Use of lung ultrasound surface wave elastography in patients with interstitial lung disease.

OBJECTIVES: To assess whether lung ultrasound surface wave elastography (LUSWE), a non-invasive measure of lung stiffness, correlates with pulmonary function and high-resolution CT (HRCT) patterns in suspected interstitial lung disease (ILD). We hypothesised that LUSWE-derived lung stiffness would be associated with functional impairment and radiological ILD patterns. SETTING: Prospective, single-centre pilot study conducted in a tertiary referral ILD unit in Barcelona, Spain. PARTICIPANTS: 68 consecutive patients (41 men and 27 women) with suspected ILD were enrolled between June 2020 and F

Comprehensive characterisation of individuals with fibrotic interstitial lung disease: baseline insights from the INJUSTIS study.

BACKGROUND: Interstitial lung disease (ILD) represents a group of complex parenchymal conditions characterised by varying clinical trajectories. The It's Not JUST Idiopathic Pulmonary Fibrosis Study seeks to identify genetic, proteomic and clinical biomarkers that distinguish rapidly progressive fibrotic phenotypes from stable phenotypes irrespective of aetiology. This manuscript presents baseline insights from the recruited cohort. METHODS: In this prospective, longitudinal study, participants with fibrotic ILDs, including idiopathic pulmonary fibrosis (IPF), fibrotic hypersensitivity pneumo

Characterization of Pulmonary Dysfunction in Systemic Juvenile Idiopathic Arthritis Using Xenon and Proton MRI.

INTRODUCTION: Systemic juvenile idiopathic arthritis-associated lung disease (SJIA-LD) is increasingly recognized and associated with potentially life-threatening complications. Diagnosis is challenging as SJIA-LD is complex and frequently presents with subtle or no respiratory symptoms, necessitating CT imaging or other tests to detect budding dysfunction. We hypothesized that xenon and proton lung MRI could identify SJIA-LD-associated structural changes and functional deficits. METHODS: Thirty-one participants aged 5-15 (16 healthy, 7 SJIA without diagnosed lung disease [SJIA-non-LD], 8 SJI

Quantitative Phase-resolved Functional Lung MRI Prediction of Disease Progression in Connective Tissue Disease-associated Interstitial Lung Disease.

Background Connective tissue disease-associated interstitial lung disease (CTD-ILD) is a spatially and temporally heterogeneous disease. Determining whether CTD-ILD is in a progressive stage is crucial for guiding clinical management. Purpose To determine whether CTD-ILD is associated with ventilation and perfusion parameters quantified with phase-resolved functional lung (PREFUL) MRI and whether these functional parameters can help predict CTD-ILD progression. Materials and Methods In this prospective study performed from May 2024 to April 2025, healthy participants without pulmonary disease

Association of prior interstitial lung disease awareness with knowledge, attitudes, and practices in Saudi Arabia: a national cross-sectional study.

INTRODUCTION: Interstitial lung diseases (ILDs) are a heterogeneous group of diffuse parenchymal lung disorders, including idiopathic interstitial pneumonias, connective-tissue-disease-associated ILD and sarcoidosis. Patients often experience prolonged diagnostic delays and non-specific symptom profiles that overlap with more prevalent conditions. While these delays are well documented, the contribution of low public awareness to these delays is not well described in the Middle East. We aimed to determine the prevalence of ILD awareness in Saudi Arabia and evaluate its independent association

Diagnostic methods of rheumatoid arthritis-associated interstitial lung disease: a systematic review.

Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) is a potentially life-threatening extra-articular complication affecting 10-60% of patients with rheumatoid arthritis. Despite its clinical significance, the optimal diagnostic strategy remains uncertain. This systematic review assessed the diagnostic value of currently available methods for detecting RA-ILD. The review followed PRISMA guidelines. Databases were searched for studies evaluating imaging modalities (chest X-ray, high-resolution computed tomography [HRCT], lung ultrasound), pulmonary function tests (forced vital ca

Pulmonary hemorrhage as a first manifestation of Sjögren's disease in a young male adult.

Sjögren's syndrome (SS) is a chronic autoimmune disorder primarily affecting exocrine glands, characterized by dry mouth (xerostomia) and dry eyes (keratoconjunctivitis sicca). However, systemic manifestations, including pulmonary complications, are increasingly recognized. Diffuse alveolar hemorrhage (DAH) is a rare but life-threatening presentation of SS, typically associated with vasculitis or coagulation abnormalities. We report the case of a 19-year-old male presenting with dyspnea and hemoptysis, later diagnosed with primary Sjögren's syndrome (pSS). Initial clinical assessment revealed

The Role of Nailfold Videocapillaroscopy in the Diagnosis and Monitoring of Interstitial Lung Disease Associated with Rheumatic Autoimmune Diseases.

Interstitial lung disease (ILD) is a severe complication of certain connective tissue diseases (CTDs) such as systemic sclerosis (SSc), mixed connective tissue disease (MCTD), idiopathic inflammatory myopathies (IIM), rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE), and it is associated with nailfold videocapillaroscopy (NVC) changes and increased morbidity and mortality rates. Early diagnosis is crucial in order to prevent the progression of ILD, prevent respiratory failure and enhance the patient's overall quality of life. The most common paraclinical investigations are high

Validation of a semi-quantitative method to assess interstitial lung disease severity and progression in systemic sclerosis by standard and low-dose HRCT scans.

BACKGROUND: While the presence of distinct imaging abnormalities by high-resolution CT (HRCT) defines interstitial lung disease (ILD), there is a relative lack of validated methods to quantify these abnormalities in clinical practice, limiting ILD severity and progression assessments. We aimed to validate a semi-quantitative method for lung fibrosis assessment in patients with systemic sclerosis associated ILD (SSc-ILD) by standard and low-dose HRCT, considering lung structure and function as integral components of ILD evaluation. METHODS: SSc patients from Oslo and Zurich with HRCT images, p

Surgical pathology of diffuse parenchymal lung disease in patients with polymyalgia rheumatica.

AIMS: Polymyalgia rheumatica (PMR) is a chronic autoimmune disorder that mainly affects older adults. Pulmonary disease in PMR is rare but may be under-recognized and pathological descriptions thereof are few. We aimed to characterize diffuse parenchymal lung disease (DPLD) in PMR. METHODS AND RESULTS: Institutional archives were searched for patients having PMR and DPLD with lung tissue sampling. After excluding cases with infection, concomitant rheumatoid arthritis, or smoking-related DPLD only, 11 patients (9 women, median age 75 years) were enrolled. Clinical history and pathology slides

Long-term clinical prognosis of anti-aminoacyl-tRNA synthetase antibodies and interstitial lung disease.

INTRODUCTION: Anti-aminoacyl tRNA synthetase (anti-ARS) antibody is the most common myositis-specific antibody subtype. Anti-ARS antibody-positive myositis is often complicated by interstitial lung disease (ILD), but the clinical progression of anti-ARS antibody-positive ILD (ARS-ILD) remains unclear. METHOD: A prospectively collected, single center longitudinal myositis database was used to retrospectively investigate 131 patients with ARS-ILD based on subtypes of anti-ARS antibodies (Jo-1, PL-7, PL-12, EJ, OJ, and KS). We investigated the occurrence and associated risk factors for pulmonary

The Clinical Efficacy and Safety of Nintedanib in the Treatment of Interstitial Lung Disease Among Patients With Systemic Sclerosis: Systematic Review.

Systemic sclerosis (SSc) is predominantly characterized by an array of cutaneous manifestations including Raynaud's phenomenon, calcinosis, telangiectasias, and skin fibrosis contributing toward substantial morbidity and diminished quality of life. The monumental impact of the disease regarding mortality is due to its pulmonary involvement known as SSc-associated interstitial lung disease (SSc-ILD). Currently, treatment is chiefly directed toward impeding disease progression with the mainstay treatment approaches involving the utilization of cyclophosphamide, mycophenolate mofetil, rituximab,

Short telomere length is associated with accelerated lung disease progression in rheumatoid arthritis-associated interstitial lung disease.

BACKGROUND: Shorter leukocyte telomere length (LTL) has been reported in patients with rheumatoid arthritis (RA)-associated interstitial lung disease (ILD) and linked to increased disease severity and mortality in idiopathic pulmonary fibrosis, which shares similarities with RA-ILD. We aimed to evaluate the impact of short LTL on baseline respiratory disease severity, disease progression and survival in patients with RA-ILD. METHODS: Patients diagnosed with RA-ILD following multidisciplinary assessment were enrolled in a prospective French observational study. LTL was measured at enrolment us

A case of rapidly progressive interstitial lung disease.

We describe a rare presentation of rapidly progressive interstitial pneumonia with autoimmune features (IPAF) associated with MDA5 autoantibodies. Unlike typical MDA5-positive dermatomyositis, this patient lacked skin or muscle involvement, expanding the clinical spectrum of this antibody-associated disease. An elderly male presented with acute-onset dyspnea and hypoxemic respiratory failure. High-resolution computed tomography revealed diffuse ground-glass opacities and interstitial changes. Autoimmune screening identified strongly positive anti-MDA5 antibodies, without clinical or laboratory

Anti-synthetase Syndrome Presenting Primarily as Interstitial Lung Disease in a Young Adult: A Diagnostic Challenge in Acute Medicine.

Anti-synthetase syndrome (ASyS) is a rare autoimmune connective tissue disorder (CTD) characterized by the presence of autoantibodies targeting tRNA synthetase, most notably the anti-Jo1 antibody. Clinically, it typically manifests with interstitial lung disease (ILD), inflammatory myopathy, arthritis, Raynaud's phenomenon, and the so-called mechanic's hands. Diagnosis can be challenging due to clinical overlap with other pulmonary and rheumatological disorders. We report a case of a young woman in her early 20s who presented at our same day emergency care (SDEC) with a four-week history of ex
