Chronic apical periodontitis (CAP) is a persistent periradicular inflammatory condition driven by intraradicular infection and remains a common indication for primary nonsurgical root canal therapy. This retrospective cohort study compared 2 calcium silicate-based bioceramic root canal sealers (C-Root SP vs iRoot SP) used during obturation in adults treated for CAP between January 2022 and December 2024. CAP was confirmed by compatible clinical findings and radiographic evidence of periapical radiolucency; cone-beam computed tomography (CBCT) was obtained when clinically indicated. The primary

Special care in dentistry : official publication of the American Association of Hospital Dentists, the Academy of Dentistry for the Handicapped, and the American Society for Geriatric Dentistry2026eng
BACKGROUND: Kearns-Sayre syndrome (KSS) is a rare mitochondrial DNA deletion disorder characterized by progressive external ophthalmoplegia, pigmentary retinopathy, and cardiac conduction abnormalities. Dental management may be complicated by developmental enamel defects, high caries burden, limited cooperation, and anesthetic considerations related to multisystem mitochondrial disease. Published dental case reports focused specifically on pediatric KSS remain extremely sparse.
CASE DESCRIPTION: We describe the staged dental management of a 10-year-old girl with genetically confirmed KSS and

Copper, predominantly present in bones, plays a crucial role in bone formation. However, when copper homeostasis is disrupted, excessive copper can trigger harmful inflammation and a novel form of cell death known as cuproptosis. The impact of cuproptosis on bone metabolism remains unclear. In this study, we demonstrated that excessive copper acts as an aggravator in osteoclastogenesis and bone resorption. We observed that the expression levels of the copper importer SLC31A1 and dihydrolipoamide S-acetyltransferase (DLAT) were positively correlated with bone loss in both human chronic apical p

This case report describes the micro-endodontic retreatment and comprehensive management of a complex left maxillary second molar presenting with multiple complications. The patient, a 41-year-old female, presented with recurrent gingival swelling associated with the tooth for more than six months. The tooth had a history of root canal treatment, fiber post placement, and full crown restoration two years earlier. Cone-beam CT (CBCT) revealed a separated metallic instrument approximately 5 mm in length in the apical region of the mesiobuccal root, a fiber post in the palatal root, underfilled r

Bacterial biofilm persistence in the endodontic system is often linked to post-treatment apical periodontitis, which continues to be a significant therapeutic problem. Because of their ability to penetrate and their photothermal effects on resistant microbes, laser technologies have shown great promise in enhancing root canal decontamination. The aim of this brief case report was to assess whether laser decontamination of the root canal system is beneficial for a patient who has persistent apical periodontitis following endodontic treatment. Clinical and radiographic examination of the patient

Background: The effective elimination of root canal microbiota is essential for the treatment of apical periodontitis. The smear layer formed during instrumentation limits the penetration of irrigants into dentinal tubules, making chelation a critical component of irrigation protocols. While ethylenediaminetetraacetic acid (EDTA) is commonly used for smear layer removal, etidronate has been proposed as an alternative due to its chemical compatibility with sodium hypochlorite. The aim of this study was to compare the effectiveness of different irrigation protocols in eliminating microorganisms

BACKGROUND: Whether alterations in the gut microbiota play a causal role in chronic apical periodontitis (CAP) remains uncertain. A two-sample Mendelian randomization (MR) analysis was used in this study to assess the potential causal effect of gut microbiota on CAP risk.
METHODS: Gut microbiota summary statistics were from the MiBioGen consortium's largest GWAS, and CAP data from the FinnGen project. The inverse variance-weighted method was the main analytic approach, and sensitivity analyses verified instrument robustness.
RESULTS: Two microbial taxa showed positive causal effect with CAP:

Background: Successful healing of chronic apical periodontitis after endodontic treatment requires a reduction in the size of the radiolucent area and the healing of the bone. This study aimed to compare the effects of different irrigation activation techniques on healing in single-rooted mandibular premolar teeth with periapical lesions of endodontic origin. Methods: A total of 132 systemically healthy patients with mandibular single-rooted premolar teeth and a periapical index (PAI) score ≥ 3 were assigned to five experimental groups (Sonic activation, Passive ultrasonic irrigation, Photon-I

BACKGROUND: Persistent Enterococcus faecalis infection remains a primary cause of endodontic treatment failure, necessitating the development of more effective disinfection protocols. This retrospective case-control study compared the antimicrobial efficacy of semiconductor laser irradiation combined with sodium hypochlorite irrigation vs. sodium hypochlorite irrigation alone for the elimination of E. faecalis from infected root canals.
METHODS: A total of 102 patients with chronic apical periodontitis and confirmed E. faecalis infection who underwent root canal treatment between May 2024 and

Dens invaginatus (DI) is a developmental anomaly characterized by the invagination of enamel and dentin into the pulp space, predisposing affected teeth to pulpal and periapical pathologies. The complex and irregular canal anatomy often complicates endodontic management. This case report describes the nonsurgical endodontic retreatment of a maxillary lateral incisor (22) with Oehler's Type II DI in a 32-year-old female patient presenting with chronic apical periodontitis. Cone beam computed tomography revealed a highly complex root canal anatomy, consisting of a large, irregular main canal, nu

Oral and craniofacial diseases are common, yet their genetic basis and links to systemic health are incompletely understood. We performed genome-wide association analyses of 67 oral phenotypes in 500,348 FinnGen participants, identifying 102 genome-wide significant loci, including 45 previously unreported associations. 48 loci remained significant after category-level Bonferroni correction. Fine-mapping revealed 14 coding variants, such as a missense variant in USP31 for caries and in MANBA for oral leukoplakia, and a stop-gained variant in GPNMB for temporomandibular disorders. Human leukocyt

Odontogenic cutaneous sinus tracts represent an uncommon clinical manifestation of chronic dental infection and are frequently misdiagnosed due to the absence of dental symptoms and their resemblance to dermatologic lesions. This case report describes the nonsurgical endodontic management of a cutaneous sinus tract of dental origin in a 13-year-old patient. The patient presented with a persistent extraoral lesion in the mandibular region, initially evaluated by non-dental specialists. Clinical and radiographic examination revealed a necrotic mandibular first molar associated with a periapical

Furcation involvement in multirooted teeth represents a significant challenge for endodontic treatment due to anatomical variability, the presence of accessory canals, and limited accessibility for adequate disinfection. This article presents a clinical case of non-surgical single-visit endodontic treatment of chronic apical periodontitis associated with furcation involvement in tooth 4.7. Root canal disinfection was performed using 3.25% sodium hypochlorite with intracanal heating up to 150 °C plugger. After chemo-mechanical preparation, definitive root canal obturation was carried out using

This case report describes a maxillary first molar with chronic apical periodontitis (CAP) and a separated endodontic instrument migrated to the maxillary sinus floor, which was successfully managed with digital guide-assisted endodontic microsurgery. A 20-year-old female patient presented with long-standing occlusal pain and discomfort in the left maxillary first molar. Clinical examination revealed a temporary restoration, tenderness to percussion, and no gingival swelling. During root canal treatment, the mesiobuccal (MB2) canal was found to be calcified and non-negotiable, and a small segm

Cemento-osseous dysplasia (COD) is an uncommon, non-neoplastic benign fibro-osseous lesion of the jaws, characterized by cementum-like tissue deposition. It primarily affects middle-aged women of African and East Asian descent. Generally asymptomatic, this condition is frequently identified incidentally through radiographs showing radiopacities with radiolucent rims. This report presented a case of florid cemento-osseous dysplasia (FLCOD) in a 45-year-old Asian female. The patient exhibited secondary caries extending to the pulp chamber on the left mandibular first molar, showed no signs of pu

Spontaneous preterm birth (sPTB; delivery before 37 weeks) remains a leading cause of neonatal mortality, and intrauterine infection is implicated in a substantial proportion of cases. Emerging evidence links oral pathogens such as Porphyromonas gingivalis and Fusobacterium nucleatum to adverse gestational outcomes; however, the mechanistic pathways connecting chronic apical periodontitis (CAP) with gestational risk remain incompletely defined. In pregnant C57BL/6J mice, CAP was established by root canal inoculation and complemented by a tail-vein challenge model using bacterial suspensions (1
