Primary reconstruction of alveolar clefts using recombinant human bone morphogenic protein-2: clinical and radiographic outcomes.

BACKGROUND: This novel method for primary alveolar cleft bony reconstruction avoids donor site morbidity and does not require close approximation of alveolar segments as necessitated by gingivoperiosteoplasty. The study aims to provide radiographic evidence of de novo synthesis of bone using recombinant human bone morphogenic protein-2 (rhBMP-2). METHODS: This institutional review board-approved class IV study retrospectively evaluated primary alveolar cleft patients from 2004 to 2006. Subjects chose an off-label application of rhBMP-2 impregnated on an absorbable collagen sponge carrier to r

Nasal cavity changes and the respiratory standard after maxillary expansion.

UNLABELLED: Mandibular cross-sectional deficiency is a dentofacial defect in connection with the narrowing of the mandibular arch width. This abnormality is a significant etiopathogenic factor and it is often associated with nasal breathing difficulties. This atresia may be treated through Rapid Maxillary Expansion or Surgically Assisted Rapid Maxillary Expansion, depending on the patient's age. Both procedures will change the craniofacial structure, especially the nasal cavity. AIM: Based on literature review, the purpose of this paper was to report the relationship among maxillary expansion

[Influence of orthodontic anomalies on periodontal condition].

INTRODUCTION: Three main reasons are routinely cited to justify the provision of orthodontic treatment: improvement of facial and dental aesthetics and of dental health and function. However, association between malocclusions and periodontal condition is still controversial. The aim of the present study was to further investigate the correlation between certain orthodontic anomalies and periodontal condition. MATERIALS AND METHOD: One hundred and one subjects were included in this study. Information regarding oral hygiene habits and accessibility to professional oral health care were obtained

Long-term outcome in a patient with a dentoskeletal open-bite malocclusion treated without extraction.

This case report describes the treatment of a patient with a Class II malocclusion with an anterior open bite. The patient, a girl 16 years of age, had a significant anteroposterior discrepancy and a high-angle tendency. Her face was convex, with competent lips. Intraorally she had an anterior open bite of 3 mm, space in the mandibular arch, and an overjet of 2 mm. High-pull headgear, anterior intermaxillary elastics, and appropriate wire bending were used to close the bite and to correct the anteroposterior dental relationship. Modification of a tongue thrust habit helped to correct this sign

Osteodistraction in the craniofacial region.

In the specific field of maxillofacial surgery, the use of osseous distraction is always more and more helpful not only in the rehabilitation of malformation pathologies, but also in the clinical situations that require bone deficit correction resulting from traumatic events and postsurgical effects, for example oncologic surgery. The reason for this versatility in the distraction protocols is, undoubtedly, due to the fact that, at present, they are valid surgical methods in alternative to or supporting maxillofacial surgery, since they are feasible from a very early age and they obtain a leve

Managing congenitally missing lateral incisors. Part II: tooth-supported restorations.

Three treatment options exist for the replacement of congenitally missing lateral incisors: canine substitution, a tooth-supported restoration, or a single-tooth implant. Selecting the appropriate treatment option depends on the malocclusion, the anterior relationship, specific space requirements, and the condition of the adjacent teeth. The ideal treatment is the most conservative alternative that satisfies individual esthetic and functional requirements. This article closely examines the three options when replacing a missing lateral incisor with a tooth-supported restoration. These options

Relationship between transverse dental anomalies and skeletal asymmetry.

The recognition and elimination of dental compensation is essential in presurgical orthodontic treatment to achieve successful stabilization of the occlusion after surgery. However, the relationship between a transverse dental anomaly and skeletal asymmetry is not fully understood. To evaluate this relationship, frontal cephalometric and 3-dimensional dental model analyses were carried out on 44 adult Japanese Class III patients (mean age 21 years 11 months) who required surgical orthodontic treatment. The patients were divided into 2 groups: a facial asymmetry group in which the mandibular tr

Modified nasal alveolar molding appliance for management of cleft lip defect.

Nasal alveolar molding is used effectively to reshape the nasal cartilage and mold the maxillary arch before cleft lip repair and primary rhinoplasty. It provides aesthetic and functional benefits of nasal tip and alar symmetry and improved dental arch form. At The Craniofacial Center at the University of Illinois at Chicago, the authors have developed a modification of a nasal alveolar molding appliance previously described in the literature. The key modification is the use of an orthodontic wire from the palatal prosthesis with an acrylic bulb positioned inside the nose, underneath the apex

Molding of the regenerate in mandibular distraction: clinical experience.

Initial clinical experience with distraction osteogenesis has demonstrated the risk of developing postdistraction malocclusion that requires secondary orthodontic correction. In addition, optimal mandibular form is not always achieved. Both animal studies and preliminary clinical investigations have suggested that the regenerate can be successfully "molded" during active mandibular distraction. The authors have applied this concept clinically to obtain a more desirable occlusal relationship in a group of mandibular distraction patients. Eleven patients are described in whom angulation of the d

[Orthodontic approach to asymmetry].

From an orthodontic point of view, asymmetries can be gathered in three great clinical entities: mandibular lateral deviations, dental asymmetries without skeletal involvement, skeletal asymmetries. Once the therapeutic aims and the principles of the orthodontic approach of these dysmorphoses have been recalled, the authors present the various orthodontic means implemented in this type of treatment. Four cases treated illustrate those types of treatment. Vertical non-surgical asymmetry may have an obvious local origin, for instance, a unilateral damage to a nerve. but usually, there is no evid

An adult case of skeletal open bite with a severely narrowed maxillary dental arch.

Surgically assisted rapid maxillary expansion is proposed as an efficient approach for adult patients with transverse maxillary deficiency. This article reports the treatment of an 18-year, seven-month old male patient with an anterior open bite and a severely narrowed upper dental arch. A posterior crossbite was present on both sides. For the correction of the posterior crossbite, a lateral maxillary expansion of more than 8 mm was required. A surgically assisted rapid maxillary expansion with Le Fort I corticotomy and mandibular setback with a sagittal splitting ramus osteotomy were determin
