Pathogenesis and immune regulation of rheumatoid arthritis-associated interstitial lung disease: from basic research to clinical implications.

Interstitial lung disease (ILD) is one of the most common extra-articular manifestations of rheumatoid arthritis (RA). Some patients with RA-ILD may develop progressive pulmonary fibrosis, leading to severe impairment of lung function and respiratory failure, which impacts quality of life and can even be life-threatening. This review identified genetic susceptibility, environmental factors, and immune dysregulation as key contributors to the etiology and pathogenesis of RA-ILD. We highlight that autoantibodies, adaptive immune abnormalities, and tertiary lymphoid organ formation significantly

Effect of manual diaphragmatic release on ventilatory function and quality of life in women with rheumatoid arthritis: a randomized controlled trial.

INTRODUCTION: Rheumatoid arthritis (RA) could lead to several respiratory dysfunctions, impairing quality of life. OBJECTIVE: To evaluate the effect of manual diaphragmatic release (MDR) added to aerobic training (AT) on ventilatory function, diaphragmatic motion, aerobic capacity, and quality of life, compared with AT alone, in women with RA. METHODS: Eighty women with RA were equally assigned to either the MDR group, which received MDR and AT, or the control group, which received AT only, for 8 weeks. Outcomes included forced vital capacity (FVC) and total lung capacity (TLC), diaphragmati

Emerging prospective evidence shows that lung ultrasound can reliably detect rheumatoid arthritis-associated ILD: need for standardised protocols and consideration in future guidelines.

Lung ultrasound (LUS) is emerging as a sensitive tool for RA-ILD detection. Though HRCT is the gold standard, LUS may allow earlier risk identification, fewer scans and better care. Future guidelines should standardise protocols, definitions and training. https://bit.ly/4hbA7tq

Lung Ultrasound for the Detection of Radiologic Progression of Interstitial Lung Abnormalities in Patients With Rheumatoid Arthritis.

OBJECTIVE: Interstitial lung disease (ILD) is an important extra-articular manifestation of rheumatoid arthritis (RA) associated with high mortality and morbidity. Lung ultrasonography (LUS) has recently emerged as a possible screening tool. We assessed the role of LUS in monitoring radiologic progression and the development of new interstitial lung abnormalities (ILAs). METHODS: In this prospective pilot study, patients with RA underwent high-resolution computed tomography (HRCT), pulmonary function tests (PFTs), symptom assessment, and LUS at baseline and 12 to 18 months later. B-lines were

Microbiota-driven mechanisms in multisystem diseases: integrative evidence across cardiovascular, metabolic, neurological and autoimmune disorders.

The human microbiota represents one of the body's most influential biological systems, engaging in constant metabolic, immunological, and neuroendocrine communication with the host. Disruption of this intricate ecosystem, or dysbiosis, has emerged as a fundamental determinant in the onset and progression of numerous chronic diseases. This review consolidates contemporary evidence on how alterations in microbial composition, metabolite production, and barrier integrity contribute to pathophysiological changes across multiple organ systems. Gut-derived metabolites-including short-chain fatty aci

Challenging the status quo for therapeutic clinical trials in fibrotic interstitial lung diseases.

BACKGROUND: Fibrotic interstitial lung diseases (fILDs) are chronic lung diseases with rising incidence and prevalence. Patients affected become increasingly breathless, dependent on supplementary oxygen and generally die from respiratory failure. Despite the availability of antifibrotic therapies, treatment options that can positively impact the quality of life of patients with fILD remain limited. OBJECTIVE: To review the current clinical trial landscape in fILD, and explore emerging strategies to accelerate the development of therapies that improve outcomes that are meaningful to patients.

Microbiome in connective tissue diseases associated interstitial lung disease.

Microbiome consists of a large community of bacteria, yeast, protozoa, and viruses that co-exist in symbiosis with human hosts. Changes in microbiome, named "dysbiosis," alter the interplay between microbiome and immune system triggering inflammation and contributing to the pathogenesis of connective tissue diseases (CTD). Interstitial lung diseases (ILD) are a group of pulmonary disorders characterized by lung tissue fibrosis and impaired lung function. The existence of a gut-lung axis is well demonstrated; however, it is not established whether gastrointestinal dysbiosis contributes to ILD d

Regulatory Roles of Long Non-Coding RNAs in Methotrexate Pharmacology: Mechanistic and Translational Insights.

BACKGROUND: Methotrexate (MTX) remains a cornerstone therapy in autoimmune diseases and oncology; however, substantial interindividual variability in efficacy and toxicity persists. While variability in MTX pharmacokinetics (PK) and pharmacodynamics (PD) has been linked to transporters, folate-cycle enzymes, and intracellular polyglutamate accumulation, the contribution of long non-coding RNAs (lncRNAs) as regulatory modifiers of these processes is not yet systematically defined. OBJECTIVE: To synthesize current evidence on lncRNA-mediated regulation of MTX response and to organize these inte

Physical Fitness Is Negatively Associated With DNA Methylation-Based Risk of Aging-Related Diseases.

Physical fitness is a key determinant of health, yet the molecular pathways linking fitness to the risk of aging-related diseases remain unclear. We examined associations between DNA methylation-based protein level estimates (EpiScores) and five fitness traits-VO2max, GripStrength, JumpMax, body mass index (BMI), and cognition-in a cohort of 290 mostly old individuals (mean age of 60 ± 11 years). EpiScores for 109 plasma proteins were obtained using the MethylDetectR tool and tested for associations with fitness traits. We found 33 significant fitness predictor-EpiScore associations independen

Elevated serum Slit3 independently predicts disease activity and interstitial lung disease in rheumatoid arthritis.

INTRODUCTION: Rheumatoid Arthritis (RA) is a chronic inflammatory disease often linked to Interstitial Lung Disease (ILD). Slit guidance Ligand-3 (Slit3), a member of the axon guidance molecule superfamily, mediates a series of biological activities, including the regulation of the inflammatory response and fibrosis. This study explored Slit3's clinical significance as a serum biomarker for RA-ILD. METHODS: Between November 2022 and April 2024, patients diagnosed with RA were included in the Foshan First People's Hospital. The clinical data of patients were collected from hospital records, an

Mitochondrial dysregulation in rheumatoid arthritis: From pathogenic mechanisms to therapeutic innovations.

The pathogenesis of rheumatoid arthritis (RA) involves complex interactions among genetic predisposition, environmental factors, and immune dysregulation. Emerging evidence indicates that mitochondrial dysfunction plays a central role in RA pathology and its systemic complications, including rheumatoid arthritis-associated interstitial lung disease. This review systematically examines the synergistic effects of mitochondrial DNA mutations, oxidative stress, and metabolic reprogramming in promoting RA progression, with particular focus on activation mechanisms of key signaling pathways includin

Multidisciplinary management of interstitial lung disease in autoimmune rheumatic diseases: an Italian Delphi consensus.

BACKGROUND: Although guidelines recommend a multidisciplinary team (MDT) approach for the diagnosis and management of systemic autoimmune rheumatic disease-associated interstitial lung disease (SARD-ILD), they lack guidance on MDT composition and function. This Delphi consensus project aimed to define a shared MDT model for managing patients with SARD-ILD. METHODS: A questionnaire was circulated to an expert panel of 77 Italian pulmonologists, rheumatologists, immunologists, and internal medicine specialists, with statements rated over two voting rounds using a 5-point Likert scale. RESULTS: R
