Traumatic dental injuries in the primary dentition: exploratory medico-legal framework for temporary damage assessment.

Traumatic dental injuries (TDIs) in the primary dentition are frequent in early childhood and may lead to sequelae affecting both injured primary teeth and their permanent successors. They also raise medico-legal questions when premature loss of a primary tooth results in a time-dependent functional, aesthetic, and psychosocial impairment before the permanent successor can be evaluated. This retrospective single-center study reviewed the records of 419 children (262 males, 157 females; mean age 3.23 ± 1.85 years) treated for TDIs to the primary dentition between 2013 and 2022, comprising 428 t

Ectopic Third Molar in the Mandibular Condyle: An Extraoral Surgical Approach and 10-Year Follow-Up.

This report describes the surgical management and long-term follow-up of a dentigerous cyst associated with an ectopic mandibular third molar located in the condylar process. A 55-year-old female presented with pain, limited mouth opening, and a "cyst near the ear." Imaging revealed a radiolucent lesion in the left mandibular ramus associated with an impacted tooth in the condyle. Histopathologic analysis following incisional biopsy confirmed a dentigerous cyst. Given the lesion size and the inaccessible position of the tooth, surgical excision was performed through a preauricular approach und

Radiographic evolution of a dentigerous cyst associated with a deeply impacted mandibular first molar in a pediatric patient: a CBCT-based longitudinal case report with minimal-visit orthodontic management.

A deeply impacted mandibular first molar associated with a dentigerous cyst is an uncommon clinical presentation, particularly in pediatric patients. This case report describes a 9-year-old boy who presented with delayed eruption of the mandibular right first molar. Panoramic radiography revealed a well-defined unilocular radiolucent lesion associated with a deeply impacted tooth, while cone-beam computed tomography (CBCT) demonstrated buccolingual cortical expansion and thinning, displacement of the adjacent second molar germ, and close proximity to the inferior alveolar canal. Based on clini

Digital Evaluation of Pathologic Root Resorption in Primary Mandibular Molars and Its Implications on Succedaneous Unerupted Premolars.

BACKGROUND: Pulpal infection from primary molars may have periapical implications resulting in pathological root resorption (PRR). There is little clarity regarding its effects on succedaneous teeth. Therefore, the aim of the study was to evaluate the effects of PRR on the root formation and eruption of their succedaneous premolars. MATERIALS AND METHODS: Digital orthopantomograms (OPGs) were obtained of subjects aged between 6 and 9 years. A split-mouth study design was planned, wherein the affected side had to contain a carious mandibular primary molar (MPM) showing PRR. The corresponding t

Development and user-centric evaluation of a clinical decision support website application for esthetic crown lengthening planning.

A gummy smile and altered passive tooth eruption pose a facial aesthetic challenge. Esthetic Crown Lengthening [ECL] requires the precise integration of aesthetic principles [Golden Proportion] and periodontal diagnosis [Coslet Classification] to achieve optimal outcomes. The complexity of manual planning necessitates a standardized, objective approach. This study describes the development of a web-based Clinical Decision Support System [CDSS] for ECL planning. It evaluates user satisfaction and the perceived clinical utility of its diagnostic logic among dental professionals. A client-side we

Primary tooth emergence in young children is associated with a stress biomarker.

Early tooth emergence may reflect developmental stress exposures and compose part of a broader set of growth measures that may be accelerated in response to early stress exposure. We test the hypothesis that timing of primary tooth emergence is associated with stress exposure and stress biology in young children. Tooth emergence was tracked prospectively in a diverse sample of initially caries-free 1-3 year-olds; socio-economic and psychosocial determinants of oral health, oral microbiology, and salivary stress markers were assessed at baseline and at 6-monthly intervals for 2 years. The numbe

Surgical Removal of an Impacted Palatal Mesiodens Prior to Orthodontic Treatment: A Case Report.

An impacted mesiodens is the most common type of supernumerary tooth and may interfere with the eruption, alignment, and orthodontic movement of permanent maxillary incisors. Accurate localization is essential for safe surgical planning and minimizing damage to adjacent anatomical structures. This case report describes the diagnosis and surgical management of a palatally impacted mesiodens in a 14-year-old male patient referred for orthodontic treatment because of dental crowding and malalignment. Routine panoramic radiography revealed an impacted supernumerary tooth in the anterior maxilla, a

Graft materials and regenerative strategies in secondary alveolar bone grafting: A scoping review of randomized clinical trials.

BACKGROUND: Secondary alveolar bone grafting (SABG) places bone in the alveolar cleft, crucial for managing cleft lip and palate. SABG restores ridge continuity, aids tooth eruption, and stabilizes the maxilla. Autogenous iliac crest grafting is the standard treatment option. However, varied evidence on donor-site morbidity, graft resorption, and outcomes prompts exploring alternative grafts and adjuncts. OBJECTIVE: To systematically map and critically synthesize randomized controlled trial (RCT) evidence on graft materials, adjunctive biologics, and outcome assessment strategies in SABG, and

Innovative treatment strategies for impacted maxillary central incisor and case comparisons: Case report.

BACKGROUND: The present article compares innovative treatment strategies for impacted maxillary central incisors, emphasizing the critical importance of prioritizing root development over rapid crown alignment. METHODS: Two case reports using distinct clinical approaches are presented. Case 1 utilized a traditional combination of closed-eruption surgical exposure and conventional orthodontic traction, while Case 2 utilized a novel root-oriented traction technique, which ensures the development of tooth roots and utilizes the natural eruption power. RESULTS: Both techniques achieved successfu

Spatiotemporal distribution of α-smooth muscle actin-positive mesenchymal cells during tooth development and mechanical remodeling of the murine periodontal ligament.

The periodontal ligament (PDL) contains heterogeneous mesenchymal stem/progenitor cells that contribute to periodontal homeostasis and regeneration. However, the developmental origin and behavior of α-smooth muscle actin (αSMA)-positive cells remain poorly understood. This study investigated the spatiotemporal distribution of αSMA-positive cells during tooth development and their response to altered mechanical loading following extraction of the opposing mandibular molars in mice. Before tooth eruption, αSMA-positive cells were widely distributed throughout the dental follicles but became prog

Window decompression surgery for the treatment of large mandibular dentigerous cysts in the mixed dentition stage: a case report.

Dentigerous cyst is a common type of odontogenic jawbone cyst in clinical practice, often involving immature permanent tooth germ in children. Traditional cyst curettage may cause damage to the developing permanent tooth germ and surrounding important anatomical structures. This article reports a case of a large mandibular dentigerous cyst in a 9-year-old female child, which was treated with negative pressure drainage through window decompression surgery. During the operation, a lateral opening drainage tube was prepared and fixed with flowable resin. The cyst significantly shrank 6 months aft
