Susceptibility to systemic autoimmunity rather than the presence of the HLA-DR4 peptide binding domain alone leads to severe inflammatory arthritis following inhalation of crystalline silica.

BACKGROUND: The mucosal origins hypothesis of rheumatoid arthritis (RA) posits that inhalant exposures, such as cigarette smoke and crystalline silica (c-silica), trigger immune responses in the lungs that contribute to joint disease onset. However, the relationship between inhalants, lung inflammation, and inflammatory arthritis remains poorly understood. RESULTS: This study compared the development of inflammatory arthritis in two genetically susceptible mouse strains, the BXD2/TyJ (BXD2) and chimeric HLA-DR4-IE transgenic (DR4-Tg), following delivery of c-silica to the lungs via oropharyng

From Dyspnea to Diagnosis: Neutrophilic Pleural Effusion With Elevated Adenosine Deaminase As the First Clue to New-Onset Rheumatoid Arthritis.

Rheumatoid arthritis (RA) rarely announces itself with a pleural effusion, yet when it does, the biochemical and pleural fluid profile often leads to a suspicion of infectious etiology at first. We describe a 44‑year‑old woman whose initial manifestation of seropositive RA was a large, unilateral, neutrophil‑rich, adenosine‑deaminase (ADA)-positive pleural effusion. The case draws attention to a diagnostic pitfall: elevated pleural fluid ADA-although classically associated with tuberculous pleuritis-can also occur in rheumatoid pleuritis, highlighting that ADA levels must be interpreted in the

Assessment of myositis specific antibodies in primary chronic arthritis: single centre prospective study.

OBJECTIVES: Among the myositis specific antibodies (MSAs), the antisynthetase (anti-ARS) and the anti-MDA5 antibodies are those more frequently characterised by the occurrence of joint involvement. We aim to define the prevalence of MSAs in patients with established rheumatoid arthritis (RA), psoriatic arthritis (PsA), or undifferentiated polyarthritis (UPA). METHODS: From January 2021 to December 2024, all RA, PsA and UPA patients prospectively followed in our Early Arthritis Clinic (EAC), were evaluated. Changes in diagnosis, clinical/laboratory signs of muscle/lung/skin involvement at onse

Pulmonary Veno-Occlusive Disease in Rheumatoid Arthritis: A Rare Pathological Entity Independent of Interstitial Lung Disease.

We present the case of an 83-year-old woman with a long-standing history of rheumatoid arthritis (RA) who was found collapsed at home. The patient presented with cardiopulmonary arrest and could not be resuscitated. A postmortem examination was performed to determine the cause of death. Postmortem computed tomography (CT) ruled out intracranial hemorrhage but revealed diffuse bilateral pulmonary consolidations and signs of bronchial obstruction. The autopsy revealed severe pulmonary edema and marked right ventricular hypertrophy. Microscopic examination of the lungs demonstrated characteristic

Circulating Activin A and Follistatin-like Proteins in Rheumatoid Arthritis with Interstitial Lung Disease: A Cross-Sectional Comparative Study.

Background/Objectives: Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) represents one of the major contributors to morbidity and mortality in Rheumatoid arthritis (RA), yet its underlying molecular mechanisms remain incompletely defined. Activin A, a member of the transforming growth factor-β (TGF-β) superfamily, has emerged as a key regulator of inflammation, fibroblast activation, and tissue remodeling. However, its role in RA patients with interstitial lung disease (ILD) has not been fully elucidated. We aimed to investigate circulating levels of Activin A, Follistatin-Li

Advances in the Diagnosis of Rheumatoid Arthritis-Associated Interstitial Lung Disease: Integrating Conventional Tools and Emerging Biomarkers.

Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) is one of the most common extra-articular manifestations of rheumatoid arthritis (RA) and a leading cause of mortality in RA patients. The diverse and nonspecific clinical presentations of RA-ILD make early diagnosis particularly challenging. In recent years, with a deeper understanding of the pathogenesis of RA-ILD and rapid advancements in medical imaging, artificial intelligence (AI) technologies, and biomarker research, notable progress has been achieved in the diagnostic approaches for RA-ILD. This review summarizes the la

Therapeutic Potential of Stem Cells in the Treatment and Management of Immunomodulatory Disorders.

INTRODUCTION: Immunomodulatory disorders, such as autoimmune diseases, inflammatory conditions, and viral infections, stem from immune system dysregulation and often resist conventional therapies. Stem cells, particularly mesenchymal (MSCs) and hematopoietic stem cells (HSCs), possess immunomodulatory and regenerative properties, offering a promising therapeutic alternative. METHODOLOGY: A systematic literature review was conducted using databases, including PubMed, Scopus, Web of Science, and Google Scholar, for studies published between 1996 and 2025. A total of 287 articles were screened,

Identifying the strongest novel gene-respiratory disease interactions for rheumatoid arthritis risk.

OBJECTIVE: We aimed to identify the strongest individual gene-respiratory interactions for rheumatoid arthritis (RA) risk. METHODS: This case-control study used the Mayo Clinic (MC) and Mass General Brigham (MGB) biobanks for discovery and validation, respectively. We matched criteria-confirmed incident RA cases to four non-RA controls on age, sex, and duration electronic health record (EHR) history. Genetic exposures included known RA risk alleles. We identified respiratory diseases by codes and defined "respiratory burden" as the total number of respiratory codes divided by EHR duration bef

Rheumatoid arthritis and risk of lung diseases in East Asians and Europeans: Evidence from a Mendelian randomization study.

BACKGROUND: Rheumatoid arthritis might increase the risk of lung diseases, such as interstitial lung disease and lung cancer. However, the causal relationships remained unclear. Herein, we utilized Mendelian randomization to study the causal relationships. MATERIAL AND METHODS: Genetic Data from European ancestry and East Asian ancestry were used. The main Mendelian randomization approaches included inverse variance weighted, weighted median, and Mendelian randomization-Egger. We also conducted a series of analyses to validate the robustness of the Mendelian randomization estimates by integra

Causal association between rheumatoid arthritis and interstitial lung disease: A two-sample bidirectional Mendelian randomization study.

Epidemiological observations suggest a potential causal relationship between rheumatoid arthritis (RA) and interstitial lung disease (ILD), and vice versa. Alternatively, this association could be due to simple co-occurrence of RA and ILD resulting from shared population genetic or environmental risk factors. However, the causal nature of this relationship remains uncertain. This study aims to investigate the genetic causality between RA and ILD. Two-sample bidirectional Mendelian randomization (MR) was employed to test the causal effects of RA on ILD and ILD on RA, utilizing genome-wide assoc
