Enamel fluorosis occurs when fluoride concentrations in or in the vicinity of the forming enamel are excessive during its pre-eruptive development. Fluoride concentrations in plasma, enamel and other tissues reflect the difference between intake and excretion, i.e. fluoride balance. In addition to the diet, modern sources of ingested fluoride include a variety of dental products, some of which have been identified as risk factors for fluorosis. Fluoride absorption is inversely related to dietary calcium which, at high concentrations, may cause net fluoride secretion into the gastrointestinal t

Periodontal diseases are infections. They have a number of properties in common with infections in other parts of the body, but have unique features resulting from the passage of the tooth through the soft tissue integument into the oral cavity. The tooth provides a solid, non-shedding surface for the colonization of potentially pathogenic bacterial species as well as a wide range of host-compatible species. Periodontal pathogens frequently colonize the periodontal area for prolonged periods of time prior to disease initiation. Disease is caused by a finite set of bacterial species leading to

We analyzed the development of 10 hips in 10 consecutive patients with neuromuscular disease (9 with spasticity, 1 with Charcot-Marie-Tooth disease) who had undergone Chiari osteotomy for painful hip subluxation or dislocation. The patients were 11 (5-19) years old at surgery and follow-up time was 8 (6-11) years. The Chiari osteotomy particularly improved and maintained femoral head coverage. These parameters did not show the postoperative deterioration noted in some other studies. The osteotomy did not improve femoral head lateral displacement. Throughout the postoperative period, the config

Measurements of the microbial growth dynamics in natural biofilm communities are almost non-existent. In a recent study, the biofilm formation on teeth was examined. A previously unknown active period of bacterial division occurred at a certain density of plaque bacteria on tooth enamel. The density-dependent cell-division phase of plaque formation contributed 90% of the biomass in the first 24 hrs of plaque formation. This suggested that growth was induced by the bacteria. In vitro assays were developed for rapid evaluation of the growth of surface-linked bacteria by the measurement of cellul

Microbial biofilms, where organisms are intimately associated with each other and a solid substratum through binding and inclusion within an exopolymer matrix, are widely distributed in nature and disease. In the mouth, multispecies biofilms are associated not only with dental plaque and tooth decay but also with soft tissues of the buccal cavity and with most forms of periodontal disease. Organization of micro-organisms within biofilms confers, on the component species, properties which are not evident with the individual species grown independently or as planktonic populations in liquid medi

Methodological problems in the low level lead studies are reviewed using the Aarhus lead study as an example. It is shown that a lead effect can be found in an area where the background blood lead level is as low as 37 micrograms/l (geometric mean). Even in such an area it is worthwhile looking for populations at risk. It is shown that attrition causes confounding with a directional bias towards the null-hypothesis. Longitudinal studies are associated with this type of bias. Misclassification as for past exposures will also have a bias of this type. Studies depending on blood-lead measures are

The polyglandular autoimmune syndromes (PGA) are well known and are distinguished into type I, type II and type III. PGAI, also called APECED (autoimmune polyendocrinopathy, candidiasis and ectodermal dystrophy), is an autosomal recessive disorder, appearing in childhood and typically characterized by hypoparathyroidism (unusual in PGAII and PGAIII) and adrenal insufficiency. In APECED, autoimmune destruction of the pancreatic beta cells with development of insulin-dependent type 1 diabetes is possible, but less frequent than in the other PGAs, especially PGAII. The pathogenesis of this unique

The purpose of the study was to evaluate the effect of early orthodontic treatment of functional unilateral posterior crossbite (FUPC) and to evaluate temporomandibular function in the short and long term. Orthodontic treatment consisted of slow expansion of the maxillary dental arch by means of a removable expansion plate, with flat coverage of the occlusal surfaces of the left and right posterior teeth. Evaluation of the occlusion showed a strong correlation between the crossbite side and the direction of the RCP-ICP slide and with the side of first occlusal contact in RCP. In 26 of 27 child

Nance-Horan syndrome (NHS) is a rare X-linked condition comprising congenital cataract with microcornea, distinctive dental, and evocative facial anomalies. Intellectual handicap was mentioned in seven published NHS patients. We performed a clinical study focused on psychomotor development, intellectual abilities, and behavior in 13 affected males in four NHS families, and present the results of a neuropsychological evaluation in 7 of them. Our study confirms that mental retardation (MR) can be a major component of the NHS. Combining our data with those from the literature leads to a frequency

The mouse Otlx2 gene is a new member of the paired-like family of homeobox genes whose human homologue, RIEG, is involved in Rieger syndrome, an autosomal-dominant disorder. One of the cardinal features of Rieger syndrome is dental hypoplasia, indicating that Otlx2/RIEG activity is essential for normal tooth development. Here, we analyzed the expression of Otlx2 during mouse tooth development and studied its regulation in dental explants. Otlx2 expression distinguishes stomatodeal from other ectoderm as early as Embryonic Day 8.5, well before tooth initiation. Thereafter, its craniofacial expr

Mice in which the genes encoding the parathyroid hormone (PTH)-related peptide (PTHrP) or the PTH/PTHrP receptor have been ablated by homologous recombination show skeletal dysplasia due to accelerated endochondral bone formation, and die at birth or in utero, respectively. Skeletal abnormalities due to decelerated chondrocyte maturation are observed in transgenic mice where PTHrP expression is targeted to the growth plate, and in patients with Jansen metaphyseal chondrodysplasia, a rare genetic disorder caused by constitutively active PTH/PTHrP receptors. These and other findings thus indicat

The development of the tooth at gene level is beginning to be understood. This paper reviews current knowledge and the advances in research on human genes whose defect leads to dental anomalies. Amelogenesis imperfecta (AI) is a diverse group of hereditary disorders characterized by a variety of developmental enamel defects including hypoplasia and hypomineralization, some of which have been revealed to be associated with defective amelogenin genes. The human amelogenin genes on X and Y chromosomes have been cloned and investigated extensively. Although autosomally inherited forms of AI are mo

Fetal alcohol syndrome (FAS) is characterized by congenital anomalies traditionally associated with hearing disorders. The present study sought to (a) evaluate possible central hearing loss; (b) verify and extend previous observations on sensorineural and conductive hearing losses; (c) evaluate possible vestibular disorders; (d) examine the relationships between hearing, speech, language, vestibular, and dentofacial disorders in FAS patients; and (e) evaluate the influence of patient age, race, and gender on the expression of these morbidities. A biracial group of 22 FAS patients (aged 3 to 26

The pathogenic potential of Fusobacterium nucleatum and its significance in the development of periodontal diseases, as well as in infections in other organs, have gained new interest for several reasons. First, this bacterium has the potential to be pathogenic because of its number and frequency in periodontal lesions, its production of tissue irritants, its synergism with other bacteria in mixed infections, and its ability to form aggregates with other suspected pathogens in periodontal disease and thus act as a bridge between early and late colonizers on the tooth surface. Second, of the mi

The congenital insensitivity-to-pain syndrome is one of several entities of sensory neuropathies in which pain sensation is absent from birth. This report describes a female child with the syndrome, who in all other aspects was normal. The most severe oral consequence of her disorder was self-inflicted exfoliation at an early age of all single-rooted primary teeth and one primary molar (a total of 13 teeth). The child was monitored from age 8. The primary aim in management was to monitor the eruption of the permanent teeth and occlusal development. At eruption of the mandibular incisors a prot

IE, although an infrequent event, remains a serious and frequently lethal complication in patients at risk. Oral microorganisms undoubtedly play a significant role in the development of IE in such patients. For example, in a retrospective review of prosthetic cardiac valve candidates (156) at the Cleveland Clinic Foundation, 15% were found to have at least one abscessed tooth, whereas approximately 17% were found to have severe (class IV) periodontal disease. Cardiac defects in conjunction with bacteremias from such dental pathoses clearly elevate the risk, although a small but increasing numb

OBJECTIVES: To measure the prevalence of disease of mandibular third molars referred for removal and to estimate the risk for development of pathoses for two cues.
STUDY DESIGN: A prospective cohort study on molars subjected to removal was performed. The prevalence of different diseases and the patient's age, angular position, and degree of impaction of the molars were registered. Odds ratio for molars with different positions and impaction states were estimated.
RESULTS: Pericoronitis was found in 64% of cases, caries in the third molar in 31%, periodontitis in association with 8%, caries i

Practising dentists must include in their diagnostic skills the ability to identify the patient with a high intake of alcohol. Dental status can often reflect the patient's perception of health and disease. The development of an appropriate treatment plan for these patients needs to take into account their generally unreliable nature. Appointments are often not kept; there is poor compliance with general health-care information, and compliance with preventive advice on dental care is achieved with difficulty. Symptomatic presentation is common with the alcoholic patient. The dentist should avo

Amelogenin is a major protein constituent of the developing enamel matrix. This protein is now well characterized from the data of amino acid sequences which have been shown to be at a high degree of homology between all species investigated to date. The gene structure of this protein has been demonstrated and it is confirmed that there are two amelogenin genes, one on the X-chromosome and the other on the Y-chromosome in humans. The mapping of human amelogenin gene in the p22 region on the X-chromosome and the gene on the Y-chromosome was established. It has been confirmed that several types

Many studies involving small animals have clearly shown that malnutrition affects tooth formation and causes increased dental caries. We have conducted two cross-sectional studies and one longitudinal study among Peruvian children to investigate the effect of early malnutrition on oral health. The cross-sectional studies have shown that in malnourished children the pattern of caries development as a function of age is significantly altered as a result of a delayed eruption and exfoliation of the deciduous teeth. This phenomenon has made difficult the observation by other scientists of a clear
