late morbidity in patients with HL cover the era of older treatment modalities. This real-world study consisted of 2912 patients diagnosed with HL between 2000 and 2019 in Finland and aimed to clarify the incidence of late-onset cardiopulmonary morbidity compared to matched controls. Patient data were collected from four nationwide Finnish healthcare registries. We reported a higher occurrence of coronary artery disease in patients aged < 50 years at diagnosis than in controls (RR 3.80 (2.16-6.74), p < 0001).

Late-Diagnosed Cystic Fibrosis with Co-Occurring CFTR 3849+10kbC>T and 5T Variants: A Case Report.

Background: The deep-intronic variant 3849+10kbC>T preserves residual CFTR function and is recurrently associated with pancreatic sufficiency, normal or borderline sweat chloride and diagnosis beyond early childhood. A co-occurring poly-T 5T allele adds a second layer of uncertainty: its penetrance depends on the TG repeat carried in cis, and its contribution to the phenotype can be weighed only once its phase relative to the second variant has been established. Case Presentation: A boy with recurrent respiratory infections from the age of three was diagnosed with cystic fibrosis at 12 years,

D-Dimer in Viral Respiratory Infections: Established Evidence in COVID-19 and Emerging Evidence in Other Respiratory Viruses.

Viral respiratory infections remain a major cause of global morbidity and mortality, with severe disease increasingly recognized to involve profound interactions between inflammation, endothelial dysfunction, coagulation activation, and immunothrombosis. D-dimer, a fibrin degradation product and established biomarker of fibrin turnover, has gained considerable attention because of its strong association with adverse outcomes in coronavirus disease 2019 (COVID-19). This narrative review compares the evidence for D-dimer in COVID-19 with that available for other respiratory viruses and deliberat

Tracking eco-evolutionary patterns among oropharyngeal pathobiome members in a pediatric cystic fibrosis cohort.

UNLABELLED: Polymicrobial airway colonization is common in individuals with cystic fibrosis (CF). Pathogen communities typically follow an ecological succession in which early colonizers, such as Haemophilus influenzae and Staphylococcus aureus, are later joined by Pseudomonas aeruginosa. Although adaptation to the lung environment is well described for single pathogens in adult people with CF, much less is known about the role of pathogen interactions and how changes in individual pathogens influence community dynamics at the early stages of disease. To address these questions, we combined ge
