Application of Macrophage Subtype Analysis in Acute Lung Injury/Acute Respiratory Distress Syndrome.

Acute lung injury (ALI)/acute respiratory distress syndrome (ARDS) is a common critical illness. Supportive therapy is still the main strategy for ALI/ARDS. Macrophages are the predominant immune cells in the lungs and play a pivotal role in maintaining homeostasis, regulating metabolism, and facilitating tissue repair. During ALI/ARDS, these versatile cells undergo polarization into distinct subtypes with significant variations in transcriptional profiles, developmental trajectory, phenotype, and functionality. This review discusses developments in the analysis of alveolar macrophage subtypes

Elastic static power, its correlation with acute respiratory distress syndrome severity: A Bayesian post-hoc analysis of the Mechanical Power Day cross-sectional trial.

OBJECTIVE: The relationship between different power equations and the severity of acute respiratory distress syndrome (ARDS) remains unclear. This study aimed to evaluate various power equations: total mechanical power, total elastic power (comprising elastic static and elastic dynamic power), and resistive power, in a cohort of mechanically ventilated patients with and without ARDS. Bayesian analysis was employed to refine estimates and quantify uncertainty by incorporating a priori distributions. DESIGN: A Bayesian post-hoc analysis was conducted on data from the Mechanical Power Day study.

Deep learning enhanced transmembranous electromyography in the diagnosis of sleep apnea.

Obstructive sleep apnea (OSA) is widespread, under-recognized, and under-treated, impacting the health and quality of life for millions. The current gold standard for sleep apnea testing is based on the in-lab sleep study, which is costly, cumbersome, not readily available and represents a well-known roadblock to managing this huge societal burden. Assessment of neuromuscular function involved in the upper airway using electromyography (EMG) has shown potential to characterize and diagnose sleep apnea, while the development of transmembranous electromyography (tmEMG), a painless surface probe,

Immune dysregulation as a driver of bronchiolitis obliterans.

Bronchiolitis obliterans (BO) is a disease characterized by airway obstruction and fibrosis that can occur in all age groups. Bronchiolitis obliterans syndrome (BOS) is a clinical manifestation of BO in patients who have undergone lung transplantation or hematopoietic stem cell transplantation. Persistent inflammation and fibrosis of small airways make the disease irreversible, eventually leading to lung failure. The pathogenesis of BO is not entirely clear, but immune disorders are commonly involved, with various immune cells playing complex roles in different BO subtypes. Accordingly, the US

Consequences of Early Maternal Deprivation on Neuroinflammation and Mitochondrial Dynamics in the Central Nervous System of Male and Female Rats.

Early life stress (ELS) is associated with an increased risk for neuropsychiatric disorders, and both neuroinflammation and mitochondrial dysfunction seem to be central to mental health. Herein, using an animal model of ELS, a single episode of maternal deprivation (MD, 24 h on pnd 9) extensively documented to elicit behavioural anomalies in male and female Wistar rats, we investigated its consequences in terms of neuroinflammation and mitochondrial dynamics in the prefrontal cortex (PFC) and the hippocampal formation (HCF). MD differentially affected the brain content of cytokines: MD induced

Succinate Regulates Endothelial Mitochondrial Function and Barrier Integrity.

Endothelial dysfunction is a hallmark of several pathological conditions, including cancer, cardiovascular disease and inflammatory disorders. In these conditions, perturbed TCA cycle and subsequent succinate accumulation have been reported. The role of succinate as a regulator of immunological responses and inflammation is increasingly being recognized. Nevertheless, how endothelial cell function and phenotype are altered by elevated intracellular succinate has not been addressed yet. Thus, we employed numerous in vitro functional assays using primary HUVECs and diethyl succinate (DES), a cel

Potential Use of GLP-1 and GIP/GLP-1 Receptor Agonists for Respiratory Disorders: Where Are We at?

Chronic respiratory disorders are the third leading cause of mortality globally. Consequently, there is a continuous pursuit of effective therapies beyond those currently available. The therapeutic potential of the glucagon-like peptide-1 (GLP-1) and the glucose-dependent insulinotropic polypeptide/GLP-1 (GIP/GLP-1) receptor agonists extends beyond the regulation of glycemia, including glucometabolic, cardiovascular, and renal effects, rendering them viable candidates, due to their mechanisms of action, for the possible treatment of respiratory disorders. This manuscript aims to provide a comp

Exploring the Interplay of Gut Microbiota and Systemic Inflammation in Pediatric Obstructive Sleep Apnea Syndrome and Its Impact on Blood Pressure Status: A Cross-Sectional Study.

Obstructive sleep apnea syndrome (OSAS) is prevalent among children and is associated with elevated blood pressure (BP), posing a risk for future hypertension and cardiovascular diseases. While the roles of gut microbiota and systemic inflammation in OSAS pathogenesis are recognized in adults and animal models, their impact on pediatric BP remains less understood. This cross-sectional study explored the relationships between polysomnographic parameters, gut microbiota, systemic inflammation, and BP in 60 children with OSAS. Significant associations between specific microbial profiles-including

Thermal Cameras for Continuous and Contactless Respiration Monitoring.

Continuous respiration monitoring is an important tool in assessing the patient's health and diagnosing pulmonary, cardiovascular, and sleep-related breathing disorders. Various techniques and devices, both contact and contactless, can be used to monitor respiration. Each of these techniques can provide different types of information with varying accuracy. Thermal cameras have become a focal point in research due to their contactless nature, affordability, and the type of data they provide, i.e., information on respiration motion and respiration flow. Several studies have demonstrated the feas

Machine learning-based forecast of Helmet-CPAP therapy failure in Acute Respiratory Distress Syndrome patients.

BACKGROUND AND OBJECTIVE: Helmet-Continuous Positive Airway Pressure (H-CPAP) is a non-invasive respiratory support that is used for the treatment of Acute Respiratory Distress Syndrome (ARDS), a severe medical condition diagnosed when symptoms like profound hypoxemia, pulmonary opacities on radiography, or unexplained respiratory failure are present. It can be classified as mild, moderate or severe. H-CPAP therapy is recommended as the initial treatment approach for mild ARDS. Even though the efficacy of H-CPAP in managing patients with moderate-to-severe hypoxemia remains unclear, its use ha

Expression and Regulation of Hypoxia-Inducible Factor Signalling in Acute Lung Inflammation.

Hypoxia-inducible factors (HIFs) are central regulators of gene expression in response to oxygen deprivation, a common feature in critical illnesses. The significant burden that critical illnesses place on global healthcare systems highlights the need for a deeper understanding of underlying mechanisms and the development of innovative treatment strategies. Among critical illnesses, impaired lung function is frequently linked to hypoxic conditions. This review focuses on the expression and regulation of HIF signalling in experimental models of acute lung injury (ALI) and clinical studies in cr

Use of Bipolar Nerve Stimulator as an Alternative to Neural Integrity Monitor During Hypoglossal Nerve Stimulator Placement.

Hypoglossal nerve stimulation is a key step in insertion of a hypoglossal nerve stimulator for sleep apnea. The hypoglossal nerve stimulator works through assessment of apneic events that result in stimulation of the protrusion fibers of the hypoglossal nerve causing protrusion of the tongue and opening of the oropharynx resulting in ability for the patient to adequately breath. Therefore, during insertion of the hypoglossal nerve stimulator, a key portion of the procedure is inclusion of the protrusive fibers of the hypoglossal nerve and exclusion of the retrusive fibers. In the past, this wa
