Clinical phenotype profile and risk factors for sinusitis and obstructive sleep apnea in children with adenoid hypertrophy.

BACKGROUND: Adenoid hypertrophy (AH) is a common childhood condition frequently associated with multiple comorbidities. However, its specific clinical phenotype and the independent risk factors for key complications, particularly in children requiring surgery, remain inadequately elucidated. METHODS: This retrospective cohort study included 161 children undergoing surgery for AH. Demographic data, disease severity indicators, comorbidities, and laboratory results were collected. Descriptive statistics, chi-square tests, and multivariate logistic regression models were used for data analysis.

Continuous Positive Airway Pressure Versus Mandibular Advancement Devices Impact on Cardio-Metabolic Outcomes in Patients With Obstructive Sleep Apnea: A Systematic Review and Meta-Analysis of Randomised Controlled Trials.

Obstructive sleep apnea contributes to cardiovascular morbidity, and its treatment may mitigate this risk. Continuous positive airway pressure and mandibular advancement devices are established therapies, but their comparative cardiometabolic effects remain uncertain. We conducted a systematic review and meta-analysis of randomised controlled trials from multiple major databases through May 2024, pooling dichotomous outcomes as risk ratios and continuous outcomes as mean differences with 95% confidence intervals using StataMP version 17. Across 14 randomised trials including 1241 patients, CPA

Effects of high-dose supplemental oxygen therapy on pulmonary haemodynamics at 3250 m in high-altitude residents at risk for pulmonary hypertension.

QUESTION ADDRESSED BY THE STUDY: Hypoxic pulmonary vasoconstriction increases systolic pulmonary artery pressure (sPAP) and may contribute to high-altitude pulmonary hypertension (HAPH). While supplemental oxygen therapy (SOT) lowers sPAP during acute hypoxic exposure, its haemodynamic effects in permanent high-altitude residents remain unclear. In particular, the relative contribution of fixed vascular remodelling to chronically elevated sPAP is uncertain. PARTICIPANTS AND METHODS: In this open-label, non-randomised, sequential interventional trial, Kyrgyz highlanders living >2500 m with tri

GLP-1 receptor agonists for obstructive sleep apnea: a meta-analysis of randomized controlled trials.

PURPOSE: Weight reduction is recommended in all overweight and obese patients with obstructive sleep apnea (OSA). Glucagon-like peptide-1 (GLP-1) receptor agonists-based therapies have demonstrated clinical benefits in reducing weight and could potentially improve OSA outcomes. We aim to analyze the efficacy and safety of GLP-1 receptor agonists-based therapies in patients with moderate-to-severe OSA. METHODS: We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) that compared GLP-1 receptor agonists-based therapies versus control in adults with moderate-to

Beyond Snoring, Sleepiness and Surgery: Update on the Prevalent Symptoms of and Treatment Effects in all Age Groups With Pediatric Obstructive Sleep Apnea.

INTRODUCTION: Screening and treatments for pediatric obstructive sleep apnea (pOSA) often use adult symptoms and conventional surgical approaches. This study tested the hypothesis that symptoms and treatment efficacy for OSA differ across age groups. METHODS: Single-center cohort study of 0 to 18-year-olds who were managed and underwent polysomnography (PSG) in a pediatric-dedicated sleep medicine center from 2020 to 2024. RESULTS: Of 491 children, 489 had pOSA, 57.7% were male, the median age was 6 years old (IQR1-3, 4-9), and 41% already had an adeno- or adenotonsillectomy prior to the PSG

Evolving Breathing Support Strategies in Infants Born Preterm: An Overview for Primary Care Clinicians.

Primary care clinicians who provide medical homes for neonatal intensive care unit (NICU) graduates may benefit from an enhanced understanding of new breathing support strategies available in NICUs. Understanding the severity of a patient's respiratory course in the NICU may help anticipate the patient's tolerance of future viral illnesses. Primary care clinicians may also serve as the leaders of neonatal resuscitation teams in referring hospitals. Understanding new equipment options in the delivery room setting and the rationale for these changes can help resuscitation teams adopt new practic

Phenotype-Guided Surfactant Therapy in ARDS: From Failed Replacement to Targeted Biological Intervention, Narrative Review.

PURPOSE OF REVIEW: Acute respiratory distress syndrome (ARDS) is associated with profound pulmonary surfactant dysfunction, yet clinical trials of exogenous surfactant have not demonstrated consistent outcome benefits in adults. This review reassesses these failures in light of current understanding of ARDS heterogeneity, surfactant biology and intrapulmonary drug delivery. RECENT FINDINGS: Surfactant dysfunction in ARDS reflects not only quantitative depletion but also inhibition by plasma proteins, altered lipid and protein composition, impaired synthesis and recycling, epithelial injury an

Glymphatic function in humans: indicators, modulators, and measurement strategies.

PURPOSE: The glymphatic system is an important pathway for brain waste clearance and is increasingly implicated in neurodegenerative, cerebrovascular, traumatic, and sleep-related disorders. Clinical translation remains challenging because glymphatic function cannot currently be comprehensively or directly quantified in vivo in humans, and assessment relies on heterogeneous measures differing in physiological specificity and measurement directness. This review integrates human glymphatic indicators, physiological modulators, and measurement strategies within a unified framework. METHODS: A na

Presumed transdermal superwarfarin toxicity in a pest control worker.

A man in his early 60s presented with dyspnoea superimposed on a 2-month to 3-month history of spontaneous mucocutaneous bleeding. Laboratory testing revealed unmeasurably elevated international normalised ratio (INR) and activated partial thromboplastin time. CT of the neck demonstrated supraglottic soft tissue swelling with severe airway narrowing, requiring intubation for airway protection. Further history revealed occupational exposure to rodenticides during pest control work without consistent use of personal protective equipment. Markedly reduced activity of vitamin K-dependent clotting

Cuproptosis and the bioinorganic basis of copper-induced cell death.

Copper is an essential yet potentially toxic biological trace element, whose functions in cells arise from its unique redox and coordination chemistry. The same chemical properties that make copper indispensable for a variety of fundamental biological processes, including respiration and antioxidant defense, also render it highly toxic when intracellular concentrations exceed homeostatic levels. Recently, a copper-dependent form of regulated cell death (RCD) termed cuproptosis has been identified, in which excess copper triggers cytotoxicity by directly binding the metabolic machinery of mitoc

Advances in MRI of glymphatic system dysfunction associated with obstructive sleep apnea and enlarged perivascular spaces.

Obstructive sleep apnea (OSA) is closely associated with an increased risk of cerebral small-vessel disease and cognitive impairment. OSA not only impairs cognitive function but also promotes the occurrence and development of enlarged perivascular spaces (EPVS) by impairing glymphatic system (GS) function. Perivascular spaces (PVS) are an important imaging marker of cerebral small-vessel disease and a morphological indicator of the functional status of the GS. However, the pathogenesis of OSA-related EPVS remains unclear, with GS dysfunction considered to be at its core. Magnetic resonance ima

Longitudinal NODDI-DTI assessment of brain recovery following CPAP in obstructive sleep apnea: Tissue-compartment changes in white and gray matter.

OBJECTIVES: Continuous positive airway pressure (CPAP) can partially reverse brain abnormalities associated with obstructive sleep apnea (OSA), but the biological substrates underlying this recovery remain unclear. We investigated whether diffusion MRI could identify tissue-compartment changes associated with brain recovery following CPAP. METHODS: Thirteen adults with OSA underwent paired brain MRI before and after 3 months of CPAP treatment. Diffusion tensor imaging (DTI) and neurite orientation dispersion and density imaging (NODDI) were analyzed using tract-based spatial statistics (TBSS;
