Clinical and Radiographic Success of Pulpotomy in Permanent Teeth With Irreversible Pulpitis: A Systematic Review.

Irreversible pulpitis in permanent teeth has traditionally been managed by root canal treatment. However, currently, vital pulp therapy, especially full and partial pulpotomy, is gaining popularity as a conservative technique to maintain pulp vitality and preserve tooth structure. This systematic review was carried out following the guidelines of the PRISMA 2020. A literature search was conducted using electronic databases PubMed, Cochrane library, and Google Scholar for studies published from 2010 to 2026. Following screening and eligibility assessment according to predefined inclusion criter

Biomimetic Cuspal Rehabilitation Using Indirect Ceramic Onlay After Pulpal Preservation: A Case Report.

BACKGROUND: Preservation of pulp vitality after pulpal exposure using vital pulp therapy (VPT) is now considered a predictable alternative to root canal treatment for normal or reversible pulpitis. With the growing demand for minimally invasive dentistry, indirect ceramic onlays offer good esthetic outcomes and durability with minimal tooth reduction. CASE REPORT: A mandibular left first molar with deep caries was treated with direct pulp capping using mineral trioxide aggregate (MTA). Following initial setting of MTA, immediate dentin sealing (IDS) was performed, and tooth was prepared for a

BMAL1 in Anterior Cingulate Cortex Pyramidal Neurons Relieves Pulpitis-Induced Nocturnal Pain.

AIM: Nocturnal exacerbation of pulpitis-induced pain severely disrupts patients' sleep and quality of life, increasing emergency dental care burden. However, the exact mechanisms underlying nocturnal pain remain poorly understood, limiting the development of targeted analgesic strategies. Here, we explored the circadian characteristics of pulpitis-induced pain in mice and the role of clock genes in its nocturnal exacerbation. METHODOLOGY: An experimental pulpitis model was established via pulp exposure, with stability confirmed by Micro-computed tomography and haematoxylin and eosin staining.

Pulp sensibility assessment following permanent tooth pulpotomy: a scoping review of clinical studies.

Vital pulp therapy (VPT), particularly pulpotomy, has emerged as a predictable treatment option for permanent teeth with deep caries and even in the presence of symptomatic irreversible pulpitis. Although numerous clinical studies have reported favorable outcomes, considerable variation exists in the methods used to assess postoperative pulp sensibility. This scoping review aimed to systematically map the pulp sensibility assessment methods, diagnostic methods, and follow-up schedules employed in clinical studies evaluating permanent tooth pulpotomy. This scoping review was conducted according

Dimensionally Engineered Nanotherapeutics for Pulpitis and Periodontitis: From Structural Design to Artificial Intelligence-Enabled Precision Therapy.

Pulpitis (Pul) and periodontitis (PD) are oral infectious diseases with extremely high global prevalence. These pathologies can induce severe pain, significantly elevate the risk of tooth mobility and eventual tooth loss, and have been confirmed to be closely correlated with multiple chronic systemic disorders including diabetes mellitus and cardiovascular diseases. Conventional clinical interventions for these diseases are frequently characterized by incomplete debridement and inefficient drug delivery. Additionally, the growing threat of antimicrobial resistance presents an ongoing challenge

Endodontic Inflammatory Disease: Systemic Consequences.

Endodontic inflammatory diseases (EIDs), such as pulpitis and apical periodontitis (AP), are common chronic oral inflammatory conditions, which are most commonly caused by untreated dental caries. Over the past two decades, several studies have reported positive bidirectional associations between AP and various systemic conditions, including cardiovascular disease (CVD), diabetes, hepatic disease, inflammatory bowel disease (IBD), hematological disease, and adverse pregnancy outcomes (APOs). These associations indicate that chronic oral inflammation may contribute to systemic low-grade inflamm

An antioxidant, injectable hydrogel with mitochondrial fusion effect promotes inflamed dental pulp repair via immunomodulation and reactive oxygen species scavenging.

Vital dental pulp is crucial for the self-repair and long-term retention of teeth with pulpitis; pulp capping materials used for vital pulp therapy must involve controlling inflammatory cascade and regulating inflammatory microenvironment at the same time. Here, we designed a dual-effect hydrogel with immunoregulatory and antioxidant properties to achieve inflamed pulp tissue repair. MASM7, a "mitochondrial glue" promoting mitochondrial fusion, could modulate THP-1-derived macrophages (THP-1-M) polarization to the M2 type under LPS-stimulated inflammatory conditions. Pulpotomy vs. pulpectomy for equivocal pulp status in primary molars: a retrospective cohort study with propensity score matching.

Background Diagnosing pulp status in primary molars is often challenging when clinical and radiographic findings fall into an equivocal range between reversible and irreversible pulpitis. This uncertainty complicates treatment decisions between pulpotomy and pulpectomy. This study aimed to compare long-term survival outcomes of pulpotomy and pulpectomy in primary molars with diagnostically equivocal pulp status and to identify potential predictors of treatment success.MethodsA retrospective cohort study was conducted on 312 primary molars from children aged 3-8 years treated at Hefei Stomatolo

Dysregulation of miR-140-3p is involved in the pathogenesis of pulpitis via the regulation of ACSL1.

Pulpitis is an inflammatory disease affecting the dental pulp tissue, impacting patients' oral health and daily life. MicroRNAs (miRNAs) are involved in the progression of pulpitis. This study aims to elucidate the regulatory role of the miR-140-3p/ACSL1 axis in pulpitis, particularly its molecular mechanisms within dental pulp cells. The levels of miR-140-3p and long-chain acyl CoA synthetase 1 (ACSL1) was measured by RT-qPCR. The receiver operating characteristic (ROC) curve represented the diagnostic capability of miR-140-3p. Lipopolysaccharide (LPS) derived from Escherichia coli was applie
