Bone age determination in a paediatric population as an indicator of nutritional status.

The United Mission to Nepal Tansen Project is in the hill country, 300 km West of Kathmandu. This 40-year-old service centre's major emphasis has been on clinical and community medicine and education. Agronomy, forestry, water resources development, adult literacy, etc., have also been part of the project's outreach. We had long noted that the children of all of the various tribal groups in the area are small for their age as compared with children in developed countries. Fracture care X-rays were examined to reveal delay in ossification and in physeal closure. Using the standards found in the

Maxillary sinus polyps secondary to dental extraction.

Polyps of the maxillary sinus were obtained from six patients who had reported upper tooth extraction with oroantral perforation prior to the development of symptoms, from 11 patients with chronic sinusitis, and from 12 patients with allergy. Histopathological features, scanning electron microscopy of the polyp epithelium and clinical data were compared in those groups of patients. The post-traumatic polyps differed from those of other aetiologies by showing the presence of granulomas, less numerous inflammatory cells with very few eosinophils, nearly normal surface epithelium (smaller surface

Fusobacterial infections: an underestimated threat.

The involvement of fusobacteria in a wide range of human and animal infection has long been recognised. Slow-growing anaerobes, often in polymicrobial culture, they are not always identified but are present mainly in the oropharynx, from where they are bloodborne to other sites or aspirated into the lung. Fusobacterium nucleatum is commonly found in periodontal disease and produces tissue irritants such as butyric acid, proteases and cytokines. It has strong adhesive properties due to the presence of lectins, and these outer-membrane proteins mediate adhesion to epithelia and tooth surfaces, a

Diagnosis and management of supra-esophageal complications of reflux disease.

Supra-esophageal reflux disease may be manifested in numerous ways, including reflux laryngitis, chronic cough, chronic sinusitis, and dental enamel loss. The mechanisms of pharyngeal and laryngeal reflux are not clearly defined, and standard reflux testing does not consistently demonstrate supra-esophageal reflux. The diagnosis is usually based on clinical suspicion when other causes of symptoms are not found and on the patient's response to empiric acid suppression. With the development of triple-probe pH monitoring, through which pharyngeal pH can be assessed along with esophageal pH, the p

Prevalence of tooth transposition, third molar agenesis, and maxillary canine impaction in individuals with Down syndrome.

Alterations in the size, morphology and number of teeth are among the many inherited disorders that have been reported in individuals with Down syndrome. By contrast, third molar agenesis and tooth transposition have been largely ignored and the prevalence of canine impaction has not been reported. The intention of this study was to quantify the occurrence of these anomalies in a group of individuals with Down syndrome, using standardized records, which included a clinical examination, dental casts, and a panoramic radiograph. The results show a notably high prevalence of third molar agenesis

Anti-adhesive strategies in the prevention of infectious disease at mucosal surfaces.

Binding of microbial cell surface adhesins to host receptor molecules is a critical early step in microbial infection and pathogenesis. Anti-adhesive strategies aimed at blocking this interaction offer an attractive means of preventing infection at an early stage. The strategy should reduce the likelihood of resistant strains of microorganisms emerging, since those that do not bind will not be subjected to sustained selective pressure, as may occur with antibiotic therapy. Three classes of adhesion-blocking agent have been investigated, namely anti-adhesin antibodies, adhesin analogues and rec

[Pacifier use: risks and benefits].

The use of a dummy as a comforting object in childhood is widespread in civilized societies. The advisability of this practice, as well as the shape, material, or time of use of these objects, is controversial. Data on the possible beneficial effects of these objects are numerous, but there is also a long list of disorders which have been associated with their use. Their soothing effect is the most widely recognized beneficial effect and the possible relationship with a lower incidence of sudden infant death syndrome is the newest. The most important risks of this nonnutritive sucking habit ar

A longitudinal epidemiologic study of signs and symptoms of temporomandibular disorders from 15 to 35 years of age.

AIMS: To study the development over 20 years of signs and symptoms of temporomandibular disorders (TMD) in an epidemiologic sample and to analyze possible correlations between these signs and symptoms and some other variables. METHODS: The original group comprised 135 randomly selected 15-year-old subjects who were examined clinically and by means of a questionnaire for signs and symptoms of TMD. The examination was repeated after 5, 10, and 20 years by the same methods. After 20 years, when the original group had reached the age of 35 years, 124 individuals (92%) could be traced, and they we

Relationship between premature loss of primary teeth and the development of temporomandibular disorders in children.

AIM: This study evaluated the prevalence of signs of temporomandibular disorders (TMD) in children with and without premature loss of primary teeth. SAMPLE: Fifty-eight children, aged 4-6 years, with missing primary molars, were compared with 58 age- and sex-matched control children with complete primary dentitions. METHOD: All the children underwent an examination that consisted of palpation and assessment of the joints and associated muscles for tenderness and joint sounds. Maximum vertical opening and deviation during jaw opening were also recorded. Children with any single anomalous find

Use of the feed additive sodium hexametaphosphate to prevent dental calculus in squirrel monkeys (Saimiri spp.).

Dental calculus and associated periodontal disease are serious clinical problems in captive squirrel monkeys. Calculus begins to appear as early as 2 years of age, with subsequent development of periodontal disease, dental abscessation, tooth loss and other sequelae. When used as a feed additive, sodium hexametaphosphate (HMP) retards the growth of calculus on previously cleaned teeth in rhesus monkeys, lemurs, and other species. We wanted to determine whether HMP would reduce dental calculus in squirrel monkeys (Saimiri spp.) whose teeth had not been pre-cleaned. The study animals were divide

The syndromic tooth--the aetiology, prevalence, presentation and evaluation of hypodontia in children with syndromes.

Hypodontia can provide significant challenges to the clinician in both diagnosis and management. It is important that each patient is assessed not only by the dental team but also has genetic counselling to determine whether the hypodontia is a stand-alone condition or is associated with other anomalies. The advances in the understanding of tooth development and genetic control of tooth morphology not only allows clinical research to broaden its understanding of tooth agenesis but provides a basis for the clinician to provide the optimum clinical care. This paper presents an overview of the ae

Suggestion can help.

One cannot practise dentistry without realising that for the patient, the control of pain and fear is extremely important. Modern technical advances have made painless dentistry a reality and yet research has shown that more people avoid dental treatment through fear of pain than all other factors combined. Dental surgeons and psychologists agree that patients frequently magnify their unpleasant dental experiences. There are deep-seated psychological reasons for this exaggerated fear; the mouth being a highly charged erotogenic region, is a primary zone of interaction with the environment and

Involvement of PTCH gene in various noninflammatory cysts.

Constitutional hemizygous inactivation of PTCH, the Shh signaling pathway gene that moderates the signal, manifests itself as nevoid basal cell carcinoma syndrome or Gorlin syndrome, a condition variably characterized by a number of developmental disorders and malformations, and by predisposition to some malignancies, basal cell carcinoma in particular. Loss of heterozygosity for the PTCH region was found several years ago in the epithelial lining of odontogenic keratocysts, the cyst type with highly increased incidence in nevoid basal cell carcinoma syndrome. This finding confirmed the expect

Biological functions of connexin genes revealed by human genetic defects, dominant negative approaches and targeted deletions in the mouse.

Gap junction channels in mammalian organs can be built up of at least 13 different connexin proteins, most of which are expressed in only few cell types, although many cells express more than one connexin protein. Recently, the consequences of missing or defective connexin proteins were studied in human patients with defects in connexin32 (Cx32; beta 1; X-linked Charcot-Marie-Tooth disease) or in Cx26 (beta 2; non-syndromic sensorineural deafness), and in mice with targeted deletions in the Cx26, Cx32, Cx37 (alpha 4), Cx43 (alpha 1), Cx46 (alpha 3) or Cx50 (alpha 8) genes. Some effects of domi

The spectrum of mutations in TBX3: Genotype/Phenotype relationship in ulnar-mammary syndrome.

Ulnar-mammary syndrome (UMS) is a pleiotropic disorder affecting limb, apocrine-gland, tooth, hair, and genital development. Mutations that disrupt the DNA-binding domain of the T-box gene, TBX3, have been demonstrated to cause UMS. However, the 3' terminus of the open reading frame (ORF) of TBX3 was not identified, and mutations were detected in only two families with UMS. Furthermore, no substantial homology outside the T-box was found among TBX3 and its orthologues. The subsequent cloning of new TBX3 cDNAs allowed us to complete the characterization of TBX3 and to identify alternatively tra

The gene for cherubism maps to chromosome 4p16.

Cherubism is an autosomal dominant disorder that may be related to tooth development and eruption. It is a disorder of age-related bone remodeling, mostly limited to the maxilla and the mandible, with loss of bone in the jaws and its replacement with large amounts of fibrous tissue. We have used a genomewide search with a three-generation family and have established linkage to chromosome 4p16. Three other families affected with cherubism were also genotyped and were mapped to the same locus. The combined LOD score is 4.21 at a recombination fraction of 0, and the locus spans an interval of app

Third molar management: a case against routine removal in adolescent and young adult orthodontic patients.

In the nearly two decades since the National Institutes of Health conference, controversy and uncertainty have continued with respect to the diagnosis and treatment of impacted, nondiseased third molars in adolescents and young adults. Articles published over the past 10 years have studied the issue from the vantage point of risk management. Those who favor prophylactic removal justify this action on three premises: 1. All impacted third molars are potentially pathologic; therefore, prophylactic removal reduces or eliminates risk of future disease. 2. The presence of third molars can cause lat

Dosage requirement of Pitx2 for development of multiple organs.

Pitx2 is a homeodomain transcription factor that is mutated in Rieger syndrome, a haploinsufficiency disorder affecting eyes and teeth. Pitx2 also has a postulated role in left-right axis determination. We assessed the requirements for Pitx2 directly by generating hypomorphic and null alleles. Heterozygotes for either allele have eye abnormalities consistent with Rieger syndrome. The ventral body wall fails to close in embryos homozygous for the null allele, leaving the heart and abdominal organs externalized and the body axis contorted. In homozygotes for either allele, the heart tube undergo

Clinical nosologic and genetic aspects of Joubert and related syndromes.

Joubert syndrome is an autosomal-recessive disorder characterized by cerebellar hypoplasia, hypotonia, developmental delay, abnormal respiratory patterns, and abnormal eye movements. The biochemical and genetic basis of Joubert syndrome is unknown and a specific chromosomal locus for this disorder has not been identified. Review of this disorder and related syndromes suggests that (1) hypoplasia of the cerebellar vermis in Joubert syndrome is frequently associated with a complex brain stem malformation represented as the "molar tooth sign" on magnetic resonance imaging, (2) the "molar tooth si
