Hyaline or amyloid-like substances may be seen in several oral lesions. Some of these originate from epithelial cells and some from connective tissue components. In this study, amyloid-like material causing gingival enlargement in three patients, with eyelid involvement in two, was examined using histochemistry, immunohistochemistry and electron microscopy. These accumulations did not share the classical features of amyloid such as green birefringence under polarized light after Congo red staining. The etiology of generalized gingival enlargement is very heterogeneous. Pregnancy, drugs, systemic diseases, poor oral hygiene, and nutritional impairment can lead to gingival enlargement. This defect can also be presented in several malformation syndromes. Gingival fibromatosis, that may exist as an isolated abnormality or as part of a syndrome, is another condition with gingival enlargement. In this paper, the malformation syndromes with gingival hypertrophy and those with gingival fibromatosis are listed in tables.

Although it is established that the immunosuppressant cyclosporin-A (CsA) and calcium antagonists [Nifedipine (Nif) and Diltiazem (Dz)] can independently induce gingival enlargement, little has been documented on the significance of the salivary CsA levels and the combined effect of CsA and a calcium antagonist upon gingival tissues. Ten patients manifesting gingival enlargement were chosen for this study. Five patients had been treated with 4 mg nifedipine per day as a treatment for angina pectoris, while the other five patients treated with phenytion as an anticonvulsant drug. Clinical and histological data showed that nefidipine induced gingival hyperplasia which is of close similarity to that induced by phenytion despite the differences in target tissue.

Fibrotic gingival enlargements induced by phenytoin or nifedipine were examined with special reference to type VI collagen expression. Immunolocalization studies showed abnormal accumulation of type VI collagen around the collagen fiber bundles in the fibrotic gingival enlargements. Examination of total RNA extracted from fibroblasts and tissues of enlarged gingivae demonstrated increased type VI collagen steady-state mRNA levels. These results suggest that excessive deposition of type VI collagen in drug-induced gingival enlargement is attributed to increased expression of the collagen genes.

A 15-year-old female patients suffered from recurrent unusual enlargement of the gingiva together with rapidly progressive alveolar bone loss. Cyclosporin-induced gingival enlargement was presented. Enlargement was noted at several sites in the mouth of five patients taking Cyclosporin following liver transplantation. Papillary gingival enlargement was observed to affect the entire maxilla and mandible. Gingival enlargement due to the immunosuppressive drug, cyclosporin A (CsA), will resolve following discontinuation of drug therapy or extraction of adjacent teeth. The distribution of fibronectin (FN) in the healthy, inflamed and hyperplastic human gingiva was investigated by indirect immunofluorescence. FN appeared as a fibrillar structure in the lamina propria of the healthy gingivae. In the inflamed specimens, FN demonstrated parallel fibres, especially in the coronal areas of the tissue. Cyclosporin-A and Nifedipine-induced gingival enlargement have been reported to have similar clinical and histopathological features as phenytoin. The present case report describes the clinical course and treatment of a young heart-transplant patient who developed gingival hyperplasia under both Nifedipine and Cyclosporin-A. After withdrawing Nifedipine from his daily medication and a second step of surgical excision, the patient presents no recurrence of gingival enlargement, 6 months post-treatment despite his Cyclosporin-A regimen and poor oral hygiene.

The purpose of the present study was to investigate, on a longitudinal basis, the effectiveness of a specific preventive dental program for patients who are taking phenytoin for seizure control. The results confirm that a preventive dental program, consisting on frequent prophylaxis and plaque control, is effective in minimizing clinically gingival enlargement associated with phenytoin therapy, even in patients who present histological aspects of gingival hyperplasia.

To investigate the mechanism of cyclosporine (CS)-induced fibrotic gingival enlargement, the effect of CS on the collagenolytic activities of 14 different human gingival fibroblast strains derived from healthy individuals with non-inflammed gingiva was examined in vitro. The periodontal conditions of 2424 pregnant and 1565 non-pregnant women were assessed according to the community periodontal index of treatment needs (CPITN). The aim of this survey was to obtain information which is necessary for the planning of preventive programs of periodontal disease for pregnant women. 95% of the pregnant women and 96% of the non-pregnant women had some signs of periodontal disease.
