Successful Nonsurgical Resolution of a Large Periapical Lesion With Pre-Existing Sealer Extrusion and Adjacent External Inflammatory Root Resorption: A Case Report.

The primary aim of root canal treatment is to eliminate intracanal infection, achieve an apical and coronal seal, and create conditions that allow repair of the periapical tissues. In root-filled teeth, persistent apical periodontitis may be associated with extensive periapical lesions, external inflammatory root resorption affecting the treated or adjacent teeth, and extrusion of endodontic materials. These complications may lead clinicians to consider surgical approaches. However, when canals are adequately cleaned, disinfected, and hermetically obturated, favorable healing may occur followi

Tumor Necrosis Factor-Alpha Inhibitors: Oral Health and Orthodontic Considerations.

Autoimmune diseases affect approximately 4.6% of the US population and are frequently managed with immunomodulatory therapies, including tumor necrosis factor-alpha (TNF-α) inhibitors. These biologic agents, such as infliximab, adalimumab, etanercept, golimumab, and certolizumab pegol, target TNF-α, a key cytokine involved in inflammatory signaling and bone remodeling. While TNF-α inhibitors have demonstrated efficacy in reducing disease activity across a range of autoimmune conditions, their impact on oral health and orthodontic treatment remains underexplored. Clinical studies suggest that T

METTL3-mediated m6A methylation of lipopolysaccharide-binding protein exacerbates periodontitis by promoting M1 macrophage polarization.

BACKGROUND: Periodontitis is a chronic inflammatory disease driven by dysregulated immune responses and is closely associated with macrophage polarization. N6-methyladenosine (m6A) methylation represents the most prevalent RNA modification; however, its role in periodontitis remains unclear. This study aimed to investigate the mechanism by which m6A modification regulates macrophage polarization during the progression of periodontitis. METHODS: A periodontitis mouse model was established by ligature induction. RAW264.7 macrophages were treated with Porphyromonas gingivalis-derived lipopolysac

The role of cementoblasts in root resorption induced by orthodontic treatment: from mechanosensation to repair potential.

While alveolar bone has high remodeling activity under physiological conditions, cementum has a strong antiresorptive capacity. Cementoblasts are the primary functional and mechanosensitive cells on the cementum surface that maintain the structural homeostasis of the tooth root. However, excessive or sustained orthodontic force applied during treatment often induces irreversible orthodontically induced inflammatory root resorption (OIIRR), characterized by impaired cementoblast function and defective root surface repair. Although the precise molecular mechanisms underlying cementoblast dysfunc

A Case of CHARGE Syndrome With Tooth Eruption Disturbance Caused by Severe Calculus Accumulation.

Individuals with disabilities who are unable to maintain oral intake are at high risk of inadequate oral hygiene because of reduced masticatory stimulation and impaired self-cleansing, leading to excessive dental calculus accumulation. However, the effects of this on tooth eruption are poorly understood. Herein, we report the case of an 18-year-old boy with CHARGE syndrome managed with gastrostomy feeding since childhood. Following prolonged inadequate oral care, severe dental calculus accumulated throughout the dentition. Thick calculi encased the mandibular right first and second deciduous m

Regenerative endodontic treatment for premolars with apical root segment separation and extensive periapical lesions secondary to severe inflammatory root resorption: A report of two cases.

Inflammatory root resorption (IRR) frequently causes hard-tissue defects within the root canal system. The present report describes two cases in which regenerative endodontic treatment (RET) was used to manage premolars complicated by severe IRR, apical root segment separation and extensive periapical lesions. The first case involved an 11-year-old male patient with a fractured premolar (tooth 20) that exhibited IRR-induced apical segment separation. Case 2 involved a 15-year-old male patient presenting with severe IRR in tooth 20, characterized by an open, anomalous apical foramen and a large

Beyond the Midline: An Unusual Presentation of Acanthomatous Ameloblastoma in the Anterior Maxilla.

Acanthomatous ameloblastoma is an uncommon histopathological variant of ameloblastoma characterised by extensive squamous metaplasia and keratin formation within odontogenic epithelial islands. It predominantly affects adults between 30 and 60 years of age and most commonly occurs in the posterior mandible; involvement of the anterior maxilla is rare. A 55-year-old man presented with a progressively enlarging swelling in the anterior maxilla accompanied by pain during mastication. Intraoral examination revealed a diffuse swelling that crossed the midline, involved the labial vestibule, and cau

Alveolar Ridge Split Technique With Simultaneous Implant Placement: A Clinical Case Series With One-Year Follow-Up.

Ridge resorption following tooth loss may limit ideal three-dimensional implant placement. The alveolar ridge split technique (ARST) is used for horizontal ridge augmentation and may permit simultaneous implant placement in selected narrow ridges. This case series aimed to assess horizontal bone gain following ARST with simultaneous implant placement and evaluate short-term ridge stability one year after loading. Three systemically healthy patients presenting with maxillary horizontal bone width of less than 6 mm were included. All underwent ARST using piezosurgery and chisels with simultaneou

Digitally customized bone block allograft with enamel matrix derivative for ridge reconstruction: Case series. Histological report in humans.

BACKGROUND: Alveolar ridge resorption following tooth extraction is a common clinical challenge that can compromise esthetics, implant placement, and prosthetic rehabilitation. Digital customization of bone block allografts offers precise adaptation to the recipient site, improving graft stability and reducing surgical time. Enamel matrix derivative (EMD) has demonstrated potential to enhance osteogenic differentiation and cell adhesion; however, its application in combination with digitally customized bone block allografts (DCBBAs) remains scarcely reported. This study aimed to evaluate, bone

Exploring Decoronation and Root Submergence: Potential for Alveolar Bone Preservation in Growing Individuals.

Severe luxative injuries not only put the dental pulp at peril, but also the periodontal ligament (PDL), often resulting in an inflammatory process on the external root surface. By losing the protection of the PDL, the tooth is prone to ankylosis and replacement resorption. In adults, such teeth might be monitored or replaced. Nevertheless, in growing individuals an un-aesthetic alveolar ridge deformity and tilting of adjacent teeth due to infraposition might be the sequelae of trauma. Hence, leaving such cases untreated in children and growing individuals is not an option. Nowadays, an arsena

RUNX2 promotes osteoclast differentiation and bone resorption via the newly identified LncRNA-ROD: implications for alveolar remodeling.

Delayed tooth eruption is a hallmark of cleidocranial dysplasia (CCD) caused by RUNX2 haploinsufficiency, yet the underlying molecular mechanism remains undefined. Here, we show that RUNX2 regulates osteoclast-mediated alveolar bone remodeling, a process closely associated with tooth eruption. Runx2+/- mice exhibit impaired osteoclast differentiation and reduced alveolar bone resorption, resulting in delayed mandibular molar eruption-a phenotype that recapitulates human CCD. Mechanistically, RUNX2 transcriptionally activates LncRNA-ROD (Regulator of Osteoclast Differentiation), which promotes
