Management of a Complex Case of Gingival Enlargement Associated With Underlying Arteriovenous Malformation: A Five-Year Follow-Up Case Report.

Arteriovenous malformations (AVMs) are high-flow congenital vascular anomalies characterized by abnormal direct communication between arteries and veins, bypassing the capillary bed. Gingival enlargement or gingival overgrowth is a common gingival disease characterized by an increase in the size of the gingival tissues. The main factors of gingival enlargement are inflammation, drugs, systemic conditions or diseases, neoplastic or false enlargements, and local factors, such as plaque, calculus, and/or overhanging restorations. The purpose of this article is to present an interesting case of th

Risk Factors and Management of Gingival Enlargement: A Systematic Review and Meta-Analysis.

OBJECTIVE: To identify risk factors/indicators of gingival enlargement (GE) and to summarise evidence for its management. METHODS: A systematic review was undertaken following PRISMA guidelines and divided into PECOTS and PICOTS questions, related to risk factors/indicators and treatment studies, respectively. Literature search was performed on the electronic databases PubMed in Medline and Ovid in EMBASE. RESULTS: From an initial screening of 3609 entries, 349 papers were eventually included, including 299 for PECOT and 50 for PICOT. In most studies, GE was drug-influenced (DIGE), while in

Acute promyelocytic leukaemia presenting with gingival hypertrophy: an atypical oral presentation.

Acute promyelocytic leukaemia (APL) is a subtype of acute myeloid leukaemia characterised by the PML-RARα fusion gene and a high risk of early mortality if treatment is delayed. Gingival hypertrophy is an uncommon manifestation of APL and is more typically associated with monocytic and myelomonocytic AML. We report a 26-year-old male who presented with a three-week history of progressive gingival enlargement, initially managed as gingivitis for two weeks, resulting in delayed haematological evaluation. Microgranular APL was confirmed by bone marrow morphology and fluorescence in situ hybridisa

Management of Chronic Inflammatory Gingival Enlargement Using Conventional and Electrosurgical Techniques in an Orthodontic Patient: A Case Report.

BACKGROUND: Chronic inflammatory gingival enlargement associated with orthodontic appliances may interfere with oral hygiene maintenance and orthodontic treatment. When non-surgical therapy is unsuccessful, gingivectomy may be indicated. Conventional scalpel and electrosurgical techniques are both established treatment modalities for gingival tissue excision, each with distinct clinical characteristics. This case report aims to describe the management of chronic inflammatory gingival enlargement in an orthodontic patient treated with conventional scalpel and electrosurgical gingivectomy. CASE

Gingival Enlargement During Orthodontic Treatment: A Narrative Review.

Gingival enlargement is commonly seen during orthodontic treatment, specifically in individuals with fixed appliances. Its clinical significance stems from its multifactorial etiology. The purpose of this narrative review is to evaluate the available evidence related to the causes, influential components, symptomatic profiles, and therapeutic approaches to gingival enlargement during orthodontic treatment. An electronic literature research was carried out in PubMed without any limitations. A total of 10 studies were chosen, incorporating one randomized clinical trial and nine observational stu

KCNK4 c.730G>C/p.Ala244Pro Promotes Gingival Fibroblast Proliferation and Fibrosis Through MAPK/ERK1/2 Activation in Gingival Fibromatosis.

INTRODUCTION AND AIMS: KCNK4-associated gingival fibromatosis (K-GF) is a rare condition characterised by progressive gingival enlargement and excessive connective tissue accumulation. Although pathogenic variants in KCNK4 have been linked to syndromic gingival overgrowth, the cellular and molecular mechanisms underlying the gingival phenotype remain incompletely understood. This study aimed to investigate the biological effects of a rare heterozygous KCNK4 c.730G>C variant identified in a paediatric patient with K-GF and to explore its potential role in gingival fibroblast dysfunction. Puberty-Associated Gingival Enlargement Exacerbated by Polycystic Ovary Syndrome: A Case Report.

BACKGROUND: Polycystic ovary syndrome (PCOS) is a common endocrinopathy affecting reproductive-aged women, characterised by metabolic abnormalities, polycystic ovaries, hyperandrogenism, and chronic anovulation. This pathology has been attributed to hormonal imbalances, including elevated levels of insulin, growth hormone (GH), ghrelin, LEAP-2, GnRH, LH/FSH ratio (>2:1), androgenic hormones, and estrogen. While puberty-associated gingival enlargement is primarily triggered by dental plaque, systemic hormonal fluctuations can significantly exacerbate the tissue response. CASE PRESENTATION: A 1

Differential Expression of Caspase-3 and Ki-67 in Hereditary Gingival Fibromatosis and Inflammatory Gingival Hyperplasia.

OBJECTIVE: The nuclear protein Ki-67 and caspase-3 regulate cell proliferation and apoptosis as well as the preservation of tissue homeostasis in response to genetic and inflammatory signals that have a role in the pathogenesis of different conditions such as gingival overgrowth. Yet, the distinct pathogenesis of these lesions could be determined by variations in cellular homeostasis mechanisms. The aim of this study was to assess and compare the immunohistochemical expression of Ki-67 and caspase-3 in hereditary gingival fibromatosis (HGF) and inflammatory gingival hyperplasia (IGH), measure

The Potential Allergic Role of Mast Cells to Orthodontic Appliance in Orthodontic-Induced Gingival Enlargement.

BACKGROUND: Gingival mast cells (MCs) are strategically scattered throughout connective tissue and adhere to the endothelium to convert local impulses into systemic signals and attract additional immune effector cells. Numerous mediators, such as histamine, cytokines, and proteases, modulate these interaction pathways. OBJECTIVES: The purpose of this study was to determine and compare MC count in the gingival enlargement tissue of patients receiving orthodontic treatment versus patients having plaque-induced gingival enlargement without orthodontic treatment, utilising hematoxylin and eosin (

Comparative evaluation of chlorhexidine with or without hyaluronic acid on early periodontal wound healing.

Achieving uncomplicated tissue recovery remains a major challenge after periodontal surgery. Therefore, it is of interest to evaluate the 10 gingivectomy patients presenting with gingival enlargement. Post-operatively, participants used either a combined 0.2% CHX+HA rinse or CHX alone. The CHX+HA cohort exhibited significantly superior early-stage wound healing outcomes. Thus, data shows combining chlorhexidine with hyaluronic acid enhances early periodontal tissue healing.

Plasma Cell Gingivitis: A Clinicopathological Case Report Highlighting a Systematic Diagnostic Approach and Step-by-Step Surgical Management.

Plasma cell gingivitis (PCG) is a rare benign inflammatory condition characterized by diffuse erythematous gingival enlargement with dense plasma cell infiltration. Its clinical presentation overlaps with inflammatory, autoimmune, hematological, and plasma cell proliferative disorders, making accurate diagnosis challenging and necessitating a systematic clinicopathological evaluation. This report describes a 23-year-old male who presented with persistent gingival enlargement and bleeding involving the maxillary and mandibular anterior regions for two years despite repeated nonsurgical periodon

Plasma Cell Gingivitis With Cheilitis in an Adolescent: A Case Report.

Plasma cell gingivitis (PCG) is an uncommon plasma-cell-rich inflammatory disorder of the gingiva that may clinically resemble plaque-associated enlargement, granulomatous disease, hematologic disease, or plasma-cell neoplasia. Its association with cheilitis is rare, and diagnosis requires clinicopathologic correlation. An 18-year-old female presented with a 1-year history of progressive gingival enlargement and upper-lip swelling. Intraoral examination showed generalized erythematous marginal and papillary gingival enlargement with loss of stippling, bleeding on probing, pseudopocketing, and

Assessment of physicians' awareness and clinical practice regarding gingival enlargement caused by calcium channel blockers.

BACKGROUND: Gingival enlargement is a side effect of calcium channel blockers (CCBs), which are usually used to treat cardiovascular conditions. Physicians have an important role in identifying and managing the disease, but their level of awareness and ability to apply their knowledge in clinical practice remain unclear in Iraq. OBJECTIVE: This study aims to assess physicians' knowledge of calcium channel blocker (CCB)-induced gingival enlargement and determine whether they apply this knowledge in their clinical practice. METHODS: A cross-sectional survey was conducted among 331 physicians w
