Platelet-Rich Fibrin with Minocycline versus Arestin in Non-surgical Periodontal Therapy: A Split-Mouth Randomised Controlled Clinical Trial.

PURPOSE: Periodontitis is a chronic inflammatory disease and while non-surgical periodontal therapy (NSPT) remains the first choice of treatment, its capability to completely resolve inflammation may be limited. Arestin® (1 mg minocycline microsphere) is an established adjunct, whereas platelet-rich fibrin (PRF) has recently emerged as a promising local drug-delivery scaffold. This split-mouth randomised controlled clinical trial compared the clinical efficacy of adjunctive minocycline delivered via conventional Arestin® versus incorporation within a PRF scaffold for the treatment of 4-6 mm pe

Clinical Crown Length Changes Following Maxillary Molar Distalization: The Role of Three-Dimensional Tooth Movement and Third Molar Position.

INTRODUCTION: Maxillary molar distalization is widely used for space gain and anterior retraction. However, gingival recession may occur around the maxillary second molar following distalization, and the tooth movement factors associated with changes in clinical crown length (CCL) remain unclear. This study investigated the associations of CCL changes with three-dimensional movement of the maxillary second molar and the position of the maxillary third molar. METHODS: This retrospective study included 58 maxillary second molars from 30 adult patients who underwent skeletal anchorage-assisted m

Phenotype-Driven Treatment Planning for the Interdisciplinary Patient. Part I: Definition, Diagnosis, and Treatment Options.

A recent update in periodontal terminology includes a shift from the term biotype, replacing it with the term phenotype. Periodontal phenotype evaluation and diagnosis is possible when findings from the clinical examination to establish the gingival phenotype are combined with assessment of the bone morphotype, commonly using CBCT imaging technology. Such analysis is critical to treatment planning, particularly for interdisciplinary dentofacial therapy (IDT) patients whose treatment often involves clinical interventions such as surgery, orthodontic tooth movement, and/or extensive restorative

Tunneled Coronally Advanced Flap and Connective Tissue Graft Wall for the Treatment of Isolated Infrabony Defects Associated with Gingival Recession: Technical Report with Cases.

The integrity and phenotype of periodontal soft tissues significantly influence the outcome of sur-gical periodontal regenerative therapy. In cases with a thin gingival phenotype, treating infrabony defects surgically can worsen gingival recession and loss of papillae. This report outlines a surgical approach for addressing infrabony defects at sites with gingival recession and a thin phenotype. The treatment involves using a tunneled coronally advanced flap to obtain access for defect de-bridement, root instrumentation, graft placement, and tissue advancement for root coverage. The Use of a Connective Tissue Graft Wall as an Adjunct to Regenerative Treatment for Deep Intrabony Defects Associated with Gingival Recession: A Retrospective Case Series with a 5-Year Follow-up.

The adjunctive use of connective tissue grafts (CTGs) in the periodontal regeneration of intrabony defects has been proposed to prevent or limit postoperative gingival recession. However, there is limited evidence regarding the long-term clinical performance of this approach. This article presents the 5-year follow-up outcomes of a combination therapy using CTGs, bone substitutes, and biologic materials for the treatment of deep intrabony defects associated with gingival recession. Twelve healthy, nonsmoking patients with 12 deep intrabony defects participated in the study. Before undergoing t
