Roles of Wnt signaling pathway in cementum formation, cementum regeneration, and cementocyte function.

Cementum, a mineralized connective tissue that covers the tooth root, is crucial in protecting the root from resorption, maintaining occlusal relationships, and supporting tooth function. Cementocytes are embedded within the cementum matrix and extend dendritic processes through the canaliculi. They are thought to be mechanosensitive, responding to changes in mechanical loading, and are physiologically responsive cells associated with the formation of cellular cementum in response to variations in functional demands on the tooth. The wingless and int (Wnt) signaling pathway, which controls cel

Caspase-1-targeting siRNA nanoparticles suppress pyroptosis to prevent force-induced root resorption.

Mechanical force-induced hard tissue defects are clinically prevalent, particularly in dentistry. Excessive mechanical stimulation could induce pathological tooth root resorption (RR), which leads to tooth mobility and even tooth loss. This process is closely associated with dysregulated biological alterations in adjacent periodontal ligament stem cells (PDLSCs) and disrupted crosstalk between PDLSCs and osteoclast precursors, yet the specific molecular and cellular mechanisms remain insufficiently understood, severely limiting the development of targeted therapeutic strategies for such hard t

Historical Insights on Intentional Replantation for Tooth Retention Contrasted with Contemporary Practices.

The use of intentional replantation procedures to retain compromised teeth has a long and tortuous history of trial and error, doubt and conviction. Even in contemporary times there are both advocates and opponents to the use of this procedure, often citing it as a "last resort to retain the teeth." This article will look at the pros and cons of this technique as indicated by both our forefathers in the pertinent literature on this subject over the past 200+ years and present-day proponents of its routine use.

Evaluation of Systemic Nonsteroidal Anti-inflammatory Drugs on Orthodontic Tooth Movement Rate: A Clinical and Biomarker-Based Study.

BACKGROUND: Pain management is a critical component of orthodontic treatment, often necessitating pharmacological intervention. Nonsteroidal anti-inflammatory drugs are commonly prescribed; however, their mechanism of action involves the inhibition of cyclooxygenase enzymes and prostaglandins, which are essential mediators of bone remodeling. MATERIALS AND METHODS: A randomized, double-blind clinical trial was conducted with 30 orthodontic patients requiring bilateral maxillary first premolar extractions and subsequent canine retraction. Patients were randomly allocated into two groups: Group

Alveolar Ridge Preservation in Posterior Extraction Sockets: A Literature Review and Proposal of a Novel Therapeutic Classification.

BACKGROUND: Alveolar ridge preservation (ARP) is a critical procedure used to maintain post-extraction socket dimensions and the residual bone morphology is a key determinant of surgical outcomes. However, existing classification systems predominantly focus on the timing of implant placement rather than ARP protocols. Limited studies have developed classifications specifically for ARP, and none have addressed the unique anatomical challenges of posterior extraction sockets. METHODS: A comprehensive review of the literature was conducted in PubMed, Web of Science and Scopus from January 2000 t

[Strategic orthodontic treatment for multiple mesioangular impacted molars: a case report].

Tooth impaction is a prevalent dentofacial developmental anomaly, mostly observed in third molars, whereas second molar impaction occurs with lower frequency. This article presents a case of a male adolescent with mesially impacted second molars resulting in the external root resorption of multiple first molars. The patient received orthodontic and surgical treatments simultaneously, achieving dental alignment and enhanced occlusal function. The treatments mitigated the long-term adverse complications of external root resorption.

Conservative Management of Class III Invasive Cervical Resorption Using the Heithersay Technique: A Case Report.

Invasive cervical resorption (ICR) is an uncommon condition involving the gradual loss of cervical tooth structure due to the invasion of fibrovascular tissue because of predisposing factors such as trauma, orthodontics, and intracoronal restorations. A 45-year-old female patient presented with a class III cervical lesion and a missing restoration on the maxillary left lateral incisor following trauma. Radiographic examination revealed a "moth-eaten" appearance and a negative pulp test when cold testing was performed. Since it was a Class III lesion, the Heithersay technique was the choice of

Periodontal ligament cells regulate macrophage polarization via CCL7/CCR1 signaling.

Orthodontically induced root resorption (OIRR), a common complication of orthodontic treatment exacerbated by excessive force application, is linked to pro-inflammatory M1 macrophage polarization. This study investigates whether periodontal ligament cells (PDLCs) under heavy compression force regulate macrophage polarization via the CCL7/CCR1 axis to exacerbate OIRR. In the animal experiment, male Wistar rats were subjected to heavy orthodontic force to establish the orthodontic tooth movement model, with the inhibitor group receiving CCR1 antagonist BX471. Histological and immunohistochemical

Achieving Predictable Facial Bone Reconstruction at Immediate Implant Placement Utilizing the Facial Allograft Shield Technique.

Resorption following tooth loss often presents challenges to both functional and esthetic outcomes of dental implant treatment for lost dentition, especially in the esthetic zone. Inadequate bone volume at the implant site is common, and bone augmentation is often required. While autogenous bone is often preferred, it is not applicable in many cases. This can necessitate multiple surgeries and extended healing times for patients. In such situations an alternate protocol, the facial allograft shield technique using an allogeneic laminar bone membrane, appears to offer a minimally invasive, effe

From mechanical force to cellular response: the role of autophagy in orthodontic tooth movement.

Orthodontic tooth movement (OTM) is a process responding to orthodontic forces, which converts mechanical stresses into biochemical events to reconstruct the periodontal ligament and alveolar bone. As a lysosome-dependent catabolic process induced by various cellular stress conditions, autophagy maintains the homeostasis of cells, tissues, and organs and is widely involved in different physiologic or pathologic processes in the tooth. Recently, accumulating studies have investigated the role of autophagy in OTM. In this review, we focus on the autophagy induced by orthodontic forces, discuss t
