Gingival Creep Failure: A Viscoelastic Theory of Recession in Thin Periodontal Phenotypes.

Gingival recession is commonly linked to alveolar bone dehiscence, inflammatory burden, traumatic brushing, or excessive orthodontic forces. However, recession is also observed in some patients despite apparently mild or "biologically acceptable" loading, particularly in thin periodontal phenotypes. Here, we propose the Gingival Creep Failure Theory, a hypothesis-driven conceptual framework in which gingival soft tissues undergo time-dependent viscoelastic deformation (creep) under sustained or repetitive tensile microstrain. Over time, accumulated deformation and microstructural fatigue may r

Multimodal Prediction of Periodontitis Using Root Exposure in Intraoral Images and Age.

INTRODUCTION AND AIMS: Despite advances in AI-based periodontitis screening, quantifiable and interpretable biomarkers from intraoral photographs remain underexplored. Therefore, this study aimed to develop a deep learning pipeline for exposed root area quantification from photographs and to evaluate its predictive value for periodontitis risk within a multimodal framework integrating age. METHODS: Intraoral photographs of the mandibular anterior sextant and covariate questionnaires were obtained from 269 participants. A fine-tuned YOLOv11 segmentation model quantified tooth and exposed root

Three years long-term impact of powered vs. manual toothbrushes on hypersensitivity and gingival recession: A randomized controlled trial.

OBJECTIVES: Gingival recessions (GR) are often associated with dentin hypersensitivity (HYP) and can significantly affect patients' oral health-related quality of life. This subgroup-analysis of a previously published study assesses the long-term effects of oscillating-rotating powered (PT) and manual toothbrushes (MT) on HYP in teeth with pre-existing GR and no interproximal attachment loss (RT1) over 36 months. METHODS: Ninety participants with RT1 were divided into PT-/MT-group (n = 47/n = 43). All participants brushed twice daily with the same fluoride toothpaste. HYP scores were reassess

Bottom-up suturing approach with biologic and grafting support for RT2 gingival recessions with papilla deficiency: Two case reports.

BACKGROUND: The management of multiple recession type 2 (RT2) gingival recessions remains a major clinical challenge because of interproximal attachment loss and limited predictability. This study aimed to present a graft-driven bottom‒up suturing concept for simultaneous root coverage and interdental papilla reconstruction. METHODS: Two systemically healthy patients with RT2 multiple gingival recessions in the mandibular anterior region were treated using a bilaminar approach with basal positioning of a connective tissue graft. Interproximal bone grafting and biologic adjuncts were used to p

Comparative evaluation of remineralisation potential using BioHAP, CPP-ACP and Enamelon on root dentin: An in vitrostudy.

Root caries and cervical dentin hypersensitivity affect older adults with gingival recession, requiring effective remineralisation strategies for root dentin preservation. Therefore, it is of interest to compare remineralisation potential of BioHAP, CPP-ACP and Enamelon on artificially demineralised root dentin from 60 extracted premolars using a 28-day pH cycling protocol. All agents significantly increased microhardness versus control (p<0.001), with BioHAP achieving superior recovery at 68.42±7.83% compared to other groups. SEM analysis confirmed BioHAP's enhanced mineral deposition and den

A Twelve-Month Observation Analysis of Real-Time Contact Pressure-Controlled Powered Tooth Brushing as a Subgroup Analysis of a 36-Month RCT.

OBJECTIVE: Bristle splaying may be associated with reduced cleaning efficacy; however, the relationship between the bristle splaying index (BSI) and contact pressure (CP) during brushing with a powered toothbrush (PT) with real-time CP control for oscillating brush head movement (range: 0.8-2.5 N) remains unknown. METHODS: This subgroup analysis of a randomised controlled trial (RCT), which included 47 subjects with gingival recession (GR), included digitally recorded data from 42 PT users: total brushing time (Btime; in minutes), brush head usage time (BHtime; in days), mean CP (in N), time

De-epithelialized Free Gingival Graft for Gingival Augmentation: A Case Report.

Gingival recession is a commonly encountered mucogingival condition that may lead to root exposure, dentinal hypersensitivity, and compromised aesthetics. Various surgical techniques have been proposed for root coverage, among which the de-epithelialized free gingival graft (DFGG) combines the advantages of both free gingival grafts (FGGs) and connective tissue grafts. A 26-year-old male patient presented with gingival recession in relation to mandibular central incisors (31 and 41). An FGG was harvested from the palatal donor site using a scalpel and subsequently de-epithelialized extraorally

Immediate implant placement in Type II socket using vestibular socket therapy with pericardial membrane versus simultaneous guided bone regeneration (randomized controlled clinical trial).

INTRODUCTION: Immediate implant placement with flap elevation and guided bone regeneration in Type II sockets often leads to midfacial gingival recession and compromised esthetics. To address these limitations, vestibular socket therapy (VST) has been proposed as an alternative technique. Our objective is to compare vestibular socket therapy using pericardium membrane versus conventional flap and guided bone regeneration (GBR) for immediate implant placement in type II sockets regarding bone and soft-tissue outcomes. MATERIALS AND METHODS: Twenty patients with Type II sockets were divided int

Enhancing graft stability in coronally advanced tunnel with connective tissue graft for Cairo RT1 and RT2 gingival recession: a case series.

Gingival recession (GR) in the mandibular anterior region presents a clinical challenge because factors such as shallow vestibular depth, frenal pull, and limited soft tissue thickness may compromise graft stability during root coverage procedures. This case series describes the management of Cairo RT1 and RT2 GR defects using a modified coronally advanced tunnel technique combined with subepithelial connective tissue grafting and tooth-anchored encircling sutures for graft stabilization. Five systemically healthy patients presenting with eleven recession defects were treated. At the 3-month f

Management of Gingival Recession of the Maxillary Left Canine Using a Coronally Advanced Buccal Flap and Palatal Connective Tissue Graft.

Gingival recession in the anterior maxillary region presents both functional and esthetic concerns, particularly when it affects the smile zone. Root exposure may lead to dentin hypersensitivity, plaque accumulation, and compromised gingival harmony. Periodontal plastic surgery techniques have been developed to restore gingival coverage and improve soft tissue thickness, demonstrating predictable results with generally favorable and well-documented outcomes. This case report describes the treatment of localized gingival recession affecting the maxillary left canine (tooth #11, Universal Number
