Study of antibiotic prescribing among dental practitioners in Shiraz, Islamic Republic of Iran.

Inappropriate prescribing of antibiotics by health care professionals is a worldwide concern. This study evaluated the knowledge and practices of dental practitioners in the city of Shiraz, Islamic Republic of Iran regarding their therapeutic use of antibiotics for patients with dentoalveolar infections. Of 219 (48.6%) dentists responding to the questionnaire more than 40% would prescribe antibiotics for localized fluctuant swelling and for problems for which antibiotics are not required according to good practice guidelines (acute pulpitis, chronic apical infection, periodontal abscess, chron

Improving oral health in the Middle East - recommendations from the first Middle East Oral Hygiene Advisory Board meeting.

Chronic periodontitis may be a sequela of chronic gingivitis, usually because of accumulation of plaque and calculus. Consistent good dental hygiene can help prevent gingivitis and periodontitis. Mechanical removal of plaque through frequent and efficacious brushing and flossing is the principal means of preventing periodontal diseases and diminishing the risk of caries. According to the American Dental Association (ADA), antimicrobial mouth-washes may provide additional oral health benefits, in addition to brushing and flossing, for preventing and reducing gingivitis and plaque. There is a mu

Effect of smoking on Langerhans and dendritic cells in patients with chronic gingivitis.

BACKGROUND: Previous literature showed contrasting results regarding dendritic cell (DC) counts in patients with periodontal diseases. Although smoking decreases the number of DCs in the lungs, the effect of smoking on the quantitative distribution of Langerhans cells (LCs) and DCs in patients with chronic gingivitis has not been investigated to our knowledge. METHODS: Gingival samples were obtained from 30 patients (15 smokers and 15 non-smokers). Immunohistochemical staining was performed to identify CD1a+ immature LCs and CD83+ mature DCs. The inflammatory infiltrate was evaluated and coun

Salivary blood group antigens and microbial flora.

OBJECTIVE: Secretor is an individual with ability to produce blood group reactive substances in the exocrine glands and to secrete these substances in the body fluids such as saliva. In saliva the blood reactive antigens are found primarily on mucins which exhibit microheterogenicity and different subtypes of these molecules. This study was done to find out if the salivary blood group antigens have any role in the adherence of certain selected microorganism in the oral cavity. METHODS: Unstimulated whole saliva and sub gingival plaque samples were collected from subjects with clinically healt

Use of chlorhexidine varnishes in preventing and treating periodontal disease. A review of the literature.

The literature includes numerous clinical trials to assess the effects of chlorhexidine varnishes in patients with chronic gingivitis and periodontitis. The purpose of this study is to review the literature systematically in order to ascertain the clinical effects of the different chlorhexidine varnishes at the periodontal level. The application of chlorhexidine varnishes seems to have beneficial effects in patients with chronic gingivitis, improving their plaque accumulation and bleeding levels and reducing their gingival index. It is possible to maintain this beneficial effect for prolonged

Common dental infections in the primary care setting.

Family physicians commonly encounter patients with dental infections, such as dental caries and periodontal disease. Dental caries is caused by bacteria that destroy the enamel and dentin; it can be detected by an oral examination that shows stained pits or fissures on the tooth surface. Use of fluoride is the most effective prevention measure for dental caries. Untreated caries may progress to pulpitis and, eventually, to necrosis of the pulp. In irreversible pulpitis, the tooth dies and the patient may have a localized abscess that can spread to surrounding tissue. Periodontal infections are

Differential patterns of receptor activator of nuclear factor kappa B ligand/osteoprotegerin expression in human periapical granulomas: possible association with progressive or stable nature of the lesions.

Receptor activator of nuclear factor kappa B ligand (RANKL) and osteoprotegerin (OPG) are expressed in apical periodontitis, suggesting a role for these molecules during lesion development. However, the profiles of RANKL/OPG expression in periapical lesions remain unknown. In this study we investigated the patterns of RANKL and OPG mRNA expression by real-time polymerase chain reaction in human periapical granulomas (N = 44) and compared them with sites presenting characteristic bone resorbing activity: healthy (n = 14) and orthodontically stretched and compressed periodontal ligament (n = 26)

Familial multiple myeloma associated with disorders of chronic inflammation: first report from Turkey.

Multiple myeloma (MM) is a malignancy arising from mature plasma cells in the bone marrow and usually presents with bone destruction, hypercalcemia, anemia, renal damage, and increased susceptibility to infection. The etiology of MM is unknown, with no established lifestyle, occupational, or environmental risk factors. Because MM is an uncommon disease, etiologic assessments can be difficult. It has been reported to be in association with sarcoidosis, and in a few cases, rheumatoid arthritis. Familial type of MM with an autosomal dominant heredity pattern has also been reported. The genetic lo
