Orthognathic surgery has significant functional and aesthetic impacts on patients. While its benefits in individuals with Class III malocclusion-improving self-esteem, quality of life, and perceived attractiveness-are well documented, its role in facial feminization surgery (FFS) remains underexplored. This review highlights orthognathic surgery as an essential adjunct in FFS for patients with concave profiles and Class III malocclusion, offering simultaneous correction of malocclusion and feminization of facial contours. We review important considerations for treatment planning, operative seq

PURPOSE: The paranasal region is a frequent source of aesthetic complaints in patients with dentofacial deformities, and soft tissue projection in this area does not always follow the skeletal movements achieved in orthognathic surgery. Sticky Bone-a particulate bone graft matrix agglutinated by platelet-rich fibrin (PRF)-has an established role in regenerative procedures in implant dentistry; however, to the best of the authors' knowledge, its use for paranasal volume enhancement during orthognathic surgery had not been previously described. The present report introduces this novel applicatio

BACKGROUND: In the statutory health system in Germany, orthodontic treatment is determined through the KIG index system. Category D describes all malocclusions with a positive overjet. Skeletal characteristics are not recorded. The aim of this study was to analyse the dento-skeletal characteristics of patients exhibiting KIG grades D4 and D5. Additionally, the similarities and differences compared to the characteristics of Angle Class II/1 malocclusion should be identified.
METHODS: N= 225 patients (116 female, 109 male) aged 11.41 ± 1.77 years were included in the study. The patients were cl

INTRODUCTION: Although Class II elastics are widely used, evidence on their dentoalveolar effects in adults is limited. This study aimed to radiographically compare the sagittal and vertical dentoalveolar effects of Class II correction using elastics alone vs miniscrew-assisted maxillary total-arch distalization (MTAD), with nighttime vs full-time elastic wear in adults treated with completely customized lingual appliances.
METHODS: A total of 79 consecutively treated adult patients with Class II malocclusion were included and grouped by protocol: elastics only (n = 34), MTAD with nighttime e

Orthognathic surgery has increasingly incorporated digital workflows and patient-specific surgical guides to enhance accuracy and predictability. While these technologies offer improved planning and execution, their intraoperative application in segmented maxillary osteotomies remains technically demanding. Limited literature exists describing the practical challenges encountered during guide-assisted osteotomy, particularly in anatomically constrained regions. This case report describes the use of a patient-specific cutting guide in a 24-year-old male patient undergoing anterior two-piece max

BACKGROUND: The periodontal phenotype plays a critical role in periodontal stability and orthodontic risk assessment. However, an integrated evaluation of soft- and hard-tissue periodontal characteristics across various sagittal and vertical skeletal patterns and dental crowding remains limited. This study applied an innovative cluster-based classification of periodontal phenotype to evaluate these variances.
METHODS: This cross-sectional study included 180 participants and 1080 maxillary anterior teeth. Using superimposed cone beam computed tomography and intraoral scan data, the thickness o

OBJECTIVES: To quantify the depth of the Curve of Spee (CoS) in untreated orthodontic patients, evaluate its variations across Angle classifications, and identify its skeletal and dental morphological correlates.
MATERIALS AND METHODS: This retrospective study analyzed digital intraoral scans and lateral cephalograms of 240 untreated orthodontic patients (age range: 13-17 years). Subjects were categorized into four groups: Angle Class I, Class II/1, Class II/2, and Class III. CoS depth was measured using OrthoCAD, while skeletal and dental variables were assessed via AX.Ceph software. Statist

PURPOSE: Dental caries and malocclusion are prevalent in children and frequently coexist. Whether malocclusion severity is independently associated with elevated caries risk, remains poorly characterised, particularly in Saudi Arabia. The aim was to examine whether malocclusion severity in the primary dentition is independently associated with caries risk level in Saudi preschool children.
MATERIALS AND METHODS: A single-centre cross-sectional survey was conducted at Qassim University, enrolling 250 Saudi children aged 3-6 years with complete primary dentitions. Malocclusion severity was grad

OBJECTIVES: Epidermolysis bullosa (EB) is a rare genetic disease characterized by skin and mucosal fragility caused by pathogenic variants in structural protein genes. Malocclusions have been previously described in patients with EB, mainly in case reports, with no cohort studies characterizing dentofacial characteristics across all EB types. This study aims to describe and compare the dentofacial characteristics of patients with EB across all types in a nationally representative cohort.
MATERIAL AND METHODS: A cross-sectional study examined 101 patients with genetically confirmed EB diagnosi

This case report discusses the genetic influence and treatment of class III malocclusion in identical twin sisters, in their early 20s, who presented with mandibular prognathism. Both exhibited concave profiles with hypodivergent growth patterns, but Sister A had a more pronounced concavity. Diagnosis revealed skeletal class III malocclusion with mandibular prognathism and negative overjet (9 mm in Sister A, 6 mm in Sister B). Treatment involved pre-surgical orthodontics, bilateral sagittal split osteotomy (BSSO) with mandibular setbacks (7 mm for Sister A, 5 mm for Sister B), followed by post

Mechanical orthodontic treatment is regarded as a classical strategy for directional tooth movement in confront of malocclusion. However, prolonged remodeling period and adverse complication risks, which are mainly ascribed to inadequate sensation of bio-mechanical cues, predominantly restrict orthodontic clinical outcomes. This study demonstrates that construction of biophysical static magnetic fields (SMFs) is highly involved in accelerated tooth movement and bone remodeling in manners dependent on amplification of mechanical sensing. The arrayed magnetic orthodontic accelerator (AMOA) with

Skeletal diagnosis and upper-airway health should be the foundation of risk assessment for the interdisciplinary patient, as the diagnosis impacts treatment planning and outcomes. Currently, there is an epidemic of dental arch deficiencies with multifactorial etiologies that contribute to dentofacial disharmony malocclusions. Assessing each dimension of the periodontal phenotype in the context of the planned intervention, termed phenotype-driven treatment planning (PDTP; as outlined in Part I of this series), offers an opportunity to evolve our treatment-planning concepts and allows for the se

Skeletal Class III malocclusions are frequently associated with craniosynostosis syndromes, resulting in distinctive facial and dental anomalies. Among these, Crouzon syndrome is characterised by mutations in the FGFR2 gene, leading to premature fusion of the cranial vault, cranial base, orbital and maxillary sutures. The phenotypic expression of this congenital condition varies according to the timing and pattern of sutural fusion. This report details the clinical features and management of a 16-year-old male patient presenting with moderate skeletal Class III malocclusion as a manifestation
