Impact of surgeon experience level on outcomes of three different root coverage procedures for RT1 gingival recession.

BACKGROUND: Gingival recession (GR) is a common dental condition. Root coverage procedures (RCPs) aim to restore the gingival margin to its original position and maximize root surface coverage. The impact of surgeon experience on surgical outcomes has not been systematically evaluated. This retrospective study aimed to investigate the impact of surgeon experience level on the outcomes of 3 different RCPs for RT1 GR by comprehensively assessing clinical and esthetic parameters before and after surgery. RESULTS: A total of 304 patients (1,020 teeth) were included in this study. Surgeons with mo

Clinical, Volumetric and Microcirculatory Outcomes of Multiple Gingival Recession Treatment with a Porcine Acellular Dermal Matrix: A Case Series.

BACKGROUND: Recession Type 1 (RT1) multiple adjacent gingival recessions (MAGR) represent a clinically relevant condition and may compromise esthetics and function. Connective tissue grafts (CTG) remain the reference treatment but involve donor-site morbidity. Porcine acellular dermal matrices (PADM) have been proposed as substitutes. This prospective exploratory case series evaluated the clinical performance of the modified coronally advanced tunnel (MCAT) combined with a PADM, while also exploring microcirculatory behavior assessed by laser speckle contrast imaging (LSCI) and volumetric soft

PTHrP Overexpression in Periosteum Promotes Alveolar Bone Formation.

Alveolar bone regions with low innate thickness are prone to dehiscence, potentially causing harmful gingival recession. In situ alveolar bone thickening via local bioagent administration has recently emerged as a promising research area. Parathyroid hormone-related protein (PTHrP), encoded by the Pthlh gene, plays a pivotal role in skeletal development and bone remodeling. In this study, we developed a recombinant adeno-associated virus (rAAV) vector overexpressing Pthlh (rAAV9-Pthlh). Using a split-mouth design, we administered rAAV9-Pthlh via a single submucosal injection into the right max

Comparative Evaluation of Effectiveness of Oxygen-Releasing Gel With Scaling and Root Planing (SRP) With That of Scaling and Root Planing Alone in the Management of Chronic Periodontitis Patients: Study Protocol for Cross-Sectional Survey.

BACKGROUND: Gingival recession and periodontal pockets are the results of the gradual deterioration of the alveolar bone and periodontal ligament caused by periodontitis, which is caused by inflammation of the tissues supporting the tooth. The primary goal of periodontal therapy is to eradicate these aberrant traits. Despite being a popular treatment, scaling and root planing (SRP) has limitations, such as difficulty accessing deeper pockets and root concavities. OBJECTIVE: The aim of this study is to assess the effectiveness of oxygen-releasing gel as a supplement to nonsurgical treatment fo

[Application progress in measurement methods for interdental papilla deficiency].

Currently, there is an increasing demand for the treatment and reconstruction of interproximal papilla defects in the anterior aesthetic zone. However, the lack of standardized measurement methods and indicators for assessing anterior interproximal papilla defects undermines the scientific rigor and accuracy of related research, and hampers comparability across studies. Common measurement methods for interproximal papilla defects include direct periodontal probe measurement, intraoral photographic measurement, and intraoral scanning data measurement. The direct periodontal probe method is simp

Effects of albumin-platelet-rich fibrin as an adjunct to non-surgical periodontal therapy in patients with periodontitis: a randomized controlled clinical trial.

BACKGROUND: Although platelet-rich fibrin (PRF) is a biological material with high regenerative potential, its rapid resorption limits its clinical effectiveness. Albumin-platelet-rich fibrin (Alb-PRF), developed in recent years, exhibits prolonged resorption time. The aim of this study was to evaluate the clinical, radiographic, and biochemical effects of Alb-PRF application into periodontal pockets following non-surgical periodontal therapy (NSPT) using a randomized split-mouth design. METHODS: This study included 23 patients diagnosed with Stage III Grade B periodontitis. Following complet

Treatment of single gingival recessions using iPRF-biofunctionalized collagen matrix: A randomized clinical trial.

BACKGROUND: The biofunctionalization of collagen matrices (CMs) with injectable platelet-rich fibrin (i-PRF) has been proposed to enhance their bioactivity and clinical efficacy in the treatment of gingival recessions (GRs). This study evaluated the use of a volume-stable collagen matrix (VCMX) biofunctionalized with i-PRF as an adjunct to the coronally advanced flap (CAF) for single GRs. METHODS: Seventy-five patients with single RT1 GR were randomly assigned to CAF, CAF+VCMX, or CAF+VCMX+iPRF. Clinical, esthetic, and patient-centered outcomes were assessed at baseline and at 6 months. RESU

Treatment of isolated gingival recession by tunneled coronally advanced flap and orthodontics: A case report.

Gingival recession (GR) in malpositioned teeth with a thin periodontal phenotype poses a high risk during orthodontic movement. Phenotype modification before orthodontic treatment may reduce complications and improve clinical outcomes. A 27-year-old woman presented with a 3-mm RT2 gingival recession and<1 mm of keratinized tissue (KT) in tooth 31, which was labially displaced. Cone-beam computed tomography (CBCT) revealed the absence of the vestibular bone, and clinical evaluation confirmed a thin periodontal phenotype. To prevent GR progression during orthodontic movement, a tunneled coronall

Wedge vs screw implants in narrow ridges: A multiple split-mouth case report in the posterior maxilla with a 14-month follow-up.

PURPOSE: A recently introduced narrow osseointegrated wedge-shaped implant has been proposed for the treatment of thin alveolar ridges that are not suitable for placement of root form screw-type implants without bone augmentation procedures. The wedge implant was designed with the aim of preserving the buccal bone plate while maintaining adequate mechanical and prosthetic performance. This multiple split-mouth case report in the posterior maxilla compares wedge-shaped and screw-type implants. MATERIALS AND METHODS: Wedge implants (1.8 mm thickness) were placed in a narrow ridge (mean width 4.

Three-year results of a prospective, controlled clinical study evaluating treatment of intra-bony defects with PRF or EMD- a randomized control trial.

BACKGROUND: Periodontal regenerative surgery is a well-established treatment modality for intrabony periodontal defects. Enamel matrix derivative (EMD) is one of the most extensively documented biologic agents, whereas platelet-rich fibrin (PRF) has emerged as a promising autologous alternative. This prospective randomized controlled clinical study aimed to compare the long-term clinical outcomes of PRF and EMD in the treatment of periodontal intrabony defects. METHODS: Thirty patients with periodontitis Stage III/IV, grade B/C and at least one intrabony defect [probing pocket depth (PPD) ≥ 6

Long-term stability of periodontal plastic surgery featuring traditional autogenous graft and coronally advanced flap alone.

BACKGROUND: Coronally advanced flap (CAF), either alone or in combination with a connective tissue graft (CTG), represents one of the most extensively investigated surgical approaches for the treatment of gingival recessions, demonstrating high rates of mean root coverage (MRC) and complete root coverage (CRC). However, evidence regarding their long-term performance remains limited. AIM: This review critically analyzes the long-term clinical performance of CAF alone and in combination with CTG, examining root coverage outcomes, stability of the gingival margin, keratinized tissue (KT), and gi

Modified coronally advanced flap for multiple anterior gingival recessions: Case report.

BACKGROUND: The frontal envelope coronally advanced flap (CAF), designed for multiple adjacent gingival recessions (MAGR) of the anterior maxilla, involves tunneling of the midline papilla. However, when treating frontal and lateral MAGR in a single session, the tunneled midline papilla can act as a fixed point, which may limit coronal advancement or lead to mucosal folds in the gingival margin. METHODS: This case report introduces a modification of the CAF technique in which a V-shaped split-thickness surgical papilla is elevated at the midline to enhance flap mobility. Two patients with com
